Objective To study the effects and clinical significance of plasma thromboxane B2(TXB2), platelet alpha granule membrance protein-140(GMP-140), 6-Keto-PGF1?(6-K-PGF1?), endothelins(ET), tissue type plasminogen activator(tPA) and plaminogen activator inhibitor-1(PAI-1) in patients with cerebral arteriosclerisis and cerebral thrombosis. Methods Plasma TXB2 ,GMP-140,6-K-PGFla and ET levels were determined by ra-dioimmuno-assay. Plasma tPA and PA1 were determined by the chromogenic peptide substrate method in controls and in patients with cerebral arteriosclerisis and cerebral thrombosis. Results The levels of plasma TXB2 and GMP-140 were significantly higher in patients with cerebral arteriosclerisis and cerebral thrombosis than in the control group(P0.05) .The levels of tPA and tPA/PAI were significantly lower in patients with cerebral arteriosclerisis and cerebral thrombosis than in controls. Although the PAI level increased in patients with cerebral arteriosclerisis and cerebral thrombosis,no difference was found in statistics. Conclusions Activation of platelets, injury of endothelial cell and decreased activity of fibrinolysis in patients with cerebrovascular disease may be involved in the development of these diseases. Appropriate treatment should be adopted on the above abnormality.

OBJECTIVE:To establish the quality standards for Qiqilian capsule. METHODS:TLC was used to identify the As-tragali Radix,Phellodendri chinensis,Coptidis rhizom. HPLC was used to determine the contents of ginsenosides Rg1,ginsenosides Rb1 and notoginsenosides R1. The column was Shim-pack VP-ODS C18 with mobile phase of acetonitrile-water(gradient elution) at flow rate of 1.0 ml/min,detection wavelength was 203 nm,column temperature was 20 ℃. RESULTS:TLC of Astragali Radix,P. chinensis,C. rhizom showed cleer sports and good separation. The linear range was 0.9-9.0 μg for ginsenosides Rg1,0.94-9.4 μg for ginsenosides Rb1 and 0.3-3.0 μg for notoginsenosides R1(r≥0.999 5);RSDs of precision,reproducibility and stability test were lower than 3.0%;recoveries were 96.08%-99.75%(RSD=1.52,n=6),97.03%-99.75%(RSD=1.10,n=6)and 96.38%-98.55%(RSD=0.90,n=6),respectively. CONCLUSIONS:The method is simple,good reproducibility,and can be used for the quality control of Qiqilian capsule.

OBJECTIVETo establish a method of TLC identification for Dida commonly used in Tibetan medicine from different species.

METHODWith silica gel G as the stationary phase, and chloroform-methanol (40: 1) as mobile phase, oleanolic acid from different species of Dida was separated and identified.

RESULTOleanolic acid was detected in 70 kinds of Dida derived from the Gentianaceae Swertia, Halenia, Gentianopsis, Lomatogonium, and Saxifragaceae saxifrage, except for the saxifrage, there are some differences among different genera or subjection.

CONCLUSIONThis TLC method can be used for identification of oleanolic acid in Dida from different species except saxifrage.

OBJECTIVETo compare the difference in the clinical efficacy on nonspecific low back pain (NLBP) treated with the dragon-tiger fighting needling method, the uniform reinforcing-reducing method and the intermediate frequency physiotherapy.

METHODSNinety cases of NLBP were randomly divided into a dragon-tiger fighting needling group (group A), an uniform reinforcing-reducing needling group (group B) and an intermediate frequency physiotherapy group (group C), 30 cases in each one. In the group A, the dragon-tiger fighting needling method was used. In the group B, the uniform reinforcing-reducing method was applied. Two groups of acupoints were prescribed. One group included Shenshu (BL 23), Dachangshu (BL 25), Weizhong (BL 40) and Ashi points. The other group included Qihaishu (BL 24), Guanyuanshu (BL 26), Kunlun (BL 60), Yaoyangguan (GV 3). These two groups of acupoints were used alternatively in the above two groups. In the group C, the intermediate frequency physiotherapy was adopted in the pain area of the lumbar region. The treatment was given once per day in each group. Six treatments made one session. Totally, 2 sessions were required. The Visual Analogue Scale (VAS), the Oswestry Disability Index (ODD and the clinical efficacy were observed in each group.

RESULTSThe scores of VAS and ODI were reduced obviously after treatment in each group (P < 0.05, P < 0.01). The score reducing in the group A was much more remarkable than those in the other two groups (all P < 0.05). The clinical curative rate was 30.0% (9/30), 23.3% (7/30) and 16.7% (5/30) in the group A, the group B and the group C, respectively. In comparison, the clinical efficacy in the group A was superior to that in either of the other groups (all P< 0.05).

CONCLUSIONThe dragon-tiger fighting needling method achieves the much better efficacy on NLBP compared with either the uniform reinforcing-reducing method or the intermediate frequency physiotherapy. It is one of the more effective needling method for analgesia.

Objective To establish an HPLC with UV method for determination of remifentanil in human blood.Methods Human blood with internal standard was extracted by butyl chloride.The sample was separated on Zobar SE-CN (4.6 mm × 250 mm,5μm) column with a mobile phase of mixture of acetonitrile,methanol and phosphate buffer,the temperature was set at 30℃ with flow rate of 1.5 mL·min~(-1).UV detection at 210 nm. Results The calibration curves were found to be linear at the range 1-20 ng·mL~(-1) of remifentanil in whole blood with a good correlation (γ=0.956).Conclusion This method is rapid,accurate and sensitive,which is very suitable for investigation of clinical blood concentration of remifentanil.

OBJECTIVETo evaluate the risk factors of postoperative renal failure (RF) in the patients with type A dissection of aorta operated on with cerebral perfusion and deep hypothermia circulatory arrest (DHCA).

METHODSFrom January 2004 to October 2007, 157 patients with type A dissection of aorta underwent surgical procedures with cerebral perfusion and DHCA. There were 115 male patients and 42 female patients with the age from 17 to 76 years old. Antegrade selective cerebral perfusion through axillary artery was performed for 129 patients and retrograde cerebral perfusion from superior cava vein was performed for 28 patients. All the factors underwent univariate and multivariate analysis.

RESULTSMean cardiopulmonary bypass duration was (188.0 +/- 10.8) min and mean cerebral perfusion time was (36.0 +/- 3.1) min. Fifteen patients died in hospital and the hospital mortality was 9.6%. Permanent neurological dysfunction (PND) occurred in 8 patients (5.1%). Postoperative RF was observed in 20 patients (12.8%). Multivariate analysis showed the preoperative renal dysfunction (P = 0.042, OR = 4.41) and over seventy-year-old patients (P = 0.049, OR = 4.94) were found to be the risk factors of postoperative RF. There was a higher incidence of death (45%, P = 0.001) and PND (25%, P = 0.009) in the patients of postoperative RF when compared with the other patients.

CONCLUSIONThe preoperative renal dysfunction and elderly patients were found to be the risk factors of postoperative RF after type A dissection of aorta surgery.

In order to construct an eukaryotic expression vector for gene of multiple myeloma mucin1 (muc1-2vntr) gene and to express it in COS-7 cells in vitro, so to provide the basic material for further research of multiple myeloma DNA vaccine. muc1-2vntr coding gene was used as a research gene and a KOZAK sequence was inserted before the gene Hind III and XbaI restriction sites were inserted before and after the coding gene. Then the whole sequence was synthesized and inserted into pcDNA3.1/myc-his B vector, and the resulted recombinant vector was transformed into E.coil competent cells to get an engineering strain, the recombinant plasmid pcDNA3.1-2vntr/myc-his B identified by restriction analysis and DNA sequencing were transfected into COS-7 cells by liposome-mediated gene transfer method. Finally, fluorescent microscopy was used to assess GFP expression and Western blot analysis using muc1 monoclonal antibody was used to recognize vntr, confirming the expression of vntr. The results showed that the full length of synthesized muc1-2vntr gene, as expected, was 140 bp. Both restriction analysis and DNA sequencing demonstrated that pcDNA3.1-2vntr/myc-his B included the whole translation frame region and muc1-2vntr gene. Furthermore, the fluorescence microscopy proved that the recombinant plasmid had been successfully transfected into COS-7 cells. The expression of mucin-1 protein was observed both in the transfected cell and the cell supernatant by Western blot. It is concluded that the pcDNA3.1-2vntr/myc-his B has been successfully constructed and expressed in COS-7 cells in vitro, which provides the basic material for further researches of mucin-1 function and possible multiple myloma DNA vaccine.

Objective To study the family rearing pattern of attention deficit hyperactivity disorder(ADHD)with or without anxiety disorder and to explore its risk factors.Methods 9495 children and their parents were sampled at random in Hunan province,using two-stage investigation.Those who were diagnosed ADHD and the normal control filled out Egna Minnen av Barndoms Uppfostran and family adaptability and cohesion scale bv themselves.Results The comparison of factors as:actual family cohesion,parents' punishments,reiection,mother's excessive protection,intervention and father's excessive protection were significantly difierent between ADHD with or without anxiety disorder and normal children(P＜0.05).The comparison of parents' punishments,reiection,excessive protection and intervention were obviously different between ADHD with anxiety disorder and simple ADHD(P＜0.05).Mother's reiection was the influencing factor of simple ADHD,with OR as 1.122.Ideal family cohesion,mother's rejection and father's punishments were the influencing factors of ADHD with anxiety disorder,with OR as 0.966.1.215 and 1.089 respectively.Conclusion There were some problems in the parental rearing pattern of ADHD with or without anxiety disorder.Mother's rejection,father's punishments and ideal family cohesion were suggested to be correlated with ADHD and anxiety disorder.

Objective·To analyze the basic medical insurance policies for children in Shanghai in 2011 and 2017, and compare with those of the other three municipalities to understand the basic medical security status of children in four municipalities in China and analyze the existing problems. Methods?·?The current effective children's basic medical insurance policies and child critical illness insurance policies and related insurance systems in four municipalities were collected systematically. The types of basic medical insurance for children in various municipalities and their funding standards, their own expenses and the proportion of government subsidies, the proportion of outpatient emergency payoff lines and reimbursement at all levels of medical institutions, and the coverage and coverage of major illness insurance reimbursement were collected and compared mainly. Results?·?The basic medical insurance for children in Shanghai is a "double insurance" system, which contained children's hospitalization fund and basic medical insurance for urban and rural residents. The basic medical insurance for children in other municipalities mainly consisted of basic medical insurance for urban (rural) residents. Compared with 2011, the level of financing and the reimbursement of the basic medical insurance for urban and rural residents and of the children's hospitalization fund in Shanghai in 2017 was increased. In 2017, the level of financing of basic medical insurance for urban and rural residents in Shanghai was 1?100 yuan, which was slightly lower than that of Beijing medical insurance for urban residents and new rural cooperative medical insurance and was higher than that of basic medical insurance for urban and rural residents in Tianjin and Chongqing, and the individual payment level of Shanghai was the lowest. The hospitalization payoff lines in Shanghai were lower than those of other municipalities. Shanghai's outpatient and inpatient reimbursement levels were higher those of than other municipalities. There wasn't a capping line in Shanghai. There were capping lines in other municipalities. There was a great difference in the insurance policy of the major diseases in four municipalities. Conclusion?·?According to their own actual conditions, models for children's medical care systems that suited each municipalities’ social and economic development have been established. Shanghai children's basic medical insurance basically meets the needs of children's growth but needs improvement in terms of the type and level of medical security for major illnesses.
