1.Investigation of Microcystin in Tai Lake Water and Fish

Objective To know the pollution situation by microcystin MC in Tai Lake in Wuxi. Methods The MC content in 12 water samples and 28 fish samples in the Tai Lake were determined by ELISA. Results The positive rates of MC in water samples and fish samples were 100%. The mean content of MC in water samples was higher in July 9 874.8 pg/ml than in November 142.8 pg/ml. The mean content of MC in the livers was 27.84 ng/g, in the muscle it was 3.97 ng/g. Conclusion The Tai Lake has been contaminated by microcystin and it can be detected in the fish living in the lake.
2.Cobalt-chromium alloy with zirconium nitride coating influences adhesion properties of bacteria

BACKGROUND:Bacterial adhesion is closely related to the surface properties of cobalt-chromium al oys, and therefore, the surface modification technology has become the focus of research in this area. OBJECTIVE:To verify whether the cobalt-chromium al oy with zirconium nitride coating can improve the bacterial adhesion of metal denture materials. METHODS:Magnetron sputtering deposition method was used to plate zirconium nitride film on the surface of cobalt-chromium al oys (experimental group), and cobalt-chromium al oy specimens uncoated served as control group. Streptococcus mutans, Candida albicans, and Actinomyces viscosus were respectively inoculated on the two kinds of test specimen, and at end of culture, the colony counting was done. RESULTS AND CONCLUSION:In the bacterial adhesion test, the number of colonies of three kinds of bacteria in the experimental group was significantly lower than that in the control group (P<0.05). The number of bacterial adhesion in the control group was significantly higher than that in the experimental group. These findings indicate that the cobalt-chromium al oy covered with zirconium nitride coating can significantly reduce the adhesion amount of Streptococcus mutans, Candida albicans and Actinomyces viscosus, and thus improve bacterial adhesion properties of cobalt-chromium al oys.
3.Clinical analysis of 42 patients with cesarean scar pregnancy

Objective To investigate the clinical features ,diagnosis and treatment of cesarean scar pregnancy (CSP) .Methods We retrospectively analyzed the medical history ,clinical manifestation ,diagnoses ,treatments and outcomes of 42 patients with ec‐topic pregnancy in the cesarean scar .Results All cases got diagnosed by transvaginal ultrasound .The error rate of first diagnosis was 40 .4% (17/42) .There were 25 cases of conservative therapy ,in which 12 cases were resolved with laparoscopic surgery and 1 case resolved with open surgery ;in the first process 15 cases were carried out laparoscopic surger in which 1 case were converted to laparotomy ;abdominal surgery were underwented in 1 case and were cured .Only one case underwent abdominal total hysterectomy as of failure after uterine artery embolization .After initial conservative treatment 3 cases were hospitalized again and 2 cases under‐went reoperation .41 patients were successfully retained the uterus and normal menstrual cycle returned at two months after opera‐tion .Conclusion The early diagnosis for CSP mainly depended on ultrasound and the thickness of incision muscle layer is an impor‐tant basis for choice of treatment .Checking the thickness of incision muscle layer for CSP mainly depended on ultrasound ,and lapa‐roscopic surgery is an ideal method for diagnosis and treatment of CSP .
4. Chronic spontaneous urticaria: therapeutic mechanism of omalizumab and assessment of its clinical efficacy

Chronic spontaneous urticaria (CSU) is characterized by recurrent wheals with severe itching, and greatly affects the life quality of patients. The European guideline on chronic urticaria recommends the anti-IgE monoclonal antibody omalizumab as the only third-line therapy for patients with CSU whose condition can not be controlled by high doses of antihistamines. Although a lot of researches have shown that omalizumab is effective and safe for the treatment of CSU, its therapeutic mechanisms have not yet been fully elucidated. This review summarizes therapeutic mechanisms of omalizumab in the treatment of CSU, and indices for predicting and monitoring its clinical efficacy.
5.The correlation between lean non-alcoholic fatty liver disease and metabolic parameters in a young and middle-aged population undergoing physical examination

Objective:To explore the correlation between lean nonalcoholic fatty liver disease (NAFLD) and metabolic indicators in a young and middle-aged population undergoing physical examination.Methods:It was a cross-sectional study. A total of 8 250 individuals who underwent routine physical examinations at the Health Medical Center of the Second Affiliated Hospital of Chongqing Medical University from January to December 2021 and met the inclusion and exclusion criteria were selected as the research subbjects. The general examination, fasting blood glucose, blood lipids, liver function, renal function, and fasting color ultrasound examination results were analyzed retrospectively to assess the correlation between lean NAFLD and major metabolic indicators using independent sample t-test, chi-square test, and multivariable logistic regression. Results:The prevalence of lean NAFLD was higher in men than in women (50.7% vs. 49.3%, χ2=97.261, P<0.001). After stratifying the age of onset of lean NAFLD, the peak age of onset was found to be between 45 and 59 years, with the prevalence gradually increasing with age. When stratified by body mass index (BMI), the peak incidence of lean NAFLD was observed in individuals with a BMI of ≥20 and <23 kg/m 2, with the prevalence showing a significant upward trend as BMI increased. The systolic blood pressure, diastolic blood pressure, total cholesterol, triglyceride, low-density lipoprotein, fasting blood glucose, alanine aminotransferase, aspartate aminotransferase, γ-glutamyltransferase, and serum uric acid in lean NAFLD groupwere all significantly higher than those in lean non-NAFLD group (all P<0.01), and the level of high density lipoprotein was significantly lower than that of lean non-NAFLD group ( t=23.755, P<0.001). The logistic analysis showed that systolic blood pressure ( OR=1.258, 95% CI: 1.081-1.465), diastolic blood pressure ( OR=1.282, 95% CI: 1.056-1.557), total cholesterol ( OR=1.712, 95% CI: 1.525-1.923), triglyceride ( OR=4.115, 95% CI: 3.621-4.676), alanine aminotransferase ( OR=1.467, 95% CI: 1.104-1.950), γ-glutamyltransferase ( OR=1.482, 95% CI: 1.242-1.769), fasting blood glucose ( OR=2.479, 95% CI: 2.092-2.939) and serum uric acid ( OR=1.390, 95% CI: 1.236-1.563) were independent metabolic risk factors for lean NAFLD (all P<0.05). Conclusions:The levels of various metabolic markers in young and middle-aged patients with lean NAFLD increase, and the risk of lean NAFLD increases. Metabolic markers are helpful to screen people at risk of lean NAFLD.
6.Protective effects of Elabela on kidney injury in db/db diabetic mice and its possible mechanism

Objective:To investigate the protective effects of Elabela(ELA) on the renal injury of db/db mice and its possible mechanism.Methods:Sixteen eight-week-old male db/db mice were intraperitoneally injected with ELA(5 mg·kg -1·day -1) or equivalent normal saline( n=8) for 8 weeks. Eight age-matched male db/m mice received equivalent normal saline injection as normal control. At the end of the experiment, blood and urine samples were obtained for HbA 1C and urinary albumin/creatinine(ACR) measurements. Immunohistochemistry was used to observe the expression of ELA. Histopathological changes in kidney tissue were observed by HE staining and Masson staining. The levels of collagen type Ⅳ(Col-Ⅳ) and transforming growth factor-β1(TGF-β1) as well as Yes-associated protein(YAP) phosphorylation in kidney tissue were examined by western blot. Results:Immunohistochemistry results showed that ELA expression was decreased in the renal tissue of db/db mice as compared with that of db/m mice( P<0.05). After ELA treatment, ACR and blood pressure were markedly decreased in db/db mice( P<0.05), but without significant changes in the body weight and HbA 1C. Renal tubular epithelial cells edema, basement membrane thickening, and increased collagen fiber in db/db were improved by ELA administration. Compared with db/m mice, the levels of TGF-β1 and Col-Ⅳ expression, as well as YAP phosphorylation were significantly increased in renal tissue of db/db mice(0.98±0.08 vs 0.68±0.10, 1.10±0.14 vs 0.51±0.08, 3.38±0.72 vs 0.81±0.13, all P<0.05), which were down-regulated after ELA administration(0.80±0.06, 0.51±0.05, 2.21±0.22, all P<0.05). Conclusion:ELA may improve the renal injury of db/db mice by regulating the signaling pathway of YAP, thereby delaying the development of diabetic nephropathy.
7.Signal Conversion and Isolation Processing Technology Used in the Cerebrovascular Stroke Detector.

By using Doppler sensor and pressure sensor, the cerebrovascular stroke detector can be used to measure the blood flow velocity and blood pressure of the carotid artery. In this study, a variety of signal conversion and isolation processing techniques are proposed for processing and feature extraction of the output signals from the sensors. Finally, effective signal output waveforms that can be used to evaluate the cerebrovascular hemodynamics index (CVHI) are obtained, and the sound signal outputs that can reflect the change characteristics of blood flow velocity and blood pressure signals are generated, which realizes the application functional requirements of the detector.

8.A Prospective Phase Ⅰ Clinical Study of Docetaxel with Concurrent Late-course Hyperfractionated Radiotherapy After Breast-conserving Surgery for Stage T1-T2 Breast Cancer

Objective To evaluate prospectively the side effects and tolerance of docetaxel with concurrent late-course hyperfractionated radiotherapy after breast-conserving surgery for stage T1-T2 breast cancer, and to assess the value of this treatment in shortening the treatment time and reducing the economic burden among patients. Methods A total of 20 patients with T1-T2 breast cancer were recruited after they underwent breast-conserving surgery. The acute radiation response classification, treatment completion rate, disease-free survival, hospital stays, and treatment costs were observed. Radiotherapy for all patients was started before the last single-agent docetaxel chemotherapy. Results The completion rate of treatment and the good rate of cosmetic effect reached 100%. The main adverse reactions were hematological toxicity (leukopenia) and skin reactions, which were tolerated. The median follow-up time was 30.1 months, and the follow-up rate was 100%. The average total treatment time of this hyperfractionated radiotherapy with concurrent docetaxel was four weeks, and the total hospitalization cost savings was approximately 10, 000 yuan. The 21-month disease-free survival rate was 100%. Conclusion Stage T1-T2 breast cancer can tolerate hyperfractionated radiotherapy with concurrent chemotherapy after a breast-conserving operation. The procedure results in good local control and satisfactory cosmetic effects, with high health and economic value.
9.Oncologic outcomes of early stage cervical cancer performed operation by different laparoscopic surgical procedures: analysis of clinical data from mutiple centers

Objective:To evaluate the oncologic outcomes of different laparoscopic radical hysterectomy.Methods:From January 2011 to December 2014, the laparoscopic operation cases of cervical cancer at stage Ⅰb1, Ⅰb2, Ⅱa1 and Ⅱa2, including the histologic subtypes of squamous-cell carcinoma, adenocarcinoma and adenosquamous carcinoma, were collected in five clinical centers. The data were divided into two groups according to the surgical procedures, that is, modified laparoscopic-vaginal radical hysterectomy (mLVRH) and total laparoscopic radical hysterectomy (TLRH). The overall survival rate (OS), disease-free survival rate (DFS) at 5 years were retrospectively analyzed in this study.Results:There were 674 cases in total, including 377 cases of mLVRH, 297 cases of TLRH. (1) The OS at 5 years: the mLVRH was 96.1% and the TLRH was 92.0%, and the mLVRH was higher than that of TLRH ( P=0.010). Stratify analysis, including stage of disease (Ⅰb1 and Ⅱa1), histologic subtypes (squamous-cell carcinoma, adenocarcinoma), lymph node metastasis, revealed that, ① Stage of disease: in stage Ⅰb1, the OS at five years of mLVRH was higher than that in TLRH group (98.6% vs 93.6%, P=0.012). In stage Ⅱa1, there was significant difference between the two groups, the OS at five years of mLVRH and TLRH were 93.6% and 77.6% ( P=0.007). ② Histologic subtypes: for the OS at five years of squamous-cell carcinoma, mLVRH and TLRH were 96.1% and 92.3%, and there was significant difference ( P=0.046); for adenocarcinoma, the OS at five years were 91.0% and 88.6%, and there was no difference between two groups ( P=0.230). ③ Lymph node metastasis: the mLVRH and TLRH with lymph node metastasis, the OS at five years were 98.6% and 96.4%; the mLVRH and TLRH without lymph node metastasis, the OS at five years were 89.3% and 80.8%. There were no significant differences between the two groups,respectively ( P=0.156, P=0.093). (2) The DFS at 5 years: there was no significant difference between mLVRH and TLRH (94.1% vs 90.9%, P=0.220). Stratify analysis for stage of disease, the mLVRH group was higher than that in the TLRH group in stage Ⅰb1 (97.0% vs 92.8%, P=0.039). However, for stage Ⅱa1, there was no significant difference between mLVRH and TLRH group (88.2% vs 75.8%, P=0.074). Conclusions:The results of this retrospective study indicated that different laparoscopy surgical procedures had diverse oncologic outcomes. The OS at 5 years of the mLVRH is superior to the TLRH. The DFS at 5 years in Ⅰb1 stage, the mLVRH is higher than the TLRH. Therefore, the modified laparoscopy is still an alternative surgery for early cervical cancer patients when following the principle of no-tumor-exposure.
10.Clinical application of critical temperature threshold measurement in cold contact urticaria

Objective:To investigate clinical application of critical temperature threshold (CTT) measurement by using a temperature tester in the diagnosis of cold contact urticaria, and to explore relationships of CTTs with disease activity, disease control condition and quality of life in patients with cold contact urticaria.Methods:A total of 20 patients with cold contact urticaria were collected from Department of Dermatology, Peking University First Hospital from October 2017 to March 2019. Disease activity was assessed by the patients themselves at the first visit; the CTT was measured, and dermatology life quality index (DLQI) was evaluated by the patients themselves at the first visit and subsequent 2 follow-up visits. At the second follow-up visit, disease control condition was evaluated in the patients. Spearman correlation analysis was used to analyze correlations of CTTs with disease activity, CTT improvement and disease control condition. Friedman test was used to compare the improvement of CTTs and DLQI during treatment, and Bonferroni multiple test was used for multiple comparisons.Results:Based on the patients′ self-assessed disease activity, there were 3 patients with mild cold contact urticaria, 8 with moderate cold contact urticaria, and 9 with severe cold contact urticaria, and the disease activity was positively correlated with CTTs ( r s = 0.573, P = 0.008) . Clinician assessment of disease control condition showed 3 patients with poor disease control, 6 with fair disease control and 9 with good disease control, and the disease control condition was positively correlated with the improvement of CTTs ( r s = 0.516, P = 0.020) . The CTTs were (20.10 ± 4.67) ℃, (10.75 ± 5.30) ℃, and 5.50 (4.00, 10.75) ℃ (expressed as median[ P25, P75]) at the first visit and 2 follow-up visits respectively, and significantly differed among different visits ( χ2 = 34.16, P < 0.001) ; meanwhile, the DLQI scores were 19.75 ± 3.81, 8.45 ± 6.27, 0.50 (0, 9.00) (expressed as median[ P25, P75]) respectively, and there was a significant difference among different visits ( χ2 = 35.23, P < 0.001) ; the CTT and DLQI were significantly lower at the 2 follow-up visits than at the first visit (all P < 0.001) . Conclusion:CTTs can reflect disease activity in patients with cold contact urticaria, and are correlated with the quality of life and disease control condition.
