1.PRELIMINARY OBSERVATION OF THE ULTRASTRUCTURE OF SYNAPTONEMAL COMPLEX OF SPERMATOCYTES OF SEVERAL EXPERIMENTAL RODENTIAL ANIMALS.

Synaptonemal complex was first discovered by Moses in 1956. Synaptonemal complex of karyocytes of meiosis of pachytene are observed by electron microscope. They are bivalent chromosomal central substructure. We have observed synaptonemal complex of guinea-pig, rabbit, rat, and mice (Jin Bai Ⅰ, Ⅱ etc.). The basic structure of synaptonemal complex are similar. They contain two lateral elements and one central element which situated in the central space.The end of the synaptonemal complex is attached to the nuclear membrane,. where the inner nuclear membrane forms a thick platelike structure. Lateral elements of spermatocytes of the mouse are divided into two or three sections. The sex vesicle and associated nucleolus appear clearly in the mouse.The axial filaments of X and Y chromosome are attached with an angle of 30 degrees between each other. A typical central element and transverse filaments are formed in the space between. Several nodules are situated on the central element. The number of nodules is from 1-4 in the several experimental rodential animals studied.
2.Analysis of the interference factors of indwelling gastric tube and nursing countermeasure for patients undergoing laryngeal cancer surgery

Objective To investigate the interference causes of indwelling gastric tube for patients undergoing laryngeal tumor surgery,and explore the appropriate clinical nursing countermeasures.Methods 60 cases of laryngeal cancer patients with indwelling gastric tube were divided into the control group and the observation group with 30 cases in each group.The control group used indwelling gastric tube with routine nursing,while in the observation group,the influencing factors for indwelling gastric tube were analyzed and per-tinent neasures were adopted timely.The satisfaction degree of patients,incidence of postoperative compli-cations,hospitalization time and effective rate of treatment were compared.Results The factors influ-encing use of indwelling gastric tube were tracheal extubation by patients,trachea cannula exodus,pipe blockage and postoperative complications.After timely expectant treatment and positive nursing,the nutri-tion intake and postoperative recovery were not affected.The related indexes of the observation group were better than those of the control group.Conclusions Patients after laryngeal cancer surgery need longtime indwelling gastric tube.Measures such as strengthening of education and propaganda,close observation of patients′ condition and timely expectant treatment should be given in order to ensure the normal use of gastric tube and reach desirable treatment effect.

5.Investigation of self-efficacy and countermeasures for the nursing activities of nurses in operation room

Objective To investigate the self-efficacy for the nursing activities of nurses in the operation room and explore the ways of improving the self-efficacy of nursing activities of nurses in the operation room.Methods 160 nnrses in the operation rooms of four hospitals in Binzhou city were chosen as the observation group.160 non-operation room nurses in these four hospitals were chosen as the control group.The self-efficacy of nursing activities questionnaires were used to survey the self-efficacy of nurses.According to the analysis of survey results,measures to improve the self-efficacy of nursing activities were discussed.Results The self-efficacy scores for 160 operation room nurses were lower than those non-operation room nurses.Among 5 dimensions,nursing skill was with the lowest score in 160 nurses who had been investigated.The difference of self-efficacy scores of care activities for nurses in operation rooms of different hospitals was significant.Conclusions Nursing activity in the operation room has its own specialty.The self-efficacy of nursing activities was lower than non-operation room nurses generally,especially in nursing skills.Improving leadership and working environment of nurses,giving positive social support and re-education programs are the scientific ways to improve self-efficacy of nursing activities in the operation room.
6.Application of holistic nursing in operation room for surgical children patients

Objective To explore the clinical effect of holistic nursing in the operation room for surgical children patients. Methods 120 patients required surgical treatment were selected and divided into the control group(45 cases)and the observation group(75 cases)randomly.The control group was given routine nursing,the observation group was given holistic nursing including preoperative interview,psychological care and all the preparations,strict check upon going into the operation room and before surgery,to help children keep graceful position,to cooperate closely with the anesthetist and surgeon,to observe the condition of patients and give good infusion management,to give continued observation of the disease condition after surgery,to make children safe escort back to the ward,and to make transfer with ward nurses.The treatment effect was compared between two groups. Results Two groups of children passed the period of operation and anesthesia safely,without appearing accidental situation.The satisfaction degree of the observation group was significantly higher than the control group.One case appeared operation accident and two cases appeared anesthesia awareness in the control group,and they were correctly handled.No one appeared operation accident and anesthesia awareness in the observation group.The differences between the two groups were significant. Conclusions Application of holistic nursing in the operation room for children patients puts forward higher requirements for nurses.It can help children cooperate with surgery better to achieve better surgical results,which is worthy of promotion.
7.An exploratory clinical study of attention deficit hyperactivity disorder in young adulthood

Objective The exploratory study aimed to characterize the clinical features and the patterns of comorbidity and psychosocial functioning of adult attention deficit hyperactivity disorder (ADHD). Methods The study initially recruited 88 children who diagnosed with ADHD at age 6~16 years. They were reevaluated and administered structured psychiatric interviews for DSM-IV when they were at least 18 years old. A total of 59 adults with ADHD and 29 controls with GAF>70 were included in the study. Results Fifty-one(86.4%)of the adult ADHD were diagnosed as predominantly inattentive subtype of ADHD. The most common adult ADHD symptoms included "often has difficulty organizing tasks and activities (98.3%); often avoiding doing work projects where effort at thinking a lot is required (96.6%); often has trouble paying close attention to details (94.9%); often has problems following through on instructions and finishing job tasks (96.6%); often has difficulty sustaining attention (88.1%); often is distracted easily by extraneous stimuli (72.9%)". The majority (66.1%) of the adult ADHD had one or more psychiatric axis-I/axis-II comorbidity. Among them, 39.0% had psychiatric axis-I comorbidity and 49.2% had personality axis-II comorbidity. The average GAF score was significant lower in adult ADHD than in controls ( t =12.96, P <0.001). The majority (74.6%) of the adult ADHD had mild or moderate impairment or distress of overall functioning. Conclusions The main feature of adult ADHD is attention deficit. Patients with adult ADHD usually have comorbid conditions such as other psychiatric disorders and personality disorders and their psychosocial functioning is relatively poor.
8.The Pregnancy Improvement of IVF-ET by Mental Intervention

Objective To study the effect of mental intervention on pregnant result in patients undergoing IVF-ET treatment.Methods 206 patients undergoing IVF-ET treatment in the hospital from February 2012 to Jan-uary 2013 were selected and randomly divided into two groups. The experiment group (n=103) was followed by mental intervention after the patients enrolled the IVF-ET treatment, the control group (n=103) enrolled the IVF-ET treatment without mental intervention.Results The HAMA and HAMD quantity grades for anxious and depression of patients in the experiment group were significantly lower than those of control ( <0.01), and the pregnancy rate of experiment group (45.63%) was higher than that of control (30.10%) <0.05. Conclusion Mental intervention for patients undergoing IVF-ET treatment could alleviate their anxiety-depression and improve the pregnancy rate of IVF.
9.Practice and evaluation of creating magnetic nursing work environment in hospital

Objective To explore the moves that creating magnetic work environment in hospital, in order to improve the nurses job satisfaction and reduce the turnover rate then stable the nursing team. Methods Through a series of measures such as the establishment of scientific rational allocation of human resources, implementing fair performance evaluation and allocation mechanism, carrying out the nurse grading management system to create magnetic nursing work environment, the nurse job satisfaction and turnover rate were compared before and after the measures. Results In January 2014 a total of 520 questionnaireswere issued, 519 valid questionnaires, and the effective recovery rate was 99.81%(519/520);And 559 were valid of 577 issued questionnaires, which was 96.88% (559/577). The overall score of nurses′job satisfaction increased from (3.20±0.46) points in 2013 to (3.41±0.52) points in 2014 (t=9.40, P= 0.000); The nurse turnover rate fell to 1.97%(13/661) from 5.13% (30/585) (χ2= 10.27, P= 0.003). Conclusions That is very important for remain stable and high quality nursing team to carry out scientific and fine management according to the demand of nurses, so as to create a safe, harmonious, positive, cooperative magnetic nursing work environment which have development direction and space.
10.Efficacy of retinal inner limiting membrane peeling and intravitreal injection of triamcinolone acetonide for secondary epiretinal membrane

Objective To observe the efficacy of retinal inner limiting membrane peeling and intravitreal injection of triamcinolone acetonide for secondary epiretinal membrane.Methods Forty-two patients (42 eyes) with secondary epiretinal membrane underwent vitretinal surgery in our hospital from June 2011 to June 2015 were retrospectively studied.All patients underwent pars plana vitrectomy + epiretinal membrane peeling + retinal inner limiting membrane peeling and intravitreal injection of triamcinolone acetonide.The follow-up time was from 3.0 months to 18.0 months,averaged (8.3 ± 2.4) months.Postoperative best corrected visual acuity (BCVA),macular morphology and complications were observed.Resules At the final follow-up,BCVA improved in 30 eyes,unchanged in 8 eyes and decreased in 4 eyes,and there was significant difference compared with that before operation (t =3.145,P =0.000).The preoperative and postoperative thickness of meuroepithelium in the centual macular area were(315.62 ±132.12) μm and (233.42 ± 146.32) μm,and there was significant difference(t =4.322,P =0.000).Small amount of retinal hemorrhage happened in 6 eyes after operation,vitreous hemorrhage was in 1 eye,transient high intraocular pressure was in 6 eyes;Prepheral retinal hole occurred in 3 eyes,which were cured by laser treatment and C3F8 tamponade.Conclusion Intravitreal injection of triamcinolone acetonide can reduce the macular edema,decrease the reoccurrence of epiretinal membrane and imporve the visual acuity.The retinal inner limiting membrane peeling in secondary macular epiretinal membrane surgery can release the macular pucker,lifting and traction on the fovea,improve the metabolism of macular local,is conducive to the recovery of macular function.
