1.Stroke incidence of the household population inShanghai39;s Qingpu District in 2017 - 2022

Objective To understand the characteristics and temporal trends of stroke incidence in the household population of Shanghai39;s Qingpu District and to provide a basis for the development of comprehensive prevention and control strategies. Methods The stroke case database for Qingpu District from 2017-2022 was obtained from the Shanghai Stroke and Acute Myocardial Infarction Registry and Reporting Information System. The average age of onset, incidence rate, standardised incidence rate, and constitutive ratio were calculated. Independent samples t-tests were used for comparisons between groups, 2-tests and 2-trend tests for comparisons of rates, and the Joinpoint regression model for calculating the annual percentage change (APC) to analyse the temporal trend of rates. Results Between 2017 and 2022, the average age of stroke onset in the household population of Shanghai39;s Qingpu District was 73.69±11.60 years. The average annual incidence rate was 556.62/100 000, with an average annual standardised incidence rate of 333.76/100000. There was an increasing trend in the incidence and standardised incidence of stroke in males (APC=7.06%, t=3.44, P=0.03, APC=5.32%, t=3.04, P=0.04). The incidence of stroke increases with age, with cases mainly concentrated in those aged 65 years and above, accounting for 79.47%. Ischemic stroke dominates the stroke typology, accounting for 91.08% of cases, while the incidence of hemorrhagic stroke shows an increasing trend (APC=4.64%, t=4.59, P=0.01). Conclusion The occurrence of stroke in the general population of Shanghai’s Qingpu District is concerning. The study indicates that males, individuals aged 65 years and above, and ischaemic stroke are significant factors that require attention for stroke prevention and control.
2.Rapid Identification of the Chemical Components of Mongolian Medicine Sanzi Powder by HPLC-Q-Exactive-MS/MS

Abstract
OBJECTIVE To quickly qualitatively analyze the chemical components in Sanzi powder compound prescription by HPLC-Q-Exactive-MS/MS. METHODS SHIMADZU GIST C18 column(4.6 mm×150 mm, 5 μm) was used, using 0.1% formic acid water-methanol as mobile phase with gradient elution, the flow rate was 0.5 mL·min-1, the column temperature was 30 ℃. Under positive and negative ion modes, the primary and secondary mass spectrometry information of Sanzi powder was scanned. Qualitative attribution of each component in Sanzi powder was carried out based on the mass spectrometry information, molecular formula, and retention time of molecular ion peaks and fragment ions analyzed using total ion flow diagrams, combined with the molecular formula and structural formula searched in the Chemspider database and references. RESULTS Based on the analysis of the mass spectrometry cleavage rules and references of various components, 95 possible chemical components were preliminarily inferred, including 39 phenolic acids, 20 tannins, 9 organic acid esters, 5 monoterpenoids, 12 iridoids, 8 triterpenes and 2 flavonoids. Among them, 57 components were from Chebulae Fructus, 30 were from Gardeniae Fructus, 10 were from Toosendan Fructus, and among these compounds, rutin from Chebulae Fructus, Gardeniae Fructus and Toosendan Fructus. CONCLUSION The HPLC-Q-Exactive-MS/MS detection method has good separation and high sensitivity, and can quickly and efficiently infer various components in Sanzi powder. It establishes a fast and efficient analytical method for identifying the chemical components in Sanzi powder.
3.Effect of continuous nursing intervention on nursing quality and postoperative function in elderly patients after femoral intertrochanteric fracture

Objective To explore the effect of continuous nursing intervention on limb function and nursing quality after proximal femoral nail antirotation (PFNA) internal fixation for femoral intertrochanteric fracture in the elderly. Methods From February, 2017 to November, 2018, 100 elderly patients with femoral intertrochanteric fracture who underwent PFNA internal fixation in our hospital were randomly divided into control group (n = 50) and observation group (n = 50), who accepted routine nursing and continuous nursing respectively for three months. They were assessed with Harris score and visual analogue scale for pain (VAS) before and after the intervention. The postoperative nursing effect was compared. Results The Harris score increased in both groups after the intervention (t > 45.98, P < 0.001), and increased more in the observation group than in the control group (t = 15.03, P < 0.001). The VAS score decreased in both groups after the intervention (t > 16.33, P < 0.001), and decreased more in the observation group than in the control group (t = 9.749, P < 0.001). The effect of nursing was better in the observation group than in the control group (Z = -2.272, P = 0.023). Conclusion Continuous nursing intervention can significantly improve the limb function and nursing satisfaction of elderly patients with femoral intertrochanteric fracture after PFNA.

Dysphagia is often associated with incoordination between breathing and swallowing, resulting in the occurrence of aspiration, cough and aspiration pneumonia. This article reviewed the specific physiological mechanisms of respiration and swallowing and their clinical applications, which aimed to focus on the relationship between respiration and swallowing systems in health and disease, and to explore its guiding value in the clinical treatment of dysphagia.
5.Clinical Characteristics and Treatment of Autonomic Dysreflexia after Spinal Cord Injury (review)

Autonomic dysreflexia (AD) is an emergency commonly happened in patients with spinal cord injury (SCI) above T6 and especially in chronic SCI patients. AD is usually triggered by stimuli below the injury level. Sudden elevated blood pressure is the main clinical symptom and cardiovascular and/or cerebral complication even death if effective treatments are not provided. Mechanisms contributed to AD possibly are dysfunction of supra spinal control, plastic changes of neurons and receptors. Preventive measures and managements such as removal of risk factor, adjustment of body position, and pharmacological treatment are now in use.
6.Effects of Children39;s Crawling-Promotion-Training-Robot on Rehabilitation of Children with Spastic Diplegia

Objective To observe the effects of Children39;s Crawling-Promotion-Training-Robot on gross motor function and cognitive function in children with spastic diplegia.Methods From January to December, 2017, 60 children with spastic diplegia were selected and randomly divided into three groups, with 20 cases in each group. All the groups received routine comprehensive rehabilitation therapy. In addition, group I received manual crawling training, group II was treated with crawler-training therapy, and group III was treated with Children39;s Crawling-Promotion-Training-Robot. They were treated ten minutes every day, five days a week for twelve weeks. Before and after treatment, the gross motor development, the muscle tension and cognitive function were evaluated with Gross Motor Function Measure Scale-88 (GMFM-88), modified Ashworth Scale (MAS) and developmental quotient (DQ) in Gesell Developmental Scale (GDS), respectively.Results After treatment, the overall percentage of GMFM-88 and the score of C dimension which were tightly tied to crawling and kneeling improved in all the groups (t>17.438, P<0.001), and the score was better in groups II and III than in group I (P<0.05), especially in group III (P<0.05); the score of MAS improved in all the groups (t>2.144, P<0.05), and no significant difference was found among them (F=0.199, P>0.05); the score of DQ in GDS improved in groups II and III (t>3.040, P<0.001), and the score was better in groups II and III than in group I (P<0.05), especially in group III (P<0.05).Conclusion Children39;s Crawling-Promotion-Training-Robot could improve the gross motor and cognitive function of children with spastic diplegia, which is better than manual crawling training and crawler-training.
