1.The Challenge and Measures after Dialysis Therapy Entered in Rural Medical Insurance of Serious Diseases

Objective: To analyze the challenge and measures of current status in China after dialysis therapy entered in rural medical insurance of serious diseases. Methods: Based on the recent data from renal registry network in China, the primary disease, morbidity, therapy patterns and costs of end stage renal disease (ESRD) patients were analyzed the challenge and issue. Results: The number of ESRD patients is continuous growing during recent years. Hemodialysis is the main dialysis therapy pattern of ESRD and the cost is enormous. The dialysis centers and associated physicians in China are not enough. Conclusion: Prevention of ESRD in the early stage should be recommended, and medical quality of dialysis register should be administrated. It is needed to enhance medical quality management, establish county-level blood purifying center, encourage the business insurance and use medical community, chain operation and technology export to decrease medical costs.
2.Professor FU Weimin's Experience of Kaiqiao Huatan Tongyu Method to Cure Viral Encephalitis Sequela

[Objective]Discussion on clinical experience and academic ideas of Professor FU Weimin in treatment of viral encephalitis sequela.[ Method] From the etiology and pathogenesis, and the treatment method to expound the clinical experience and academic viewpoints of Prpfessor FU Weimin, summarize his law of characteristic medication and diagnosis thought,and put an example to exemplify that. [Results] Professor FU Weimin is good at treating this disease from phlegm and blood. He believes that the real standard based on six exogenous pathogenic factors or plague pathogen and other than evil not clear, long into the blood collaterals, phlegm heat and blood stasis obstruction, disturbing the brain orifices, are the symptoms of disease;and the virtual caused by the empty brains, heart and kidney failure and the losed senses and deities, are the causative organism. The pathological features are mainly composed of real or virtual inclusions.Treatment with the kaiqiao huatan tongyu methods are the basic principles, combined with tonifying the kidney and marrow, trafficing heart and kidney, which embodies the compatibility characteristics of eliminating pathogen without forgetting the acual. [Conclusion] Professor FU Weimin 's treatment of viral encephalitis sequela is effective, and can effectively control the disease, reduce the recurrence rate and improve patients ' self-care ability and quality of life.It's worthy of our study and promotion.
3.Bacterial adhesion of zirconia ceramics versus nickel chromium alloy as oral materials

BACKGROUND: Experimental and clinical experiences show that the surface roughness of dental restoration materials directly affects bacterial adhesion; in addition, the material composition and physicochemical properties are also important influencing factors. OBJECTIVE: To compare the bacterial adhesion of zirconia ceramics and nickel chromium al oy as oral materials. METHODS: Zirconia ceramics and nickel chromium al oy were respectively cut into 10 pieces of 3. 0 cm ×3.0 cm×0. 2 cm plate specimens. For each material, five pieces were subjected to surface polishing treatment, and the other five pieces were glazed. Surface roughness value of specimens was detected. Experimental strains of Streptococcus mutans was cultured on the specimen surface at 37 ℃ for 48 hours, and the amount of bacteria adhering to the specimen surface was detected. RESULTS AND CONCLUSION: Surface roughness of four groups of specimens showed no difference. The amount of bacteria that adhered to the polishing specimens of zirconia ceramics was significantly lower than that of nickel chromium al oy (P < 0.05), but no difference was found in the amount of bacteria adhering to the glazing specimens of nickel-chromium al oy and zirconia ceramics. These findings indicate that zirconia ceramics has better ability to inhibit bacterial adhesion than nickel-chromium al oy, especial y after glazing.
4.Safety and Efficacy of Danhong Injection Combined with Edaravone in the Treatment of Acute Cerebral Infarction:A Systematic Review

Objective To assess the safety and efficacy of danhong injection combined with edaravone for acute cerebral infarction ( ACI) . Methods To search CBM、CNKI、VIP、WF、MEDLINE for randomized controlled trials ( RCTs) of related articles from inception to November 2014.After assessed the quality of studies included,RevMan5.2 software was used to analyze data. Results 23 studies,were included and 2 001 patients were involved.The results showed that the effective rate of danhong injection combined with edaravone for ACI was better than that of control group with statistical significance [ RR=1.23, 95%CI (1.17,1.29), P<0.000 01].Meta-analysis on the improvement of neurological deficit and activities of daily living also showed a better effectiveness of experimental group than control group with statistical significance [MD=-3.09,95%CI(-3.68,-2.50),P<0.000 01];[MD=15.15,95%CI(12.69,17.62), P<0.000 01].But the result showed no statistical significance in safety evaluation of two groups [RR=1.88,95%CI(0.76,4.63),P=0.17]. Conclusion Although the results support the conclusion that danhong injection with edaravone is safe and superior to either danhong injection or edaravone or conventional treatment, further large sample and high quality randomized controlled trials should be carried out.
5.A preliminary study of relationship between endothelial dysfunction and insulin resistance in obese subjects

Objective To explore the relationship between endothelial dysfunction and insulin resistance (IR) during reduction of body weight in obese subjects. Methods 260 obese subjects without metabolic disorders, cardiovascular disease and other clinically detectable diseases were enrolled in this study. Among them 157 patients were found to have impaired endothelial function and 103 with normal endothelial function. They were randomly divided into body weight reduction intervention group and nonintervention group. The measures of intervention included food restriction, exercise, and drugs for weight reduction, and they were continued for two years. 87 subjects with normal weight and without endothelial dysfunction served as normal control. IR index (HOMA-IR) and flow-mediated endothelium-dependent dilatation function were measured before and after the study period. 6.THE EFFECT OF RETINOIC ACID ON DENDRITIC CELL DERIVED FROM CORD BLOOD AND ITS PATHWAY

Objective: To investigate the role of retinoic acid (RA) in the differentiation, and maturation of dendritic cells (DCs) derived from cord blood monocytes, and its mechanism. Method: Nine cord blood samples were collected from healthy and well nourished newborns. Monocytes were isolated and cultured in vitro. Cells were divided into 4 groups: control group; retinoid acid receptor ? (RAR? )agonist group (RA group); RAR? antagonist group (RO group) and agonist plus antagonist group (RA+RO group). Part cells were collected in D9, and the surface markers on cells were detected by flow cytometer. The mixed lymphocyte reaction (allo-MLR) was used to evaluate the ability of DC to induce proliferation of T cells and the influence by RA and RAR?. Th cytokines were measured in protein level by ELISA and in mRNA levels by RT-PCR to decipher the effect of RA and RAR? in the regulation of Th1/Th2 balance by DCs. Results: The differentiation and maturation of DCs were inhibited significantly in RA group compared with control group. When RO was added into the culture, the effects of RA can be reversed. After MLR , the suppression effect of RA on allo-T cell proliferation was inversed by RO. During the influence of DC on Th cell polarization, RO could inhibit the down-regulation of Th1 cytokine (IFN-?) as well as the up- regulation of Th2 cytokines (IL-4, IL-10) by RA, whether on protein levels or on mRNA levels. Conclusion: Retinoic acid inhibited the differentiation and maturation of DC derived from cord blood monocytes, reduced the ability to stimulate allo-T lymphocytes proliferation by DCs, and made immune response bias to Th2. RAR? plays a very important role in the regulation of RA on DCs.
7.EFFECT OF VITAMIN A ON TRACHEAL MUCOSAL IMMUNITY

Objective: To investigate the effects and mechanisms of vitamin A on tracheal mucosal immunity. Method: The rat model with vitamin A deficiency (VAD) was established. By use of immunohistochemical, RT-quantity PCR and ELISA methods, the number of dendritic cells (DCs, OX-62 positive cells), the maturation of DCs (the positive areas of CD80 and CCR7), the pathogen recognition receptors (the positive areas of TLR2 and of TLR4) and TLRs signal transduction (the mRNA level of MyD88), as well as cytokines production in tracheal mucous membrane were detected. Results: In VAD rats, the number of DCs in trachea was increased, but the expressions of CD80 and CCR7 were similar with control group. The TLR2 and MyD88 mRNA levels were up-regulated, and IL-12 production was also raised. However, Th2 cytokines IL-4 and IL-6 were reduced significantly in protein and mRNA levels, and the production of IL-10 protein was declined compared with the control. Conclusion: In VAD rats, the number of DCs, receptors of pathogens recognition, and cytokines production in trachea mucosa were significantly changed. The affection of VAD on DCs may play important role in the impaired mucosal immunity of trachea.
8.Roles of ERK and cPLA2α in biphasic regulation of renal proximal Na+-HCO3-transport by angiotensin Ⅱ

Objective To clarify the signaling mechanisms underlying angiotensin Ⅱ biphasic regulation of renal proximal Na+-HCO3- transport. Methods Different concentration Ang Ⅱ to the responses of Na+-HCO3- cotransporter (NBC) activity in isolated proximal tubules, with or without ATR, MAPK, cPLA2α, P450 blockade was compared in wild-type and Ang Ⅱ type 1a receptor (AT1aR)-deficient mice. The phospholipase of ERK was examined by Western blotting. AT1aR mRNA was examined by RT-PCR from kidney proximal tubules. Results (1)In isolated wild-type mouse, renal proximal tubules showed biphasic effects of Ang Ⅱ on NBC activity. Low concentration Ang Ⅱ (10-10 mol/L) increased NBC activity, but high concentration Ang Ⅱ (10-6 mol/L) decreased NBC activity. Olmcsartan (AT1 antagonist) blocked both stimnlatory and inhibitory effects of Ang Ⅱ on NBC activity, but PD98059 (mitogen-activated protein kinase inhibitor) blocked only the stimulatory effect of low concentration Ang Ⅱ ( 10-10 mol/L). (2)In AT1aR-deficient mice, only the stimulatory effect by high concentration of Ang Ⅱ (10-6 mol/L) was observed, which was blocked by olmesartan and PD98059. (3)In wild-type mice, pharmacological blockade of cPLA2 or P450 converted the inhibition effect by high concentration Ang Ⅱ (10-6 mol/L) to the stimulation, which was blocked by olmesanan and PD98059. These results indicated that the extracellular sigual-regulated kinase (ERK) activation via AT1 mediated only the stimulatory effect of Ang Ⅱ, while the cPLA2α/P450 activation via AT1 mediated the inhibitory effect of Ang Ⅱ independently of ERK. The analysis of ERK phosphorylation by Ang Ⅱ also supported a view that the cPLA2α/P450 pathway worked to suppress the ERK activation. Conclusions Ang Ⅱ activates ERK and cPLA2α with different concentration dependency via AT1. The balance between ERK and cPLA2α activities determines the final responses to Ang Ⅱ in intact proximal tubules.
9.Clinical effect of gonadotropin releasing hormone-analogue and marvelon in the treatment of ovarian endometriosis after laparoscopy

Objective To study the clinical effect of gonadotropin releasing hormone-analogue (GnRH-a) and marvelon in the treatment of ovarian endometriesis after laparoscopy.Method The postoperative women with ovarian endometriosis were divided into three groups:19 treated with GnRH-a and marvelon(greup A),20 treated with GnRH-a(group B),20 without endocrinal therapy(group C).The symptoms scores,the recurrence and side effects were compared.ResultsThree years after laparescopy,the recurrent rate and symptoms scores in group C was the highest (P<0.05),the recurrent cyst size [(12 + 1)mm] and symptoms scores [(1.2 + 0.9)scores] in group A was the lowest among three groups.Conclusions Marvelon and GnRH-a can be used in the treatment of ovarian endometriesis after laparoseopy.The effect of marvelon combined with GnRH-a is prior to GnRH-a.
