1.Treatment and correlation factors of aqueous misdirection during surgery on glaucoma patients

Objective To investigate the emergency treatment and correlation factors of aqueous misdirection during glaucoma and cataract surgery in glaucoma patients.Design Retrospective case series.Participants 126 cases(132 eyes)of angle-closure glaucoma patients with aqueous misdirection during surgery,100 cases(100 eyes)of angle-closure glaucoma patients without aqueous misdirection during surgery and 100 cases(100 eyes)of senile cataract patients.Methods We performed pars plana vitreous water-bag aspiration or anterior vitrectomy on glaucoma patients with aqueous misdirection during surgery.All the factors in observation group and control groups were compared and analyzed by logistic regression.Main Outcome Measures Preoperative highest intraocular pressure(HIOP),intraocular pressure after maximum dose(IOP),anterior chamber depth(ACD),lens thickness(LENS),axial length (AXL),mean keratometry(K),intraocular lens diopter within-0.5D corrected by SRK-T formula(IOLD).Results Among all the 132 angle-closure glaucoma eyes whose posterior chamber pressures were increased during surgery,we performed pars plana vitreous water-bag aspiration in 123 eyes(93.18%),and anterior vitrectomy in 9 eyes(6.8%).Postoperative complications included shallow anterior chamber in 20 eyes(7 eyes with malignant glaucoma,9 eyes with choroidal detachment,4 eyes with delayed local suprachorodal hemorrhage),and vitreous local hemorrhage in 1 eye.Means of all the observation indexes on three groups showed significant difference(all P
2.Comparison of the streamlined liner pharynx airway and endotreacheal intubation in the radic mastectomy under general anesthesia

ObjectiveTo compare the effects of streamlined liner pharynx airway(SLIPA) and endotracheal intubation in the radical mastectomy under general anesthesia,and to evaluate the efficacy and safety.MethodsSixty patients scheduled for radical mastectomy were randomly divided into 2 groups.SLIPA group( S,n =30 ) and endotracheal intubation group(T,n =30).The data were kept regarding the insertion time and successful rate,MAP,HR,BIS were recorded at 6 time points:before induction ( T0 ),during SLIPA incursion or trachel intubation ( T1 ),3min after intubation( T2 ),before extubation( T3 ),after extubation ( T4 ),3 min after extubation ( Ts ).Ppeak,PetCO2 and SpO2 were measured after successful airway placement at 10min,30min,60min and to observe intraoperative and postoperative throat complications.ResultsThe time of endotracheal intubation group is significantly longer than SLIPA group (P <0.05).In group T,MAP and HR at T1,T2,T4,T5 were higer than at T0 and the same time points in group S(all P < 0.05 ).No regurgitation and aspiration was found in two groups ( all P > 0.05 ).The incidence of complicaton in group T is significantly higher than that in group S ( all P < 0.05 ).ConclusionSLIPA is easier placement,safety and efficacy when it is used for the radical mastectomy under general anesthesia without significant influences on hemodynamics and fewer complications of throat after operation.
3.Initial stress distribution of the maxillary anterior teeth, periodontal ligament and alveolar bone by different intruding loadings A three-dimensional finite element analysis

BACKGROUND: It is very important to explore an optimal loading force that can both allow effective intrusion and cause no damage to the teeth.OBJECTIVE: To investigate the optimal loading conditions, including loading force values and directions, in the intruding mechanics of maxillary anterior teeth.METHODS: A three-dimensional finite element model (3D FEM) of the maxillary anterior teeth was constructed to include the periodontal ligament (PDL), alveolar bone, and appliance (brackets and wire). The initial stress distribution and force of the maxillary anterior teeth were analyzed when the 3D FEM was loaded with different intruding force values and directions. RESULTS AND CONCLUSION: The optimal value of the intruding force for 6 anterior teeth fell within the range from 0.5 N to 1.0 N. The maximum stress was detected at the distal cervical level of the lateral incisor, and a relatively even distribution of initial stress was observed at 20° palatal deviation to the Y axis. The results suggest that using mini-screw implants at the labial region between the canine and lateral incisor and a combined 0.5-1.0 N intruding force with a distal force could be an optimal intrusion force system by which patients with normal occlusion could obtain a pure intrusion of anterior teeth.

6.Inter-rater Reliability of Myoton-3 Myometer for Assessing Muscle Tone in Healthy Adults

Objective To examine the inter- rater reliablity of Myoton-3 Myometer in assessment of muscle tone in healthy subjects.Methods 20 healthy volunteers were assessed their muscle tone of biceps brachii and flexor carpi radialis muscles in relaxed state with Myoton-3 by 2 testers within 24 h. The frequency of damping oscillations (F value) measured by Myoton-3 was as the characteristic of the muscletone. The intraclass correlation coefficient (ICC) and Bland-Altman analysis were performed. Results The ICC was 0.72~0.88 and 0.79~0.89 in triple scan and ten-time scan pattern, respectively. The Bland-Altman analysis revealed no systematic errors between testers. ConclusionThe Myoton-3 Myometer is reliable between testers for measuring the muscle tone in healthy adults.
7.Change and clinical significance of serum homocysteine acid, interleukin-6 and high sensitive C-reactive protein during acute stroke

ObjectiveTo observe the changes of serum homocysteine acid ( Hcy),interleukin-6 (IL-6) and high sensitive C-reactive protein (hs-CRP)and elucidate the clinical significance in patients with acute stroke. MethodsA total of 126 acute stroke patients were enrolled in the investigation. Based on the MESS,patients were divided into different groups according the severity and prognosis, and 108 physically healthy subjects were selected as controls. The concentration of serum Hcy, IL-6, and hs-CRP were determined in all cases after being attacked at day 3,day 14 and day 28 respectively. Results①The levels of Hcy, IL-6, hs-CRP in patients with acute stroke were significantly higher than controls ( (58.24 ±9.86) μmol/L vs. ( 17.12 ±4.23) μ mol/L, (59.64 ±13.82) ng/L vs. (18.46 ±4.62) ng/L,(19.78 ±6.12) mg/L vs. (2.28 ±0.82) mg/L,all P<0. 01 ). ②The levels of Hcy,IL-6, hs-CRP in patients with mild, moderate and severe acute stroke were significantly higher than controls (P < 0.01 ) at 3 day after the onset; and the Hcy, IL-6, hs-CRP were also significantly lower in mild cases with acute stroke compared with moderate and severe cases (P < 0. 01 ). At day 14 ,the moderate and severe patients had significantly higher levels of Hcy, IL-6, hs-CRP than mild cases and controls (P < 0.01 ). At day 28 the severe patients had significantly higher levels of Hcy, IL-6, hs-CRP than mild and moderate cases and controls (P < 0.01 ) ③At day 28, the levels of Hcy, IL-6, hs-CRP in basically recovered and remarkably improved cases were significantly lower than unrecovered cases [ ( 16.12 ± 4.74) μmol/L, ( 1 8.42 ± 5.02) μ mol/L vs. ( 48.69 ± 7.89)μmol/L; ( 19.52 ± 5.67 ) ng/L, ( 20. 74 ± 6. 13 ) ng/L vs. ( 51.26 ± 11.66 ) ng/L, ( 3.21 ± 1.36) mg/L,(3.24 ± 2.51 ) mg/L vs. ( 8.86 ± 1.32 ) mg/L respectively, all P < 0. 01 ]. ConclusionsThe levels of serum Hcy, IL-6, hs-CRP are significant biomarkers to evaluate the severity and prognosis of acute stroke.
9.Therapeutic Observation of Acupuncture at Myofascial Trigger Points for Cervical Spondylosis of Vertebral Artery Type

Objective To observe the therapeutic efficacy of acupuncture at myofascial trigger points (MTrPs) in treating cervical spondylosis of vertebral artery type. Methods Ninety-eight patients with cervical spondylosis of vertebral artery type were randomized into a treatment group of 51 cases and a control group of 47 cases. The treatment group was intervened by acupuncture at the MTrPs, while the control group was by injection of medical solution for anti-inflammation and analgesia. The positive rate of rotate-cervix test, dizziness score, and visual analogue scale (VAS) of the head and neck pain were compared before and after intervention. Results After intervention, the positive rate, dizziness score, and VAS score were significantly changed in both groups (P<0.05). There were significant differences in comparing the positive rate, dizziness score, and VAS score between the two groups after intervention (P<0.05). The total effective rate was 96.1%in the treatment group versus 76.6%in the control group, and the difference was statistically significant (P<0.05). Conclusion Acupuncture at the MTrPs can improve the dizziness and head-neck pain in patients with cervical spondylosis of vertebral artery type.
10.High-mobility group box protein1 increases tumor necrosis factorαexpression in human bronchial epithelial cells

Objective To investigate the effect of high-mobility group box protein1 (HMGB1) on the expression of TNF-αand its mechanism in 16HBE in vitro. Methods groups with different HMGB1 (0, 100, 500, 2 000 ng/mL) concentration was set; RAGE antagonizing groups were as control, HMGB1-2000ng, anti-RAGE and anti-RAGE+HMGB1. The changes of TNF-αmRNA and secretion were determined by quantitative PCR and ELISA. RAGE protein level was measured by western blotting. Results HMGB1 intervention and TNF-α expression of 16HBE presented a positive dose-dependent relationship. Thechanges of RAGE was HMGB1positively concentration dependent. In comparison with HMGB1 2 000 ng/mL group, anti-RAGE+HMGB1showed a remarkable reduction of TNF-α secretion. Conclusion In vitro, HMGB1 increases TNF-α expression in 16HBE with a dose-dependent manner through RAGE.
