Objective To determine the serum level of pepsinogenⅠ,Ⅱ(PGⅠ,PGⅡ) and PGⅠ/Ⅱin the residents from Zhuanghe county,a high risk area of gastric cancer in North China, and to explore their distribution as well as related factors. Methods Serum PGⅠand PGⅡlevels were detec- ted with ELISA method in 6990 subjects.Gastric diseases were diagnosed by endoscopy and histopatho- logic examination.Serum H.pylori-IgG antibody was determined by ELISA method. Results The me- dian values for PGⅠ,PGⅡ,PGⅠ/Ⅱwere 86.9/?g/L,10.6/zg/L and 8.1 respectively.Serum PGⅠand PGⅡin male(95.2?g/L,12.1?g/L) were significantly higher than those in female(79.7?g/L, 9.4?g/L;P=0.000),PGⅠ/Ⅱratio(7.9) was significantly lower in male (8.3,P=0.000).There were significantly decrease in PGⅠ/Ⅱratio along with age increase.PGⅠ/Ⅱratio decreased signifi- cantly following with progression of gastric mucosa from normal (10.4) to non-atrophic lesions(8.8) and to atrophic lesions (6.6).Serum PGⅠand PGⅡin H. pylori positive subjects (88.7?g/L,11.4?g/L) were significantly higher than those in H.pylori negative subjects (81.4?g/L,8.4?g/L;P= 0.000),PGⅠ/Ⅱratio(7.7) was significantly lower in H.pylori positive subjects (9.6,P=0.000). For patients with atrophic lesions, the area under the PGⅠ/ⅡROC curve was 0.622.The best cut-off point for PGⅠ/Ⅱwas 6.9,with sensitivity of 53.2%,and specificity of 67.5%.Factors linked to PGⅠ/Ⅱwere identified using multinomial logistic regression:male (OR:1.151,95% CI:1.042—1.272, P=0.006),age=61(OR:1.358,95% CI:1.188—1.553,P=0.000),atrophic lesion(OR:2.075,95% CI:1.870—2.302,P=0.000),and H.pylori infection (OR:1.546,95% CI:1.368—1.748,P= 0.000).Conclusions The serum PG levels are significantly skewed from normal distrubition in the residents of Zhuanghe county, and affected by age and gender,as well as associated with gastric diseases and H.pylori in- fection.Compared with PGⅠand PGⅡalone,PGⅠ/Ⅱis more suitable for screening gastric cancer.

Objective To observe the effects of the Ossatron high-energy shock waves on bone nonunion and delayed union. Methods Between October 2004 and October 2005,18 patients with nonunion and four patients with delayed union were treated with high-energy extracorporeal shock wave therapy(Ossatron,HMT Co., Swiss).Facture lines were located with C-arm X-ray before treatment.Parameters of shock waves were decided according to fracture sites.Patients with unstable fixation received plaster or bracing for 6 to 8 weeks after treatment. After therapy,follow-up radiography was performed at 6 and 12 weeks and monthly afterwards until the fracture united. Results The mean follow-up was 6.7 months(range,3 to 12 months). There were no complications except local petechial hemorrhage and local swelling in 10 patients.Thirteen patients(59.1%)got bony union, including 12 patients(66.7% )with stable internal fixation, and the average healing time was 4.3 months(range,6 weeks to 6 months).Nine fractures(40.9%)still suffered nonunion. Conclusion High-energy shock waves are a good alternative therapy for nonunion and delayed union, with obvious advantages of being non-invasive and safe.

Chinese Journal of Orthopaedic Trauma 2004;0(08):-
Objective To investigate the histological and biochemical responses associated with periprosthetic osteolysis, including the changes of tumor necrosis factor-?(TNF-?),osteoprotegerin(OPG) and os- teoprotegeria ligand (OPEL) in the interface membrane in rat models so as to understand the mechanisms of these interactions. Methods Twenty-four male SD rats were used in this study.A titanium alloy plug was inserted into each distal femur through the knee joint.Polyethylene particles were injected into the left knee at 2,4,6,8 and 10 weeks after implantation of the plugs.Saline solution was injected into the right knee as the control. The animals were killed 12 weeks after implantation. The distal femur was taken from each knee to make histological sections. The samples were made decalcified and stained with hematoxylin and eosin. The histopathological changes were observed under a light microscope. The concentrations of TNF-?in the membrane were assayed by a ELISA kit and the mRNA expressions of OPG and OPEL extracted from the tissue around the plug were detected by RT-PCR. Results The interface membrane taken from the experimental side was found to contain extensive fibrous tissues which extended deeply into the surrounding hone.TNF-?concentrations in the membrane of the experimental side were higher than in the control membrane. There was a significant decrease of OPG mRNA and a significant increase of OPGL mRNA in the membrane around the plug at the experimental side compared with the control side.Con- clusions Polyethylene particles can induce inflammatory responses around the titanium alloy plug. The increase of OPGL expression and reduction of OPG expression in membrane at the experimental side may be induced by TNF-?. Such changes might contribute to the differentiation of osteoclasts and result in the periprosthetic osteolysis.

Objective To investigate CT findings in gastric stromal tumors(GST).Methods Both plain and enhanced spiral CT findings in 46 cases with gastric stromal tumor were retrospectively analyzed. In all patients, diagnosis was confirmed with immunohistochemical markers.CT features were retrospectively studied and summarized.Statistical analysis of the shape, growth pattern, necrosis and enhancement patterns was performed with X2 test in 43 cases with single gastric stromal tumor. Results Of the 46 GST patients,43 patients had single GST and multiple GST was detected in 3 cases. In the 43 cases with single GST,tumors were found in the gastric body in 24 cases, gastric fundus in 16 cases, and in the gastric antrum in 3 cases.GST mostly grow along the vertical plane of gastric wall, with a large size but local attachment. The tumor size was less than 5era in diameter in 14 cases.Of them, ten cases had a regular shape,10 cases showed homogeneous enhancement, and 4 cases exhibited central necrosis,7 tumors showed intra-luminal growth and 5 tumors showed extra-luminal growth, while the other 2 cases involved both intra and extra lumina.Twenty-nine cases had tumors larger than 5cm in diameter.Of them,24 cases had irregular shape,27 cases showed inhomogeneous enhancement,24 cases had central necrosis,5 tumors showed intra-luminal growth and 9 tumors showed extra-luminal growth, while 15 cases involved both intra and extra lumina. The tumor size of GST closely was related to the shape, growth pattern, necrosis and the inhomogeneous enhancement patterns of the GST(P<0.05).The enhancement of the tumor was more intense in venous phase and delayed phase.Five cases showed septal enhancement,4 tumors exhibited marked enhancement in arterial phase with up to 60 HU.Conclusions CT can precisely display the location, shape and size of gastric stromal tumors. It is very helpful to provide useful information for early diagnosis and evaluation.

OBJECTIVETo analyze the biologic principle of orthodontic treatment in patients with skeletal Class III malocclusion and open bite.
METHODSEleven pre-adolescent patients with severe skeletal Class III malocclusion and open bite (age range 7 - 9 years old, mean age 8.3 +/- 0.8) were included. All patients were surgical cases but the patients rejected surgery. The treatment methods used were face mask, rapid maxillary expansion occlusal splint and fixed appliance. Lateral cephalometric films were taken before and after treatment. Cephalometric analysis was performed.
RESULTSAfter the treatment SNA changed from (79.0 +/- 1.2) degrees to (81.9 +/- 0.8) degrees (P < 0.01). And the inclination of lower incisors was decreased from (25.6 +/- 2.1) degrees to (20.1 +/- 1.4) degrees when measured to the NB line (P < 0.01). The direction of the facial growth was maintained.
CONCLUSIONSGood orthodontic results could be achieved in patients with skeletal Class III malocclusion and open bite.
