Chinese Acupuncture & Moxibustion 2008;28(5):375-377
UNLABELLEDFor cupping-bloodletting therapy of Saudi Arabia, the patient takes a sitting position or lying position, fully exploring the positions for bloodletting, mainly locating at the back and the neck, corresponding to bilateral Geshu (BL 17), Fengchi (GB 20), Dazhui (GV 14) and Ashi points in Chinese acupuncture. The local hair of the patient is cut off and then cupping is given to the related points for 5-10 minutes. After the cup is removed, the local part for blood-letting, and the mouth and internal wall of the cup are sterilized with 75% alcohol. The doctor wears a sterilized gloves and a mask, and a mono-use surgical blade is used to superficially cut the skin inside each of the cupping marks for about 10 cuts. And cupping is again given at the local parts, with blood drawn into the cup. 5-10 minutes later, the blood in the cup becomes solid, then the cup with the blood is removed, and the skin is cleaned with 75% alcohol and the wounds are sterilized with 2% iodine and are covered with gauze and is fixed with adhesive plaster for 24 hours.

A 20-year-old male patient presented with myalgia of upper limbs and myasthenia of extremities for more than 1 month. Physical examination showed diffuse erythema on the cheeks, upper eyelids, upper chest, neck and dorsa of the hands. The myodynamia of the proximal and distal muscles of upper and lower extremities was grade Ⅳ, Ⅴ, Ⅲ and Ⅴ respectively. Laboratory examinations revealed that the serum levels of creatine kinase, CK-MB and lactate dehydrogenase were 2103 U/L, 83 U/L and 489 U/L respectively, which were all above the normal range. Electromyogram revealed myopathic abnormality and normal nerve conduction velocity. Histopathology of gastrocnemius muscle showed hypertrophy and swelling of muscle fibers, disappearance or fuzziness of transverse striation, and intermuscular lymphoid cell infiltration. A biopsy of the skin lesion from the upper chest showed liquefaction degeneration of and colloid bodies in basal cell layer, perivascular lymphoid cell infiltration in the dermis. A diagnosis of dermatomyositis was established based on the clinical and laboratory findings. After management with intravenous prednisolone 80 mg once daily and symptomatic treatment for 4 weeks, the myodynamia of upper limbs was improved, serum levels of creatine kinase,CK-MB and lactate dehydrogenase reached the normal ranges. However, the myodynamia of lower limbs progressively deteriorated with the emergence of paresthesia. Enhanced MRI scan showed a tumor in the vertebral canal at the level of thoracic vertebra 11 to 12. A spherical encapsulated tumor measuring 3 cm in diameter was surgically removed. The tumor was diagnosed as paraganglioma in vertebral canal according to pathological and immunohistochemical findings. The patient was finally diagnosed with paraganglioma in vertebral canal superimposed on dermatomyositis.

OBJECTIVETo investigate the process of separating and purifying flavonoids from Smilax glabra.
METHODWith the yield of flavonoids as index, the optimum process of separating and purifying flavonoids from S. glabra Roxb was screened by static and dynamic adsorption tests.
RESULTThe static saturated adsorption capacity of D101 macroporous resin to flavonoids of S. glabra was 45.6 mg x g(-1) (dry resin). The optimum conditions of dynamic adsorption and elution were as that the pH, the concentration, the adsorption velocity of the extracting solution, and the adsorption capacity were 6.00 +/- 0.20, 4.2 mg x mL(-1), 2 mL x min(-1) and 15 mL, respectively. The adsorbed resin column was washed by 100 mL 60% ethanol with pH value of 8.00 +/- 0.20 at the eluting velocity of 3 mL x min(-1) after washed by 100 mL distilled water.
CONCLUSIONThe flavonoids of S. glabra was able to be easily separated and purified by D101 macroporous resin under the optimum conditions above, and the recovery rate was higher than 90%. The content of obtained flavonoids reached 62.6%, which was 2 times of the content before purification.
