1.Tension-free mesh plug hernioplasty under local anesthesia in out-patient clinic: Report of 65 cases

Objective To discuss the feasibility and safety of tension-free mesh plug hernioplasty under local anesthesia in out-patient clinic.Methods Tension-free mesh plug hernioplasty under local anesthesia was performed in 65 cases of unilateral inguinal hernia in out-patient clinic from April 2003 to May 2005.There were 56 cases of indirect inguinal hernia(including 1 case of recurrent hernia) and 9 cases of direct hernia.Results The hernioplasty was successfully completed in all the 65 cases,with good anesthetic results registered.The operation time was 20~65 min(mean,32 min),and the length of clinic stay,3~20 h(mean,(4.8 h)).Incisional fat liquefaction occurred in 1 case.Follow-up visits for 2~24 months(mean,9.5 months) found no recurrence or testicular atrophy.Conclusions Tension-free mesh plug hernioplasty under local anesthesia is a safe,reliable,and cost-effective out-patient procedure for patients with type Ⅱ~Ⅲ unilateral inguinal hernia.
2.Cloning of survivin-an inhibitor of apoptosis proteins and expressed in pichia pastoris system

Objective To clone human survivin gene and express survivin-an inhibitor of apoptosis proteins in Pichia pastoris eukaryotic expression system. Methods Full length survivin gene was amplified by PCR using survivin specific primers. The verified survivin gene by sequencing was subcloned into pPic9k. The recombinant plasmid was linearized by restriction enzyme cutting. The linear survivin gene was introduced into GS115/ His-cells by electroporation. The transformants were transferred onto YPD plates that contained different concentrations of G418 for screening the positive clones. The integrated survivin gene in positive clones was confirmed by PCR. Selected transformants were cultured in BMMY medium with 1 % methanol for inducing the expression of survivin protein. The expressed survivin protein was analyzed by ELISA. Results The cloned human survivin sequence was the same as that in the GeneBank. The recombinant pPic9k-survivin was constructed in Picha pastoris eukaryotic expression vector. After the linear digestion, the recombinant vector was introduced into GS115/ His-cells, the 6 positive clones against G418(4 mg/mL) were obtained and confirmed by PCR; the highest survivin protein was expressed when expressed for two days in 1 % methanol BMMY medium. Conclusions Improved survivin protein yield may be reached by modifying the experimental conditions. This will help to further study the biological functions of survivin and its roles in tumor developments.
3.Effects of Allopurinol on Inhalation Pulmonary Injury in Rats

Six hours after inhalation pulmonary injury,the pulmonary microvascular permeability (PMVP),pulmonary malondialdehyde (MDA) level,superoxide dismu-tase activity (SOD),the contents of protein and leucocytes in broncho-alveolar lavage fluid(BALF),and serum MDA level were determined in rats pretreated with allopurinol.In addition,the pathological changes of the lung tissues were also studied.It was found that early administration of allopurinol resulted in marked reduction of PMVP,and the protein content and leucocyte count in BALF.His-topathological examinations revealed that allopurinol could reduce the leucocyte accumulation in pulmonary vessels,interstitial leucocyte infiltration,interstitial and alveolar edema and hemorrhage,and vascular congestion.The serum and pulmonary levels of MDA also markedly decreased.The changes of pulmonary level of MDA were parallel to those of PMVP,and the protein content and leucocyte count in BALF in time course and in degree.The results listed here imply that the mechanism of the therapeutic effects of allopurinol on inhalation pulmonary injury is to reduce the production of superoxide anions through the inhibition of xanthine oxidase,in turn to inhibit the peroxidation of pulmonary lipids to alleviate pulmonary damage.
4.Put into perspective clinical advantages and limitations of all-in-one femtosecond laser corneal refractive surgery

The all-in-one femtosecond laser corneal refractive surgery,especially small incision lenticule extraction (SMILE),has gained more and more attention in clinic,because of its small incision,“no flap” and corneal biomechanical stability advantages.However,like any other kind of corneal refractive surgery,the all-in-one femtosecond laser refractive surgery not only exists its clinical advantages,but also has its limitations.It is very important to put into perspective clinical advantages and limitations of all-in-one femtosecond laser corneal refractive surgery.
6.Research progress of epileptic encephalopathy

Epileptic encephalopathies are an epileptic condition characterized by epileptiform abnormalities associated with progressive cerebral dysfunction.These syndromes include early myoclonic encephalopathy and Ohtahara syndrome in the neonatal period,West syndrome and Dravet syndrome in infancy,myoclonic status in nonprogressive encephalopathies,and Lennox-Gastaut syndrome,Landau-Kleffner syndrome,and epilepsy with continuous spike waves during slow wave sleep in childhood and adolescences.Other epileptic syndromes such as migrating partial seizures in infancy and severe epilepsy with multiple independent spike foci may be reasonably added.In this paper,an overview of epileptic encephalopathies including clinical features,etiology,electroencephalogram characteristics,treatment and prognosis are provided.
7.Inborn errors of metabolism and epilepsy in children

Epilepsy is a common disorder of neurological system in children.Inborn errors of metabolism (IEMs) is one of an important etiology of epilepsy.Seizures may be the first and the major presenting feature of IEMs.IEMs are a relatively rare cause of epilepsy,but their recognition and diagnosis is very important because several disorders are treatable.This review will concentrate on diseases where epilepsy is the predominant clinical manifestation and especially where the disease course can be positively influenced by specific metabolic therapies.

Generalized epilepsy with febrile seizures plus(GEFS+)is a familial in herited epileptic syndrome characterized by phenotypic heterogeneity from the mi lder febrile seizures to the severest epileptic encephalopathy such as severe my oclonic epilepsy in infancy(SMEI).GEFS+ is a disorder with a genetic heterogen eity.Molecular genetics have revealed that four genes are associated with the p athogenesis of GEFS+.These include mutations in genes encoding subunits of neur onal voltage-gated sodium channels(SCN1A,SCN1B,SCN2A)and ?2 subunit of the gamma amino-butyric acid(GABA)A receptor(GABRG2).These genes have be en confir med as having a role in autosomal dominant GEFS+ families.In addition,the phen otypes of the affected members may depend on the types and locations of these ge ne mutations.This review states the molecular genetic progress of GEFS+ in brie f.
9.The clinical efficacy of rhubarb acupoint sticking joint liver qi stagnation diet for chronic functional constipation in patients with depressive anxiety

Objective To investigate the clinical efficacy of rhubarb acupoint sticking joint shuganjieyu diet in the treatment of chronic functional constipation patients with depressive anxiety.Methods 120 patients of chronic functional constipation with depression anxiety were selected,they were randomly divided into control group and obser-vation group,60 cases in each group.The patients in the control group were given promoting gastrointestinal motility drugs mosapride capsules,and orally given laxatives rhubarb with sodium bicarbonate.The patients in the observation group were treated with topical rhubarb acupoint,and orally given medicated shuganjieyu diet.7 days for a course of treatment,the patients were treated for four courses.The clinical efficacy of the two groups,defecation interval and fecal nature,SAS,SDS scale score changes and quality of life (PAC -QOL)scale score changes before and after treatment and adverse reactions during treatment were recorded.Results The total effective rate of the observation group was 90.00%,which of the control group was 91.67%,the difference between the two groups was not significant (χ2 =0.32,P >0.05).Before treatment,the bowel movement and stool ratings between the two groups had no statis-tically significant differences (t =0.23,0.33,all P >0.05).After treatment,the two points of the two groups were significantly decreased,but the differences between the two groups were not significant (t =0.18,0.41,all P >0.05).Before treatment,the SAS,SDS scale scores between the two groups had no statistically significant differences (t =0.78,0.69,all P >0.05).After treatment,the SAS,SDS scores of the two groups were significantly decreased, which of the observation group decreased more significantly than those of the control group (t =4.18,4.72,all P <0.05).Before treatment,the PAC -QOL scale scores for each dimension level had no statistically significant differ-ences (t =0.16,0.21,0.42,0.20,all P >0.05).After treatment,the dimensions of the two groups showed a decline trend,and those of the observation group decreased more significantly than the control group (t =2.10,3.56,2.76, 2.34,all P <0.05).In the control group during the treatment,2 cases of diarrhea,three cases of dry mouth,the patients were improved after given symptomatic treatment.No serious adverse event occurred during therapy in the observation group.Conclusion Rhubarb acupoint joint shuganjieyu diet in the treatment of chronic functional constipation patients with depression anxiety has significant clinical effect,which can significantly improve patients'depression and anxiety scores,quality of life,and without significant adverse reactions,it is worthy of clinical applica-tion.
10.Advances of research in ion channels in idiopathic epilepsy

Epilepsy is a kind of brain dysfunction syndrome caused by so many reasons instead of a certain disease. Abnormal neuron discharge can cause epilepsy. Idiopathic epilepsy refers to epilepsy or epilepsy syndrome without any latent reasons but inherited trait. Idiopathic epilepsy is confirmed as an ion channelopathy at present. The first genemutation was found in idiopathic epilepsy in 1995, and a lot of genes coding either voltage-gated or ligand-gated ion channels have been found since then. In the present review, some new advances in research on ion channels dysfunction in idiopathic epilepsy are summarized.
