1.Meta-analysis of lumbar posterolateral fusion versus circumferential fusion in the treatment of the lumbar disease.

OBJECTIVETo evaluate the efficacy of lumbar posterolateral fusion versus circumferential fusion in the treatment of the lumbar disease.
METHODSSearched MEDLINE (January, 1966 to December, 2007), EMBASE (January, 1984 to December, 2007), Cochrane Central Register of Controlled Trial (4th Quarter 2007), The China Biological Medicine Database (1984 to December, 2007), and hand searched several related journals, such as Spine, European Spine Journal, The Journal of Bone and Joint Surgery, Chinese Journal of Surgery, Chinese Journal of Orthopaedics, Chinese Journal of Spine and Spinal Cord, and so on. Searched the reviews, the clinical results and some other related studies on the two fusion techniques, and the quality of included trials was evaluated. Data were extracted by two reviewers independently with a designed extraction form. RevMan 5.0.5.0 software was used for data analysis of the fusion rate, the complication rate, the re-operation rate, the operative blood loss, the clinical outcome, and the operation time.
RESULTSFour randomized clinical trials (RCTs) involving 437 patients were included. The results of Meta-analysis indicated that in the fusion rate [OR 0.47, 95%CI (0.24, 0.94), P = 0.030], the complication rate [OR 0.53, 95%CI (0.32, 0.87), P = 0.010], and the operative blood loss [weighted mean difference (WMD) = -349.95, 95%CI (-561.64, -138.26), P = 0.001], the circumferential fusion group was significantly higher than the posterolateral fusion group. And in the re-operation rate [OR 2.28, 95%CI (1.30, 3.98), P = 0.004] the posterolateral fusion group was significantly higher than the circumferential fusion group. There were no statistically significant differences in the clinical outcome [OR 1.04, 95%CI (0.64, 1.68), P = 0.870] and the operation time [WMD = -90.24, 95%CI (-190.20, 9.71), P = 0.080].
CONCLUSIONSTo compare with the posterolateral fusion, the circumferential fusion can increase the fusion rate and reduce the re-operation rate, but it can also increase the complication rate and the blood loss. More high quality large-scale randomized controlled trials are required.

2.A study on the quality of life for children with chronic cough

Objective To explore the impact of chronic cough on children’s life quality, and to observe their life quality after drugs and psychological intervention. Methods One hundred 9 to 12 years old children with chronic cough were randomly selected. The drugs and psychological intervention were administrated. The children had been follow-up. The children’s quality of life was assessed by“Inventroy of Subjective Life Questionnaire”before and after treatment. Meanwhile 100 healthy children were randomly selected as a control group. Results With the prolonged treatment, the recovery rate and effective rate in children with chronic cough increased. Before the treatment, the scores of family life, peer interaction, self cognitive, experience of depression, experience of anxiety, cognitive and emotional component, and overall satisfaction were significantly lower in children with chronic cough than those in healthy children (P<0.05). After the treatment, the scores of family life, peer interaction, self cognitive, experience of depression, experience of anxiety, physical experience, cognitive and emotional component, and overall satisfaction were significantly improved in children with chronic cough (P<0.05), even the scores of physical experience and emotional component were significantly higher in children with chronic cough than those in healthy children (P<0.05). Conclusions The quality of life in children with chronic cough decline, however, drug and psychological intervention can improve their quality of life.
3.Effect of drought stress on growth and physiological-biochemical characteristics of Stellaria dichotoma.

A pot experiment was conducted to study effect of drought stress on leaf physiological characteristics and growth of one year old Stellaria dichotoma seedlings. The result showed that plant height and shoot dry weight significantly decreased with decrease in soil water content; however, root length and root dry weight increased at light drought stress and decreased at severe drought stress. The result also showed that with the decrease of soil water content, proline content in S. dichotoma leaves decreased then increase, while solube protein content decreased. Activities of SOD and POD in S. dichotoma leaves significantly decreased as soil water content decreased, while activity of CAT significantly decreased at severe drought stress. Membrane permeability in S. dichotoma leaves increased, while MDA content decreased then increased as soil water decreased. These results suggest that S. dichotoma had osmotic stress resistance ability and reactive oxygen scavenging capacity at light drought stress, which caused S. dichotoma growth was no inhibited at a certain extent drought stress.

4. Quality evaluation of Notopterygii Rhizoma et Radix slices by entropy weight and gray relative analysis method

Objective: The entropy weight method and grey relative analysis (GRA) method were used to evaluate the quality of prepared Notopterygii Rhizoma et Radix (NRR) slices. Methods: The content of extracts (water and ethanol), volatile oil, notopterol and isoimperatorin in 42 batches of NRR slices from different areas was determined. Using GRA, the weight of the entropy weight method was used as the resolution coefficient ρ to construct the quality research model of the slices. Results: The relative correlation degree of each evaluation unit sequence was 0.241 1-0.679 5, which indicated that the quality of NRR slices from different producing areas and manufacturers were different. The implementation of standardized management of NRR traceable slices had a relatively high degree of correlation, and the quality ranking was in the front, which indicated that the implementation of decoction slices standardized management was beneficial to the quality control of NRR slices. The top five samples were S16, S18, S17, S34, and S33 based on these two methods. Conclusion: The comprehensive quality evaluation of NRR slices can be effectively evaluated by GRA and entropy weight method. Entropy weight method improves the reliability of GRA method and the scientificity of quality evaluation of NRR slices.
5.The expression and methylation of AXIN2 gene in hepatocellular carcinoma

Objective To investigate AXIN2 mRNA expression level in hepatocellular carcinoma (HCC) , and to analyze the effect of AXIN2 gene methylation status on its mRNA expression and HCC genesis and development. Methods Fifty-three surgical excised HCC specimens and paired adjacent non-cancerous specimens, seven normal liver specimens and five HCC cell lines were collected. The expression of AXIN2 at mRNA level and the methylation status of AXIN2 gene promoter were determined by quantitative PCR. Results The expression of AXIN2 mRNA was lower in HCC tissues (0.1629 + 0.0679) than that in adjacent non-cancerous tissues (0. 4155 + 0. 2330), and there was significant difference (Z= -2. 567, P = 0. 010). The methylation level of AXIN2 gene in HCC and adjacent non-cancerous tissues (39. 77% ±3. 89%, and 36. 92% ±2. 81%) was significantly higher than that in normal liver tissues (7. 38% ±2. 40% , t=-3. 663 ,P = 0. 009;t= -4. 591 ,P = 0. 007).AXIN2 gene was hypermethylated in all five HCC cell lines. There was a negative correlation between AXIN2 mRNA expression level and the degree of methylation ( r = -0. 458, P = 0. 032). The methylation level was higher in TNM Ⅲ patients of HCC than that in TNM Ⅰ and Ⅱ patients (P =0.008). Conclusion The down-regulation of AXIN2 gene mRNA expression is correlated with its hypermethylation status. The low expression of AXIN2 mRNA and the abnormal methylation of promoter may be one of the important mechanism of HCC genesis and development.
6.Rheological properties of sucrose acetate isobutyrate in situ gel.

To study the rheological properties of sucrose acetate isobutyrate (SAIB) in situ gel and the influencing factors. Measurements of shear stress and viscosity were carried out at different shear rate. The rheological properties of SAIB solution were similar to those of Newtonian fluid. The factors such as the type of solvent, concentration, additive, drug and temperature had effect on the rheological properties. Ethanol was a suitable solvent compared with ethyl lactate and N-methylpyrrolidone (NMP). The solution viscosity of SAIB was reduced from 1.29 to 0.11 Pa x s with only increasing the content of ethanol from 10% to 20%. Polylactic acid (PLA) and risperidone could increase the intermolecular force and viscosity. However, adding 10% (w/w) PLA, the initial release of risperidone was reduced from 20.2% to 3.5%. The solution viscosity reduced significantly by stepping up the temperature. The results obtained support the using of SAIB is satisfactorily injectable in situ gel formulation.

7.Effects of purine nucleotide on the expressions of FSH and LH and the ultrastructure of endocrine cells in the pituitary gland of heroin-addicted male rats.

OBJECTIVETo investigate the effects of purine nucleotide on the expressions of follicle-stimulating hormone (FSH) and luteotrophic hormone (LH) and the ultrastructures of the distal somatotrophic and gonadotrophic cells in the pituitary gland of heroin-addicted and -withdrawal rats.
METHODSNinety-two male Wistar rats were randomly divided into a control group (ip saline for 14 d), a nucleotide group (ip AMP and GMP for 10 d), a heroin group (ip heroin for 10 d), a heroin + nucleotide group (ip AMP and GMP + heroin for 10 d), a 3 d withdrawal group (ip heroin for 10 d and killed at 14 d), a 9 d withdrawal group (ip heroin for 10 d and killed at 20 d), a 3 d nucleotide group (ip nucleotide for 3 d after 10 d heroin administration and killed at 14 d), and a 9 d nucleotide group (ip nucleotide for 9 d after 10 d heroin administration and killed at 20 d). Changes in the mRNA expressions of FSH and LH in the pituitary gland of the rats were analyzed by semi-quantitative RT-PCR, and alterations in the ultrastructures of the distal somatotrophic and gonadotrophic cells were observed under the microscope.
RESULTSThe expression of FSH mRNA was significantly increased in the nucleotide, heroin + nucleotide, 3 d nucleotide and 9 d nucleotide groups (0.099 +/- 0.018, 0.177 +/- 0.046, 0.151 +/- 0.030 and 0.184 +/- 0.028) as compared with the control group (0.045 +/- 0.009) (P < 0.01); and so was that of LH mRNA in the heroin + nucleotide, 3 d nucleotide and 9 d nucleotide groups (0.950 +/- 0.169, 0.990 +/- 0.171 and 0.960 +/- 0.147) in comparison with the control group (0.700 +/- 0.099) (P < 0.01). In the heroin group, the nuclei of the distal somatotrophic and gonadotrophic cells exhibited morphological abnormality, unclear membrane, slightly pyknotic matrix, marginal and agglutinated heterochromatin, dilated rough endoplasmic reticula, swollen mitochondria, broken and vacuolated cristae in the cytoplasm, obviously decreased number of secretory granules, and myelin bodies in some cells. However, the heroin + nucleotide group showed no significant changes in the ultrastructures of somatotrophic and gonadotrophic cells compared with the control group.
CONCLUSIONShort-term use of heroin does not obviously affect the expressions of FSH and LH mRNA in the pituitary gland of rats, while heroin + nucleotide, or nucleotide following heroin withdrawal can enhance their expressions significantly. Heroin damages the ultrastructures of the distal somatotrophic and gonadotrophic cells in the pituitary gland of male rats, and purine nucleotide can diminish or inhibit this damage.

8.Preliminary investigation of coblation for early glottic carcinoma.

OBJECTIVETo establish whether Coblation is a suitable modality for removal of early glottic carcinoma.
METHODSFourteen patients with early glottic carcinoma (Tis-T2) without lymph node metastasis underwent resection of laryngeal cancer lesions using transoral endoscopic coblation (TEC), without pre- or post-operative radiotherapy and chemotherapy.
RESULTSNo severe complication such as bleeding and dyspnea occurred in the cases. Only mild postoperative pain happened to the patients. All patients could eat and pronounce on the surgery day. With following-up of 25 - 37 months, no recurrence was observed in 13 cases. One case, with poorly differentiated squamous cell lesion in the anterior commissure invading subglottic, recurred 3 months postoperatively. The patient received the re-resection of laryngeal lesion by coblation, but another recurrence happened to the patient 6 months postoperatively, and then was cured by partial laryngectomy, with recurrence-free survival 21 months postoperatively.
CONCLUSIONThe observation suggests that transoral endoscopic coblation is a reliable and safe modality for the resection of early glottic carcinoma.

9.Study on the relationship between collagen type Ⅰ alpha 2, osteocalcin genes polymorphisms, serum calciotropic hormone levels and dental fluorosis

Objectives To investigate the relationship between fluorosis polymorphisms in collagen type Ⅰ alpha 2 (COL1A2) and osteocalcin (OC) gene, and serum calciotropic hormone levels. Methods The children between 8 and 12 years of age in Kaifeng and Tongxu cities of Henan Province were chosen to be the object of observation. Accoding to situation of dental fluorosis, they were divided into three groups: dental fluorosis group, non-dental fluorosis group from high fluoride areas, and control group form the control areas. The Pvu Ⅱ and Rsa Ⅰ markers of COL1A2 gene as well as HindⅢ marker of OC gene were genotyped by PCR-RFLP procedure. Calcitonin and osteocalcin levels in serum were measured using radioimmunassays. Results The frequency distribution of COL1A2 PvuⅡ genotype was pp 49.3%(37/75), Pp 32.0%(24/75), PP 18.7%(14/75) in children with fluorosis; pp 43.5% (30/69), Pp 52.2% (36/69), PP 4.3%(3/69) in children without fluorosis from high fluoride areas; and pp 43.8% (42/96), Pp 40.6% (39/96), PP 15.6% (15/96) in the children without fluorosis from control areas respectively. Childrens with the homozygous genotype PP of COL1A2 Pvu Ⅱ had a significantly increased risk of dental fluorosis(OR=4.85, 95%CI: 1.22-19.32) compared to children with the homozygous genotype pp in anendemic fluorosis area. The frequency distribution of COLIA2 Rsa Ⅰ genotype was rr 50.7% (38/75), Rr 36.0% (27/75), RR 13.3%(10/75) in children with fluorosis; rr 46.4%(32/69), Rr 46.4%(32/69), RR 7.2%(5/69) in children without fluorosis from high fluoride areas, and rr 45.8% (44/96), Rr 45.8% (44/96), RR 8.3% (8/96) in the children without fluorosis from control areas respectively. There were no significant differences in the three groups (P>0.05). The frequency distribution of OC Hind Ⅲ genotype was hh 48.0% (36/75), Hh 34.7% (26/75), HH 17.3% (13/75) in children with fluorosis; hh 43.5% (30/69), Hh 43.5% (30/69), HH 13.0% (9/69) in children without fluorosis from high fluoride areas, and hh 47.9%(46/96), Hh 40.6%(39/96), HH 11.5%(11/96) in children without fluorosis from control areas respectively. There were no significant differences in the three groups (P>0.05). Additionally, fluoride levels in urine and OC levels inserum were found to be significantly lower in controls from non-endemic areas compared to cases(P<0.05). However, the differences in urine fluoride and serum OC levels were not observed when cases were compared to controls from high fluoride areas(P>0.05). Conehlsions This study provides the evidence of an association between polymorphisms in the COL1A2 gene with dental fluorosis in populations exposed to high fluoride. There were no correlation between OC Hind Ⅲ genotype and the dental fluorosis.
10.Children's dental fluorosis and estrogen receptor α gene Xba I polymorphism

Objectives To explore the relationship between polymorphism in estrogen receptor alpha (ERα)gene Xba I and child dental fluorosis.Methods Qiulou township of Kaifeng and Sunying township of Tongxu counties of Henan province were chosen as the investigation spots in 2006.An area of water drinking endemic fluorosis(high fluoride area)and a non-endemic area(control area)were chosen in every spot,where dental fluorosis of children aged 8 to 12 years old were examined and diagnosed by using the Dean method.The children in the high fluoride areas were divided into dental fluorosis group and control group of the endemic areas according to dental fluorosis status,and the children in the control areas as control gruop of non-endemic areas.The Xba I polymorphism in the ERα gene was genotyped using the PCR-RFLP procedure.The fluoride levels in the urine samples from the three groups were detected by fluoride ion selective electrode and over standard rate of the urine was counted.Results The prevalence rate of dental fluorosis in high fluoride areas was 51.7%(74/143)and the community fluorosis index was 1.310.No dental fluorosis case was checked out in the control and the community fluorosis index was 0.021.The over standard rate of urine fluoride in dental fluorosis group[84.6%(121/143)]was significantly higher than that of control in non-endemic area[9.6%(9/94);χ2=125.95,P<0.01].The frequency distribution of ERα Xba I genotype was XX 6.8%(5/74),xx 36.5%(27/74),xx 56.8%(42/74)in dental fluorosis group;XX 15.9%(11/69),Xx 37.7%(26/69),xx 46.4%(32/69)in the eontrol of the endemic areas;XX 14.9%(14/94),Xx 43.6%(41/94),xx 41.5%(39/94)in children from the control in non-endemic area,respectively.No significant difference was found among the three groups(χ2= 3.450, P > 0.05). Allele frequency of ERα Xba I genotypes was X 22.7%(30/132), x 77.3%(102/132) in dental fluorosis group and X 35.5%(39/110),x 64.5% (71/110) in the control in endemic area when urine fluorosis of children was exceeding standard and significant difference was found in this two groups(χ2 = 4.768, P < 0.05; OR = 0.535,95% CI:0.305 - 0.941). Conclusion Children who carried X allele frequency of ERα Xba I genotypes have a lower risk of dental fluorosis when children with high-loaded fluoride status.
