5.Clinical Analysis of 124 Children with Henoch-Sch(o)nlein Purpura

Objective To study the clinical characteristics of Henoch-Sch(o)nlein purpura (HSP) in children.Methods We retrospectively analyzed the clinical data of 124 children with HSP.Results Among these 124 children,54.84% of the children presented upper respiratory tract infection before the onset of HSP.The purpura recurred in 30 of 47 children with Henoch-Sch(o)nlein purpura nephritis and in 22 of 77 children without Henoch-Schonlein purpura nephritis,respectively (P<0.05).The incidence of Henoch-Sch(o)nlein purpura nephritis was significantly higher in children who had purpura,gastrointestinal symptoms,and joint symptoms simultaneously than in those only with purpura (P<0.05).The elevated levels of IgA and C reactive protein were found in 67.35% and 39.39% of the children,respectively.Conclusion The most important predisposing factor of HSP is upper respiratory tract infection.The children with recurrence of purpura or those who have purpura,gastrointestinal symptoms,and joint symptoms simultaneously are more likely to present renal damage.

Takayasu arteritis (TA) is a devastating vasculitis of the aorta and its major branches,coronary and pulmonary arteries.The clinical manifestations in children are less specific than in adnlts:the disease in children presents with fever,arthralgias,vomiting,weight loss and hypertension.Conventional angiography,which is recognized as the golden standard in evaluating vascular lesions in TA,combined with computer tomography angiography (CTA),magnetic resonance angiography (MRA),ultrasonography,could not only provide important information for early diagnosis,but also detect disease activity.New immunosuppressive agents and biological therapies,such as TNF-a blocking agents,have been verified to be effective although corticosteroids and conventional immunosuppressive agents are still basic treatment.

Rational microenvironment contributes actively to cancer development through multiple mechanisms including triggering genomics instability,providing shield,promoting immune escape,inducing differentiation and permissive nich.Tumorigenesis reversely promotes the form of tumor microenvironment composed of hypoxia,decreasing pH,angiogenesis and nutrition deficiency.Tumor microenvironment not only is the reasons but also results in cancer development and progression.A better understanding of how the tumor environment affects cancer progression would provide the new strategy for cancer therapy.
8.Roles of mitochondria in tumorigenesis

As semi-autonomous organelles,mitochondria are easily damaged by various factors.Mitochondrial dysfunction such as abnormal oxidative phosphorylation,metabolism alterations,apoptosis prevention,damaged mitophagy,immune escape and deregulated signaling pathway have been hypothesized to be involved in tumorigenesis.Modulated mitochondria may prevent the occurrence of tumors.
9.Meta-analysis of fusion with and without instrumentation in the treatment of chronic low back pain

Objective To evaluate the efficacy of fusion with and without instrumentation in the treatment of chronic low back pain.Methods To search Medline(from 1966 to Deeember.2006).Embase (1984 to December,2006),Cochrane Central Register of Controlled Trial f4th Quarter 2006),Current Controlled Trials.The China Biological Medicine Database(1984 to December,2006).and hand searched several related journals.The quality of included trials was evaluated.Data were extracted by two reviewers independently with a designed extraction form RevMan 4.2.8 software was used for data analysis.Results Eight studies involving 656 patients were included.The results of meta-analysis indicated that statistically significant difierences were observed between the two operative procedures in the fusion rate(OR=2.26,95% CI 1.51-3.38,P=0.0001),clinical outcome(OR=1.54,95%CI 1.09-2.19,P=0.01),incidence of re-operation (OR=2.16.95% CI 1.08-4.33,P=0.002).There were no statistically significant difierences in back pain scores (WMD=-0.22,95% CI-0.81-0.36),leg pain scores(WMD=0.22,95%CI-0.50-0.93),complications(OR=1.68,95%CI 0.78-3.63),satisfaction of surgery by patients(OR=1.54,95%CI 0.93-2.55).Three studies described the mean surgical time,perioperative blood loss,days in hospital after surgery,which revealed that single fusion was superior to fusion with instrumentation.Three studies described the work status of patients post-operatively.Two studies indicated smoking could decrease the fusion rate.Conclusion To compare with single fusion,fusion with instrumentation can increase the fusion rate and improve clinical outcome slightly,but it also increases the incidence of re-operation.More high quality large-scale randomized controlled trims are required.
