Objective The study was to evaluate DWI for quantifying liver fibrosis. Methods A total of 12 volunteers, 47 patients who had chronic HBV or HCV hepatitis and underwent liver biopsy [Scheuer score for fibrosis(S) and inflammation(G)] were enrolled in this study. They were scanned using a 1.5 T MR unit with b value of 0,250,500,750, 1000 s/mm~2. ADCs at b_(250-1000) and b_(500-1000) were the average ADCs of b=250, 500, 750, 1000 s/mm~2 and b=500, 750, 1000 s/mm~2. The studied the correlation between Scbeuer scores and ADC values, and conducted Mann-Whitney U test and Logistic regression to evaluate ADC for prediction of fibrosis scores. Results The average ADCs were (1.41± 0.11),(1.37±0.09), (1.27±0.05), (1.26±0.04), (1.22±0.06) mm~2/s respectively from SO to S4, stage at b=750 s/mm~2 (F=18.31, P<0.01). With the increase of fibrosis score, the average ADC decreased gradually, the two were better negatively correlated at b_(250-1000)(r=-0.727, P<0.01) than other b values. Using b_(750) and the two combined b values, the found significantly lower ADCs in S2 or greater versus S1 or less and in S3 or greater versus S2 or less fibrosis (P<0.01). The best predictor for S2 or greater was b_(750) with the largest AUC of 0.909, sensitivity of 85.7%, and specificity of 100.0% (ADC ≤1.35×10~(-3) mm~2/s). The best predictor for S3 or greater was b_(250-1000) with the largest AUC of 0.864, sensitivity of 69.6%, and specificity of 95.8% (ADC≤1.53×10~(-3) mm~2/s). Conclusion DWI can be a good predictor for scoring liver fibrosis for S2 or S3 stage above, while b_(750) and the combined b values are suitable for evaluation.

Objective To discuss the influence about three kinds of nasal feeding methods on ICU critical patients with gastrointestinal complications, thus providing guidance for ICU patients to choose reasonable way of nasal feeding pump injection. Methods 120 ICU patients were randomly assigned into three groups, which is the continuous 6-hour and intermittent 2-hour every three time pump group and continuous 24-hours pump group and intermittent 24-hour(continuous 2-hour and intermittent 1-hour every eight time units)pump group ,each group has 40 cases. To observe the the influence about three different way of nasogastric on ICU critical patients with gastrointestinal complications. Results The incidence of diarrhea and constipation among the three groups was not statistically significant (P>0.05) . The gastric retention rate of the continuous 24-hour pump group was significantly higher than that of the continuous 6-hour and intermittent 2-hour pump group and the intermittent 24-hour pump group, the differences were statistically significant (P < 0.05). There was no statistically significant difference between the continuous 6-hour and intermittent 2-hour pump group and the intermittent 24-hour pump group, (P > 0.05). Conclusions The continuous 24-hour pump group in gastric retention was slightly worse than the other two pump way. Compared with ICU class three a week and feeding times , the continuous 6-hour and intermittent 2-hour pump method is more suitable for ICU patients.

Objective To investigate and optimize the condition of purification and crystallization of oncogenic protein MDM2.Methods MDM2 was expressed in E.coli expression system, and purified by Ni-NTA chelating affinity chromatography and molecular sieve chromatography.The secondary structure of purified protein was analyzed by circular dichroism spectroscopy (CD).Then the crystallization condition of MDM2 was screened and optimized by sitting-drop vapor-diffusion method.Results High purity of MDM2 was obtained by Ni-NTA chelating affinity chromatography and molecular sieve chromatography purification.CD analysis indicated the secondary structure of MDM2 was ordered.Protein-crystallisation experiments illustrated that MDM2 was prone to crystallization under lower pH.Conclusion The optimum pH of MDM2 protein crystallization is 5.5, the optimum protein concentration is 10 mg/mL.

Objective To discuss the pathogenesis,diagnosis and treatment of delayed traumatic intracerebral hematoma.Methods The clinic data of traumatic delayed intracranial hematoma patients in this hospital were retro- spectively analyzed.According to clinic observation and CT re-examination,47 cases were diagnosed as delayed trau- matic intracranial hematoma(45 cases by operative treatment,and the other 2 by conservative treatment).Results There were 21 cases of recovery,10 cases of slight disability,8 cases of severe disability,8 cases of death.The total mortality rate was 17 %.Conclusion Brain contusion,subarachnoid hemorrhage and skull base fracture were impor- tant factors of DTICH.Fine-observation and prompt CT re-examination offered excellent results for DTICH.

Objective To explore the effects of silybin-phosphatidylcholine compound (SPC )on lipopolysaccharide (LPS)-induced activation of nuclear factor-?B (NF-?B) and phosphorylation and degradation of inhibitors of NF-?B?(I?B?).Methods Phagocyte were collected in abdominal cavity of Kunming mousse aged 6 to 8 weeks,cultured phagocyte(2?105 mL-1) were divided into control, LPS and SPC groups randomly, phagocyte in control group were added into the same volume of 9 g/L sodium chloride.Phagocyte in LPS group were added into a single bolus of LPS(10 ?g/mL LPS) for 24 hours, phagocyte in SPC groups were preincubated with different concentration of SPC for 2 hours followed by a 24 hours incubation with 10 ?g/mL LPS. Immunocytochemistry were used to measure the contents of NF-?B, phosphorylated I?B? in phagocyte.Results The content of NF-?B p65 located in the nuclear in control group was little. The content of NF-?B p65 located in the nuclear in LPS group markly higher than that in control group(P

Objective To study the expression of active efflux pump AdeABC in clinical Acineto-bacter baumannii islates and whether this efflux pump confers resistance to antibiotics. Methods The anti-biotic susceptibility and the function of efflux pump inhibitor were tested by micro-dilution broth method. The expression of adeB was examined by RT-PCR. The controlling gene adeRS was amplified by PCR and se-quenced. Results Thirty multidrug resistance Acinetobacter baumannii isolates and 5 sensitive isolates for PCR were both abtained the expected products of adeB and adeRS. The mRNA expression of adeB in 15 multidrug resistance(MDR) isolates were positive, but there was no expression of adeB in 5 sensitive iso-lates. The mutations of adeRS existed in 2 MDR isolates. Conclusion The expression of AdeABC may in-volved in the resistant mechanisms of the clinical MDR Acinetobacter baumannii isolates.

Objective To study the expression of active efflux pump AdeABC,AdeIJK,AdeFGH,AbeM,AbeS,CraA,MdtL in clinical Acinetobacter baumannii isolates and whether the efflux pumps confers resistance to antibiotics.Methods Thirty-two multi-drug resistant Acinetobacter baumannii islates and 10 sensitive isolates were collected.Genes of the exporter protein were amplified by PCR.The expression of adeB,adeJ,adeG,abeM,abeS,craA,mdtL were examined by real-time fluorescence quantitative RT-PCR.The controlling genes adeRS and adeL were amplified by PCR and sequenced.Results The positivity rates of adeB,adeJ,adeG,abeM,abeS,craA,mdtL were 100％,100％,100％,96.88％,100％,100％ and 93.75％ respectively in 32 multi-drug resistant Acinetobacter baumannii isolates,and all 100％ in 10 sensitive isolates.The difference of expression of adeB,abeM and mdtL were significant( P＜0.001,P =0.001,P=0.013) between 21 multi-drug resistant isolates of C clone and 10 sensitive isolates.The mutations of adeRS existed in 2 multi-drug resistant isolates,no point mutation of adeL.Conclusion The expression of AdeABC,AbeM and MdtL may involved in the resistant mechanisms of the clinical Acinetobacter baumannii islates.

ObjectiveThe purpose of this study is to discuss the diagnostic accuracy of nornalized liver ADC using the spleen and renal cortex as reference organs for the diagnosis of liver fibrosis.Methods Forty three patients with liver disease (chronic liver disease group) at compensated stage and 10 healthy volunteers (control group) were retrospectively assessed with diffusion-weighted imaging at a 3.0 T MR unit.Liver ADC,spleen ADC,renal ADC and normalized ADC (defined as the ratio of liver ADC to spleen ADC or renal cortex ADC,S-ADC and R-ADC for short) were measured in patients stratified by fibrosis stage.Spearman analysis was used to see the correlation between fibrosis stages and ADC,one-way ANOVA was used to compare the ADCs in different fibrosis stages.Logistic regression analysis was used to determine the performance of ADC for prediction of liver fibrosis,and show the area under the curve ( AUC),sensitivity and specificity choosing the optimal cutoff value that maximized the Youden index.ResultsTen volunteers belonged to SO stage.From SO to S4 stage,there were 2,5,9,12 and 15 patients,correspondingly,liver ADCwere (1.37±0.13) ×10-3,(1.33±0.16) ×10 -3,(1.17±0.16) ×10-3,(1.23±0.14) ×10-3and ( 1.12 ±0.11 ) × 10-3mm2/s,S-ADC were 1.86 ±0.18,1.68 ±0.12,1.34 ±0.14,1.48 ±0.15 and 1.34±0.10,R-ADC were 0.71 ±0.08,0.68 ±0.12,0.61 ±0.09,0.64 ±0.11 and 0.60 ±0.08respectively,and the differences among them were significant ( F =6.48,18.70 and 3.04,P ＜0.05 ).The correlation between fibrosis stage and S-ADC was stronger than between fibrosis stage and liver ADC,R-ADC (r =- 0.71,- 0.51,- 0.41 ;P ＜ 0.01 ).S-ADC was superior to liver ADC and R-ADC for detection of S2,S3 and S4 fibrosis stage (Youden index:0.91,0.58,and 0.59).ConclusionSpleen normalized liver ADC improves diagnostic accuracy for detection of liver fibrosis than liver ADC and renal normalized liver ADC.
