1.A Clinical Trial of Triclabendazole in the Treatment of Human Paragonimiasis skrjabini

Objective To observe the therapeutic effect of triclabendazole in patients with paragonimiasis skrjabini. Methods Four patients were treated with triclabendazole at dosage of 10 mg/kg bid for 3 days and were followed up for 6~10 months. The clinical symptoms, chest X ray, hemogram and liver function tests were observed before and after treatment. Results Migratory subcutaneous nodules in three patients all disappeared. Small amount of pleural effusion were absorbed in two patients. Severe headache subsided in a patient with cerebral lesion. MRI showed a bean sized calcified spot in the right frontal region. Chest X ray in 2 patients showed that pulmonary lesions were markedly absorbed and disappeared. There was no adverse effect of the drug.Conclusion Triclabendazole has marked therapeutic effect on paragonimiasis skrjabini. It is safe in administration and well tolerated.
2.The role of substance P to modulate the neuroendocrine function in asthmatic rats

Aim To investigate the role of substance P in the brain of asthmatic rats.Methods Rats were injected with aluminum hydroxide and OVA allergens to prepare the animal model of asthma. Then the content of c-fos protein in asthmatic rats' brain was detected by immunohistochemical method (SABC).The content of substance P in paraventricular nucleus (PVN), the content of corticotropin-releasing hormone (CRH) in median eminence (ME), and the content of adrenocorticotropic hormone (ACTH) and corticosterone (CORT) in peripheral blood were detected by radioimmunoassay method. Then exogenous SP, SP receptor antagonist S0145 were microinjected in PVN to observe their effect upon lung function and HPA axis in asthmatic rats. Results In asthmatic rats, the content of SP increased within the PVN. The content of CRH, ACTH and CORT decreased (P<0.05).The ratio of expiratory and inspiratory and airway resistance increased. Diaphragm discharge points and lung compliance decreased (P<0.01).After microinject of SP in PVN, there was a further decrease in pulmonary function and the content of CORT, ACTH and CRH in asthmatic rats (P<0.01).While the SP receptor antagonist S0145 might reverse the change of lung function and hypothalamus-pituitary-adrenal axis (HPA axis) in asthmatic rats.Conclusion In asthmatic rats the SP in PVN can affect the function of HPA axis, involved in asthma attacks.
3.Simultaneous Determination of 4 Components in Compound Paracetamol and Amantadine Hydrochloride Tablet by GC

OBJECTIVE:To establish a method for the simultaneous determination of paracetamol,amantadine hydrochloride, caffeine,chlorphenamine maleate in Compound paracetamol and amantadine hydrochloride tablet. METHODS:GC was performed on the column of HP-5 sillica capillary,temperature programmed,detector was FID detector,with the temperature of 300 ℃,car-rier gas was nitrogen gas,the flow rate is 1.5 mL/min,the split ratio was 20:1 and injection volume was 1μL. RESULTS:The lin-ear range was 156.0-4990.4 μg/mL for paracetamol,125.7-4023.2 μg/mL for amantadine hydrochloride,19.14-612.4 μg/mL for caffeine and 2.515-80.48 μg/mL for chlorphenamine maleate(all r=0.9999);the limits of quantification were 1.4,0.5,1.1,0.9 ng,limits of detection were 0.4,0.2,0.3,0.3 ng;RSDs of precision,stability and reproducibility tests were lower than 2.0%;re-coveries were 99.59%-101.77%(RSD=0.8%,n=9),99.56%-101.80%(RSD=0.7%,n=9),98.44%-100.83%(RSD=0.7%,n=9) and 100.05%-101.91%(RSD=0.6%,n=9),respectively. CONCLUSIONS:This method is simple,rapid,accurate and reli-able,and suitable for the simultaneous determination of paracetamol,amantadine hydrochloride,caffeine,chlorphenamine maleate in Compound paracetamol and amantadine hydrochloride tablet.
4.Detection of Chemical Constituents in Antirheumatic Traditional Chinese Medicines

OBJECTIVE:To establish an HPLC method for the detection of Prednisone acetate, Hydrocortisone, Dexamethasone acetate, Piroxicam, Diclofenac sodium, Indometacin and Ibuprofen, the 7 chemical constituents in antirheumatic traditional Chinese medicine. METHODS: The chromatographic separation was performed on a Hypersil BDS C18 column (200 mm?4.6 mm,5 ?m) using methanol-1 g?L-1KH2PO4(58∶42) (adjusted to pH 4.0?0.05 with H3PO4) as the mobile phase at a flow rate of 1.0 mL?min-1. The detective wavelength was set at 220 nm. RESULTS: Seven chemical constituents were all well-separated with good linearity within respective range of sample size. CONCLUSION: The method is rapid and convenient, and can be used for the quantitative and qualitative analyses of 7 chemical constituents in antirheumatic traditional Chinese medicine and to detect whether there is addition of chemical constituents in antirheumatic traditional Chinese medicines.
5.COMPARATIVE INVIVO OVICIDAL EFFECT OF ALBENDAZOLE AND MEBENDAZOLE ON HOOKWORM EGGS AND IN THE TREATMENT OF INTESTINAL NEMATODE INFECTIONS

Albendazole and mebendazole were comparatively evaluated in 22 adult patients for their in vivo effects on hookworm eggs. Both drugs were given 200mg twice daily for three consecutive days. Stool specimens wore collected before treatment and during the following five days. The Stool egg count was carried out by Stolls method and each specimen was also cultured by Hara-da-Mori technique for at least eight days. The mean pre-treatment percentage of incubated hookworm eggs that developed to larvae was 75.3% in albendazole group and 68.8% in mebendazole group respectively. One day following the initiation of treatment the mean percentage was remarkably reduced to 0.25% in albeadaole group and no eggs developed to larval stage beyond day 1. 0ne day and two days following the initiation of treatment the mean percentage was 16.23% and 23.13% respectively in mebendazole group. No eggs developed to larval stage on day 3 and thereafter. Albendazole seems to have better ovicidal effect than mebendazole.Albendazole and mebendazole were also comparatively evaluated in 123 adult patients with single or mixed infections of hookworm, ascarisis, and trichuris. A single dose of 400mg was used for both drugs. 2 and 4 weeks after treatment, their stools were examined by brine flotation technique. The hookworm eggs negative conversion ratss 2 weeks after treatment were 78.8% and 26.4% respectively in albeadazole and mebendazole group; while those 4 weeks after treatment were 74.1% and 25.5% respectivley. For ascariasis, the eggs negative conversion rates were 98.0% and 92.0% respectively. For trichuriasis, the negative conversion rates were only 20.6% and 27.7% respectively
6.EVACUATION OF HYPERTENSIVE INTRACEREBRAL HEMATOMA BY STEREOTACTIC TECHNIQUE

Evacuation of intracerebral hematomas in 38 hypertensive patients was reported. Hematomas were found in the internal capsule in 24 patients, subcortex in 5. cerebellum in 7 and brain stem in 2. The volume of hematoma was less than 2ml (in the brain stem) in 2 patients, 20-50ml in 15, and over 100ml in 6. The fluid hematoma was to tally aspirated with stereotactic technique in 7 patients. In 17 patients, over 80% of hematoma was evacuated, and in 14 about 60-80% of hematoma was evacuated by the same technic. The results of the operation were good in 11 patients (29.0%), while in 29% there was a mild disability, and in 12 (31.5%) there was a marked disability, 4(10.5%) died.
7.Survey on Physiological and Psychological Symptoms of Drug Abusers under Rehabilitation Period Through Labor

Objective To investigate the physiological and psychological symptoms of drug abusers under rehabilitation period through labor.Methods A self-designed clinical symptoms questionnaire and Eysenck Personality Questionaire (EPQ) were used to evaluate 1 004 drug abusers under rehabilitation period through labor, and the related symptoms were analyzed.Results The mainly physiological symptoms were fatigue, forgetfulness, aching pain of joints, arrhythmia and discomfort of limbs, and the mainly psychological symptoms are drug craving, insomnia and anxiety. The mean scores of P and E were higher in those who had the drug craving symptoms ( P<0.01), and the mean scores of N were higher in those who had the insomnia or anxiety symptoms ( P<0.01).Conclusion The mainly physiological symptoms of drug abusers under rehabilitation period through labor are fatigue, forgetfulness, aching pain of joints, arrhythmia and discomfort of limbs, which are probably caused by the unbalance of the nervous and internal secretive systems, and the plastisity of the nervous system. The psychological symptoms may due to the effect of morphine and the personality of the drug abusers.
8.Triclabendazole in the treatment of Paragonimiasis skrjabini.

OBJECTIVETo observe triclabendazole effect on Paragonimus skrjabini in experimentally infected rats,and to develop a new drug for treating paragonimiasis.
METHODSMetacercariae of Paragonimus skrjabini were isolated from crabs (Sinopotamon) collected from endemic area. Wistar rats were infected intraperitoneally. One and two months after infection, they were treated with triclabendazole at the dosage of 300 mg.kg(-1).2 d(-1), 450 mg.kg(-1).3 d(-1) and 600 mg.kg(-1).3 d(-1) respectively. Five patients with Paragonimus skrjabini were treated, with Triclabendazole dosage of 10 mg/kg bid x 3 days.
RESULTSThe worm reduction rates were 50.3%, 80.8% and 86.7% respectively one month after completion of treatment. Dead worms of sesame size recovered from muscles, liver, abdominal cavity, chest cavity and lung were greatly diminished in size and weight in comparison with that of the control group. Many large (about 1 cm) black-colored distended worm cysts were found in the lungs of the control rats. Usually there were two adult worms pairs with numerous eggs in each worm cyst. Most worm cysts in the treated groups of rats were changed into hemorrhagic-necrotic patches. All five patients were cured.
CONCLUSIONTriclabendazole was highly active against Paragonimus skrjabini in rats experimentally infected and patients.

9.Evaluation of the combination of multiple subpial transection and other techniques for treatment of intractable epilepsy.

OBJECTIVEMultiple subpial transection (MST) is one approach to the surgical treatment of intractable epilepsy with epileptogenic lesion located in functional areas. To verify the effect of MST, an experimental study was performed first, followed by clinical application.
METHODSOn the basis of the experimental study, MST was performed in 200 intractable epileptic patients from 1991 to 2000. Of them, 80 cases underwent MST only while 120 others underwent MST combined with other techniques, such as corpus callosotomy, temporal lobectomy and focus resection. A series of modifications of the surgical techniques were made.
RESULTSThe results of the experimental study indicated that MST could inhibit the formation and spreading of epileptic discharge and limit the damage to neurons in a minimal area on the epileptogenic agent injected cortex. MST does not impair major functions of the cortex. After the clinical application and modifications, 160 patients were followed up for 1 to 8 years. Complete control of seizure was obtained in 100 cases (62.5%), significant reduction (more than 75%) in 32, reduction (more than 50%) in 20 and no change in 8. The total rate of effectiveness was 95.0%, and the significant rate of effectiveness was 82.5%. No functional defects were found in any patients.
CONCLUSIONSThe results indicate that MST is an effective approach to the surgical treatment of intractable epilepsy. MST can be combined with other approaches. The outcome of the subdivision of the MST only group indicates that MST on local epileptogenic lesion without structural changes is as effective as that of the combined operation group. To evade hemispheric disturbance, MST should be done first to avoid severe complications. Hemispherectomy should be performed only on poor effected cases of MST.

10.Study of Knee Osteoarthritis Gait Index on Function Evaluation-Time Distribution at the Sole and the Vertical Rebound Force at Stand Phase

Objective To investigate time distribution of pressure of footplate in each stage and vertical anti-force from ground of patients with knee osteoarthritis (KOA).Methods 40 KOA patients, 10 aged people, and 10 common saplings were selected to test time distribution of pressure of footplate in each stage and vertical anti-force from ground in period of monopodia support.Results To unilateral KOA patient, there was not obvious difference to be observed in initial contact phase and forefoot push off phase ( P>0.05), but there was a significant difference in forefoot contact phase and foot flat phase between health side and sick side ( P<0.001), and there was obvious difference decreased buffer time and buffer force coefficient between them ( P<0.01).Conclusion To KOA patients, the time decreases in weight-bearing phase and absorption shocks, and amortization coefficient also decreases. These indexes can be used as the walking index of functional assessment and the objective evidence of curative effect.
