1.Role and Therapeutic Prospect of Intestinal Microecology in Crohn’s Disease

The etiology and pathogenesis of Crohn’s disease(CD)are not fully clear,and genetic susceptibility, immunologic disorder,intestinal barrier dysfunction and intestinal microecology are considered to be involved in the pathogenic mechanism of CD. In recent years,the relationship between intestinal microecology and CD has received much attention. Several studies confirmed that the intestinal microecology in CD patients was different from that in normal person. The change of intestinal microecology was correlated with the occurrence of CD,and modulation of intestinal flora was effective in the treatment of CD. This article reviewed the relationship between intestinal microecology and CD and the therapeutic prospect of intestinal microecology for the treatment of CD.
2.Hemocompatibility of zinc-calcium-phosphate coating on the surface of AZ31 magnesium alloy in vitro

BACKGROUND: In our previous studies, zinc-calcium-phosphate (Zn-Ca-P) coating has been successful y prepared on the surface of AZ31 magnesium al oy, which improves the corrosion resistance of the al oy. OBJECTIVE: To analyze the hemocompatibility of Zn-Ca-P coated AZ31 magnesium al oy in vitro. METHODS: Zn-Ca-P coating was prepared on the surface of AZ31 magnesium al oy using chemical transformation. Afterwards, the characteristics of the Zn-Ca-P coating were investigated using scanning electron microscope, and X-ray diffraction analyzed its components. Besides, hemocompatibility in vitro was evaluated by platelet adhesion assay, dynamic clotting time test and hemolysis test. RESULTS AND CONCLUSION: Under scanning electron microscope, a flower-like coating was formed ont surface of the AZ31 magnesium al oy, with the main chemical component of insoluble phosphates Zn3(PO4)2?4H2O. And some round distortionless blood platelets adhered to the surface of AZ31 magnesium al oy, with no pseudopodia. In dynamic clotting time test, the long curve of Zn-Ca-P coating tended to descend indicating better anticoagulant activity. And the hemolysis rate was below 5%. In conclusion, Zn-Ca-P coating has good hemocompatibility in vitro.
3.Effects of cyclic and continuous nutrition infusion on serum nutritional indicators in patients with mechanical ventilated respiratory failure

Objective To observe different effects of cyclic and continuous nutrition infusion on serum nutritional indicators, peripheral white blood cell counts (WBC), arterial partial pressure of oxygen (PaO2), arterial partial pressure of carbon dioxide (PaCO2) and acute physiology and chronic health evaluation Ⅲ (APACHE Ⅲ ) score in patients with mechanical ventilated respiratory failure (MVRF).Methods A cross-over and self-controlled trial was conducted in 48 patients with MVRF treated in a medical intensive care unit during December 2006 to June 2009, and continuous nutrition (group A) and cyclic nutrition (group B) were infused respectively for patients of the two groups.Serum levels of albumin (ALB), pre-albumin (PA), transferrin (TR), PaO2, PaCO2, WBC and APACHE Ⅲ score were measured for the patients with 24-hour continuous nutrition (group A) and 16-hour cyclic nutrition (group B)infusion.Effects of the two nutritional therapies were compared.Results After nutrition infusion, serum levels of ALB, PA and TR were (34±3)g/L, (196±28)mg/L and (2.1±0.3 ) g/L in group A, and (35 ±4) g/L, (198 ±25) mg/L and (2.0 ±0.4) g/L in group B, respectively; and PaO2 and PaCO2levels were (92 ± 12) mm Hg ( 1 mm Hg =0.133 kPa) and (42 ± 10) mm Hg in group A, and (91 ±9)mm Hg and (42 ± 10) mm Hg in group B, respectively.WBC and APACHE Ⅲ score were ( 11.8 ± 1.7) ×109/L and 38 ±7 in group A, and ( 12.6 ± 1.2) × 109/L and 40 ±6 in group B, respectively.Significant difference in serum levels of PA and TR was found between the two groups (PPA =0.019 and PTR =0.013),while there was no significant difference in other indictors between the two groups.Conclusions 24-hour continuous nutrition infusion for patients with MVRF can obviously improve their serum levels of PA and TR,but has no effect on serum level of ALB, PaO2, WBC and APACHE Ⅲ score in critical ill patients, as compared to those with 16-hour cyclic nutrition infusion.
4.The student-centered teaching clinic in general internal medicine is a new model for outpatient education

International medical community has seen the prevail of outpatient teaching for medical students in recent years. A student-centered and faculty-run teaching clinic was set up based on current outpatient resources in 2008 by Division of General Internal Medicine (DGIM) in Peking Union Medical College Hospital (PUMCH). We concluded that teaching clinic was an effective teaching mode for medical students to improve clinical skills, diagnostic proficiency and health-care professionalism in internal medicine.

6.Prognosis-relevant factors in primary vaginal carcinoma

Objective:To evaluate prognosis factors of primary vaginal cancer. Methods:Data of 80 patients who were hospitalized in the Department of Gynecology and Oncology of Guangxi Tumor Hospital from January 2003 to January 2015 were retrospectively ana-lyzed. All the patients were divided into radiotherapy group (n=49) and surpery group (n=31). Based on radiation mode, the patients with radiotherapy were divided into two-dimensional radiotherapy group (n=29) and three-dimensional radiotherapy (3DRT) group (n=20). Prognosis and complications between two subgroups were compared. Surgical patients were divided into laparoscopic surgery group (n=16) and laparotomy surgery group (n=15) with comparing therapeutic feasibility of video-laparoscopic operation and laparot-omy for primary vaginal carcinoma treatment. Results:Univariate analysis showed that FIGO stage, pathology, tumor size, and extent of vaginal mass involvement were related to prognosis (P<0.05). Multivariate analysis showed that FIGO stage and pathology were in-dependent prognostic factors. Statistical differences of 5-year survival were significant between 2DRT (20.9%) and 3DRT (58.6%) groups (P=0.022). Incidences of urinary tract (14/29, 48.27%) and gastrointestinal symptoms (15/29, 51.72%) in 2DRT group and in 3DRT (3/20,15%;4/20,20%) are different significantly (P<0.05). Hospitalization days of laparotomy surgery group (57.00 ± 41.75) were significantly longer than that of laparoscopic surgery group (29.56 ± 7.30) (P=0.024). Conclusion: Applying laparoscopic surgery and 3DRT improved quality of life without decreasing survival rate of patients with vaginal cancer.
7.Characterization of proteins in hyperlipidemia pancreas using differential gel electrophoresis and tandem mass spectrometry

Objective To investigate the mechanism of hypedipidemia on pancreas of rats by comparative proteomic analysis.Methods Ten male SD rats were randomly divided into two groups,the experimental group Was fed with high lipid forage and the control group Was fed with normal food.Pancreatic samples from the two groups were harvested six weeks later.Differential protein analysis Was performed using differential in-gel electrophoresis(DIGE),and characterizing the protein biomarkers using tandem mass spectrometry.Western blot Was used to confirm the expression of significantly changed proteins.Results Compared to the normal pancreas tissue,a total of 3 protein spot-features were found to be significantly increased and 11 significantly decreased in pancreatic samples with hyperlipidemia.Significantly increased proteins in hyperlipidemia pancreatic samples were arginaseⅡ,ribonuclease inhibitor and glyeine amidinotransferase,which increased by 2.19,1.82 and 1.12 fold,respectively.Significantly decreased proteins in hyperlipidemia group were tyrosyl-tRNA synthetase,alpha-amylase,triacylglycerol lipase,DJ-1protein,Cu,Zn superoxide dismutase,which dicreased by 2.48,2.37,1.85,1.73 and 1.65 folds,respectively.Western blot analysis revealed increased arginase Ⅱ levels and decreased alpha-amylase in pancreatic samples with hyperlipidemia.Conclusions Pancreas wag possibly injured by hyperlipidemia via increase of arginase Ⅱ.Decreased amylase and lipase may be the protection mechanism of pancreas.
8.Expression of c -Met in gastrointestinal stromal tumor and its clinical significance

Objective To study the expression of c -Met protein in gastrointestinal stromal tumor(GIST) and to evaluate its clinicopathological significance.Methods The immunohistochemical technique,EnVision method was used to evaluate the expression of c -Met in 105 cases of GISTs.Results c -Met protein positive rate in GISTs was 9.52%(10 /105),its expression rate was significantly higher in GISTs with >10 /50HPF mitotic activity(P <0.05).c -Met protein expression rate was higher in GISTs which >5cm,moderate or high -risk,or mass located outside the stomach,but the difference was not statistically significant(P >0.05 ).Conclusion c -Met protein expression may related with risk of GISTs.We think that c -Met is worthy of further study for its potential usage as a evaluation indicators of GISTs clinicobiological behavior.
9.Preparation of aliginic sodium diester liposome and measurement of its size distribution

Objective Aliginic sodium diester(ASD) liposome was prepared and its aggregation stability was evaluated.Methods ASD liposome was made by reverse phase evaporating method from egg yolk lecithin,compared with blank liposome,the size distributions and mean particle diameters of ASD liposome and blank liposome were measured respectively before and after 40 hours heating under 37℃.Results Under the condition of n(PC)∶n(CHOL) = 1∶1,the mean size diameter of ASD liposome was 4.24?m.The mean size diameter of ASD liposome changed only 0.26?m after cultivation,however,the value of blank liposome was 1.35?m.Conclusion ASD loaded in the liposome enhanced the physical stability of the liposome.
10.Effects of ischemic preconditioning on the expression of extracellular signal-regulated protein kinase and c-jun N-terminal kinase in hippocampus in a gerbil model of cerebral ischemia-reperfusion injury

Objective To investigate the effects of ischemic preconditioning (IP) on the expression of extracellular signal-regulated protein kinase (ERK) and c-jun N-terminal kinase (JNK) in hippocampus in a gerbil model of ischemia-reperfusion (I/R) injury and the role of ERK and JNK in the mechanism of ischemic cerebral preconditioning. Methods Gerbils of both sexes weighing 50-70kg were randomly divided into 4 groups : ( Ⅰ ) sham operation group; ( Ⅱ ) IP group; (Ⅲ) I/R group and ( Ⅳ ) IP + I/R group. Cerebral ischemia was produced by occlusion of bilateral common carotid arteries and confirmed by isoelectricity on EEG. In sham operation group bilateral common carotid arteries were exposed but not occluded. In IP and I/R groups the animals were subjected to 3 min (IP group) or 5 min (I/R group) cerebral ischemia respectively. In IP + I/R group the animals first underwent 3 min cerebral ischemia followed by 24h reperfusion and then were again subjected to 5 min cerebral ischemia. Open field test was performed to evaluate the behavioral deficit 1,3,5 and 7 days after ischemia. The animals were sacrificed at 15 min, 2, 4, 6h and 1, 3, 5, 7 days after ischemia. The brains were immediately removed for detection of apoptosis (TUNEL) and expression of p-ERK and JNK (immuno-histochemistry) in hippocampal CA1 and CA3 regions and microscopic examination. Results Compared with I/R group the behavioral deficit was significantly decreased and the number of living pyramidal neurons was significantly increased and apoptotic neurons significantly decreased in IP + I/R group. No p-ERK expression was detected in CA1 region in all of the 4 groups but in CA3 region the p-ERK expression was significantly higher in group IP + I/R than in group I/R. The p-JNK expression increased gradually during reperfusion in both CA1 and CA3 regions and was still detectable 7 days after ischemia and was significantly lower in CA1 region in group IP + I/R than in group I/R.Conclusion IP protects hippocampal neurons from I/R injury by inhibiting the expression of p-JNK in CA1 region and enhancing the activity of p-ERK in CA3 region
