1.Serum pepsinogenⅠandⅡin residents from Zhuanghe county in North China

Objective To determine the serum level of pepsinogenⅠ,Ⅱ(PGⅠ,PGⅡ) and PGⅠ/Ⅱin the residents from Zhuanghe county,a high risk area of gastric cancer in North China,and to explore their distribution as well as related factors.Methods Serum PGⅠand PGⅡlevels were detec- ted with ELISA method in 6990 subjects.Gastric diseases were diagnosed by endoscopy and histopatho- logic examination.Serum H.pylori-IgG antibody was determined by ELISA method.Results The me- dian values for PGⅠ,PGⅡ,PGⅠ/Ⅱwere 86.9/?g/L,10.6/zg/L and 8.1 respectively.Serum PGⅠand PGⅡin male(95.2?g/L,12.1?g/L) were significantly higher than those in female(79.7?g/L, 9.4?g/L;P=0.000),PGⅠ/Ⅱratio(7.9) was significantly lower in male (8.3,P=0.000).There were significantly decrease in PGⅠ/Ⅱratio along with age increase.PGⅠ/Ⅱratio decreased signifi- cantly following with progression of gastric mucosa from normal (10.4) to non-atrophic lesions(8.8) and to atrophic lesions (6.6).Serum PGⅠand PGⅡin H. pylori positive subjects (88.7?g/L,11.4?g/L) were significantly higher than those in H.pylori negative subjects (81.4?g/L,8.4?g/L;P= 0.000),PGⅠ/Ⅱratio(7.7) was significantly lower in H.pylori positive subjects (9.6,P=0.000). For patients with atrophic lesions,the area under the PGⅠ/ⅡROC curve was 0.622.The best cut-off point for PGⅠ/Ⅱwas 6.9,with sensitivity of 53.2%,and specificity of 67.5%.Factors linked to PGⅠ/Ⅱwere identified using multinomial logistic regression:male (OR:1.151,95% CI:1.042—1.272, P=0.006),age=61(OR:1.358,95% CI:1.188—1.553,P=0.000),atrophic lesion(OR:2.075,95% CI:1.870—2.302,P=0.000),and H.pylori infection (OR:1.546,95% CI:1.368—1.748,P= 0.000).Conclusions The serum PG levels are significantly skewed from normal distrubition in the residents of Zhuanghe county,and affected by age and gender,as well as associated with gastric diseases and H.pylori in- fection.Compared with PGⅠand PGⅡalone,PGⅠ/Ⅱis more suitable for screening gastric cancer.
2.Analysis of surveillance results of Brucellosis in human being in Guizhou province from 2005 to 2008

Objective By analyzing the surveillance result of Brucellosis in human being of Guizhou province from 2005 to 2008,to understand the current situation of relevant population with brucella infection,and then to provide the basis for the development of prevention and control measures.Methods According to the Brucella Disease Monitoring Standards (GB 16885-1997),in Guizhou province,Huaxi,Wudang,Xingyi,Dushan,Ceheng,Long Lane,Xifeng,Carey,Ziyun and so on other areas(city,county) were selected as monitoring points,and occupational groups of animal husbandry in agricultural areas,as well as farmers and students contacted with livestock were selected as monitoring subjects.Rose bengal plate agglutination test(RBPT) and tube agglutination test (SAT) were used to detect Brucellosis antibody.Results From 2005 to 2008,Brucellosis antibody detection rate was 0.63%(37/5904) in target groups of Guizhou province,specifically,the rates in Huaxi,Wudang,Xingyi and Ceheng counties(towns or districts) were 2.28%(19/832),0.16%(2/1274),1.84%(15/815) and 0.14% (1/735),respectively; the rates in livestock workers,peasants and students contacted with livestock in rural areas were 1.29% (36/2800),0.04% ( 1/2814) and 0.00% (0/290),respectively.In all antibody positive carriers,most were dairy cattle raisers which accounted for 83.78% (31/37) in the total infected persons.Conclusions Dairy cattle and goat raisers in some counties(towns or districts) of Guizhou province have infected Brucellosis,and direct contacts with brucella-carrying cattle is the major route of Brucellosis transmission in human being.Strengthen livestock quarantine and dispose infected livestock timely are the key of Brucellosis control.
3.Evaluation of serum pepsinogen I , II and osteopontin co-detection in gastric cancer screening.

OBJECTIVETo explore the value of combined assay of serum PG and OPN concentration for gastric cancer screening.
METHODSPepsinogen I , II and osteopontin (OPN) concentrations in fasting serum were measured by ELISA in 570 subjects, including 144 gastric cancer, 60 dysplasia, 113 atrophic gastritis, 70 erosion or ulcer, 92 superficial gastritis and 91 healthy control. The cut off point for PG and OPN was determined using receiver operator characteristics curves (ROC).
RESULTSUsing a serum PG I concentration < or =80 ng/ml, I: II ration < or =5.0 and OPN concentration > or =34 ng/ml or > or =30.4 ng/ml (based on ROC) for gastric cancer screening,the specificity, positive and negative predictive values were superior to that obtained by PG concentration only. Using a serumPGI concentration < or =50 ng/ml, I : II ration C 5. 0 and OPN concentration > or =35.2 ng/ml or > or =29. 2 ng/ml (based on ROC), the sensitivity, positive and negative predictive values were superior to that obtained by PG concentration only. Combining PG and OPN for gastric cancer screening, both sensitivity and specificity were more than 70% , while with OPN alone, only good specificity can be achieved.
CONCLUSIONCombining different serum PG and OPN concentration for gastric cancer screening is superior to PG or OPN only. This may be used as a new method in gastric cancer mass screening.

4.Adjuvant therapies after surgery for hepatocellular carcinoma with microvascular invasion

The high recurrence rate of hepatocellular carcinoma (HCC) after surgery cannot be ignored, but there are still no consensus statements and guideline recommendations for adjuvant therapies after HCC surgery. Current studies have shown that microvascular invasion is one of the high-risk factors for postoperative recurrence of HCC, and for HCC patients with microvascular invasion, timely postoperative adjuvant therapy may delay the time to recurrence and bring benefits to patients. At this stage, the prevention of postoperative recurrence of HCC and the prolongation of survival time through adjuvant therapy have attracted more and more attention. This article reviews the research advances in adjuvant therapy after surgery for HCC with microvascular invasion in recent years, in order to provide reference and help for postoperative adjuvant therapies for HCC patients with microvascular invasion.
5.Investigation on the ability of nurse-patient communication and Self-efficacy between baccalaureate and associate nursing students

Objective To investigate the ability of nurse-patient communication and Self-efficacy between baccalaureate and associate nursing students, and provide instruction for fostering the nurses' positive ability of nurse-patient communication and Self-efficacy. Methods Thirty baccalaureate nursing students from Grade 2005 and 30 associate nursing students from Grade 2006 in the nursing school of Mudanjiang Medical College were selected by random sampling and were investigated with the ability nurse-patient communication Scale by XuYaHong and General Self-Efficacy Scale by Ralf scchwarzer. Results The total level of nursepatient communication ability was medium or above that. There was significantly different in the ability of nursepatient communication between nurses with different educational level in terms of the total scores of nurse-patient communication, scores of coffering information and understanding patients, those in baccalaureate nursing students were higher than those in associate nursing students. Conclusions The clinicle teacher must take implemation to cultivate the ability of nurse-patient communication during the clinical practice. The result suggests that teachers should build the students' positive self-efficacy to improve their nurse-patient communication, ability.
6.Effects of HBsAg pulsed dendritic vaccination on anti-HBs production in immunosuppressed rats after liver transplantation.

OBJECTIVETo explore the effects of HBsAg pulsed dendritic vaccination on anti-HBs production in immunosuppressed rats after liver transplantation (LT).
METHODSBrown-Norway liver allografts were transplanted into Lewis recipients. The transplanted Lewis rats were injected with EK506 (2 mg/kg) and randomly divided into two groups: rats in HBsAg-DCs group (n = 15) were intraperitoneally injected with HBsAg pulsed DCs at 14 d and 28 d after LT, and rats in the HBsAg group (n = 15) were injected with HBsAg (200 mul) once a week for 12 weeks. Rats without any immunosuppressive treatment after LT served as controls (n = 5). IL-2 and IFN-gamma mRNA expression in spleen were analyzed by RT-PCR, serum IL-2, IFN-gamma and anti-HBs were detected by ELISA.
RESULTSHigh dose of FK506 resulted in the immunosuppressed in LT rats, as evident by low production of IL-2 and IFN-gamma, and without liver rejection compared to rats in the control group. HBsAg-DCs induced high titer of anti-HBs antibody, however, titer of anti-HBs were seldom detectable in the HBsAg group at 1, 2 and 3 mouth after vaccination.
CONCLUSIONThe capacity of HBsAg-DCs to induce anti-HBs in immunosuppressed rats suggested that DC vaccine may prevent HBV recurrence in liver transplanted patients.

7.Effect of no mediator on kainic acid induced behavioral seizures in rats.

AIMTo further explore the roles of endogenous nitric oxide (NO) or NO derivatives in complex partial seizures and generalized convulsions.
METHODSThe effect of pretreatment with L-nitroarginine (L-NNA), an inhibitor of nitric oxide synthase (NOS), or L arginine (L-Arg), a precursor of NO on kainic acid (KA)-induced seizure in rats and the changes in the concentration of NO2 -/NO- in the hippocampus were determined.
RESULTSThe rats appeared with wet dog shakes (WDS) at 15 min and then occurred generalized convulsions during 1 h to 3 h after administration of KA (10 mg/kg i.p.). However, the pretreatment of L-NNA (50 mg/kg) so dramatically promoted and enhanced KA-induced behavioral seizures that the latency of generalized convulsion was shorten dramatically, and the mortality was greatly high. In contrast, the pretreatment with L-Arg (40 mg/kg) markedly delayed or weakened KA-induced behavioral changes, such as increasing latency of WDS and generalized convulsion, shortening time o f seizure and none of animal died during observed time. The concentration of NO2- /NO3- in the hippocampus increased immediately at 30 min and remained to 7 d after the administration of KA. Compared with control group (pretreatment with NS), the concentration of NO2- / NO3- in the hippocampus apparently increased at 3 h and 3 d after the administration of KA in the rats with L-Arg pretreatment.
CONCLUSIONThe endogenous NO (NO or NO derivatives) mediators may play an important role against excitotoxin induced seizures in rats.

8.Effects of PPARγ on the cholesterol efflux of C57 mice peritoneal macrophage in inflammatory conditions

Objective:To explore the effects of PPARγ on the cholesterol efflux of C57 mice peritoneal macrophage.Methods:Firstly constructing C57 mice model under different metabolic situation including high-fat diet and acute inflammation then isolate and culture its peritoneal macrophage,observing the expressions of PPARγ and IκB-α,identify the character of macrophage cholesterol efflux in every group.Then pretreat the normol C57 mice peritoneal macrophage with PPARγ ligand ciglitazone and PPARγ antisense oligonucleotide,observing the effect to cholesterol efflux after simulated with LPS in vitro.Results:The level of mice serum lipids of high fat diet group was significantly higher than that of normal diet group.The results of Western-blotting showed that the expression of PPARγ protein in groups of HFD and stimulated by LPS were significantly higher than that of control group.The expression in groups stimulated by LPS was all lower significantly than in control grouph.The determination of cholesterol efflux showed that this function of macrophage with HFD was more enhanced than that of control group but was inhibited in group stimulated by LPS.To normol peritoneal macrophage pretreat with PPARγ antisense oligonucleotide and stimulated by LPS,the expression of PPARγ protein was lower than that of control group but the expression of IκB-α was depressed obviously.Conclusion:The PPARγ ligand ciglitazone can increase the cholesterol efflux of C57 mice peritoneal macrophage and weaken the inhibition stimulated by LPS.The PPARγ antisense oligonucleotide can depress it and aggravate the inhibition.
9.Effect of acupuncture on hippocampal mitochondrial proteome expression in SAMP8 mouse model with Alzheimer disease

Objective:To observe the effect of acupuncture on the expression of mitochondrial proteome in hippocampus of senescence-accelerated mouse prone g (SAMPg) mice models with Alzheimer disease (AD),and to explore the possible protective mechanism of acupuncture on mitochondria.Methods:Sixty 6-month-old male SAMP8 mice were randomly divided into an acupuncture at acupoint group,an acupuncture at non-acupoint group and a model group,20 mice in each group.The 20 male senescence-accelerated mouse/resistance 1 (SAMR1) mice of the same age were used as a normal control group.Shenshu (BL 23),Baihui (GV 20),Xuehai (SP 10) and Geshu (BL 17) were selected for acupuncture intervention in acupuncture at acupoint group.After an 8-week intervention,mitochondrial tissues were extracted from the hippocampus.Differentially expressed proteins were identified by subcellular organelle proteomics.Western blot was used to verify the expressions of some related proteins in hippocampal mitochondria.Results:Compared with the model group,there were 13 differentially expressed protein spots in the acupuncture at acupoint group,of which,9 were up-regulated,including neurofilament light polypeptide (NFL),actin (cytoplasmic 1,database ID:ACTB),tubulin beta-2A chain (TBB2A),tropomodulin-2 (TMOD2),pyruvate dehydrogenase E1 component subunit beta (PDHE1-β),NADH-ubiquinone oxidoreductase 75 kDa subunit (database ID:NDUS1),heat shock cognate 71 kDa protein (HSC71),pyruvate dehydrogenase E1 component subunit alpha (PDHE1-α) and ATP synthase beta subunit (ATP-β);4 were down-regulated,including glial fibrillary acidic protein (GFAP),pyruvate dehydrogenase phosphatase 1 (PDP1),mitochondrial-processing peptidase subunit alpha (MMP-α) and adenosine kinase (ADK).According to the information provided in the protein database,most of the differentially expressed proteins involve the regulation of mitochondrial function and structure.The expression levels of NFL and TBB2A in the normal control group and the acupuncture at acupoint group were significantly higher than those in the acupuncture at non-acupoint group (P<0.05).ATP-β and NDUS1 expression levels were significantly higher in the acupuncture at acupoint group than those in the acupuncture at non-acupoint group (P<0.05);there was no significant difference between the acupuncture at non-acupoint group and the model group (P>0.05).Conclusion:Acupuncture may achieve the potential therapeutic effect on AD by regulating the structure and functional proteins of hippocampal mitochondria.
10.Survey of attitudes and behaviors of healthcare professionals on delirium in ICU.

OBJECTIVETo assess the medical community's awareness and practice regarding delirium in the intensive care unit (ICU).
METHODSOne hundred and ten predesigned questionnaires were distributed to ICU practitioners in the affiliated hospitals of Zhejiang University.
RESULTSA total of 105 valid questionnaires were collected. Totally, 55.3% of the clinicians considered that delirium was common in the ICU. Delirium was believed to be a significant or serious problem by 70.5% of respondents, and under-diagnosis was acknowledged by 56.2% of the respondents. The incidence of ICU delirium is even more under-estimated by the pediatric doctors compared with their counterparts in adult ICU (P less than 0.05). Primary disease of the brain (agreed by 82.1% of the respondents) was believed to be the most common risk factor for delirium. None of the ICU professionals screened delirium or used a specific tool for delirium assessment routinely. The vast majority (92.4%) of respondents had little knowledge on the diagnosis and the standard treatment of delirium.
CONCLUSIONSAlthough delirium is considered as a serious problem by a majority of the surveyed ICU professionals, it is still under-recognized in routine critical care practice. Data from this survey show a disconnection between the perceived significance of delirium and the current practices of monitoring and treatment in ICU in China.
