1.Feasibility of application of high frequency jet ventilation to trachea and carinal reconstruction

Objective To evaluate feasibility of high frequency jet ventilation (HFJV) during trachea and carinal reconstruction. Methods Ten patients with lung cancer ,scheduled for elective trachea and carinal reconstruction ,were intubated with double lumen tube following routine anesthesia induction. A HFJV tube (ID=3mm) was inserted 3cm into main bronchus. HFJV was applied ,with the respiratory rate of 120/min,I:E=1:2 and drive pressure of 0.15-1.20MPa, after cut off the main bronchus of the opposite side. Blood gas analysis was made before the operation, 15min following one-lung ventilation, 5, 10, 20 min following HFJV and 15min following one-lung ventilation,respectively.Results PaO 2 was significantly higher following HFJV than that one-lung ventilation (P
2.Clinical Observation of Acupuncture by Stages for Post-stroke Swallowing Dysfunction

Objective To observe the clinical efficacy of acupuncture by stages in treating post-stroke swallowing dysfunction. Method Sixty eligible patients were randomized into a staging-acupuncture group and an ordinary-acupuncture group, to respectively receive acupuncture by the stage of swallowing and ordinary acupuncture in addition to the basic treatments. Function estimation was conducted before and after intervention in both groups to compare the therapeutic efficacies.Result According to the water drinking test, the total effective rate was 83.3% in the staging-acupuncture group versus 53.3% in the ordinary-acupuncture group, and the difference was statistically significant by rank sum test (P<0.05), and the staging-acupuncture group was superior to the ordinary-acupuncture group especially in the pharyngeal stage (P<0.05); the differences were statistically insignificant in the oral stage and esophageal stage (P>0.05); regarding the video fluoroscopic swallowing study (VFSS), the staging-acupuncture group was significantly better than ordinary-acupuncture group in improving swallowing function (P<0.05).Conclusion Acupuncture by swallowing stages is effective in treating post-stroke swallowing dysfunction, especially in improving the swallowing function in pharyngeal stage.

Aim To explore the effect of Procyanidin from Vaccinium(PC) on the myocardial fibrosis and its mechanism.Methods The model of myocardial fibrosis in rats was built by subcutaneous injection of isoprenaline(Iso) for 5 mg?kg-1?d-1 in vivo.Meanwhile,rats were treated with PC 100,200 and 400 mg?kg-1?d-1 by gastrointestinal administration.The contents of hydroxyproline(Hyp),malondialdehyde(MDA) and the activity of superoxide dismutase(SOD) in left ventricle were assayed with spectrophotometry respectively.Changes of myocardial ultrastructure were observed by electronic microscope.The cultured neonatal rat CFb was isolated by trypsin digestion method.The proliferation of CFb was induced by Angiotensin Ⅱ and assessed by thiazolyl blue(MTT) assay.PC was added in different dosage(25,50 and 100 mmol?L-1).Ⅰ,Ⅲ collagen quantification and TGF-?1 levels were determined by the ELISA.Results In vivo,compared with model group,the contents of hydroxyproline and malondialdehyde of left ventricle were significantly reduced and the activity of superoxide dismutasein left ventricle was markedly enhanced in PC three groups(P
4.Analysis on Isatidis Radix Protein in Different Areas by SDS-PAGE

Objective To identify Isatidis Radix in different areas by SDS-PAGE;To provide the basis for the identification and quality evaluation of Isatidis Radix. Methods By using SDS-PAGE technology, the protein profiles of Isatidis Radix in Heilongjiang and Hebei regions were established. Results Isatidis Radix protein in Heilongjiang contained about 13 protein subunits, and the major subunits were 10.5 kD, 23.0 kD, 24.9 kD, and 44.1 kD. Isatidis Radix protein in Hebei contained about 12 subunits, and the major subunits were 55.6 kD, 45.9 kD, 34.3 kD, 18.9 kD, 12.4 kD, and 10.5 kD. Conclusion SDS-PAGE technology can be used as one of the references to identify Isatidis Radix in different areas.
5.Preparation of the small intestinal submucosa sponge

BACKGROUND:Studies have found that smal intestinal submucosa that is directly implanted into the lesion cannoteffectively promote celgrowth and differentiationin vivoandin vitro, because of its smal pore size and poor permeability.
OBJECTIVE:To establish the smal intestinal submucosa sponge and to explore its morphological characteristics.
METHODS:Porcinesmall intestinal submucosa was prepared by physiochemical method. Thenthe small intestinal submucosa with the mass fraction of 1%, 2%, 3% and 4% was cross-linked by 50, 100 and150 mmol/L 1-ehyl-3-(3-dimethylaminopropyl) carbodimide hydrochloride, respectively, so as to obtain smal intestinal submucosa sponge, whose morphology was detected by lighting and scanning electron microscope. In the meanwhile, smal intestinal submucosa as control group, and smal intestinal submucosa sponge as test groupwere intramuscularly implanted into the back of rats,respectively. At 1, 2 and 3 weeks after implantation, histological changes andimplantdegradation were observed by hematoxylin-eosin staining.
RESULTS AND CONCLUSION:The smal intestinal submucosa sponge, which was prepared by the smal intestinal submucosa with the mass fraction of 1% and 100 mmol/L cross-linking agent, had elastic and close space structure, uniform pore size and regular structure, so it was selected as the implant into themuscle.At 1 week after implantation, in the test group,the mesh sponge had the complete structure withfew neutrophils, lymphocytes and giant cel reaction, andsoft tissue hyperplasia and migration surrounding the implant appeared;in the control group,there were numerous inflammatory cels, and wound adhesion and little migration of surrounding tissues could be found.At 3 weeks, inflammatory cels mostly disappeared, and fibroblast-like cels and vascular components appeared, with thinner and regular colagen fiber bundles, and connective tissue-like structures could be found. In contrast, the control group stil had numerous inflammatory cels and few colagen fibers. In conclusion, smal intestinal submucosa sponge isapotential material used asthe tissue-engineered skinscaffold.
6.Colonoscopic and endoscopic ultrasonography diagnosis of appendix mucocele

Objective To investigate the features and clinical value of endoscopic ultrasonography and colonoscopy for appendix mucocele. Methods The patients with mucocele of appendix who were diagnosed by endoscopic ultrasonography and colonoscopy in three hospitals all underwent surgery from January 2008 to March 2015. Appendix mucocele in these patients was confirmed by postoperative pathology and clinical data were retrospectively analyzed. Results A total of 22 patients with appendix mucocele were analyzed retrospectively. The average size of intralumen mucocele was 1. 84±1. 42 cm (0. 6-4. 5 cm) . The colonscopic finding of appendiceal mucocele showed submucosal protuberance at the appendiceal orifice with smooth surface. The appendiceal orifice was found at the edge of appendiceal mucocele. Endoscopic ultrasonogrphy showed low echo with smooth cyst wall in 8 patients, mixed equal echo and low echo in 14 cases. Appendicectomy was performed in 11 patients and resection of ileocecum in 11 others. Conclusion Endoscopic ultrasonography and colonoscopy are valuable for diagnosis and treatment in appendiceal mucocele.
7.Effect of iron on vasoconstriction in the isolated rat aorta.

The present study was to examine the effect of iron on isolated rat aortic rings, and to elucidate the underlying mechanism. The thoracic aortic rings without endothelium of male Sprague-Dawley rats were mounted on a bath system. Isometric contractions of aortic rings were measured. The results obtained are as follows. (1) Ferric ammonium citrate (FAC) (100 micromol/L) caused a phasic response with an initial transient contraction followed by a relaxation in thoracic aortic ring. The maximal contractile amplitude was 24.02+/-2.37% of the maximal contraction induced by KCl, the duration of phasic contraction lasted for about 20 min. (2) In high Ca(2+) Krebs-Henseleit (K-H) solution (twice of the normal concentration), the contractile amplitude induced by FAC was enhanced. After the aortic rings were incubated with nifedipine for 15 min to block the L-type Ca(2+) channel, the iron-induced contraction was attenuated. (3) In Ca(2+)-free K-H solution, addition of FAC caused a strong and sustained contraction in the presence of PDBu. (4) Pretreatment of FAC for 30 min decreased the KCl-induced contraction and also caused a significant reduction in the contractile response to phenylephrine (PE). Pretreatment of the arteries with DMSO, catalase or glutathione before FAC exposure prevented the decrease in contraction responses to PE (P<0.05). It is therefore concluded that iron causes phasic contraction of vascular smooth muscle, in which both extracellular Ca(2+) entry through L-type Ca(2+) channel and increase in Ca(2+) sensitivity of smooth muscle cells are involved. Exposure to iron causes inhibitory effects on KCl- or PE-induced contractions in isolated thoracic arteries. Reactive oxygen species and glutathione may be involved in iron-induced contraction dysfunction.

8.Clinical analysis of hepatic arterial thrombolysis combined with splenic arterial embolization in treatment of hepatic arterial thrombosis after liver transplantation

Objective To analyze the clinical efficacy of transcatheter hepatic arterial thrombolysis combined with splenic arterial embolization in the treatment of hepatic artery thrombosis (HAT) after liver transplantation. Methods Clinical data of 9 patients diagnosed with HAT after liver transplantation undergoing transcatheter hepatic arterial thrombolysis combined with splenic arterial embolization were retrospectively analyzed. The incidence of HAT and clinical efficacy of thrombolytic therapy were summarized. The incidence of thrombolysis related complications and clinical prognosis were evaluated. The thrombolytic therapy procedures of typical cases were analyzed. Results HAT was diagnosed at 1-66 d after liver transplantation with a median time of 10 d. The formation site of HAT was found at the anastomosis of the main hepatic artery in 8 cases and at the right branch in 1 case. Upon diagnosis, 9 patients received transcatheter hepatic arterial thrombolysis combined with splenic arterial embolization in emergency. The hepatic artery was open during operation in 4 cases and treated with postoperative thrombolytic therapy with indwelling catheter in 3 cases. The opening time for inwelling catheter was 72-96 h. The total successful rate was 7/9. The thrombolysis related complication of abdominal hemorrhage occurred in 1 case after surgery. Three cases died, including 2 cases of liver failure and infection, and 1 case of biliary ischemia and systemic infection at 70 d after interventional therapy. Conclusions Hepatic arterial thrombolysis combined with splenic arterial embolization is an efficacious treatment for HAT after liver transplantation, which can serve as the optimal therapy for patients who are unable to undergo secondary liver transplantation.
9.Relationship between hemolymph phenol oxidase and melanization of Plasmodium yoelii oocysts in Anopheles dirus

Objective To explore the relationship between hemolymph phenol oxidase and the melanization of Plasmodium yoelii oocysts in Anopheles dirus. Methods An Anopheles dirus-Plasmodium yoelii system was used Anopheles dirus were divided into 3 groups, that is, non-blood-fedding (N), normal-blood-fedding (B) and infected-blood-fedding (I). The activities of MPO and o-DPO in hemolymph from 3 groups were determined with native polyacrylamide gel electrophoresis (PAGE) and density scanning at 5, 7, 11 and 15 d after blood feeding. Results Both MPO and o-DPO activity were significantly higher in group I than group N and B (P<0.05). But with the melanization of Plasmodium yoelii oocysts, both MPO and o-DPO activity in group I were decreased in comparison with group N, especially on the 15 th day after infected-blood feeding. MPO and o-DPO activity in group B were significantly stronger than those of group N. Conclusion Blood feeding and infection of Plasmodium yoelii both can activate the cascade. The heamolymph phenol oxidase may play an important role in the melanization of Plasmodium yoelii oocysts in Anopheles dirus.
10.Treatment and analysis of the early postoperative complications of tibial plateau fractures.

OBJECTIVETo analysis the early complications of tibial fracture and its related factors, and propose a solution.
METHODSFrom December 2003 to December 2013,38 patients with early complications of tibial plateau fracture after operation were retrospectively analyzed. There were 35 males and 3 females, aged from 37 to 69 years old (averaged 42.3 years). According to Schatzker classification, 3 cases were classified as type II, 2 cases as type III, 2 cases as type IV, 19 cases as type V, 12 cases as type VI. The intervals between injury and operation ranged from 9 hours to 9 days, 26 cases within 3 days. Fifteen cases were treated with internal fixation of plates and 23 were treated by plate fixation and bone transplantation. Early complications included skin necrosis in 15 cases, infection in 6 cases, osteofascial compartment syndrome in 3 cases, common peroneal nerve injury in 2 cases, the superficial peroneal nerve injury in 3 cases, popliteal artery injury in 2 cases, loss of reduction in 7 cases.
RESULTSThe wound of 14 cases healed at the first stage and 24 cases healed delay. Hospitalization days ranged from 7 to 67 days (averaged 25.6 days). All patients were followed up for 12 to 36 months with an average of 16.4 months. The fracture healing time ranged from 3 to 9 months (averaged 6.9 months). According to Merchant knee function evaluation criteria, the results were excellent in 19 cases, good in 12, fair in 5 and poor in 2.
CONCLUSIONEarly complications of tibial fracture after operation is closely associated with the severe fracture complexity and related with preoperative preparation, surgical timing, operation incision selection and surgical technique. Early detection and timely processing reduce damage.
