1.Application of palpebral strip suspension on the outside of the lower eyelid for moderate to severe iatrogenic lateral canthal deformities

Objective:To investigate the application of the palpebral strip suspension on the outer side of the lower eyelid for moderate-to-severe iatrogenic lateral canthal deformities.Methods:From October 2019 to October 2020, patients with moderate to severe lateral canthal deformity caused by lateral canthoplasty were admitted to the Department of Medical Cosmetology of the Second Affiliated Hospital of Zhengzhou University. They received lateral tarsal sling suspension of the lower eyelid. The improvement in lateral canthus shape, lateral angle position, blepharophimosis size and the lid bulb separation was observed and analyzed.Results:Seven female patients, aged from 23 to 48 years, with 14 eyes, were included. Complete correction of the canthal deformity was achieved in all the patients. During the follow-up period of more than 6 months, all patients had complete recovery of the external canthus shape and position, as well as symmetry of the bilateral lid fissure size, and improvement of the lid bulb separation without serious complications.Conclusions:The palpebral strip suspension on the outer side of the lower eyelid for moderate-to-severe iatrogenic lateral canthal deformities is easy to perform, with satisfactory postoperative result and few complications.

3.Meningitis caused by Corynebacterium:report of two cases and literature review

Objective To report two cases of meningitis caused by Corynebacterium and review relevant literature to enhance clinical awareness of the pathogenicity of Corynebacterium species.Methods The clinical details were reported for 2 cases of meningitis caused by Corynebacterium.Similar case reports were retrieved from PubMed,CNKI,Wanfang,VIP,and CBM databases using search terms"Corynebacterium meningitis"and"cerebrospinal fluid shunt infection".Clinical data of the identified patients were reviewed and analyzed.Results Both patients with meningitis caused by Corynebacterium developed symptoms of fever,abdominal pain,and unconsciousness following lumboperitoneal shunt procedures.The culture of cerebrospinal fluid was positive for Corynebacterium.Both patients improved significantly after vancomycin treatment.Literature search yielded 29 similar cases.Overall,the 31 cases(including the two cases in this report)included 20 males and 11 females,aged between 4 weeks to 87 years.The identified Corynebacterium species included C.jeikeium in 12 cases,C.striatum in 9 cases,C.xerosis in 2 cases,C.bovis in 2 cases,C.aquaticum,C.equine,and C.minutissimum one case each,and unspecified Corynebacterium species in 3 cases.Underlying diseases were reporetd in most patients,including immune disorder in 2 cases,malignant tumor in 8 cases,genetic disease in 2 cases,premature with hydrocephalus in 4 cases,brain trauma in 2 cases,intracranial vascular malformation in 2 cases,and cerebral aneurysm in 2 cases.Risk factors for Corynebacterium meningitis included chemotherapy for malignancy in 5 cases,long term use of steroids and immunosuppressants in 2 cases,invasive procedures in 24 cases,and prior antibiotic use in 11 cases.Vancomycin was the most commonly prescribed treatment.The drainage tube and/or venous catheter were also removed or replaced in 19 cases.Twenty-five patients recovered after treatment and 6 patients died.Conclusions Corynebacterium species are emerging as opportunistic pathogens.Clinicians should be aware of the infections caused by Corynebacterium if patients have undergone invasive procedures or are immunocompromised,especially when associated with fever,altered consciousness,and clinical signs of encephalitis/meningitis.If cerebrospinal fluid test is positive for Corynebacterium,specific treatment should be prescribed promptly for Corynebacterium infection.
4.Prenatal diagnosis of Pallister-Killian syndrome: a report of two cases

This article reported two fetuses diagnosed with Pallister-Killian syndrome (PKS). Amniotic fluid samples of one pregnant woman (case 1), with an abnormal result of non-invasive prenatal test, were analyzed by karyotyping and copy number variation-sequencing (CNV-seq), and samples of the other one (case 2), with a prenatal ultrasound of a lowered spinal conus and an enhanced echo of the renal cortex, were tested for CNV-seq. The chromosome karyotype of case 1 showed 47,XN,+mar[30]/46, XN[10]. CNV-seq results revealed three to four copies of repetition of the p13.33p11.1 segment of chromosome 12, suggesting 12p tetrasomy chimerism in both fetuses. The diagnosis of fetal PKS was confirmed through the combination of multiple technologies (ultrasound, chromosomal karyotype analysis, and CNV-seq). The two pregnancies were terminated after genetic counseling.
5.Quantitative analysis of non-alcoholic fatty liver disease based on MRI proton density fat fraction in the coastal region of Qingdao

Objective:To explore the clinical application value of MRI-PDFF on different liver segments for the evaluation of non-alcoholic fatty liver disease (NAFLD).Methods:178 volunteers from March 2019 to February 2020 were included. PDFF values ??of all nine segments of the liver were measured using CSE3.0T MRI scan. The obtained average value was used to represent the average liver fat content. PDFF values of each or combined liver segment were equally compared with the average value to observe the representativeness of fat content. Receiver operating characteristic curve was used to analyze the diagnostic performance of each liver segment, and the Youden index was used to calculate the cutoff value. Paired-sample t-test or non-parametric Kruskal-Wallis test were used to compare measurement data among groups.Results:178 volunteers average liver fat content ranged from 0.89% to 42.61% with MRI-PDFF, and 71.35% (127/178) of the volunteers had PDFF > 5%. There was no significant difference between SIII, SIVb, SV, and SVIII liver segments when compared with the average value ( P > 0.05). PDFF values ??of SI, SII, and SIV a liver segments were all lower than the average value, while the PDFF values ??of SVI and SVII liver segments were all higher than the average value ( P ??< 0.05). MRI-PDFF sensitivity value for diagnosing liver steatosis of nine liver segments was 85.8% ~ 94.5%, and the specificity was higher than 96.0%. Among them, the SV liver segment had the highest sensitivity (94.5%), and the corresponding optimal diagnostic threshold value was 5.13%. Compared with single and combined liver segment, the PDFF value of SII, SV, SVI combined liver segment had the highest diagnostic performance for fatty liver, with the sensitivity and specificity of 96.9%, and 100%, respectively, and the corresponding optimal diagnostic threshold value was 5.17%. Conclusion:Compared with single and other combined liver segments, MRI-PDFF values of SII, SV, and SVI combined liver segments have higher sensitivity and specificity for the diagnosis of NAFLD, and it can be used as the first choice for the determination of liver fat content with MRI.
6.Intervention study of Orff rhythm-beat-tap training on positive emotions in elderly patients with mild dementia

Objective:To study effects of Orff rhythm-beat-tap training on positive emotions in elderly patients with mild dementia.Methods:Using the convenient sampling method, a total of 72 elderly patients with mild dementia who were treated in the Third People's Hospital of Huzhou from August 2018 to August 2019 were selected as the research objects and they were divided into two groups using the simple random sampling method. After the start of the study, 2 cases dropped out because of failure to persist in training, 1 case dropped out because of transfer during hospitalization and 1 case dropped out because of aggravation of the disease and changing drugs. A total of 68 cases were included in the study, including 33 cases in the intervention group and 35 cases in the control group. The control group received routine nursing, while the intervention group received 12-week Orff rhythm-beat-tap training on this basis. The scores of Positive and Negative Affect Schedule (PANAS) , Apathy Evaluation Scale (AES) and Dementia Mood Assessment Scale (DMAS) were evaluated before and after the intervention.Results:The score of positive emotion after intervention in the intervention group was (31.58±1.87) , higher than (26.66±2.93) in the control group, and the difference was statistically significant ( t=8.297, P<0.01) . After intervention, the score of negative emotion of the intervention group was (21.97±2.30) , the score of apathy was (44.88±5.22) , and the score of dementia mood was (23.18±4.33) , which were lower than (23.80±3.94) , (51.91±7.49) and (26.09±6.20) of the control group, and there were statistically significant differences in scores between the two groups ( t=-2.356, -4.514, -2.225; P<0.05) . Conclusions:Orff rhythm-beat-tap training is suitable for elderly patients with mild dementia, which can effectively enhance positive emotional experience of patients, improve apathy and optimize mood.
7. Application of ultra-thin oblique posterosuperior auricular fascial flap in the second stage of Nagata microtia reconstruction

Objective:
To investigate the effect and safety of ultra-thin oblique posterosuperior auricular fascial flap in the second stage of ear reconstruction.
Methods:
Fifty-six cases with congenital microtia treated in The Second Affiliated Hospital of Zhengzhou University Medical Cosmetology Department from November 2015 to November 2018 were selected as the research objects. In the first stage, the costal cartilage ear stent was implanted, and in the second stage. The ultra-thin posterior oblique fascia flap was used to cover the stent. And the cranioauricular angle was reconstructed by free skin grafting.
Results:
48 cases of the 54 cases completely survived, and 8 cases had hemorrhage after operation. The wounds were healed after dressing change. The postoperative cranioauricular angle was close to the healthy side.
Conclusions
In the second stage of ear reconstruction, the retroauricular ultra-thin fascial flap method has reliable blood supply, small wound, unswollen reconstructed ears, and good outcome. It is a good choice for covering the ear framework.
8.Fermentation, purification and immunogenicity evaluation of hepatitis E virus-like particles expressed in Hansenula polymorpha.

To develop a new recombinant hepatitis E vaccine, we used Hansenula polymorpha expression system to express recombinant hepatitis E virus-like particles (HEV VLPs), to construct a recombinant engineered strain HP/HEV2.3. The fermentation conditions and purification process were studied next. The first working seed lots were fermented in liquid culture, and the fermentation products were collected, then crushed, clarified, purified by ultrafiltration, silica gel adsorbed and desorbed, concentrated by ultrafiltration, purified by liquid chromatography and sterilized by filtration. The purity reached 99% with a yield of 33%. Electron microscopy analysis revealed that both the purified recombinant HEV VLPs from HP/HEV2.3 and natural hepatitis E virus particles appear identical of being 32 nm. The resulting DNA sequence obtained from VLPs is identical to the published HEV sequence. The SDS-PAGE analysis has revealed that the protein molecular weight of the HEV VLPs is 56 kDa, and the expression product HEV VLPs were accumulated up to 26% of total cellular protein. The expression level is 1.0 g/L. Western blotting, enzyme-linked immunosorbent assay (ELISA) results of the protein and ED₅₀ of the vaccine showed that the HEV VLPs have good antigenicity and immunogenicity. In summary, the recombinant HEV VLPs from Hansenula polymorpha can be used in the manufacture of a new genetically engineered vaccine against hepatitis E.
9.Single-cell analysis reveals an Angpt4-initiated EPDC-EC-CM cellular coordination cascade during heart regeneration.

Mammals exhibit limited heart regeneration ability, which can lead to heart failure after myocardial infarction. In contrast, zebrafish exhibit remarkable cardiac regeneration capacity. Several cell types and signaling pathways have been reported to participate in this process. However, a comprehensive analysis of how different cells and signals interact and coordinate to regulate cardiac regeneration is unavailable. We collected major cardiac cell types from zebrafish and performed high-precision single-cell transcriptome analyses during both development and post-injury regeneration. We revealed the cellular heterogeneity as well as the molecular progress of cardiomyocytes during these processes, and identified a subtype of atrial cardiomyocyte exhibiting a stem-like state which may transdifferentiate into ventricular cardiomyocytes during regeneration. Furthermore, we identified a regeneration-induced cell (RIC) population in the epicardium-derived cells (EPDC), and demonstrated Angiopoietin 4 (Angpt4) as a specific regulator of heart regeneration. angpt4 expression is specifically and transiently activated in RIC, which initiates a signaling cascade from EPDC to endocardium through the Tie2-MAPK pathway, and further induces activation of cathepsin K in cardiomyocytes through RA signaling. Loss of angpt4 leads to defects in scar tissue resolution and cardiomyocyte proliferation, while overexpression of angpt4 accelerates regeneration. Furthermore, we found that ANGPT4 could enhance proliferation of neonatal rat cardiomyocytes, and promote cardiac repair in mice after myocardial infarction, indicating that the function of Angpt4 is conserved in mammals. Our study provides a mechanistic understanding of heart regeneration at single-cell precision, identifies Angpt4 as a key regulator of cardiomyocyte proliferation and regeneration, and offers a novel therapeutic target for improved recovery after human heart injuries.
