1.Investigation of operation personnel hand hygiene cognition and compliance in operation room

Objective To investigate the realistic status of hand hygiene cognition and execution rate among the operation-room personnel,in order to provide the corresponding integrated manage strategy for surgical site infection reduction.Methods Investigation of hand hygiene cognition and execution rate were performed among the 114 operation-room personnel with 46 doctors and 68 nurses.Results Hand hygiene cognition scores and execution rate of circuit nurses respectively were (78.43 ± 7.71) and 71.39% (4 183/5 839),meanwhile (62.44 ± 9.82) and 47.81% (2 395/5 009) of the surgeon and the anesthesiologist,which emerged significant diversity between these two group (P < 0.01).Conclusions Integrated hand management should be performed to insure the hand hygiene compliance,resulting in minimizing the surgical infection.
2.Relationship between work-family balance and depression among employees in organization: A cross-lagged regression analysis

Objective:To investigate the relationship between employees'work-family balance (WFB) and depression.Methods:Totally 259 employees from 4 departments in a petrochemical enterprise were assessed with the Work-Family Balance Scale (WFBS) and Patient Health Questionnaires-9 (PHQ-9) and were conducted a 4-months follow-up study via cross-lagged regression.Results:There were significantly negative correlation between the scores of WFBS and PHQ-9 (r =-0.12--0.22,Ps < 0.05).Cross-lagged regression revealed that WFBS score at time 1 was not a predictor of PHQ-9 at time 2 (β =0.04,P > 0.05),and PHQ-9 score at time 1 was a negative predictor of WFBS score at time 2 (β =-0.13,P < 0.05).Conclusion:There may be a close relationship between work-family balance and depression,and depression could predict work-family balance among employees in organization.
3.The prognostic significance of dynamic monitoring of minimal residual disease (MRD) status in patients with newly-diagnosed multiple myeloma.

Objective: To evaluate the prognostic value of kinetic changes in minimal residual disease (MRD) status, as well as its relationship with risk stratification, therapeutic response and treatment in patients with newly-diagnosed multiple myeloma (MM) . Methods: A total of 135 patients with newly-diagnosed MM were screened, and 105 patients who achieved VGPR or more as the best responses were included into this study. The MRD status was determined by multiparameter flow cytometry (MFC) at multiple intervals after two cycles of treatment until clinical relapse, death, or last follow-up. The statistical methods included Kaplan-Meier analysis, Cox regression, etc. Results: ①In all 135 patients, 57.8% (78/135) patients achieved MRD negativity (MRD(-)) after treatment. In 105 patients who achieved VGPR and thus included in this study, the MRD(-) rate was 72.4% (76/105) , with a median interval of 3 months from starting treatment to achievement of MRD(-) status. ②The 2-year PFS rate of patients with MRD(-) status was significantly higher than that of MRD(+) status (62.2% vs 41.3%, P=0.001) , while MRD persistence (MRD(+)) was an independent factor for poor prognosis (multivariate analysis for PFS: P=0.044, HR=3.039, 95%CI 1.029-8.974) . ③Loss of MRD(-) status (i.e., MRD reappearance) showed inferior outcomes compared with MRD sustained negative ones, the PFS was 18 months versus not reach (P<0.001) and the OS was not reach for both (P=0.002) . ④The 2-year PFS and OS rates of patients with duration of MRD(-)status≥12 months were significantly higher than those of the control group (PFS: 77.7% vs 36.7%, P<0.001; OS: 96.4% vs 57.9%, P<0.001 respectively) . Duration of MRD(-) status was associated with a marked reduction in risk of relapse or death (univariate analysis for PFS: P<0.001, HR=0.865, 95%CI 0.815-0.918; for OS: P=0.001, HR=0.850, 95%CI 0.741-0.915 respectively) . ⑤Moreover, even in patients carrying high-risk cytogenetic abnormalities (CA) or ineligible for ASCT, MRD negativity remained its prognostic value to predict PFS (high-risk CA medianPFS: not reach vs 19 months, P=0.006; ineligible for ASCT medianPFS: not reach vs 25 months, P=0.052 respectively) . ⑥Last, treatment with the bortezomib-based regimens contributed to prolonged MRD(-) duration (median MRD(-) duratio: 25 months vs 10 months, P=0.034) . Conclusion: Our findings supported MRD(+) status as an independent poor prognostic factor in MM patients, which implicated that duration of MRD(-) status also played a significant role in evaluation of prognosis, while loss of MRD(-)status might serve as an early biomarker for relapse. Therefore, monitoring of MRD kinetics might more precisely predict prognosis, as well as guide treatment decision, especially for when to start retreatment in relapsed patients.

Objective To compare the quality of Angelica ultrafine grinding and Angelica pieces.Methods Methods of extraction and chromatographic is more optimized than method in Pharmacopoeia 2010.The method of ultrasonic extraction was used for 30 min by 70%metha-nol.High performance liquid chromatography ( HPLC ) method for the chromatographic conditions: Column: Kromasil C18 column (4.6 mm × 250 mm,5μm) , flow rate:1.0 mL· min -1 , detection wavelength:316 nm, column temperature:25 ℃, mobile phase:acetonitrile-water gra-dient 0.05% phosphoric acid system.Results The content of ferulic acid of water -soluble components in Angelica was higher than that of Angelica pieces.Conclusion Ultrafine grinding Angelica can improve the dissolution of the water -soluble components of ferulic acid.
5.Application of centipede oil with hydrocolloid paste in high-risk drugs extravasation

Objective To explore the effects and clinical nursing of combination of centipede oil with hydrocolloid paste in high-risk drug extravasation.Methods 90 patients with high-risk drug extravasation were randomly divided into control group 1,control group 2 and experimental group.Control groups 1 and 2 received centipede oil only and hydrocolloid paste only respectively.The experimental group was given both centipede oil and hydrocolloid paste.The medicine in each group was all applied to sore,blister,scleroma,erythema and hematoma parts of the patients.The effects of the treatment were compared.Results No significant difference was found between control group 1 and control group 2 (P > 0.05).However,the effectiveness of experimental group was 96.7% and was significantly higher than that of control groups 1 (73.3%) and 2 (70.0%),the differences were statistically significant (x2 =6.41,7.68,respectively; P < 0.05).Conclusions Combination of centipede oil with hydrocolloid paste in high-risk drugs extravasation treatment was effective for tissue repair.
6.Compliance of lung cancer screening with low-dose computed tomography and influencing factors in urban area of Henan province

Objective:To evaluate the compliance of low-dose computed tomography (LDCT) screening for high-risk groups of lung cancer and influencing factors in urban area of Henan province during 2013-2017.Methods:Cluster sampling method was used to select the residents of 40-74 years old in Henan for cancer risk factor investigation and lung cancer risk assessment. Subjects with high risk of lung cancer received LDCT screening. The differences of LDCT receiving rates between groups were compared with χ2 tests, and the time trend of rates were tested with the Cochran- Armitage trend test. The potential factors correlating to the compliance of LDCT screening were identified with multivariate logistic regression models. Results:Overall, 35 672 participants who met the inclusion criteria were included in this analysis, and 13 383 of them received LDCT screening, the receiving rate was 37.52%. The receiving rate varied greatly across cities, ranging from 38.47% to 26.73% ( P<0.05). Moreover, the receiving rate varied greatly across periods, ranging from 29.22% during 2013-2014 to 43.30% during 2014-2015, and the receiving rate increases gradually as the screening year increases ( P<0.001). The multivariate logistic regression analyses showed that: being female, age 45-69 years, with education level of junior high school/high school, previous smoking, drinking or previous drinking, infrequent physical exercise, history of tuberculosis, history of chronic bronchitis, history of emphysema, history of asthma bronchiectasis and family history of lung cancer were positive factors for receiving LDCT screening (All P<0.05). Conclusions:The overall compliance of LDCT screening in high-risk population of lung cancer was still not high in urban area of Henan. Implementation of effective interventions targeting the specific high-risk populations might improve the overall compliance of LDCT screening in the future.
7. Effect of HR-HPV positive,load and SCC-Ag positive on recurrence rate after radical resection for cervical cancer

OBJECTIVE: To investigate the effect of HR-HPV positive,HR-HPV load and SCC-Ag positive on the recurrence of cervical cancer after radical resection.METHODS: The clinical data of cervical cancer patients who underwent radical resection of cervical cancer in People's Hospital of Zengcheng District from January 2010 to January2019 were retrospectively collected.The patients were followed up regularly,and the preoperative HR-HPV positive,loading,the amount of SCC-Ag expression were determined;the recurrence and metastasis of cervical cancer were analyzed,and the value of HR-HPV positive,HR-HPV loading and SCC-Ag in predicting the recurrence and metastasis of cervical cancer were analyzed.RESULTS: A total of 438 patients with cervical cancer were included.There was no significant difference in pathological type,or postoperative Meigs-Brunschwig pathological staging between the recurrence group(n=42)and the non-recurrence group(n=396)(P>0.05).The difference in the proportion of HRHPV positive(40/42 vs. 144/396),HR-HPV loading and SCC-Ag positive(34/42 vs. 64/396)was statistically significant between non-recurrence group and recurrence group(P<0.05).The difference in HR-HPV positive in pelvic recurrence and distant metastasis was statistically significant(P<0.05).There was no significant difference in HR-HPV load or SCC-Ag in pelvic recurrence and distant metastasis(P>0.05).Multivariate logistic regression analysis showed that distant metastasis,FIGO staging of cervical cancer,HR-HPV positive,and SCC-Ag were independent factors affecting cervical cancer recurrence(P<0.05).When predicting by individual indicator,the specificity and positive predictive value of HR-HPV positive for predicting cervical cancer recurrence were 99.18% and 95.23% at the highest,and the negative predictive value of HR-HPV was87.37% at the highest.When SCC-Ag was used to predict cervical cancer recurrence,the sensitivity was up to 33.33%.The sensitivity of combined prediction was 64.51%,the specificity was 99.46%,the positive predictive value was97.41%,and the negative predictive value was 94.44%.CONCLUSION: Distant metastasis,FIGO staging,HR-HPV positive,and SCC-Ag are independent factors affecting cervical cancer recurrence.The combination of HR-HPV positive,HR-HPV loading and SCC-Ag has certain value for predicting recurrence of cervical cancer,and the prediction value is the highest.
8.Clinical Anslysis of Primary Adrenal NK/T-Cell Lymphoma.

OBJECTIVE:
To investigate the clinical characteristics, diagnosis, and treatment of one patient with primary adrenal natural killer/T-cell lymphoma (PANKTCL), and to strengthen the understanding of this rare type of lymphoma.
METHODS:
The clinical manifestations, diagnosis and treatment process, and prognosis of the patient admitted in our hospital were retrospectively analyzed.
RESULTS:
Combined with pathology, imaging, bone marrow examination, etc, the patient was diagnosed with PANKTCL (CA stage, stage II; PINK-E score 3, high-risk group). Six cycles of "P-GemOx+VP-16" regimen(gemcitabine 1 g/m2 d1 + oxaliplatin 100 mg/m2 d 1 + etoposide 60 mg/m2 d 2-4 + polyethylene glycol conjugated asparaginase 3 750 IU d 5) was performed, and complete response was assessed in 4 cycles. Maintenance therapy with sintilimab was administered after the completion of chemotherapy. Eight months after the complete response, the patient experienced disease recurrence and underwent a total of four courses of chemotherapy, during which hemophagocytic syndrome occurred. The patient died of disease progression 1 month later.
CONCLUSION
PANKTCL is rare, relapses easily, and has a worse prognosis. The choice of the "P-GemOx+VP-16" regimen combined with sintilimab help to improve the survival prognosis of patient with non-upper aerodigestive tract natural killer /T-cell lymphoma.

9.Clinical Efficacy of Modified Qingyitang Combined with Blood Purification on Severe Acute Pancreatitis Complicated with Multiple Organ Dysfunction Syndrome

Objective::To explore the effect of modified Qingyitang combined with continuous blood purification in the adjuvant treatment of severe acute pancreatitis (SAP) complicated with multiple organ dysfunction (MODS) caused by heat accumulation of viscera. Method::Totally 100 cases of patients of SAP complicated with MODS, who were diagnosed as heat accumulation of viscera by traditional Chinese medicine(TCM) and treated in ICU of the First Affiliated Hospital of Hunan University of Chinese Medicine during May 2015 and May 2019, were randomly divided into two groups, namely control group and observation group, with 50 cases in each group. The patients in control group were treated with fasting and abstinence, gastrointestinal decompression, inhibition of trypsin secretion, gastric mucosal protection, early jejunal nutrition, reduction of inflammatory reaction, continuous blood purification (CBP), mechanical ventilation and circulatory support. The patients in observation group were treated by nasojejunal tube according to syndrome differentiation in addition to routine comprehensive therapy. Modified Qingyitang was injected for 7 days. The remission time of abdominal pain and distention, the time of first exhaust and defecation, the time of ICU residence, the number of samples falling off, the cause of death and the number of cases were recorded. Relevant indexes were measured before treatment, on the 3rd and 7th day of treatment, including the evaluation indexes of pancreatitis: blood amylase (AMS), blood lipase (LPS), and modified computed tomography severity index (MCTSI), inflammatory response indexes were interleukin-6 (IL-6) and hypersensitive C-reactive protein (hs-CRP). Dry eye is a multifactorial disease, which seriously affect the work and life of patients. Since the pathogenesis of dry eye has not been clarified so far, there is no standard animal model available now. Non-Sjögren syndrome dry eye is one of the main types of dry eye, which requires further research. Studies on dry eye mechanisms have led to the development and improvement of animal models. In this paper, the methods of animal models of non-Sjögren syndrome dry eye, including drug treatments, surgery, exogenous injury, behavioral patterns, changes in diet, are reviewed. In addition,different models are described, the problems are analyzed and perspectives and reflections on future research are offered.
