1.The exploration of TBL curriculum setting of preclinical theoretic learning in orthodontics postgraduate education

TBL (Team-based learning) model is a new teaching method.By evaluating the effect of TBL model in theory teaching on Chinese orthodontic postgraduate program in the department of Orthodontics,Guanghua School of Stomatology,Sun Yat-sen University,and comparing the traditional teaching model which centered on teachers,we have probed into the application experience of TBL teaching method in curriculum setting of preclinical theoretic learning in orthodontics postgraduate education.The result shows TBL has significantly enhanced students' enthusiasm for learning,sense of collaboration,and has improved the quality of education.TBL method is better,even though there is something that needs to be improved.
2.Effects of lingual orthodontics on function and aesthetics of stomatognathic system

Objective To evaluate the functional and aesthetic disturbance of patients treated with lingual orthodontic surgery and to investigate the key points of laboratory procedure and clinical treatment of lingual orthodontic deformity. Methods A total of 30 cases of Angle class Ⅰ with moder-ate crowding were selected. They were equally divided into two groups: one group was treated using lingual brackets, and the other using conventional labial brackets. Each patient completed a four-part survey right after the bracket placing and 7, 28, 56 days after the treatment. The differences of tongue-lip-cheek soreness, eating and speech difficulties and aesthetic problems were evaluated be-tween two groups. Lateral cephalograms were analyzed at the start of the treatment and the end of the overall treatment. Results Speech difficulties of lingual orthodontics were significantly greater (P<0.01) than that in the labial orthodontics group in the first week of the treatment. Eating difficulties were greater (P<0.01) in the lingual orthodontics group by 28 days, whereas aesthetic problems were greater in group using labial appliance than group lingual appliance. Conclusions In the first two months of the orthodontic treatment, eating and speech difficulties are the most severe problems for the lingual group. Lingual orthodontics offers important aesthetic advantages over labial orthodontics, and it will be one of the development directions of current orthodontics.
3.99Tcm-EHIDA SPECT combined with planar hepatobiliary scintigraphy for the differentiation between infant biliary atresia and neonatal hepatitis

Objective To evaluate the value of 99Tcm-EHIDA SPECT combined with planar hepatobiliary scintigraphy (combined scintigraphy) in the differentiation of infant biliary atresia (BA) from neonatal hepatitis (NH).Methods This is a retrospective study on 32 infants with cholestasis (18 males,14 females;age:28-83 d) from June 2013 to June 2014.All infants underwent sequential 99Tcm-EHIDA hepatobiliary planar scintigraphy and combined scintigraphy,and the diagnostic efficacy of the 2 methods for differentiating infant BA from NH was analyzed by x2 test.Results In sequential planar scintigraphy,the diagnostic sensitivity,specificity,accuracy,positive predictive value and negative predictive value were 76.2% (16/21),6/11,68.8% (22/32),76.2% (16/21) and 6/11,respectively.In combined scintigraphy,the diagnostic parameters were 95.2% (20/21),8/11,87.5% (28/32),87.0% (20/23) and 8/9,respectively.Combined scintigraphy correctly identified 4 BA cases that were misdiagnosed as NH by planar scintigraphy.The sensitivity of combined scintigraphy was significantly higher than that of planar scintigraphy (x2 =4.285,P<0.05),while no significant difference was found in the other diagnostic parameters (x2 =0.061-0.405,all P>0.05).Conclusions Combined scintigraphy has incremental value for the differentiation of infant BA from NH.
4.Antihepatocarcinoma effect of Celecoxib by inhibiting COX-2 and PD-1

Objective:To investigate the role of celecoxib in inhibiting liver cancer and to explore the potential molecular mech-anisms.Methods:A total of 65 cases of patients with primary hepatocellular carcinoma(HCC) underwent surgical treatment in our hospital from February 2012 to June 2016 were recruited as study object.Another 15 cases of normal liver tissue offered by the department of hepatobiliary surgery were selected as control.Immunohistochemical staining(IHC) was used to detect the expression level of COX-2 and PD-1 in tumor and normal tissue samples.Pearson correlation analysis were used to evaluate the correlation between COX-2 and PD-1 in HCC patients.Construction of H22 hepatoma cells bearing BALB/c mice model,randomly divided them into control group,celecoxib group and PD-1 antibody group(15 mice for each group).Sacrificed all mice alive at the end of the 4th weeks after the treatment and removed the tumors then.The tumor growth curves and survival curves were drawn to observe the anti-tumor effect.IHC were used to evaluate the expression of COX-2,PD-1 as well as the number of CD8+T and foxp3 positive Treg cells in tumor tissue.Flow cytometry were used to determine the number of CD8+T and CD4+CD25+Treg cells in peripheral blood.The peripheral blood mononuclear cell(PBMC) of BALB/c mice were separated and scramble or PD-1 siRNA were transfected then,Western blot analysis were used to detected the level of COX-2,PD-1,CD8 and Foxp3 after the treatment of celecoxib.Results:IHC results showed that the expression level of COX-2 and PD-1 in tumor tissue of HCC patients were significant higher than control(P<0.001).Pearson correlation analysis showed COX-2 were both positive correlated with PD-1 in the tumor tissue of HCC patients(R2=0.673,P<0.001).The tumor growth curves and survival curves in celecoxib or PD-1 antibody groups were significant better than in control group (P<0.05).There were no significant difference of tumor growth curves and survival curves between celecoxib group and PD-1 antibody group(P>0.05).IHC and flow cytometry analysis showed that celecoxib treatment significant decreased the expression level of PD-1(P<0.05).Both celecoxib and PD-1 antibody treatment significant increased the number of CD8+T cells while decreased the number of Treg cells in the tissue or the peripheral blood(P<0.05).Western blot analysis showed that celecoxib significant decreased the level of COX-2,PD-1,CD8 and Foxp3 in PBMCs,while it did not affect the level of CD8 and Foxp3 in PD-1 siRNA transfected PBMCs.Conclusion:The expression level of COX-2 and PD-1 were increased in the tumor tissue of HCC patients.Celecoxib may inhibit liver cancer by regulating PD-1 mediated tumor immune via inhibiting COX-2.
5.Inhibition of curcumin on proliferative vitreoretinopathy in rabbit eye

Background Our previous study demonstrated that curcumin can induce the apoptosis of retinal pigment epithelial (RPE) cells and herein inhibit the proliferation of RPE cells,and it is proved that the intravitreous injection of 0.1mg curcumin has less adverse effect to ocular tissue, inferring a good applicative prospect in clinic. Objective The goal of this experiment was to evaluate the effectiveness of curcumin on the prevention and treatment of experimental proliferative vitreoretinopathy (PVR). Methods PVR models were induced by injection of 0.1ml RPE cells (containing 2×106 cells) into vitreous cavity in 40 eyes of 20 healthy and mature New Zealand albino rabbits.0. 1ml curcumin(0. 1 mg) was then injected into lateral eye of each model rabbit immediately following the injection of RPE cells,and the equal volume of normal saline solution containing 0. 5‰ DMSO was injected into the fellow eye of each model rabbit as controls. On 1,3,7,14,21 and 28 days after injection, the changes of cornea, aqueous humor, lens, vitreous and fundus were examined and recorded by slit lamp biomicroscope, indirect ophthalmoscope,fundus color camera and B-type ultrasonograph to evaluate the inflammatory response. The incidence rate of retinal detachment was calculated and compared between curcumin group and control group. Results The inflammatory reaction in anterior chamber and misty opacity in vitreous were found from 1 day through 3 days after injection, but no obvious proliferative strap and retinal detachment in all of the experimental eyes. On the 7th day after injection, inflammatory reaction was extinct in the anterior chamber of rabbit eyes, and proliferative strap occurred in 14 eyes(75% ) in the control group but only 2 eyes (10% ) in curcumin group,showing significant difference between these two groups (P<0. 01). No retinal detachment was seen in both the two groups. On 14,21 and 28 days after injection, the incidence rate of retinal detachment was 55% ,80% ,95% respectively in control group and that of curcumin group was 10% ,15% ,15% respectively,presenting considerably differences between two groups (P<0. 01, P<0. 01 ,P<0. 01 ). Conclusion Injection of curcumin into vitreous cavity can effectively inhibit the occurrence and development of PVR in rabbit.
6.Diagnostic value of procalcitonin detection in Candida bloodstream infection

Objective To discuss the clinical application value of serum procalcitonin(PCT) in patients with Candida bloodstream infection.Methods The data of 783 hospitalized patients of Tianjin Medical University General Hospital including blood culture and serum PCT test were retrospectively analyzed,and the medical records of patients with Candida or bacterial bloodstream infection were evaluated by univaxiate and multivariate logistic regression analysis.The comparison of PCT value were carried out among the different blood culture groups using the Mann-Whitney U test.A receiver operating characteristic(ROC) curve was used to determine the diagnostic performance of the PCT.Results The PCT was 0.21 (0.06,1.02) ng/mL in the 510 patients with negative blood culture,but in 121 patients with Candida infection and 152 patients with bacteria infections,the PCT levels were 1.15 (0.38,6.85) ng/ mL and 2.34 (0.77,15.12) ng/mL,respectively.There were statistically significant differences in PCT levels among three groups(P<0.05).According to ROC,when the value of PCT was 0.355 ng/mL,the sensitivity was 76.9%,and the specificity was 60.8% with 0.726 area under the curve (AUC) (P<0.01) for the identification of Candida infection by blood cultures.Conclusions Serum PCT levels have a certain diagnostic value for Candida bloodstream infection.In critically ill patients with factors associated with candidemia,the combination of clinical symptoms with PCT as an adjuvant diagnostic marker and other laboratory findings can be used to make a prompt and effective initiation of antifungal therapy.
7.Changes of platelet endothelial cell adhesion molecule-1, tissue type plasminogen activator and plasminogen activator inhibitor-1 expression in the lung tissue of neonatal rats after intraperitoneal injection with lipopolysaccharide.

OBJECTIVETo further explore the pathogenesis of neonatal acute lung injury and neonatal pulmonary hemorrhage by establishing the animal model of neonatal acute lung injury (ALI) and by investigating the changes of platelet endothelial cell adhesion molecule-1 (PECAM-1), tissue type plasminogen activator (t-PA) and plasminogen activator inhibitor-1 (PAI-1) in ALI.
METHODSTotally 88 neonatal rats which were divided into 8 groups randomly including one normal saline control group and 30 min, 1 h, 2 h, 4 h, 8 h, 16 h and 24 h post injection groups. The changes of lung pathology in newborn rats were observed at different time after LPS was injected intraperitoneally. The changes of PECAM-1 protein, t-PA and PAI-1 mRNA expression were measured by immunohistochemistry and RT-PCR.
RESULTSThe expression of PECAM-1 protein and mRNA was decreased and the lowest level was reached at 8 h and 16 h post injection, respectively. The average values were 95.1 +/- 9.76 and 0.861 +/- 0.016, respectively, which were significantly lower than those in the control group (129.5 +/- 6.15, 1.192 +/- 0.035, P < 0.01). The expression of t-PA and PAI-1 mRNA was increased after LPS was injected. The highest level of t-PA mRNA expression was observed at 2 h after injection. The average value was 1.195 +/- 0.036, which was significantly higher than that in the control group (0.781 +/- 0.017, P < 0.01). The highest level of PAI-1 mRNA expression was observed at 2 h, 4 h and 8 h post injection. The average values were 1.178 +/- 0.069, 1.153 +/- 0.036 and 1.176 +/- 0.044, respectively, which was significantly higher than those of the control group (0.681 +/- 0.019, P < 0.01).
CONCLUSIONSThe expression of PECAM-1 protein and mRNA was decreased after LPS injection, suggesting the disruption of the tissue protective mechanism; the expression of t-PA and PAI-1 mRNA was increased, indicating the presence of a hypercoagulability state. At the same time, the expression of t-PA mRNA was increased which caused the extra-cellular matrix degradation at the early phase after LPS injection. These three phenomena might be the contributory factors to pulmonary hemorrhage.

8.Case-control study on the effect of core strength training on the function of anterior cruciate ligament reconstruction.

OBJECTIVETo observe the effect of core strength training on knee joint function and postural stability after anterior cruciate ligament reconstruction (ACLR).
METHODSA total of 80 ACLR patients were randomly allocated into conventional rehabilitation training group and core strength training group from May 2013 to May 2015 with 40 patients in each group. The patients in conventional rehabilitation training group underwent conventional ACLR rehabilitation training, in which 28 males and 12 females. The mean age was(30.5±5.2) years old(ranged, 22 to 42 years old). The mean BMI was(23.8±2.4) kg/m²(ranged, 18.2 to 25.9 kg/m²). Thirty patients had injuries on the dominant side and 10 patients had injuries on the non-dominant side. The core strength training group received conventional ACLR rehabilitation training and core strength training, in which 31 males and 9 females. The mean age was(31.1±4.8) years old(ranged, 21 to 45 years old). The mean BMI was(24.1±2.7) kg/m²(ranged, 18.5 to 26.1 kg/m²) . Twenty-seven patients had injuries on the dominant side and 13 patients had injuries on the non-dominant side. The Lysholm score, tibial anterior transition measured by KT-1000 before and after treatment, and the Star Excursion Balance Test results after treatment were compared between the two groups.
RESULTSSix months after rehabilitation training, the tibial anterior transition of the conventional rehabilitation training group and the core strength training group were(3.4±1.0) mm and(3.3±1.2) mm respectively, which were less than(12.1±1.8) mm and(12.5±2.0) mm before treatment. But there was no significant difference in anterior tibial translation between two groups(>0.05). The Lysholm score of the conventional rehabilitation training group and the core strength training group were 91.8±4.3 and 92.1±3.9 individually, which were higher than 69.2±5.8 and 70.2±5.1 before treatment. But there was no significant difference in Lysholm score between two groups(>0.05). Six months after rehabilitation training, the results of Star Excursion Balance Test showed the reach distance with the support in the injured side and healthy side in the core strength training group were greater than that of the conventional rehabilitation training group in the eight directions(<0.05).
CONCLUSIONSThe core strength training could improve the dynamic balance of ACLR patients.
9.Preliminary investigation of orthodontic treatment in compliance with dentofacial development in patients with skeletal Class Ⅲ malocclusion and open bite

Objective To analyze the biologic principle of orthodontic treatment in patients with skeletal Class Ⅲ malocclusion and open bite.Methods Eleven pre-adolescent patients with severe skeletal Class Ⅲ malocclusion and open bite(age range 7-9 years old,mean age 8.3±0.8)were included.All patients were surgical cases but the patients rejected surgery.The theatment methods used were face mask,rapid maxillary expansion occlusal splint and fixed appliance.Lateral cephalometric films were taken before and after treatment.Cephalometric analysis was performed.Results After the treatment SNA changed from (79.0±1.2)°to(81.9±0.8)°(P<0.01).And the inclination of lower incisors was decreased from (25.6±2.1)°to(20.1±1.4)°when measured to the NB line(P<0.01).The direction of the facial growth was maintained.Conclusions Good orthodontic results could be achieved in patients with skeletal Class Ⅲ malocclusion and open bite.
10.Study on skin sensitization as well as liver and kidney impairment in guinea pigs treated with trichloroethylene.

OBJECTIVETo study skin sensitization as well as liver and kidney impairment in guinea pigs treated with trichloroethylene (TCE).
METHODSGuinea pig maximization test (GPMT) was applied in this study, guinea pigs were divided into 3 groups, namely negative control, positive control and TCE treatment. Animals of 3 groups were administrated with olive oil, 2, 4-dinitrochlorobenzene (DNCB), and TCE, respectively, by intradermal injection. The animal skin was observed and blood was collected after various treatment, the liver function tests were conducted, including detection of activities of ALT, AST, LDH and levels of creatinine, uric acid, and urea with automatic biochemical analyzer.
RESULTSObvious skin impairment was observed in the groups of positive control and TCE treatment, the skin impairment included erythema and edema, the sensitization rate was 100% in positive control and 83.3% in TCE treatment group. Additionally, the activities of ALT, AST and LDH increased significantly in the groups of positive control and TCE treatment when compared with the negative control.
CONCLUSIONSTrichloroethylene is one of the strong hypersensitizing substances, it could induce skin allergic reaction and liver impairment in guinea pigs.
