1.Effects of NANC neuro-transmitter on detrusor stability of lower urinary tract with obstraction

Objective To investigate the effects of nonadrenergic noncholinergic (NANC) neuro transmitter on the frequency and strength of the automatic contractility of detrusor strip. Methods Cystometry was performed to observe the occurance of detrusor instability (DI) after 6 weeks of Wistar rats LUT. Then the rats were sacrified and their detrusors were made into strips to observe the effect of NANC vasoactive intestinal peptide (VIP) and adenosine triphoate(ATP) on the frequency and the strength of automatic contractility. Results The occurrence rate of DI was 62% after 6 weeks of obstruction of LUT; VIP and ATP could significantly inhibit the automatic contraction ability of detrusor strip, decreasing the frequency and the strength of the automatic contraction of detrusor ( P 0.05). Conclusions VIP and ATP can significantly inhibit the frequency and strength of the automatic contractibility of normal and obstructive detrusor.
2.Application of Peritoneal Dialysis in Children of Congenital Heart Disease With Post-operative Acute Renal Insufficiency

Objective: To explore the effects of peritoneal dialysis on acute renal insufifciency and on relevant blood indicators in children with congenital heart disease (CHD) after the operation.
Methods: A total of 48 CHD patients received direct open heart surgery by cardiopulmonary bypass and suffered from post-operative acute renal insufifciency in our hospital from 2011-12 to 2014-12 were retrospectively analyzed. The patients were divided into 2 groups: Peritoneal dialysis group and Routine medication group,n=24 in each group. The differences of renal function indexes, the blood levels of electrolyte and inlfammatory factors were compared between 2 groups.
Results: Compared with Routine medication group, the patients in Peritoneal dialysis group presented decreased serum critinine, BUN and urine β2-micro globulin, 24-hour protein level,P<0.05; decreased blood K+ level and increased blood levels of Ca2+, HCO-3P<0.05; decreased plasma levels of hs-CRP, TNF-α, IL-6,P<0.05. The cure rate and mortality in Peritoneal dialysis group were better than those in Control group,P<0.05. No long term complication related to peritoneal dialysis was found.
Conclusion: Peritoneal dialysis may improve the renal function in CHD patients with post-operative acute renal insufifciency and optimize the blood levels of electrolyte and inlfammatory factors.
3.Relationship and mechanism between the serum level of HMGB1 and the mortality rate in patients with sepsis

Objective To investigate the relationship and mechanism between the serum level of high mobility group box protein-1(HMGB1)and the mortality rate in patients with sepsis.Methods The serum levels of HMGB1,superoxide dismutase(SOD)and malondialdehyde(MDA)in 48 patients with sepsis were determined.The clinical outcomes in those patients were recorded and anlyzed.Results After the onset of sepsis,the serum HMGB1 levels of both death group and survival group were increased gradually and peaked at 72h after the onset of the disease.The semm HMGB1 levels of death group were much higher than those of survival group except at 24h(t=6.07,6.20,24.43,all P<0.05).The activity of serum SOD of death group was markedly lower than that of survival group at 12h,24h,48h and 72h(t=10.24,20.61,11.67,33.33,all P<0.05),and the level of serum MDA of death group was significantly higher than those of survival group at all time points(t=26.06,22.17,23.86,9.49,5.95,all P<0.05).There was a significantly positive correlation between the serum HMGB1 and MDA level.Conlusioa The increase in serum HMGB1 level may be the important reasoll for the increased mortality rate in patients with sepsis;Oxidant/antioxidant imbalance may be olle reason for the increase in serum HMGB1 level.
4.Expression of HIF-1α and VEGF in synovium of knee joint in adjuvant-induce-arthritis rat

Objective:To investigate the expression of HIF-1 and VEGF and the correlation with pathological score of synovium in adjuvant-induce-arthritis rat knee joint.Methods: Models of adjuvant arthritis(AA)in rats were made successfully, the arthral pathological score was calculated, production of HIF-1α and VEGF protein was assayed by histoimmunchemical staining at different stage after CFA inoculation.Results:The HIF-1α and VEGF concentration in experimental group were higher than that in control group, and there was significant difference (P<0.01).And correlation between them was significantly(P<0.01), similar with pathological score of synovium(P<0.01).Conclusion:HIF-1α and VEGF play important roles in the development and progression of synovium of knee joint in AA rat.The interaction of them promote development and progression of neonatal vascular in synovium.
5.Short term results of laparoscopic versus open complete mesocolic excision for right-sided colon cancer

Objective To compare the clinical efficiency between laparoscopic and open complete mesocolic excision for right-sided colon cancer.Methods Between January 2011 and August 2012,a total of 134 patients with right-sided colon cancer who underwent CME at Fuzhou General Hospital of Nanjing Military Command were divided into laparoscopic (71) and open (63) groups.The intraoperative parameters,pathology,postoperative course and short-term outcomes were compared between groups.The chi-test and the student t test were used for statistics.Result There were no significant differences in the length of distal (P=0.427) and proximal margin (P=0.515),tumor diameter (P =0.440) and number of lymph nodes dissected (P =0.377).Postoperatively patients were followed for up to 12 months,no significant differences were found in local regional recurrence rates (4.2% vs 1.6%) (P =0.622) and distal metastasis rates (5.6% vs 3.2%) (P =0.684) between the two groups.The mean operative time (P =0.134) and postoperative complication rate (P =0.977) were similar.The mean intraoperative blood loss was less (P =0.000),bowel flatus passed earlier (P =0.000) and hospital stay shorter (P =0.000) in laparoscopic than that in open group.Conclusions Laparoscopic CME has the same oncologic clearance effects and short-term follow-up result compared with open CME for right-sided colon cancer,and laparoscopic CME is minimally invasive,less bleeding,less pain and quick recovery.

Objective To approach the efficacy of magnesium sulfate(MgSO4 )supplemantation to prevent junctional ectopic tachycardia(JET)during the early postoperative period in infant patients after sur-gery for congenital heart disease.Methods A total of 325 infant patients undergoing cardiac surgery with cardiopulmonary bypass were randomly divided into two groups:the trial group (n =162,50 mg /kg 25%MgSO4 )and the placebo group(n =163,normal saline).The serum magnesium concentrations were meas-ured at five time points.Continuous electrocardiographic documentation by Holter monitor was performed for 24 hours after cardiac sugery.Results At T3 time point(1 hour after 50 mg /kg 25% magnesium sulfate and equivalent saline applied in intravenous infusion),the concentration of serum magnesium in trial group was higher than that of the placebo group[(1.79 ±0.33)mmol /L vs.(0.92 ±0.18)mmol /L],and the differ-ence between two groups was significant(P =0.001 ).At T4 and T5 time points(after magnesium sulfate and equivalent saline applied 12 and 24 hours respectively),the concentrations of serum magnesium in trial group were throughout higher than those of the placebo group [T4:(1.24 ±0.2)mmol /L vs.(0.83 ±0.17 ) mmol /L;T5:(1.20 ±0.16)vs.(0.80 ±0.19)mmol /L,P =0.024,P =0.035,respectively].The overall incidence of JET was 5.2%(17 /325),the placebo group had a significantly greater occurrence of JET than that of the trial group,the difference was significant between two groups [9.2%(15 /163 )vs.1.2%(2 /162),P =0.013 ].The onset time of JET in placebo group was earlier than that of the trial group (P =0.019).A total of 14 of 17 cases(82.4%)with JET had hypomagnesaemia,the cases with JET had a signif-icant longer period of mechanical ventilation and CICU stay.Conclusion Positively supplementation with magnesium sulfate during the early postoperative period in infant patients after surgery for congenital heart diseaae could reduce the incidence of JET.Hypomagnesaemia is related to JET.
8.Diagnostic value of two kinds of imaging of extracranial carotid artery stenosis in patients with transient ischemic attack

Objective To evaluate the value of neck blood vessel colored doppler ultrasound (NBVCDU) and magnetic resonance angiography (MRA) to extracranial carotid artery stenosis in patients with transient ischemic attack (TIA).Methods After implementing NBVCDU and MRA examinations at the same time,45 TIA patients with at least one examination showing arteriostenosis in extracranial section were chosen to carry out cerebral digital subtraction angiography( DSA ),then the stenosis rate was calculated by American symptomatic carotid endarterectomy trial (NASCET) method.Results Regarding DSA as the gold standard,for 45 TIA patients that having 180 arteriostenosis in extracranial section, sensitivity,specificity,accuracy of NBVCDU examination was 93.51% ,95.15% ,94.44%, Kappa = 0.735; sensitivity,specificity,accuracy of MRA was 92.21% ,94.17% ,93.33% , Kappa =0.681; sensitivity,specificity,accuracy of NBVCDU combined with MPA was 97.40% ,99.03% ,98.33%, Kappa = 0.872.Conclusions The sensitivity and accuracy of arteriostenosis in extracranial section by NBVCDU examination is higher than that by MRA, and it is suitable in the crowd primary examination.NBVCDU combined with MRA has shown good consistence with DSA for diagnosing arteriostenosis in extracranial section,but can't replace DSA comlpetely.
9.Expression and significance of SIAH2 in epithelial ovarian carcinoma

Objective To analyze the expression and clinicopathological significance of seven in absentia homolog 2 (SIAH2) in epithelial ovarian carcinoma, and to discuss its role in the development of epithelial ovarian carcinoma. Methods The expression of SIAH2 was determined by immunohistochemical S-P method in 165 cases of ovarian samples and SIAH2 expression was examined by Western blot. In the combination with follow-up data, survival curves were calculated using the Kaplan-Meier method and relationship between SIAH2 expression and prognosis of ovarian carcinoma patients was analyzed by the log-rank test. Cox proportional hazards regression model was used to examine the independent predictive factors in the patients with epithelial ovarian carcinoma.Results SIAH2 expression in epithelial ovarian cancer (76.4%) was higher than that of borderline ovarian tumors (41.7%) , benign ovarian cysts (5.13%) and normal ovarian tissues (2.86%) , and there were significant differences (all P < 0.05). No statistical significance of SIAH2 expression was found between benign ovarian cysts and normal ovarian tissues (P> 0.05). The expression of the SIAH2 was significantly correlated to histological grade, FIGO stage and lymph node metastasis (P < 0.05).The relative expression of SIAH2 in normal ovarian tissue, benign ovarian cyst, borderline ovarian tumor and epithelial ovarian cancer was 0.12 ± 0.05, 0.11 ± 0.04, 0.57 ± 0.08 and 1.05 ± 0.10, respectively. No difference of SIAH2 expression was found between normal ovarian tissues and benign ovarian cysts (P> 0.05). The expression of SIAH2 increased from ovarian tissues/benign ovarian cysts to borderline ovarian tumor to epithelial ovarian cancer and the difference was statistically significant (all P < 0.05).The survival curves of patients with SIAH2 (+) differed from those of patients with SIAH2 (-) and the difference was statistically significant (P < 0.05). Multiple factor analysis revealed that the higher expression of SIAH2 was an independent prognostic factor for overall survival. Conclusions The over-expression of SIAH2 plays an important role in the tumorigenesis and progression of epithelial ovarian cancer. The over-expression of SIAH2 may serve as a biomarker for poor prognosis of epithelial ovarian cancer patients.

The 53rd annual meeting of the American Society of Clinical Oncology (ASCO) was held in Chicago,United States between June 2 and 6,2017.The latest advances in biliary tract cancer research from this meeting were summarized and analyzed in this paper.The adjuvant therapy in biliary tract cancer made a breakthrough in this meeting,the findings could provide the basis for a new standard of changing the current management model in the disease.The precision medicine and targeted therapy will be the development direction in the future.Doctors should attach great importance to the adjuvant and comprehensive therapy in biliary tract cancer and initiate high level multi-center clinical trials to improve the overall the diagnostic and treatment levels of biliary tract cancer.
