1.Observation of pulmonary artery motion curve of pulmonaryh ypertension with anatomical M-mode echocardiography

Objective To explore the usage of anatomic al M-mode echocardiography in observing pulmonary artery motion curve of pulmonary hypertension patients.Methods The subjects included 21 pulmonary hypertension patients complicated with tricuspid valve insufficiency, and 21 controls whose age and gender matched with the patient group. The inner diameter of main pulmonary artery and right atrium, acceleration time of pulmonary valve Doppler signal and maximum velocity of tricuspid insufficiency (ACTpv) were measured during routine echo examination. From the maximum velocity of tricuspid insufficiency the pulmonary artery systolic pressure (PASP) was calculated. The pulmonary artery motion curve and pulsating amplitude of pulmonary artery wall (PAWPA) was observed and measured with a home-made anatomical M-mode echocardiography with instrument. Objective To explore the protective effect of salidroside on early reperfusion in patients with acute cerebral infarction treated with urokinase intravenous thrombolysis.Methods 98 patients with acute cerebral infarction who were consistent with intravenous thrombolysis after screening were enrolled in the study.They were equally divided into the control group and the treatment group by sealed envelop method in one to one ratio.The control group received urokinase intravenous thrombolytic therapy on the basis of dehydration,anticoagulation,brain rehabilitation,regulating blood pressure,blood glucose and other conventional treatment.Based on the treatment in the control group,the treatment group was treated with Hongjingtian injection for 10 days.The serum malondialdehyde(MDA),superoxide dismutase(SOD),plasma endothelin-1(ET-1) and high sensitive C-reactive protein(hs-CRP),neurological deficit score(NIHSS),Barthel index and clinical effect in the two groups were observed.Results There were no significant differences in levels of MDA,SOD,ET-1,hs-CRP,NIHSS and Barthel index between the two groups before treatment (t=0.30,0.26,0.41,004,0.57,0.33,all P>0.05).After treatment,there were statistically significant differences between the control group and the treatment group in levels of MDA,SOD,ET-1,hs-CRP,NIHSS score and Barthel index[(7.54±1.14)nmol/mL vs.(6.04±1.21)nmol/mL,(92.42±15.28)U/mL vs.(108.54±19.24)U/mL,(64.5±7.4)pg/mL vs.(41.4±8.1)pg/mL,(3.84±1.04)μmol/L vs.(2.54±0.84)μmol/L,(16.5±5.4)points vs.(11.2±3.1)points,(62.1±12.4) vs.(71.4±10.4)](t=6.31,4.59,14.74,6.81,5.96,4.02,all P<0.01).The total effective rate of the treatment group was 91.84%,which of the control group was 63.39%,the difference was statistically significant(χ2=8.83,P<0.05).Conclusion Salidroside can relieve lipid peroxidation in patients with acute cerebral infarction,promote decomposition of oxyradical,reduce inflammatory reactions,improve neurological function and quality of life.The curative effect is significant.It has protective effect on early reperfusion in patients with acute cerebral infarction treated with urokinase intravenous thrombolysis.

Objective:To observe the interventative effect of Ziyin-Huatan Decoction by regulating exosomes on subcutaneous tumor of mice with gastric cancer. Methods:MGC-803 cells were randomly divided into exosome control group, low-dose group and high-dose group. The low-dose group and high-dose group were intervened with Ziyin-Huatan Decoction of 25 μg/ml and 100 μg/ml respectively. After 48 hours, the exosomes secreted by MGC-803 cells in each group were extracted. Twenty BALB/c male mice were randomly divided into exosome control group, low-dose Ziyin-Huatan Decoction group, high-dose Ziyin-Huatan Decoction group and blank control group, with 5 mice in each group. Except for the blank control group, the mice in the other groups were injected with exosomes extracted from the cells of the corresponding group through the orbit, 10 μg/time for each mouse, once every other day, a total of 15 times; the blank control group was injected with the same amount of PBS. Then SGC-7901 cells were inoculated into mice to establish a tumor model. The tumorigenic rate and body weight of mice were observed. The levels of CD31, VEGF and bFGF in tumor tissues were detected by immunohistochemistry. Results:Compared with the blank control group, the tumor weight [(170.00 ± 10.00) mg vs. (343.33 ± 20.82) mg] and the expression of CD31 (37.43 ± 0.55 vs. 63.30 ± 0.85), VEGF (11.37 ± 1.19 vs. 70.30 ± 0.72) and bFGF (43.77 ± 1.53 vs. 84.97 ± 1.86) in the high-dose Ziyin-Huatan Decoction group significantly decreased ( P<0.05). Compared with exosome control group, the expressions of CD31, VEGF and bFGF in low and high dose Ziyin-Huatan Decoction groups were decreased ( P<0.05). Conclusion:Ziyin-Huatan Decoction can significantly inhibit the growth of subcutaneous tumor of gastric cancer in mice by regulating exudation, which may be related to the inhibition of tumor angiogenesis.
5.Protective effect of nerve growth factor associated with ginkgo biloba extraction on acute glaucoma retinal ischemia reperfusion injury in rabbit

AIM:To investigate the protective effect of nerve growth factor combined with Ginkgo biloba extract on retinal ischemia-reperfusion (RIR) injury in rabbits with experimental high intraocular pressure.METHODS:Establishment of rabbit glaucoma ischemia reperfusion model.Twenty-four New Zealand white rabbits were randomly divided into three groups:nerve growth factor group, Ginkgo biloba extract group and combination group.Respectively, in the continuous administration of 1, 7, 14d.We observed the morphological changes of the tissues of the retina.The levels of superoxide dismutase(SOD), nitric oxide(NO) and malondialdehyde(MDA) in retinal tissue were measured.RESULTS:Respectively, first, in the continuous administration of 1, 7, 14d, the contents of MDA and NO in Ginkgo biloba extract group and nerve growth group were higher than that in combination group (P<0.05).Secondly, the SOD content of Ginkgo biloba extract group and nerve growth group were lower than that of combination group at each time point (P<0.05).At each time point, the number of HE staining of retinal ganglion cells (RGCs) showed that the loss of RGCs in the combination group was significantly lower than that in the other groups, and the ganglion cell count showed that the Ginkgo biloba extract group and the neuronal growth group were lower (P<0.05).CONCLUSION:Nerve growth factor combined with Ginkgo biloba extract has better protective effect on retinal ischemia-reperfusion injury.The mechanism may be related to the decrease of free radicals and increase the activity of SOD in retinal tissue.
6.Predictive value of plasma atrial natriuretic peptide level in prognostic evaluation of patients with Sepsis

Objective To evaluate the predictive value of plasma atrial natriuretic peptide (ANP) level on prognostic of patients with systemic inflammatory response syndrome by dynamic monitoring ANP levels.Methods Ninety-eight patients admitted to the intensive care unite were classified into survival group(n =78) and death group (n =20).The level of plasma ANP,procalcitonin,C-reactive protein and lactic acid were measured.Acute Physiology and Chronic Health Evaluation (APACHE) Ⅱ score were recorded.Results The plasma ANP level of patients in the death group was 0.40 (0.26) μg/L,significantly higher than that in the survival group(0.22(0.12) μg/L,P =0.000).Along with treatment scheme,the plasma ANP level decreased in survival group,and there were significant difference among three times (0.22 (0.12) μg/L,0.17 (0.09) μg/L,0.13 (0.11) μg/L,P =0.000).But there was no difference in ANP level of patients in death group along with the disease developing (0.38 (0.30) μg/L,0.39 (0.23) μg/L,0.39 (0.22) μg/L,P =0.99).ICU hospitalized time in survive group associated with APACHE Ⅱ score,ANP and PCT(r =0.735,0.628,0.487 respectively,P =0.000,0.001,0.021).Conclusion ANP is proved to be a good clinical index in prognostic evaluation of patients with sepsis.
7.Investigation and analysis of influencing factors of nursing assistant requirements of county hospital of Nanchang city

Objective To investigate and analyze the nursing assistance requirements and the influencing factors of county hospital of Nanchang city.Methods A total of 327 nurses in 6 country-level hospital of Nanchang city were surveyed by the self-made questionnaire.Results The total demand score of county hospital was (4.29±0.47) scores,which was in a high level.The helping content demand was the highest,the form of assistance needs was the lowest.From the specific items of view,prevention of incision infection,nursing training,critically ill patients with bedsore nursing were higher in the content of assistance.Send a nurse to support education,nursing training,hospital experts on-site guidance were higher in the form of assistance.Long term assistance for 1 years,short-term assistance for 4-6 months were the highest in demand of helping times.The main factors of influencing county hospital nursing assistance were work area,department,degree,title etc.The work area was the important influencing factor of nursing assistance of county hospital.Conclusion The relevant departments are recommended to develop personalized assistance scheme according to the recipient hospital needs.
8.Effects of Berberine on Inflammatory Factors, Adipokines and Fatty Acid Metabolism in 3T3-L1 Adipocytes

Objective To observe the effects of berberine on inflammatory factors, adipokines and fatty acid metabo-lism in 3T3-L1 adipocytes, and to investigate the molecular mechanism underlying berberine’s role of improving insulin re-sistance. Methods mRNA level of inflammatory molecules, adipokines, key enzymes and protein in fatty acid metabolism in 3T3-L1 cells were determined by quantitative real time polymerase chain reaction (qRT-PCR) after cells were treated with different concentrations of berberine (0, 5, 10, 20, 40μmol/L) for 24 hours and with 10μmol/L berberine at different du-rations (0,4,8,24,48 h). These factors mainly included interleukin-6 (IL-6), tumor necrosis factor-α(TNF-α), leptin, adipo-nectin, visfatin, fatty acid synthase (FAS), acetyl-CoA carboxylase (ACC), adipose triglyceride lipase (ATGL) and adipocyte fatty acid binding protein (AFABP). Results In 3T3-L1 adipocytes, transcription level of IL-6, TNF-α, leptin, FAS, AT-GL, AFABP reduced with addition of berberine dosage at 10~40μmol/L(P<0.05)while visfatin mRNA level increased(P<0.05)compared with the control group. No significant difference was found in expression of adiponectin(P>0.05). Tran-scription level of IL-6, TNF-α, leptin, AFABP, ATGL, FAS decreased with time after 10μmol/L berberine intervention (8-48 h) compared with the control group(P<0.05). On the other hand, visfatin mRNA level increased(P<0.05)compared with the control group. Adiponectin mRNA decreased only after cells were treated with berberine for 48 h(P<0.05). No sig-nificant difference was found transcription of ACC between each groups treated with berberine(P > 0.05). Conclusion mRNA level of inflammatory factors, adipokines, key enzymes and protein in fatty acid metabolism in 3T3-L1 adipocytes can be affected by berberine and this effect depend on its dose and time . This might be the mechanisms underlying berber- ine to improve insulin resistance.

10.Expression of Th17 cytokines in skin lesions of patients with psoriasis.

In order to investigate the role of Th17 cytokines in the pathogenesis of psoriasis, reverse transcriptase-polymerase chain reaction (RT-PCR) was used to detect the expression of IL-17, IL-23 (p19/p40), and IL-6 in skin lesions and non-lesions of the patients with psoriasis and skin tissues of normal subjects. The results showed that the mRNA expression levels of IL-17, IL-23p19, IL-23p40 and IL-6 in psoriasis lesion were significantly higher than those of non-lesions (1.231 +/- 0.843 vs 1.003 +/- 0.044, 1.166 +/- 0.142 vs 0.765 +/- 0.133, 1.125 +/- 0.104 vs 0.730 +/- 0.103, 1.186 +/- 0.222 vs 0.976 +/- 0.122, respectively, all P < 0.05). Meanwhile, The expression levels of IL-17 mRNA, IL-23p19 mRNA, IL-23p40 mRNA and IL-6 mRNA were higher in non-lesions than those in normal skin tissues (1.003 +/- 0.044 vs 0.620 +/- 0.104, 0.765 +/- 0.133 vs 0.584 +/- 0.078, 0.730 +/- 0.103 vs 0.000 +/- 0.000, 0.976 +/- 0.122 vs 0.656 +/- 0.121, respectively, all P < 0.05). The overexpression of Th17 cytokines in the skin lesions of patients with psoriasis may indicate that Th17 cytokines play a very important role in the immunopathogenesis of psoriasis.
