1.Relationships among attachment, parental attachment and parental rearing style of adolescent depression

Objective To explore the relationships among attachment,parental attachment and parental rearing style of adolescent depression.Methods 36 outpatients and hospital patients in Anhui Mental Health Center with depression and their parents were involved as the case group,33 age-,gender-,and education-matched high school students and their parents as the normal controls.The case group and control group were assessed with Experiences in Close Relationships Inventory (ECR) and Parental Rearing Pattern Questionnaire (EMBU).Results ① The score of attachment avoidance and anxiety dimension in the depression group were obviously higher than that of the control group(73.19 ± 16.63 vs 66.25 ± 12.91,62.76 ± 13.90 vs 51.97 ± 13.69,P < 0.01) ;the score of mother's attachment anxiety in the depression group were significantly higher than that of the control group (72.06 ± 14.23 vs 57.42 ± 12.81),and there were significant differences between two groups (P < 0.01).②The attachment avoidance was positively correlated with parental attachment avoidance in the depression group (r =0.534,r=0.488 ; P< 0.01).The attachment anxiety dimension was positively correlated with mother's attachment anxiety in depression group (r =0.532,P < 0.01).Attachment avoidance was negatively correlated with parental warmness(r =-0.406,r =-0.462,P < 0.01),however attachment avoidance had positive correlation with father's punishment and mother's rejection in depression group (r=0.395,r=0.468; P<0.05).Father's attachment avoidance was negatively correlated with father's warmness(r =-0.527,P< 0.01),and mother's attachment avoidance was negatively correlated with mother's warmness (r =-0.491,P < 0.01),which was positively correlated with mother's rejection (r =0.392,P < 0.05).③ Regression analysis showed that mother's warmness rearing style had full mediating effect on the relationship between mother's attachment avoidance and attachment avoidance in depression group,and then mother's refused rearing style partly mediated the relationship between them.But father's warmness didn(t) have mediating effect on the relationship between father's attachment avoidance and attachment avoidance in depression group.Conclusion The attachment pattern is insecure in the depression group.Parenting attachment and rearing style significantly influence on adolescent depression attachment patterns,and mother's rearing style has mediating effect on the relationship between mother's attachment and depression attachment.
2.Pathological changes of diabetic rat thyroid ultrastructure and intervention effects of insulin and aminoguanidine

Objective To investigate the thyroid ultrastructural pathological changes of diabetes mellitus (DM) rats as well as the intervention effects of insulin and aminoguanidine. Methods Totally 87 rats were treated with streptozotocin to establish DM animal models and divided into DM group(n=27),insulin intervention group(n=32) and aminoguanidine intervention group(n=28),25 rats were taken as normal controls. Twelve and 20 weeks after the animal model establishment, animals were sacrificed, thyroid tissue was taken and ultrastructure was observed. Results In the thyroid of DM rats, follicular epithelial cells present as applanate shape, microvilli were depleted, rough endoplasmic reticulum dilated to irregular vesicular. None pinocytotic vacuole and casual primary or secondary lysosome were seen. Follicular cavity was dilated, colloid in the cavity had higher electronic-density. Interstitial edema, capillary base lamian was thickened at different stage. Proteo-substance deposition with granulo-shape, cloud shape or homogeneity appeared. The number of thyroid parafollicular cells increased. But endocrine granule in parafollicular cells was few. When compared with DM group, the thyroid tissue injury of insulin intervention group and aminoguanidine intervention group were lessened to different degree. Conclusion The hypofunctional thyroid follicular cells, large quantity of proteo-substance deposition in the interstitium and increased parafollicular cells of DM rats may be related with hyperglycemia toxicity. Insulin and aminoguanidine treatment have some protection effects.
3.Expression of androgen receptor and embryonic stem cell associated transcript 4 in breast cancer patients with over-expression of human epidermal growth factor receptor 2 and their clinical significances

Objective To investigate the expression and clinical significance of androgen receptor (AR) and embryonic stem cell associated transcripts 4 (NANOG) in breast cancer patients with human epidermal growth factor receptor 2 (HER-2) over-expression, and to analyze its relationship with clinicopathologic features of breast cancer. Methods 143 breast cancer patients with HER-2 over-expression were selected from the screening of 1052 cases of invasive breast cancer according to estrogen receptor (ER), progesterone receptor (PR) and HER-2 status. The protein expression of AR and NANOG was assayed by using immunohistochemistry.The relationship between AR expression and clinicopathological features was analyzed by χ2 test. The correlation between AR expression and NANOG expression was analyzed by Spearman correlation analysis. Results The positive expression of AR was 35.7 % (51/143). The AR expression was not associated with age and menstruation status (both P>0.05), and was associated with tumor size, clinical TNM staging and lymphatic metastasis (all P< 0.05). The positive rate of NANOG were 53.1 %(76/143), and NANOG proteins were negative in adjacent normal breast tissue and benign breast lesions. The positive rate of AR was 27.6%(21/76) in NANOG-positive cases, whereas the positive rate of AR was 44.8%(30/67) in NANOG-negative cases, and the difference was statistically significant (χ2=4.526, P=0.033). There was an inverse correlation between NANOG and AR expressions (r= -0.255, P= 0.002). Conclusion AR and NANOG may be new targets for endocrine therapy and molecular biological therapy.
4.Etiology, diagnosis, and treatment of lower limb arterial occlusion in patients below 45 years old.

OBJECTIVETo evaluate the etiology, diagnosis, and treatment of lower limb arterial occlusion in patients below 45 years old.
METHODSThe clinical data of 58 patients with lower limb arterial occlusion (except for Buerger's disease) below 45 years old admitted to PUMC Hospital from July 1997 to November 2007 were retrospectively analyzed.
RESULTSAmong these 58 patients, 31 patients (53.4%) had confirmed thrombophilic risk factors, including 12 patients had two or more thrombophilic risk factors. Follow-up (ranged 2-43 months, mean 10 months) showed higher rates of revascularization and amputation in patients with thrombophilic risk factors (22.6% and 22.6%, respectively) than in those without thrombophilic risk factors (14.8% and 11.1%, respectively) (P > 0.05). Among patients with two or more thrombophilic risk factors, the rates of revascularization and amputation were 25.0% and 33.3%, respectively.
CONCLUSIONSLower limb arterial occlusion in young adults is often caused by thrombophilic risk factors. Attention should be taken to identify these etiologies before surgical treatment.

5.Identification of marine-derived shell TCM by near infrared spectroscopy.

The identification of five marine-derived shell traditional Chinese medicine (TCM) recorded in the Chinese Pharmacopoeia were studied. Using near infrared technology (NIR) combined with principal component analysis (PCA) methods, Ostreae Concha, Haliotidis Concha, and Margaritifera Concha could be efficiently distinguished from Meretricis Concha together with Arcae Concha. In the first principal components, Ostreae Concha exhibited obvious differences with high loadings in 4 236, 5 263, 7 142 cm(-1) concerning to the contents of CaCO3 and H2O in the samples. Arcae Concha and Meretricis Concha displayed significant differences with others in the second principal components, which can be illustrated by high loadings in 5 000 -4 430 cm(-1) areas. It is indicated that the second principal components might be related to organics which contained NH and CH groups, for example proteins. Meanwhile, our data showed a correlation between the function of these shell TCM and their distribution in the PCA plot. These results suggested that organic components in marine-derived shell TCM could not be neglected for their quality control.

6.Treating iliac artery occlusive disease with subintimal angioplasty.

OBJECTIVETo assess the value of subintimal angioplasty in treating patients with iliac artery occlusive disease.
METHODSWe retrospectively analyzed the clinical data of 13 patients with iliac artery occlusion who received subintimal wire placement followed by percutaneous transluminal angioplasty and stent placement in our hospital from May 2004 to August 2006.
RESULTSInitial success was obtained in all the 13 patients with an improvement in both the symptoms and the mean ankle-brachial index of 0.55 (range: 0.39-0.94, P = 0.004) without any complication. During the follow-up (range: 3-21 months; mean: 12 months), the one-year stent patency rate reached 100%.
CONCLUSIONSubintimal angioplasty is a valuable approach to treat iliac artery occlusion with reliable efficacy and safety.

7.Clinical characteristics of gastric cancer in the elderly patients.

OBJECTIVETo investigate the clinical manifestations,treatment,and prognosis of gastric cancer in the elderly patients.
METHODA total of 252 patients with gastric cancer who admitted to the Oncology Department of Peking Union Medical College Hospital were divided into elderly group (≥ 65 years) and non-elderly group (< 65 years) and the clinical characteristics of these two groups were analyzed and compared.
RESULTSThe elderly accounted for 36.0% of all gastric cancer patients in our department. The proportion of male was significantly higher in elderly group than non-elderly group (male:female = 3.74:1, P=0.020). Abdominal satiety and pain were the most common symptoms,which were significantly lower in elderly group (43.3% vs. 61.7%, P=0.005). However,the frequency of weight loss was significantly higher in the elderly group (15.6% vs. 6.2%, P = 0.015). Significantly more elderly patients with gastric cancer were found the second tumors (12.2% vs. 2.5%, P=0.002). The most common tumor location was cardia (36.7%) in elderly group and antrum (34.6%) in non-elderly group. A small proportion (2.2%) of elderly patients had multi-original lesions, which was not found in non-elderly group. The overall rate of surgery and R0 resection rate were 77.8% and 70.9% respectively, which were similar in both groups. The overall rate of chemotherapy was 98%. The ratio was one third compared with younger patients who received three and more than three lines chemotherapy (3.3% vs. 9.3%), but did not reach statistical difference. More elderly patients chose FOLFOX / XELOX regimen (73.3%) compared with younger arm. The median survival time was 26.5 months in elderly group and 28.0 months in non-elderly group (P=0.835). Subgroup analysis showed that the median survival time of stage 4 gastric cancer was longer in elderly group than in non-elderly group (22.7 months and 16.1 months, respectively; P=0.057), which was marginally statistically significant.
CONCLUSIONSMore old men may get gastric cancer. More elderly patients may present with weight loss. Cardia is the most common tumor location. The ratio of multi-original lesions and secondary tumors is higher for elderly patients. Elderly patients with good performance status can receive surgery and chemotherapy safely. The resection rate is similar between elderly and non-elderly patients. Elderly patients receive more two-drug combination regimens. The overall prognoses are similar between elderly patients and non-elderly patients.

8.Preparation and characterization of Forms A and B of benazepril hydrochloride.

OBJECTIVETo prepare Form A and Form B of benazepril hydrochloride and to compare the differences in spectrums, thermodynamics and crystal structure between two polymorphic forms.
METHODSForm A and Form B of benazepril hydrochloride were characterized by Fourier transform infrared spectroscopy (IR), thermal gravimetric analysis (TG), differential scanning calorimetry (DSC), powder x-ray diffraction (PXRD) and single crystal x-ray diffraction (SCXRD).
RESULTSPreparation method, crystal structure and polymorphic stability of Form A and Form B of benazepril hydrochloride were obtained. Based on the analysis of crystal structure of both polymorphs, Form A belonged to monoclone space group P2(1) with a=7.8655(4)Å, b= 11.7700(6)Å, c= 13.5560(7)Å, β= 102.9470(10)°, V=1223.07 (11)Å(3) and Z=2, while Form B belonged to orthorhombic space group P212121, with a=7.9353(8)Å, b=11.6654(11)Å, c=26.6453(16)Å, V=2466.5(4)Å(3) and Z=4. From the DSC and XRD results, Form B of benazepril hydrochloride could be transformed into Form A after heating treatment.
CONCLUSIONForm A and Form B of benazepril hydrochloride are both anhydrous and displayed different polymorphs due to different molecular configuration. Furthermore, Form A exhibits more stable than Form B at high temperatures.

9.Experimental research on application of hydrodynamic acoustic generator in extraction of Chinese medicine.

OBJECTIVETo study the hydrodynamic ultrasonic energy extraction technique for Chinese Medicine.
METHODThe content of baicalin in Radix Scutellariae extracts and total flavones in Folium Ginkgo extracts were as indexes respectively and measured by high performance liquid chromatography. We investigated the hydrodynamic ultrasonic energy extraction process for the extraction of Radix Scutellariae and Folium Ginkgo respectively, and compared with refluxing method, decoction and ultrasonic extraction. With the content of the extracts as indexes, Compound prescriptions of Traditional Chinese Medicine were extracted by the hydrodynamic ultrasonic extraction and decoction extraction.
RESULTThe content of baicalin of hydrodynamic ultrasonic energy extracts was significant higher than other extracts. The content of Ginkgo leaves flavones in hydrodynamic ultrasonic energy extracts and ultrasonic extracts had no significant difference. Compared with decocting extracts, hydrodynamic ultrasonic energy extracts of TCM compound prescriptions content was higher greatly.
CONCLUSIONThe hydrodynamic ultrasonic energy extraction process showed higher extraction rate of baicalin of radix scutellariae than traditional extraction methods. And it was beneficial to the dissolution of water-soluble constituents in compound prescriptions of TCM. The content of Ginkgo leaves flavones in the hydrodynamic ultrasonic energy extracts and ultrasonic extracts had no obvious difference.

SOAP is widely used in various aspects of medicine, and there are still some problems in the course of TCM. The use of SOAP model for teaching and assessment has important practical significance and value in the course of Chinese medicine internal medicine practice, and SOAP emphasize the practice of the post competence oriented teaching, which is worth promoting.
