1. The clinical significance of inflammatory cytokines in acute lower extremity deep venous thrombosis developed in hospitalized postoperative patients with abdominal/pelvic malignancies

Objective: To explore the association of inflammatory cytokines including CRP (C-reactive protein), IL-6 (interleukin 6), IL-10 (interleukin 10), NF-κB (nuclear factor-kappa B) and Es (E-selectin) with acute lower extremity DVT (deep venous thrombosis) developed in hospitalized postoperative patients with abdominal/pelvic malignancies. Methods: Serum samples were obtained to examine the serum levels of CRP, IL-6, IL-10, NF-κB and Es in hospitalized postoperative patients with abdominal/pelvic malignancies who initially developed acute lower extremity DVT (group A, n = 40), and these serum cytokine levels of group A were compared with those of hospitalized postoperative patients with abdominal/pelvic benign diseases who also developed acute lower extremity DVS (group B, n = 20), the hospitalized postoperative patients with abdominal/pelvic malignancies who did not develop acute lower extremity DVS (group C, n = 40), and the healthy subjects who volunteered as normal controls (group D, n = 20). Results: The serum levels of CRP, IL-6, IL-10, NF-κB and Es in group D were significantly lower than those in groups A, B and C (P < 0.01). There was no significant difference in CRP level among groups A, B and C (P > 0.05). The serum levels of IL-6, NF-κB and Es in group A were significantly higher than those in groups B and C (P < 0.05). The serum level of IL-10 in group A was significantly lower than those in groups B and C (P < 0.05). Conclusion: Serum CRP level may not be used as an early predictor for the diagnosis of acute lower extremity DVT developed in hospitalized postoperative patients with abdominal/pelvic malignancies, but the serum IL-6, NF-κB and Es levels can be used as early predictors for this diagnosis. 3.Comparison of Manual and Automatic Evaluation of Speech Recognition Threshold Using Mandarin Disyllabic Test

Objective To compare the results of manually -tested speech recognition threshold (SRT ) with automatically software -recorded SRT in the trial of Mandarin disyllabic test ,exploring the significance to the clini-cal applying .Methods 128 normal people of different ages without hearing loss and 57 workers exposed to noise in an automobile manufacturing was selected .These two group of volunteers speak mainly Mandarin in their daily life . MADSEN Conera (Danmark) clinical audiometr was applied .A group of double syllable word list with the same dif-ficulty of equivalence was used as test material .The initial presentation level was 20 dB HL higher than PTA .Then compared the results of manually -tested SRT with automatically software -recorded SRT .Results In the normal group ,the automatic value SRT was 7 .84 ± 3 .98 dB HL ,the manual value was 9 .19 ± 4 .47 dB HL ,and the average value of speech frequency threshold was 7 .63 ± 5 .78 dB HL .In the noise group ,the automatic value SRT was 6 .10 ± 8 .40 dB HL ,the manual value was 18 .81 ± 9 .52 dB HL ,and the average value of language frequency threshold was 27 .18 ± 19 .13 dB HL .There was significant difference between the values of SRT tested manually and recorded automatically (P<0 .01) .Conclusion There are differences between SRT valued manually and automatically .The SRT in people with normal hearing can be tested using automatic -recorded method .This method is convenient for screening in people without hearing loss .To exam in people with hearing loss ,the manual test is more appropriate .

5.Multivariate analysis of prognostic factors after radical resection for hepatocellular carcinoma

Objective To determine the prognostic factors after radical resection (RR) for hepatocellular carcinoma (HCC).Methods Altogether 144 patients who had undergone RR for HCC from 1988 through 1995 were included for a univariate and a Cox multivariate analysis.Nineteen factors contributing to overall survival rate (SR) and disease-free SR were analysed.Results The 5-year SR and disease-free SR (N=144) were 47.3% and 23.9%,separately.Multivariate analysis revealed that classification of RR was the signficant factor to overall SR,and presence of vessel invasion was the signficant factor to disease-free SR.The 5-year SR and disease-free SR in the patholngically RR and clinically RR groups were 60.2%,29.0% and 14.0%,0%,respectively.The 5-year disease-free SR in the group without (or with) vessel invasion was 27.8% (or 0%).Conclusions The classification of RR is the determinative prngnostie factor.Pathologically RR is the first option for patients with in- dications.It is essential to improve adjuvant therapy to decrease postoperative recurrence and metastasis rates.
6. Effects of the effective components group of Xiaoxuming decoction on MCAO rats

OBJECTIVE: To investigate the protective effects of the effective components group of Xiao-Xu-Ming decoction (XXM) on focal cerebral ischemia rats. METHODS: The model of focal cerebral ischemia was made by middle cerebral artery occlusion (MCAO) with nylon suture. The neuroethological score and the staying time on slope board were gotten after 48 h of the operation. The percentage of cerebral infarction volume to total brain volume was calculated by TTC staining. Furthermore, the content of MDA, the activities of SOD and GSH-Px, and the activity of NOS in the brain tissue were detected by photospectrometer analysis system. RESULTS: The effective components group of Xiao-Xu-Ming decoction could significantly reduce the scores of neurological deficits , alleviate the cerebral infarction volume, improve the activity of SOD, reduce the content of MDA, and decrease the activity of iN-OS, but have no effect on the activity of GSH-Px. CONCLUSION: The effective components group of Xiao-Xu-Ming decoction may attenuate the focal cerebral ischemia injury in rats, and the mechanism is related to regulate the balance between oxidation and anti-oxidation and decrease the activity of iNOS in the brain tissue. 7.Effect of Family Rehabilitation Plan on Motor Function of Stroke Patients with Hemiplegia

ObjectiveTo observe the effect of family rehabilitation plan (FRP) on motor function of stroke patients with hemiplegia.Methods64 patients were randomly divided into the FRP group (32 cases) and control group (32 cases). The patients in the FRP group critically followed the schedule and activities scheduled on FRP and were assessed and guided in out-patient department every two weeks. The patients in the control group made schedule and carried on activities by themselves. Motor function was assessed with Fugl-Meyer Assessment (FMA) and modified Barthel index (MBI) respectively.ResultsMotor scores of the patients in two groups significantly increased after treatment ( P<0.001), and that of the FRP group was significantly better than that of the control group ( P<0.001).ConclusionThe FRP can significantly improve motor function of stroke patients with hemiplegia.
8.Application of autologous corneal len inlays in correction of hyperopia

Background The safety,effectiveness and predictability of laser in situ keratomileusis (LASIK) to correct hyperopia are of a widely clinical concern,but the efficacy of LASIK for hyperopic correction is not satisfied in comparison to that of myopia.Objective The aim of this work was to evaluate the safety,effectiveness and predictability of autologous corneal len inlays to correct hyperopia.Methods Autologous corneal len inlays was performed on the left eye of female patient with the diopter of +3.25 DS.This patient was followed-up for 1 month.Cornea compensatory intraocular pressure (IOPcc) and imitation Goldmann IOP (IOPg) were measured,and the corneal thickness and corneal flap were observed using optical coherence topography (OCT).Corneal topography was examined to check the change of corneal curvature.Ocular response analyzer was used to evaluate the shifts of corneal hysteresis (CH) and corneal resistance factor (CRF).Uncorrected and best corrected visual acuity and refraction were compared between before and after operation.Written informed consent was obtained from the patient prior to the operation.Results No intraoperative and postoperative complications were found.Uncorrected distance visual acuity improved one line 1 day and 1 month after surgery,and near visual acuity progressed to J2 and the spherical equivalent (SE) was-0.125 D 1 month after surgery.OCT exhibited that the cornea was clear and transparent,and the graft was centered in the corneal stroma capsular bag without rejection 1 month after surgery.The IOPcc was 12.4 mmHg before operation and 11.9 mmHg 1 month after operation,and IOPg was 11.9 mmHg before operation and 10.7 mmHg 1 month after operation in the operated eye.The CH was 9.7 mmHg before operation and 8.9 mmHg 1 month after operation,and CRF was 10.9 mmHg before operation and 10.3 mmHg 1 month after operation,without remarkble change in the indexes mentioned above.Conclusions Autologous corneal implantation can be used to correct hyperopic eye with good safety,effectiveness and predictability.
