2.Establishment of evaluation index system of medical research project based on Delphi method

Objective To construct a set of evaluation index system for medical research project,to make the work justice and fairness,to manage the research work scientificly and standardized,to promote scientific research and hospital development closely,so as to better serve the society.Methods Based on the literature review,semi structure interview method and current medical research project evaluation index system and related management methods,a preliminary index system was formed.Using the Delphi method to carry out two rounds anonymous questionnaire consulting,the medical research project acceptance evaluation index system was finalized.Results The evaluation index system of medical research projects includes 5 first-level indexes and 21 second-level indexes.Ranking from high to low is based on the weight coefficient of the size.Conclusions Through the establishment of the index system of medical scientific research projects,the project acceptance management work has become scientifically planned,which guide scientific research project productive and improving the scientific management level and technological innovation ability.
3.Effect of Intermittent Low Flow Oxygen Inhalation on Cognitive Function in Patients with Vascular Dementia

Objective To investigate the effect of intermittent low flow oxygen inhalation on cognitive function in patients with vascular dementia. Methods 116 cases of vascular dementia were divided into treatment group (n=58) and control group (n=58). Both groups received the routine drugs, and the treatment group received intermittent low flow oxygen inhalation in addition. They were assessed with the Mini Mental Statement Examination (MMSE) and Activity of Daily Living Scale (ADL) before and 8, 16 weeks after treatment. Results There was not significant difference between groups in scores of MMSE and ADL 8 weeks after treatment, but was 16 weeks after treatment (P<0.01). Conclusion Intermittent low flow oxygen inhalation can improve the cognitive function and quality of life in patients with vascular dementia.

5.The comparative analysis between ultrasound and 64-slice spiral CT examination in acute appendicitis

Objective To investigate the application of ultrasound and 64-slice spiral CT in the diagnosis of acute appendicitis. Methods Sixty-five patients with pathologically or clinally confirmed acute appendicitis were underwent ultrasound and CT examina-tion before operation or conservative treatment.Compared with pathology,the accuracy of CT and B-ultrasound imaging were ana-lyzed.Results Fifty-eight cases were correctly diagnosed by CT and 48 cases were correctly diagnosed by ultrasound,the accuracy was 89.23% (58/65)and 73.85% (48/65),respectively.There was a statistically significant difference between two groups (χ2 =5.1 1,P =0.024).Conclusion The accuracy of multi-slice spiral CT is higher than ultrasound in the diagnosis of acute appendicitis.
6.Effect of "Bushen Huoxue Xiezhuo" decoction on growth of rat glomerular epithelial cells

AIM: To examine the effect of protamine sulfate, lipopolysaccharide(LPS),tumor necrosis factor(TNF), epidermal growth factor(EGF) and " Bushen Huoxue Xiezhuo" (BHX) decoction on the proliferation of extracorporeal cultured rat glomerular epithelial cells (GEC). METHODS: Their action on the proliferation of rat GEC were investigated using the -TdR incorporation. Meanwhile, the serum of rats treated with BHX decoction was extracted pharmacologically and its effects on the growth of GEC were also studied. RESULTS: LPS, protamine sulfate, TNF-? and EGF could significantly inhibit the -TdR incorporation of GEC in a dose- and time-dependent manner. However, this inhibition could be efficiently reversed by the serum containing BHX decoction. CONCLUSION: GEC is one of the main target cells on which BHX decoction act, and the protection on GEC might be one of the mechanisms underlying the role of BHX decoction in preventing the progression of nephrosis.
7.Analysis of the factors affecting ambiguous medical records in DRGs

Objective Analyze the causes of ambiguous medical records to provide evidence for the DRGs.Methods 268 Ambiguous medical records were selected from 17 751 surgery medical records between 2010 and 2014 based on major diagnosis and major surgery.The single factor Chi-square test was used to study the influence of various inpatient characteristics on the occurrence of ambiguous medical records, and those significant variables were given assignment to find influencing factors of ambiguous medical records by using logistic regression analysis.Results Ambiguous medical records account for 1.51% of all surgical medical records.Logistic regression analysis results show that such risk factors as discharging from internal medicine, death of patient, transfer between departments, elderly, long time of hospitalization, and tumor patients, as contributing to ambiguous medical records.All the results are statistically significant (P<0.05), including discharging from which departments (OR=6.595, 95%CI..5.043 ~ 8.625), death of patient (OR=3.787, 95% CI: 2.611 ~ 5.492) and transfer between departments (OR =2.746, 95% CI: 2.061 ~ 3.659), which rank important risk factors for ambiguous medical records.Conclusion Analysis of the causes of ambiguous medical records provides important evidences for the hospital to improve its medical record quality management.
8.Posterior pedicle screw fixation for treatment of fracture and dislocation of atlantoaxial spine

Objective To investigate the clinical effect of pedicle screw fixation in the treatment of fracture and dislocation of atlantoaxial spine via posterior approach .Methods 19 patients with fracture and dislocation of atlantoaxial spine in this hospital from June 2011 to December 2013 were selected and treated with open reduction and pedicle screw fixation via posterior approach . The X‐radiographs were postoperatively re‐examined at regular time for understanding the correction of fracture and dislocation and implant fusion results ,the neurological functions were evaluated according to the Japanese Orthopaedic Association(JOA) scores . Results All cases got bony fusion without the occurrence of internal fixation loosening ,broken screw or broken rod .The JOA score was improved from preoperative (7 .35 ± 2 .39) points to postoperative (13 .21 ± 2 .53) points (P<0 .05) .Conclusion The posteri‐or atlantoaxial pedicle screw fixation and fusion for treating upper cervical spine injury has satisfactory effect .
9.Percutaneous Endoscopic Gastrostomy on Patients with Central Nervous System Injuries

ObjectiveTo explore the percutaneous endoscopic gastrostomy on patients with central nervous system injuries in neurological rehabilitation and to establish enteral nutrition.MethodsThirty-two patients with central nervous system were enrolled for percutaneous gastrostomy, using the Pull technique.ResultsThe successful rate of percutaneous gastrostomy as Pull technique was 100%, 2 cases of stomach bleeding through endoscopy. Straining the fixation gasket and partial compression stoped bleeding. One patient appeared swelling and secretions in abdominal incisions, and recovered after dealing with local iodophor and changing fresh dressing. Hypostatic pneumonia occurred in one patient, and improved after anti-infective treatment. No serious complications occurred.ConclusionPercutaneous gastrostomy is a simple and safe for the patients with central nervous system injuries.
10.Effects of quality of life in patients with intermediate and advanced lung cancer undergoing different treatment regimens

Objective:
To examine the impacts of chemotherapy/radiotherapy, targeted therapy, and combined treatment on the quality of life in patients with intermediate and advanced lung cancer.
Methods:
The patients with intermediate and advanced lung cancer undergoing chemotherapy/radiotherapy, targeted therapy, and combined treatment for the first time were recruited from a tertiary hospital in Weifang City, Shandong Province, using a quota sampling method in September 2023. Basic information was collected using a general information questionnaire, and the quality of life was assessed using the Chinese version of Functional Assessment of Cancer Therapy-General. The investigation started on the 7th day of treatment, and the follow-ups were conducted at 3 and 6 months. The quality of life in patients with different treatment regimens and at different treatment time were compared using repeated measure analysis of variance.
Results:
There were 26 chemotherapy/radiotherapy patients, 32 targeted therapy patients, and 95 combination therapy patients. There were no significant differences in age, gender, place of residence, education level, self-rated economic status, medical insurance, pathological type and disease stage among the three treatment regimens (all P>0.05). The repeated measure analysis of variance showed an interaction effect between time and group among patients receiving the three treatment regimens (P<0.05). The quality of life scores of patients receiving combination therapy decreased with extended treatment time (all P<0.05). The quality of life scores of patients receiving targeted therapy at 3 and 6 months were lower than those treated for 7 days (both P<0.05). No significant differences were observed in quality of life scores among chemotherapy/radiotherapy patients with different treatment durations (all P>0.05). At 3 and 6 months, patients receiving combination therapy had lower quality of life scores compared to those receiving chemotherapy/radiotherapy or targeted therapy (all P<0.05).
Conclusion
The decline in quality of life for patients with intermediate and advanced lung cancer undergoing chemotherapy/radiotherapy and targeted therapy is less than that for patients receiving combined therapy.
