1.Hysteroscopic Surgery for Endogenous Cesarean Scar Pregnancy:Clinical Analysis of 34 Cases

Objective To investigate the applicative value of hysteroscopic surgery in the treatment of endogenous cesarean scar pregnancy ( CSP) . Methods A total of 34 patients with CSP in this hospital from April 2012 to October 2014 were treated by hysteroscopic surgery .Seven patients received uterine curettage under hysteroscopy and 27 patients got uterine artery embolization therapy before hysteroscopic surgery .All of them were regularly reviewed of β-hCG levels and ultrasound examinations . Results The surgical procedure was successfully completed in all the 34 cases.The operation time was (17.5 ±3.5) min, the intraoperative blood loss was (17.5 ±7.5) ml, and the hospitalization stay was 4-5 days.All of them had no serious complications .Their β-hCG levels were dropped to normal range within 28 days after operation , and all the patients had menstruation within 2 months after surgery . Conclusion Hysteroscopic surgery for CSP has advantages of good visibility , less bleeding, simple performance, high success rate, less postoperative complications , and shorter hospitalization time , being worthy of clinical utilization .
2.The effect of laparoscopy-assisted rectal cancer surgery on the level of Th1/Th2 balance and change of CD4+CD45R cell

Objective To explore the difference of the level of Th1/Th2 balance and change of CD4+ CD45R cell after treated by laparoseopy-assisted rectal cancer surgery and conventional open rectal cancer surgery, so to compare the effect of the two procedures on immunity function of the patients. Methods CD4+ IFN-γ+ T cell (Thl cell), CD4+ IL-4 + T cell( Th2 cell)and the ratio of Th1/Th2 were detected by flow cytometery ; The levels of IFN-γand IL-4 were measured by ELISA. The changes of CD4 + CD45RA+ T cell and CD4+ CD45RO+ T cell were detected in the different procedures so that the effect of the two procedures on eytoimmunity and immune balance were analyzed. Results Compared with baseline levels, the level of Th1 cells was decreased [ (4.51±1.52 ) %, (7.26 ±2.59) % vs. ( 12.06 ±1.82 ) % ] (P < 0.05 ) ; The level of Th2 cells was increased [ (6.70 ± 2.41 ) %, (6.70 ± 2.41 ) % vs. (4.47 ± 1.90) % ] ( P < 0.05 ) ; The level of IFN-γ was decreased [ (57.15± 23.64) ng/L, (72.70 ± 27. 31 ) ng/L vs. ( 123.47 ± 32. 52) ng/L ] ( P < 0. 01 ) ; The level of IL-4 was increased [(55.55±7.29)ng/L,(57.56 ±7.13)ng/L vs. [(41.87±5.83)ng/L] (P<0.05) on 2nd and 7th day after conventional open reeatal cancer surgery; The level of Thl, Th2, IFN-γ and IL-4 in the laparoscopy-assisted rectal cancer surgery was not different ( P > 0.05 ). At 7 day postoperatively, the level of the CD4+ CD45 RA+ T cells was in-creased (35.11 ±7.82) and the CD4+ CD45RO+T cells was decreased(61.21±9.13) compared with surgery be-fore(31.11±6.72) and (68.11 ±11.42) respectively (P< 0.05). Conclusions The laparoscopy-assisted rec-tal cancer surgery has less influence on immune balance, thus protect systemic cell immunity.
3.MTHFR C677T polymorphism contributes to unexplained recurrent spontaneous abortion

Objective To explore the relationship between methylenetetrahydrofolate reductase (MTHFR) 677C > T and unexplained recurrent pregnancy loss (URPL).Methods All patients were recruited from the outpatient department of Obstetrics/Gynaecology & Genetics of Hangzhou First People's Hospital from January 2013 to May 2014.A case-control study was performed.According to the stochastic indicator method,there were 125 subjects with a history of ≥2 times URPL as the case group,and 905 healthy parous women with no history of URPL as the control group.Polymerase chain reaction-restriction fragment length polymorphism (PCR-RFLP) was used to detect the distributions of the polymorphisms of MTHFR 677C > T,and the results were validated using oligo-chip and direct sequencing.Results The allele and genotype frequencies of MTHFR were 60.0% for C,40.0% for T,32.0% for CC,56.0% for CT,and 12.0% for TT in the case group and 67.4% for C,32.6% for T,41.4% for CC,52.0% for CT,and 6.6% for TT in the control group,respectively.The prevalence of allele T was significantly higher in the case group than in the control group (OR =1.379,95% CI =1.051-1.808,P =0.020),the frequency of genotype TT was also significantly higher in the case group than in the control group (OR =2.344,95% CI =1.220-4.503,P =0.009).Conclusion The fertile women with MTHFR 677T allele and 677TT genotype may be susceptibility to URPL in a Chinese Han population from the Hangzhou area.
4.Effect of Emodin on Hypertrophic Scar Fibroblasts

Objective To inVestigate the effect of emodin on hyPertroPhic scar fibroblasts ( HSFs ) and exPlore the underlying mechanism. Methods HSFs were treated by emodin at final concentrations of 0,20,40,and 80 μmol·L-1, resPectiVely,in the cultural media. Forty_eight hours later,the cells were subjected to MTS assay and flow cytometry assay with annexin V and ProPidium iodide as dyeing indicators. Whole cell lysates from the cells of eVery grouP were subjected to Western blotting to measure the Protein exPression leVels of ERK1∕2,Bcl_2,Mcl_1 and RIP1. Results The cell Viability of HSFs was inhibited by emodin in a dose dePendent manner. The mortality rate of HSFs treated with emodin for 48 h at the concentrations of 40 and 80 μmol·L-1 were 28. 6%and 68. 0%,resPectiVely,which was significantly higher than that of the control grouP ( P<0.01).Pretreatmentwith Z_VAD_FMK could Partially reduce the mortality caused by emodin (P<0.05).PhosPhorylation of ERK1∕2 and the exPression of RIP1 and Mcl_1 were inhibited by emodin. Conclusion Down regulation of ERK1∕2,RIP1 and Mcl_1 by emodin may account for the inhibited Proliferation and increased cell death of HSFs.
5.Antibiotic-resistant genes and multilocus sequencing typing of Pseudomonas aeruginosa

We investigated the antibiotic‐resistant genes and genetic diversity of Pseudomonas aeruginosa from patients in hospital ,the smear samples from hospital and clinic environment ,and from medical staff’ hands respectively in 2011‐2012 in Nanshan District of Shenzhen .Polymerase chain reaction were used to detect the 20 kinds of antibiotic‐resistant genes (TEM , VEB,CARB,OXA,SHV,PER,GES,GTX,SPM,GIM,IMP,VIM,DHA,oprD,Aac(6′)‐Ⅰ ,Aac(6′)‐Ⅱ ,Aac (3′)‐Ⅰ ,A ac(2″)‐Ⅰ ,qacE1‐sull and int‐Ⅰ) .Multilocus sequencing typing was used to analyze the clonal complexes .The 11 kinds resistant genes TEM ,SHV ,IMP ,DHA ,Aac(6′)‐Ⅰ ,Aac(6′)‐Ⅱ ,Aac(3′)‐Ⅰ ,Aac(2″)‐Ⅰ ,qacE1‐sull ,int‐Ⅰand oprD were detected ,for the positive rates respectively ,and which were 8 .1% ,6 .4% ,4 .8% ,9 .7% ,4 .8% ,14 .5% ,9 .7% , 56 .5% ,8 .1% ,and 8 .1% ;the loss rate of oprD gene was 61 .2% .The 19 antibiotic resistance gene profiles existed in 52 Pseudomonas aeruginosa strains .Multilocus sequencing typing found 39 sequence types and 5 clonal complexes in 62 Pseudo‐monas aeruginosa strains ,CC244 and ST856 were dominant .There were some differences of antibiotic resistance gene profiles between different samples ,the Pseudomonas aeruginosa strains from patients carried multiple resistant genes .In our research , the Pseudomonas aeruginosa had the genetic diversity and the dominant clonal complexes existed .
6.Comparative study of the effect of soft channel drainage and craniotomy in the treatment of moderate cerebral hemorrhage in basal ganglia

Objective To compare the clinical effect of minimal invasive drilling cranium from frontal part and craniotomy from temporal in the treatment of patients with cerebral hemorrhage in basal ganglia.Methods From August 2011 to August 2017,126 cases with cerebral hemorrhage in basal ganglia in the People's Hospital of Meishan were divided into observation group and control group,with 63 cases in each group.The observation group was treated by minimal invasive drilling cranium,and the control group was treated by craniotomy.The clinical treatment effect,operation indicators (operation time,amount of intraoperative bleeding,operation cost,discharge time) and nervous function recovery were analyzed.Results The operation time,amount of intraoperative bleeding,operation cost,discharge time of observation group were (31.6 ± 10.3) min,(20.9 ±5.8)mL,(2.1 ± 1.0) thousand CNY,(15.3 ± 0.9) d,respectively,which were better than those of the control group [(214.2 ± 46.8) min,(94.1 ± 56.2) mL,(15.0 ±4.8) thousand CNY,(21.4 ±0.4)d] (t =13.56,11.94,19.65,8.39,all P <0.05).The effective rate and the nerve function score between the two groups had no statistically significant differences (all P > 0.05).Conclusion The minimal invasive drilling cranium from frontal part has advantages of small surgical trauma,short operation time,less bleeding in the operation,low death rate,less cost of operation,short hospitalization time and good drainage effect,and thus it is worthy of clinical application.
7.Chylous leakage after retroperitoneoscopic upper-pole heminephrectomy for duplex kidney.

OBJECTIVE:
To discuss management of chylous leakage after retroperitoneoscopic upper-pole heminephrectomy for duplex kidney.
METHODS:
Between November 2004 and Februar y 2011, 39 patients underwent retroperitoneoscopic upper-pole heminephrectomy for duplex kidney, of these 5 patients had chylous leakage. The ages of the patients ranged from 32 to 60 years (mean 42). All the patients were treated conservatively, and the therapeutic effects were observed.
RESULTS:
Delayed chylous leakage in 5 patients occurred 5-31 days after surgery, and leakage occurred in 4 of the same 5 patients during the first 2 post-operative years. Chylous leakage after retroperitoneoscopic upper-pole heminephrectomy for duplex kidney preferentially occurred at the left side of duplex kidney. All the patients healed under conservative treatment.
CONCLUSION
Chylous leakage typically occurs after left retroperitoneoscopic upper-pole heminephrectomy for duplex kidney, and can be prevented by improving surgical technique; it can be completely relieved by conservative management with satisfactory results.

8.Effect of dexmedetomidine on emergence agitation after general anesthesia in children undergoing odontotherapy in day-surgery operating room.

OBJECTIVETo study the effectiveness of dexmedetomidine used for general anesthesia maintenance in children undergoing odontotherapy in day-surgery operating room in reducing the incidence of emergence agitation (EA).
METHODSEighty children undergoing odontotherapy and under general anesthesia in day-surgery operating room were randomized into two groups, group A (n=40) and group B (n=40). Each patient in group A was administered with a bolus dose of dexmedetomidine (1.0 μg·kg⁻¹, saline diluted to 10 mL) pump-infused after intubation and a maintenance dose of 0.1-0.4 mL·(kg·h)⁻¹ followed-up until 45 min before the end of operation. Each patient in group B was administered with a bolus dose of normal saline 10 mL pump-infused after intubation and maintenance dose of 0.1-0.4 mL·(kg·h)⁻¹ followed-up until 45 min before the end of operation. Gender, age, weight, physical status according to the American Society of Anesthesiologists, perioperative heart rate (HR), mean arterial pressure (MAP), pulse oxygen saturation (SpO₂), sufentanil dosage, duration of surgery, time of extubation, time of regaining consciousness, and time to reach modified Aldrete's score≥12 were recorded. Behavior in postanesthesia care unit was rated on the four-point agitation scale.
RESULTSCompared with group B, decreases were observed in HR and MAP at the beginning of operation, in 10 and 30 min, 1 and 2 h after the beginning of operation, and after extubation of group A (P<0.05). Sufentanil dosage and incidence of EA during recovery of group A were also lower than those of group B (P<0.05). Time to regain consciousness and time to reach modified Aldrete's score≥12 of group A were longer than those of group B (P<0.05). No statistical difference was observed between other indexes of the two groups.
CONCLUSIONSAs an anesthetic used for general anesthesia maintenance in children undergoing odontotherapy in day-surgery operating room, dexmedetomidine results in low incidence of EA during recovery and more stable vital signs.

Objective To compare differences of SRS-22 scores between male and female AIS patients.Methods From October 2007 to April 2009,298 AIS patients from 7 spine centers completed simplified Chinese edition of SRS-22 questionnaire,including 62 males and 236 females with an average age of 15.7 years old,and the average Cobb angle was 52.8°.Of 62 male patients,34 were from urban areas and 28 rural areas.Among 236 female patients,132 were from urban areas and 104 rural areas.A comparison was conducted in terms of age,Cobb angle,the ratio of urban population to rural population,and the scores of all domains and the subtotal scores of SRS-22 questionnaire between male and female patients.Results There was no difference of age,Cobb angle and the ratio of urban population to rural population between male and female patients(P>0.05).The scores of four domains and subtotal scores of SRS-22 questionnaire were significantly higher in male patients when compared with female patients(P<0.05).Conclusion Gender difference should be taken into the consideration when estimating the quality of life in AIS patients by SRS-22 questionnaire.The quality of life in male patients with AIS was better than that of female patients.
10.Mechanism of Qihuang Yiqi Shexue Prescription in Treatment of ITP Model Mice Based on Autophagy Mediated by AMPK/mTOR/ULK1 Signaling Pathway

ObjectiveTo explore the mechanism of Qihuang Yiqi Shexue prescription (QHYQSX) in the treatment of immune thrombocytopenia (ITP) model mice based on the autophagy mediated by the adenosine monophosphate-activated protein kinase (AMPK)/mammalian target of rapamycin (mTOR)/Unc-51-like kinase 1 (ULK1) signaling pathway. MethodFifty BALB/c mice were randomly divided into normal group, model group, high- and low-dose QHYQSX groups, and prednisone group, with 10 mice in each group. The ITP model was induced by intraperitoneal injection of anti-platelet serum (APS) of guinea pig. On the 8th day of the APS injection, drugs were administered by gavage for 14 days. Peripheral blood platelet (PLT) count and hemoglobin (Hb) concentration were detected. Spleen and thymus were separated, weighed, and the organ index was calculated. Sternum was sampled for bone marrow smear, and bone marrow megakaryocytes were classified under a microscope. Thrombopoietin (TPO), interleukin-6 (IL-6), IL-10, tumor necrosis factor-α (TNF-α), transforming growth factor-β1 (TGF-β1), and interferon-γ (IFN-γ) in the serum were detected by enzyme-linked immunosorbent assay(ELISA). AMPK, mTOR, ULK1, microtubule-associated protein light chain 3 (LC3), Beclin1, and p62 mRNA expression levels in the spleen were detected by Real-time fluorescence-based quantitative polymerase chain reaction (Real-time PCR). The protein expression of AMPK, p-AMPK, p-mTOR, p-ULK1, LC3Ⅱ/LC3Ⅰ, Beclin1, and p62 in the spleen was detected by Western blot. ResultCompared with the normal group, the model group showed reduced peripheral blood PLT count, Hb, and TPO levels (P<0.05,P<0.01), increased spleen and thymus indexes (P<0.01), decreased number of bone marrow megakaryocytes (P<0.01), elevated serum levels of IL-6, TNF-α, and IFN-γ (P<0.01), and reduced IL-10 and TGF-β1 levels (P<0.01). Compared with the model group, the groups with drug intervention showed increased PLT counts and TPO levels (P<0.01), decreased spleen and thymus indexes (P<0.05, P<0.01), elevated number of bone marrow megakaryocytes (P<0.05, P<0.01), reduced serum levels of IL-6, TNF-α, and IFN-γ (P<0.05, P<0.01), and up-regulated IL-10 and TGF-β1 levels (P<0.05,P<0.01). Compared with the low-dose QHYQSX group, the high-dose QHYQSX group and the prednisone group showed different degrees of significant differences in improving PLT counts and levels of cellular inflammatory factors (P<0.05, P<0.01). Real-time PCR and Western blot results showed that compared with the normal group, the model group showed up-regulated mRNA expression of AMPK, LC3, and Beclin1 and protein expression of p-AMPK/AMPK, LC3Ⅱ/LC3Ⅰ, and Beclin1 in the spleen (P<0.05, P<0.01), and down-regulated mRNA expression of mTOR, ULK1, and p62 and protein expression of p-mTOR, p-ULK1, and p62 (P<0.05, P<0.01). Compared with the results in the model group, high- and low-dose QHYQSX and prednisone could down-regulate the mRNA expression of AMPK, LC3, and Beclin1 and protein expression of p-AMPK/AMPK, LC3Ⅱ/LC3Ⅰ, and Beclin1 in the spleen (P<0.05, P<0.01), and up-regulate the mRNA expression of mTOR, ULK1, and p62 and protein expression of p-mTOR, p-ULK1, and p62 (P<0.05, P<0.01). ConclusionQHYQSX may inhibit excessive autophagy by regulating the AMPK/mTOR/ULK1 signaling pathway, thereby regulating immune intolerance and playing a role in the treatment of ITP.
