6.The development,application and implications of the Anderson Model in the field of healthcare

The Anderson model was founded in 1968, based on the three initial components of predisposing characteristics,enabling resources and need as its original components.It has been modified and improved for five times since then by increasing measurement index,adjusting structure,expanding path relation,and transforming a-nalysis path,to become a complete theoretical model and the feasibility of empirical research finally.The Anderson Model was widely recognized by the international academia,and used in the field of healthcare services research in U-nited States and Europe as a theoretical model for analyzing the main influence factors of utilization behavior of health service,such as individual medical choice, medical costs, disease screening, drug use etc.This paper aimed to summarize the development and the usage of the model in foreign countries.It will be not only be helpful to the theo-retical and applied research in the field of healthcare in China, but also be helpful to improving the medical and health policy and designing the questionnaire in the National Health Service Survey.
7.Screening of differentially expressed genes in placentas with hepatitis B virus infection by suppression subtractive hybridization technique

Objective To screen differentially expressed genes in placentas with hepatitis B virus (HBV)infection and to discuss the molecular mechanism of HBV intrauterine infection.Methods Thirty placenta tissue specimens from HBsAg and HBV DNA positive pregnant women were used as the study group and 30 placenta tissue specimens from normal pregnant women with HBsAg and HBV DNA negativity were served as the control group.The suppression subtractive hybridization(SSH)technique was used.Total RNAs of placenta tissue of the study group were mixed as the tester,and total RNAs of placenta tissue of the control group were mixed as the driver.A subtractive cDNA library was constructed by PCR-selective cDNA subtraction systems.Amplifications of the library were carried out with E.coil strain DH5? by reverse spot hybridization.RT-PCR confirmed that phosphatidylinositol 3-kinase(PI3K)was up-regulated in placenta tissue with HBV infection.Results Colony PCR showed that the clones contained 200-1000 bp inserts. Thirty five clones were confirmed by reverse spot hybridization and analyzed by sequencing and bioinformatics.Thirty three known genes and 2 genes with unknown function were obtained.RT-PCR preliminarily confirmed that PI3K gene was up-regulated in HBV infected placenta.Conclusions The differentially expressed genes in placentas with hepatitis B virus(HBV)infection using SSH technique has been screened out successfully.These differentially expressed genes encoding proteins participating in cell vital metabolism and malformation,and signal conduction-antiapoptosis pathway.This finding brings some new clues for studying the mechanisms of HBV intrauterine infection.
8.Soluble tumor necrosis factor related apoptosis inducing ligand (sTRAIL) and clinical outcome of HBV infection in human.

OBJECTIVETo explore the role of sTRAIL in the pathogenesis of HBV infection in human being.
METHODSsTRAIL in 153 patients' sera was examined with ELISA. Correlations between sTRAIL and liver functional parameters were analysed.
RESULTSThe levels of sTRAIL in patients with various clinical types of hepatitis B as well as primary hepatocellular carcinoma were all higher than that in normal persons and became almost normal in recovering stage cases. In acute and chronic HBV infection, sTRAIL level was negatively correlated with ALT, AST and total bilirubin levels, and positively correlated with serum albumin.
CONCLUSIONThe results indicated that higher level of sTRAIL expression is correlated with liver damage, and apoptosis induced by sTRAIL is one of the mechanisms of liver damage in HBV infection.

9.Preliminary phenotype analysis of RIG-Ⅰ knockout mice

Objective To explore the biological functions of retinoic acid-inducible gene-I(RIG-I) in vivo through phenotype analysis of RIG-I knockout mice. Methods The gene expression of RIG-Ⅰ in various tissues of mice was examined with Northern blotting and semi-quantitative RT-PCR.The phenotypes observed included body weight measurement,differential count of peripheral blood cells,metabolic parameters measurement and histopathologic examination. ResultsRIG-Ⅰ expressed in various tissues of mice with different levels.No gross developmental abnormalities and expected maturation arrest in granulocytic differentiation were observed in RIG-Ⅰ knockout mice.However,RIG-Ⅰ knockout mice exhibited an unexpected increase in the ratios of neutrophiles to lymphocytes in peripheral blood and increased susceptibility to bacteria infection. Conclusion RIG-Ⅰ may play an important role in immune regulation in mice.
10.Analysis on Death Cases of Oncology Department of the First Hospital of Hunan University of Chinese Medicine During 2007-2016

Objective To analyze the death cases of the oncology inpatients of the First Hospital of Hunan University of Chinese Medicine (hereinafter referred to as "this hospital") during 2007-2016; To provide the basis for the prevention and treatment of cancer, prognosis improvement and rational allocation of TCM health resources. Methods The death cases of the oncology inpatients from 2007 to 2016 were analyzed by retrospective medical history method from the aspects of causes of death, hospitalization costs and survival time. Results There were 830 death cases of the oncology inpatients in the hospital during 2007-2016, with 548 males and 282 females, and the mortality rate decreased year by year. The average hospitalization costs for patients was 39 630.86 yuan, of which TCM treatment cost 6206.98 yuan (15.66%). Patients with colon cancer had the highest proportion of TCM treatment costs (20.10%), followed by lung cancer, breast cancer, and liver cancer. The top five causes of death were lung cancer, liver cancer, breast cancer, colon cancer, and rectal cancer. The sex ratio of patients who died of lung cancer was 1.61:1. The average survival time of male patients was 446.3 days, which was far lower than the 738.3 days of female patients. The cancers with long-term survival periods were cervical cancer, breast cancer, and rectal cancer. The cancers with shorter survival periods were pancreatic cancer, liver cancer, and lung cancer. Conclusion The structure of death causes of the oncology inpatients in the hospital has a certain degree of representation. The prevention and control of lung cancer, especially male lung cancer should be strengthened; TCM health resources and prevention should be utilized rationally; the prognosis of patients with liver cancer should be improved, to improve the overall survival and quality of life of patients with breast cancer or cervical cancer, which is important for the prevention and treatment of malignant tumor.
