1.Apply value of sentinel lymph node detection in patients with cervical cancer

Objective To probe into the clinical apply value of sentinel lymph node detection in patients with cervical cancer.Methods 33 patients with cervical cancer underwent SLNS detection by using blue dye or methlene blue 4ml or lymphazurin was injected into the cervix at 4 points around the tumor at the time of radical hysterectomy and bilateral pelvic lymphadenectomy1 tumor characteristics surgical findings and specific locations of lymphatic dye uptake were recorded and correlated with final pathology results.Results Among 33 patients undergone this detection,dye uptake was not seen in 1 patient,dye uptake was seen in 32 patients,the SLNS were successfully detected in 81.25%(26/32);7 patients didn't discriminated SLN,the rate was 21.88%(7/32).There were 6 cases who were discovered pelvic cavity lymph nodes metastases in 26 patients discriminated SLN successfully,in all 12 piece metastases lymph nodes were SLN,sensitivity was 6/6,accuracy was 100%(26/26),fake negative rate was 0.Conclusion Sentinel lymph node dectection by the way of blue dye has more clinical apply value in patients with cervical cancer.
2.Improvement on Secure Solution to EMR Based on No.1 Military Medical Project

Objective To explore the solution to the secure problem of EMR based upon No.1 Military Medical Project.Methods The existing EMR system provides medical records service and the EMR are stored and managed in ordinary or established file format in the database server.Based upon above database characters related to EMR and the requirement of EMR security,the MRBACKUP system was developed to resolve EMR backup,recovery and dump.Results The MRBACKUP system safeguarded the security of EMR in the lowest cost,the least system resources and the highest efficiency.Conclusion The new secure solution to EMR can better resolve the security problems in the widely-used EMR in hospitals.
3.Change in corneal biomechanical properties and influence factors in population with myopia

Background Corneal refractive surgery has significant effects on corneal biomechanical properties.However,there are few study on large scale population with corneal biomechanical properties in the myopic population before corneal refractive surgery.Objective This study was to evaluate the change of corneal biomechanical properties and influence factors in myopic eyes using ocular response analyzer (ORA).Methods A prospective cohort study was carried out in Beijing Tongren Eye Center from April 2010 to January 2011.Corneal hysteresis (CH) and corneal resistance factor (CRF) were detected using ORA in 1 792 eyes of 896 myopic subjects who were going to receive corneal refractive surgery under the informed consent.According to different spherical equivalent (SE) the eyes were grouped into-0.25-<-3.0 D group,-3.0-<-6.0 D group,-6.0-<-9.0 D group,-9.0-<-12.0 D group,-12.0-<-15.0 D group and ≥-15.0 D group and according to different central corneal thickness (CCT),the eyes were grouped into <500 μm group,500-<550 μm group,550-<600 μm group and≥600 μm group.The CH value and CRF value in different SE groups or different CCT groups were compared,and the factors influencing CH and CRF were analyzed by Pearson correlation analysis.Results The mean CH and CRF value were (9.84±1.52)mmHg (1 mmHg =0.133 kPa) and (10.46±1.71) mmHg,respectively in the myopic eyes.The CH values were (10.35±1.53),(10.07±1.55),(9.81±1.46),(9.71±1.59),(9.35±1.55) and (9.23±1.28) mmHg in the-0.25-<-3.0 D group,-3.0-<-6.0 D group,-6.0-<-9.0 D group,-9.0-<-12.0 D group,-12.0-<-15.0 D group and ≥-15.0 D group,respectively,showing a significant difference among the groups (F=20.69,P<0.01),and the CH values in the-6.0-<-9.0 D group,-9.0-<-12.0 D group,-12.0-<-15.0 D group and ≥-15.0 D group were significantly lower than those in the-6.0-<-9.0 D group,-9.0-<-12.0 D group (all at P<0.01).There was no significant difference in the CRF values among different SE groups (F =0.65,P =0.49).CH and CRF values were increasing with the raise of CCT,with evidently differences among the CCT<500 μm group,500 μm ≤ CCT<550 μm group,550 μm≤ CCT<600 μm group and CCT≥ 600 μm group (CH:F=110.99,P<0.01;CRF:F =84.35,P<0.01),and the CH and CRF values were significantly higher in the 550 μm ≤ CCT<600 μm group and CCT ≥600 μm group than those in the CCT<500 μm group (all at P<0.01).CH showed a positive correlation with SE (r =0.24,P<0.01),and no significant correlation between CRF and SE (r =0.03,P =0.20).The CH and CRF showed the positive correlations with the CCT (r=0.42,0.57,both at P<0.01).In addition,CH and CRF values increased as the increases of SE,corneal curvature (CC) and CCT values,with the regression equation of CH =-7.87+0.08SE+0.16CC+0.02CCT (R2=0.26,P<0.01) and CRF=-11.42+0.14CC+0.03CCT (R2=0.34,P<0.01),respectively.The CH and CRF did not change with the age and gender in the subjects.Conclusions Corneal biomechanical properties including CH and CRF were positively correlated with CCT,CC and SE.
4.Comparison of metabolite extraction from Boea hygrometrica by gas chromatography/mass spectrometry.

This study is aiming to establish an efficient metabolite extraction method for exploration of molecular mechanisms of desiccation tolerance of the resurrection plant Boea hygrometrica using a metabolomics approach. The extracts of metabolite in B. hygrometrica using methanol solution (method A) and methanol-chloroform-water solution (method B) were analyzed by gas chromatography-mass spectrometry (GC-MS). The total numbers of chromatographic peaks, extraction efficiency, retention time and the peak stability were compared. The results showed that for fresh materials, the total chromatographic peak number of method B is more than that of method A; the extraction efficiency of nine representative metabolites by method B is higher than that by method A; the comparison of 10 random chromatographic peaks revealed that the relative standard deviation (RSD) values of the retention time are less than 1% for both methods, whereas the RSD values of the extraction efficiency is different. The percentage of peaks that owned RSD values of the extraction efficiency higher than 10% is 50% for method A and 100% for method B. In addition, method B was also efficient for dry materials from B. hygrometrica. The number of chromatographic peaks, RSD value of retention time and extraction efficiency of dry materials was similar to that of fresh materials using method B, but decreased sharply using method A. Putting together, our study provided evidence that method B is an efficient extraction method for further analysis of metabolites from this resurrection species.

5.Impact of different doses of dexmedetomidine on the anesthetic sedative and analgesic effect of anesthesia agents in hysterectomy of patients with uterine fibroids under laparoscopy

Objective To explore the impact of different doses of dexmedetomidine on the anesthetic sedative and analgesic effect of anesthesia agents in hysterectomy of patients with uterine fibroids under laparoscopy.Methods 90 patients with uterine fibroids with ASA class Ⅰ or Ⅱ who were going to undergoing general intravenous anesthesia laparoscopic hysterectomy were selected and they were divided into group A (dexmedetomidine 0.4μg·kg-1·h-1,n=30) and group B (dexmedetomidine 0.2μg·kg-1·h-1,n=30) and group C (no dexmedetomidine,n=30) by digital random method.The analgesia effect (VAS score) of group A,group B and group C preoperation and postoperative 1h,12h and 24h,and the sedation (Ramsay sedation score) were compared.General information,postoperative blood pressure,heart rate,pulse oxygen saturation,postoperative patients'' satisfaction on analgesic effect and the sedation effect and adverse reactions of the three groups were also compared.Results Age,body mass index and other basic information of three groups had no statistically significant differences (all P>0.05).The preoperative VAS score of the three groups also had no statistically significant difference (F=0.625,P>0.05).Postoperative 12h,24h,the VAS scores of group A and group B were lower than those of C group,which of group A were lower than group B (F=11.250,P<0.05,F=21.181,P<0.05).The Ramsay sedation score was higher than that of C group[(3.67±0.18) vs.(2.13±0.11)].The satisfaction of group A was significantly higher than group B and group C (96.67% vs.73.33%,46.67%,χ2=25.953).The Ramsay sedation score and satisfaction were higher than those of group B (all P<0.05).There were no significant differences in blood pressure (F=0.362,P>0.05),pulse oxygen saturation (F=0.978,P>0.05) and heart rate among the three groups before and after operation (F=1.268,P>0.05).There was no obvious adverse reaction in the three groups.Conclusion Dexmedetomidine can promote anesthetic sedative and analgesic effect of anesthesia agents in hysterectomy of patients with uterine fibroids under laparoscopy,and the dose 0.4μg·kg-1·h-1 of dexmedetomidine has better effect,feasibility and safety,it is worthy of clinical promotion.

7.Effect of angiotensin converting enzyme genetic polymorphism on cardiovascular response to endotracheal intubation in patients with hypertension

Objective To investigate the effect of angiotensin converting enzyme (ACE) genetic polymorphism on the cardiovascular response to endotracheal intubation in patients with hypertension.Methods The patients with primary hypertension,ASA Ⅱ or Ⅲ,aged 54-64 yr,weighing 50-70 kg,scheduled for elective operation under general anesthesia,were enrolled in this study.Polymerase chain reaction-restriction fragment length polymorphism was used to detect the polymorphism of ACE gene.The patients were assigned into 3 groups according to their genotypes:homozygote DD group (group DD),heterozygote ID group (group ID),and homozygote Ⅱ group (group Ⅱ).Systolic blood pressure (SBP),diastolic blood pressure (DBP) and heart rate (HR) were recorded before and after induction of anesthesia,and at 0,1.5 and 5.0 min after intubation (T0-4).The rate-pressure product (RPP) was calculated.The cardiovascular events were recorded.Results In groups DD,ID and Ⅱ,40,39 and 40 cases were included in the analysis respectively.Compared with group ID,there was no significant difference in SBP,DBP,HR and RPP at T0-4 in group DD (P > 0.05).Compared with groups DD and ID,SBP,DBP,HR and RPP were significantly deceased at T2,3,and SBP,HR and RPP were significantly deceased at T4 in group Ⅱ (P < 0.05).The incidences of the myocardial ischemia during intubation and cardiovascular response to intubation were significantly lower in group C than in groups DD and ID (P < 0.05).Conclusion ACE genetic polymorphism exerts an effect on the cardiovascular response to endotracheal intubation in patients with hypertension,and homozygote DD and heterozygote ID have the most influence.
8.Effect of galectin-3 on ventricular remodeling in rabbits with ischemic cardiac insufficiency

AIM: To investigate the relationship between galectin-3 (Gal-3) and myocardial fibrosis, and to clarify the role of Gal-3 in ventricular remodeling in rabbits with ischemic cardiac insufficiency.METHODS: A rabbit model of ischemic cardiac insufficiency was established by ligation of the anterior descending branch of the coronary artery.The 20 rabbits were randomly divided into sham operation group and cardiac insufficiency group by random number table method.After 4 weeks of coronary artery ligation, the cardiac function was measured by cardiac echocardiogram.Real-time PCR and Western blot were used to detect the expression of Gal-3, type I collagen and type III collagen at mRNA and protein levels in the myocardium.The serum Gal-3 contents were measured by ELISA.HE staining and Masson staining were used to observe the degree of fibrosis development in myocardial tissues after infarction.RESULTS: Compared with sham operation group, the mRNA expression of Gal-3 in cardiac insufficiency group was significantly increased.At the same time, type I collagen, type III collagen and collagen type I/III ratio were also increased significantly.The protein contents of Gal-3, type I collagen and type III collagen were increased significantly.The serum Gal-3 levels were significantly increased.The pathological changes were observed in cardiac insufficiency group as the myocardial cell morphological disorder and marked hyperplasia of fibrous tissue were seen.CONCLUSION: Gal-3 aggravates myocardial fibrosis in rabbits with ischemic cardiac insufficiency, and promotes the ventricular remodeling and the occurrence of heart failure.
9.Active components of Polysaccharide of Ophiopogon Japonicus on acute myocardial ischemia

AIM: To choose the anti myocardial ischemic ingredients from polysaccharide of Ophiopogon Japonicus(PO), then to confirm their effects. METHODS: According to molecular weight, PO was separated to three portions by using ultrafiltration technique. The effects of each separated portion on myocardial ischemia induced by isoproterenol were studied. Then the effects of PO active portions (POAP) on the convulsion of coronary artery induced by pituitrin and acute myocardial ischemia caused by the ligation of coronary artery were further studied in rats. RESULTS: POAP could resist the elevation of Tand S T phase induced by pituitrin in rats, remarkably decrease the elevated S T phase and contents of CK and LDH in serum caused by the ligation of rat coronary artery. POAP also exerted some inhibition effects on the reduction of SOD and increase of MDA caused by myocardial ischemia. CONCLUSION: POAP can protect myocardial cells, inhibit the generation of free radicals, and scavenge the oxygen free radicals generated from myocardial ischemia.
10.Application of competitive PCR for screening selectable marker-free Xa21 transgenic rice.

Polymerase chain reaction (PCR) is a simple, quick and highly sensitive method. However the accuracy of the conventional PCR assay was often affected by false positives and false negatives. In this study, a protocol competitive PCR was used to reduce the false results in screening for selectable marker-free (SMF) Xa2l transgenic rice plants. The competitive template of Xa21 was the endogenous Xa2l homologous sequence located on chromosome 11. The competitive template of the selectable marker gene, hygromycin phosphotransferase (hpt), was an additive DNA extracted from hpt transgenic Nipponbare (Oryza sativa L). Through competitive PCR analysis of transgenic T1 plants produced by double right border binary vector, false positive or false negative samples were effectively diminished, and genuine SMF Xa21 transgenic plants were obviously obtained. Comparing with the conventional non-competitive PCR, competitive PCR increased the accuracy for selecting SMF Xa21 transgenic plants. The results of bacterial blight (BB) resistance tests and hygromycin B resistance assay of SMF Xa21 transgenic plants testified the reliability of this method.
