1.Treatment of cystic craniopharyngiomas by CT-guided stereotactic neuroendoscopic resection and intratumoral chemotherapy.

Objective To investigate the treatment of cystic craniopharyngiomas by CT-guided stereotactic neuroendoscopic resection and intratumoral chemotherapy. Methods 16 cases of cystic craniopharyngiomas were partial resected by CT-guided stereotactic neuroendoscopy. Intratumoral chemotherapy with bleomycin were given postoperatively. Results The clinical symptoms improved promptly after evacuations of cyst in all patients. No death or severe complications occurred. Follow-up (ranged from 2 to 3 years) CT or MRI indicated that the tumor cysts gradually regressed or disappeared. Conclusions The treatment of CT-guided stereotactic endoscopic resection and intratumoral chemotherapy for cystic craniopharyngioma is safe and effective, which should be a very useful procedure in clinical practice.
2.Intraparenchymal Atypical Meningioma in Basal Ganglia Region in a Child: Case Report and Literature Review

Intraparenchymal meningiomas without dural attachment are extremely rare, especially when they occur in basal ganglia region in child. An 8-year-old boy was admitted at our hospital, complaining of recurrent headache and vomiting for 3 months. Neurological examination showed impaired vision and mild paresis of the left extremities. Magnetic resonance imaging demonstrated a lesion located in the right basal ganglia region extending to superasellar cistern with solid, multiple cystic and necrotic components. Computed tomography revealed calcification within the mass. Due to the anterior cerebral artery involvement, a subtotal resection was achieved and postoperative radiotherapy was recommended. Histopathological examination indicated that the lesion was an atypical meningioma. The postoperative rehabilitation was uneventful. Mildly impaired vision and motor weakness of left extremities improved significantly and the patient returned to normal life after surgery. To our knowledge, intraparenchymal atypical meningioma in basal ganglia extending to superasellar cistern was never reported. The significance in differential diagnosis of lesions in basal ganglia should be emphasized.

3.Blood-Blister Aneurysms of the Internal Carotid Artery in Tibetan and Han Populations : A Retrospective Observational Study

Objective:
: Blood-blister aneurysms (BBAs) of the internal carotid artery (ICA) are challenging lesions with high morbidity and mortality rates. Although research on BBAs is well documented in different populations, the study of BBAs in the Tibetan population is extremely rare. This study aimed to evaluate the characteristics of BBAs and analyze the treatment modalities and long-term outcomes in the Tibetan population in comparison with the Han population.
Methods:
: The characteristics of patients with BBAs of the ICA from January 2009 to January 2021 at our institution were reviewed. The features of aneurysms, treatment modalities, complications, and follow-up outcomes were retrospectively analyzed.
Results:
: A total of 130 patients (41 Tibetan and 89 Han patients) with BBAs of the ICA who underwent treatment were enrolled. Compared with the Han group, the Tibetan group significantly demonstrated a high ratio of BBAs among ICAs (8.6%, 41/477 vs. 1.6%, 89/5563; p<0.05), a high ratio of vasospasm (34.1%, 14/41 vs. 6.7%, 6/89; p=0.001), a high risk of ischemic events (43.9%, 18/41 vs. 22.5%, 20/89; p<0.05), and a low ratio of good outcomes (modified Rankin scale, 0–2) at the 1-year follow-up (51.2%, 21/41 vs. 74.2%, 66/89; p<0.05). The multivariate regression model showed that ischemic events significantly contributed to the prediction of outcomes at 1 year. Further analysis revealed that microsurgery and vasospasm were associated with ischemic events.
Conclusion
: In comparison with Han patients, the Tibetan population had a high ratio of BBA occurrence, a high incidence of ischemic events, and a high ratio of poor outcomes. The endovascular approach showed more benefits in BBA patients.
5.Study on quality grade standard of premature Forsythia suspensa

OBJECTIVE To study the quality grade stand ard of the premature Forsythia suspensa . METHODS A total of 138 batches of premature F. suspensa were collected from the main producing areas of F. suspensa in China. According to 2020 edition of Chinese Pharmacopoeia ,the contents of impurities ,moisture,ethanol-soluble extract ,volatile oil ,forsythin and forsythoside A in the premature F. suspense were determined ,and the qualified samples were screened. AHP-PCA mixed weighting method was used to give comprehensive weight to the indicators (except for the limit of impurity ). The comprehensive score of the samples was calculated. The suggestions on the quality grade division of premature F. suspensa were put forward according to cluster analysis of K-mean value. RESULTS & CONCLUSIONS The contents of impurities ,moisture,ethanol-soluble extract ,volatile oil ,forsythin and forsythoside A in the premature F. suspense were 0-7.80%,1.60%-8.18%,13.13%-61.60%,0.21%-3.47%,0.02%-2.15% and 0.79%-14.04%,respectively;average contents of them were 1.24%,4.97%,34.88%,2.01%,0.42%,6.86%,respectively. Totally 47 batches of 138 batches were qualified in all indexes. It is suggested that the quality grade of the premature F. suspense can be divided into three grades :in first grade of F. suspense ,the contents of volatile oil ,forsythin,forsythoside A , ethanol-soluble extract and moisture were ≥2.40%,≥0.59%,≥8.34%,≥38.66% and ≤4.99%,respectively;in second grade of F. suspense ,the contents of above indicators were ≥2.26%,≥0.41%,≥7.47%,≥32.58% and ≤5.33%,respectively;in third grade of F. suspense ,the contents of above indicators were ≥2.15%,≥0.32%,≥4.60%,≥31.52% and≤7.23%,respectively.
