2.High risk factors and treatment analysis of local recurrence after Dixon operation of rectal cancer

Objective To investigate the risk factors and treatment of local recurrence after Dixon operation of rectal cancer.Methods The clinical data of forty-seven patients who had local recurrence after Dixon operation of rectal cancer in our hospital from January 2011 to December 2013 was analyzed retrospectively.According to literature review,the high risk factors and treatment methods of local recurrence of rectal cancer in China and abroad were analyzed.Results The treatment methods of patients with local recurrence after anterior resection were radical resection,palliative resection and chemotherapy.The prognosis of patients with radical resection whose overall survival time was (38.202±2.411) months,better than that with palliative resection ((24.591±2.150) months),better than that with chemotherapy ((12.833±1.033) months).There was a significant difference in the survival time among the three groups (χ2=30.155,P<0.001).Conclusion Fully understanding of high risk factor can help reduce the probability of local recurrence.For patients with local recurrence of the rectal cancer,radical resection should be performed.In case that radical surgery is not possible,positive treatments should be adopted if the patients are in good physical condition.
3.Study on differentiation of murine embryonic stem cells into hepatocyte-functional cells induced by hepatocyte growth factor (HGF)

Objective To investigate the functional role of mur in e hepatocyte growth factor (HGF) in inducing in vitro the mouse embryonic st em cells (ESC) to differentiate unidirectionally into hepatocyte. Method s Both spontaneous and HGF induced differentiations of the primordial c ell elusters formed by suspended culture in vitro of ESC were daily observed with microscope for 5-7 days. After 4 weeks of culture, the cells were stained with HE and glycogen staining, their morphology were observed, The synthesis of urea nitrogen and triglycerides in the culture medium, the expression of myocard ial MHC, albumin, AFP and CK18, and the indocyanine green (ICG) staining were a lso detected. Results It was hard to control the spontaneous di fferentiation of ESC. HGF could promote the differentiation of ESC into endoderm , and more likely into myocardium. HGF could also induce the expression o f albumin, AFP and CK18, and positive staining of ICG and FDA. Conclusio ns HGF may induce the differentiation of ESC into hepatocyte, but the f unctional role is limited. It implies that a comprehensive effort of multiple fa ctors might be needed in inducing the hepatocyte differentiation.
4.CRP promotes lectin-like oxidized low density lipoprotein receptor-1 expression on THP-1 derived macrophages

Objective To explain the effects of C-reactive protein(CRP) on lectin-like oxidized low density lipoprotein receptor-1 expression on THP-1 derived macrophages and the related signal transduction pathways.MethodsTHP-1 cells were differentiated into macrophages with the stimulation of PMA.THP-1 derived macrophages were incubated with CRP and co-incubated with inhibitors of NF-?B、AP-1 and MARK signal transduction pathways.The expression of LOX-1 antigen and mRNA was analyzed by ELISA and RT-PCR.Results CRP stimulated the expression of LOX-1 antigen and mRNA on macrophages in a dose-dependent manner.NF-?B inhibitor BAY11-7085 suppressed the inducible effects of CRP on LOX-1 expression.Conclusion CRP increased LOX-1 expression on THP-1 derived macrophages at transcription and post-transcription levels.The NF-?B signal transduction pathway may be involved in such process.
5.Analysis of Articles in Chinese Medical Equipment Journal from 2002 to 2008

Objective To analyze the characteristics of the Chinese Medical Equipment Journal's articles and authors as well as their spatial & temporal distribution,and then put forward some advices for the development of the journal.Methods 4 434 articles of the journal from 2002 to 2008 were sampled for bibliometrics analysis from the aspects of column,author,region,organization,publishing delay and foundation support.Results With an abundant manuscript source mostly from the developed areas such as Beijing,Guangdong Province,Shandong Province and Tianjin,the journal set its columns reasonably and flexibly,and thus gifted with a short publishing delay.The coauthor degree and cooperation rate were less than the journals in other fields,and the number of foundation-supported articles,also less than others,but was increasing yearly.Conclusion Chinese Medical Equipment Journal,with a high-quality manuscript source and great influence in the field,can be considered as a platform for communication and academic achievements publishing.
6.Sentinel lymph node detection of rabbit tongue by indirect CT lymphography combined with nanocarbon tracer.

OBJECTIVE:
To investigate the value of indirect CT lymphography (CT-LG) combined with nanocarbon tracer suspension staining in the sentinel lymph node (SLN) detection of rabbit tongue (cancer).
METHOD:
Eight rabbits were divided into two groups, i. e. normal control group (6 rabbits) and VX2 tongue carcinoma group (2 rabbits). Pre-contrast CT images were taken for all rabbits. The mixture of iohexol and nanocarbon tracer suspension (iohexol 0.25 ml and nanocarbon tracer 0.25 ml, respectively, volume ratio = 1:1) was used as contrast agent of indirect CT-LG. Indirect CT-LG was performed as the contrast agent was injection into right ventrolateral submucosa of the tongues. CT images were acquired at 1, 5, 10 and 15 min after injection.
RESULT:
There were no cervical lymph nodes clearly identified in any group on the pre-contrast CT images. After the contrast agent injection, indirect CT-LG showed an ipsilateral SLN with rapid enhancement which was the only enhanced SLN, located lateral to the larynx-trachea region in all rabbits of VX2 control group and carcinoma group. The CT attenuation values of SLN were (58.80 ± 5.45) HU, (134.80 ± 28.92) HU, (91.00 ± 13.08) HU and (63.80 ± 4.32) HU at 1, 5, 10 and 15 minutes after indirect CT-LG. After the cervical anatomy guided by CT scan, all of the right deep SLN showed a distinct enrichment of black dye highly consistent with the locations on the indirect CT-LG images.
CONCLUSION
It is more accurate for sentinel lymph node detection by indirect CT lymphography combined with nanocarbon tracer staining.

7.Freiberg's disease treated with dorsal closed Ⅴ-shaped osteotomy and Charlotte breakage screw fixation

Objective To evaluate the clinical outcomes of treating Freiberg's disease with dorsal wedge osteotomy and Charlotte breakage screw fixation.Methods From June 2010 to June 2014,11 patients with Freiberg's disease were treated at our department.They were 5 males and 6 females at an average age of 32 years(range,from 15 to 52 years).X-ray revealed osteosclerosis and collapse of the metatarsal head.According to Smillie classification system,there were 5 cases of stage Ⅱ and 6 ones of stage Ⅲ.The duration of symptoms was from 7 to 48 months (average,23 months).The metatarsal heads were rotated to reconstruct the joint surface.After dorsal closed V-shaped osteotomy,Charlotte breakage screws were implanted.Results All wounds healed with no complications in the early postoperative period.The 11 patients were followed up for 14 to 48 months (average,19 months),showing obvious pain relief.X-ray showed that the osteotomy sites got solid union in all patients at an average time of 10 weeks (range,from 8 to 13 weeks) after operation.Average shortening of the metatarsal was 1.5 mm (range,from 1.1 to 1.8 mm).The American Orthopaedic Foot and Ankle Society(AOFAS) score,Lesser Metatarsophalangeal-Interphalangeal Scale(LMIS) score,and range of motion at one day,one month and one year after operation were significantly improved compared with the preoperative ones (P < 0.05).Significant differences were also observed between all postoperative time points (P < 0.05).No such complications happened as referred pain from the adjacent metatarsophalangeal joint,displacement,nonunion,necrosis or infection.All patients returned to sports and recreational activities at 6 months after operation,except one case of stage Ⅲ who had constant swelling in the metatarsophalangeal joint but eventually recovered at 10 months after operation.According to patients' satisfaction at the last follow-ups,including 7 cases were excellent,3 good and one poor.Conclusion Dorsal closed V-shaped osteotomy with Charlotte breakage screw fixation is an effective procedure for Freiberg' s disease of stages Ⅱ and Ⅲ,because it leads to effective reconstruction of the metatarsophalangeal joint,allows early joint motion and avoids a second operation.
8.Pleuropulmonary blastoma:one case report

ObjectiveTo investigate the early diagnosis and treatment of pleuropulmonary blastoma in children. Meth-ods The progress of pleuropulmonary blastoma from type 1 to type 3 was retrospectively analyzed.Results The male infant had no obvious abnormality in chest X-ray at 18 days. At one year old, X-ray iflm of the chest showed cystic lesions near hilum of the left lung (about 5 cm × 4 cm × 4 cm). The congenital cystic adenomatoid malformation was considered and an operation resection was suggested. However, it had been rejected by his parents. At 3 years and 2 months old, chest X-ray and CT showed left lung had a solid cystic lesion, the volume was signiifcantly enlarged, the back side was solid and the upper part had a large gas cavity. Two months later, the patient had asthmatic suffocation. The chest CT showed the tumor was completely solid. Pleuropulmonary blastoma type 3 was considered. The patient received the conventional chemotherapy after operation excision, and was followed up for 1 year. No recurrence was observed after the treatment.Conclusions Type 1 pleuropulmonary blastoma and congenital cystic adenomatoid malformation are hard to be differentially diagnosed. Early diagnosis and treatment of type 1 pleuropulmo-nary blastoma can improve the prognosis.
