5.Clinical Analysis of 124 Children with Henoch-Sch(o)nlein Purpura

Objective To study the clinical characteristics of Henoch-Sch(o)nlein purpura (HSP) in children.Methods We retrospectively analyzed the clinical data of 124 children with HSP.Results Among these 124 children,54.84% of the children presented upper respiratory tract infection before the onset of HSP.The purpura recurred in 30 of 47 children with Henoch-Sch(o)nlein purpura nephritis and in 22 of 77 children without Henoch-Schonlein purpura nephritis,respectively (P<0.05).The incidence of Henoch-Sch(o)nlein purpura nephritis was significantly higher in children who had purpura,gastrointestinal symptoms,and joint symptoms simultaneously than in those only with purpura (P<0.05).The elevated levels of IgA and C reactive protein were found in 67.35% and 39.39% of the children,respectively.Conclusion The most important predisposing factor of HSP is upper respiratory tract infection.The children with recurrence of purpura or those who have purpura,gastrointestinal symptoms,and joint symptoms simultaneously are more likely to present renal damage.

Takayasu arteritis (TA) is a devastating vasculitis of the aorta and its major branches,coronary and pulmonary arteries.The clinical manifestations in children are less specific than in adnlts:the disease in children presents with fever,arthralgias,vomiting,weight loss and hypertension.Conventional angiography,which is recognized as the golden standard in evaluating vascular lesions in TA,combined with computer tomography angiography (CTA),magnetic resonance angiography (MRA),ultrasonography,could not only provide important information for early diagnosis,but also detect disease activity.New immunosuppressive agents and biological therapies,such as TNF-a blocking agents,have been verified to be effective although corticosteroids and conventional immunosuppressive agents are still basic treatment.

As a way of gene modification,alternative splicing is an important factor of eukaryotic gene expression and regulation.It makes various transcripts from one protein-coding gene,and greatly extends the genetic information.Alternative splicing of pre-messenger RNA plays an important role in tumor cells.By alter-native splicing,some important genes can generate splicing variants different from those in normal cells.The existence of tumor-specific splicing variants leads to the occurrence and progression of tumor.Therefore,explo-ration on the alternative splicing of tumor-associated genes may be of great significance in tumor diagnosis and treatment.

As small non-coding RNAs,microRNAs (miRNAs)have been recognized as important reg-ulatory factors in the post-transcription regulation network.Studies have shown that miR-133a and miR-133b, as members of miR-133 family,can regulate the expression of target genes such as epidermal growth factor receptor and oncogene.They regulate the mitogen-activated protein kinases and protein kinase B signaling path-ways,which affect tumor cell proliferation,invasion and migration.MiR-133 plays a key role in the process of tumor development and progression,suggesting that it could be served as a new target for cancer treatment.
