1.Scalp soft tissue expansion in combination with skin flap and hair transplantation for cicatricial alopecia with large area in 36 cases

BACKGROUND: Cicatricial alopecia always occurs following the scalp trauma, and the optimal repairing method is scalp soft tissue expansion. OBJECTIVE: To verify the repairing effect of scalp soft tissue expansion therapy on the large-scale cicatricial alopecia. DESIGN, TIME AND SETTING: A case report was carried out in the Department of Burn and Plastic Surgery at the 180 Hospital of Chinese PLA (Quangzhou, Fujian, China) between January 1996 and April 2008. PARTICIPANTS: Thirty-six patients of cicatricial alopecia were adopted, with the size arranging 15 cm?9 cm to 24 cm?14 cm. METHODS: Scalp soft tissue expansion was performed in 36 cases of cicatricial alopecia. In later stage, the residual alopecia with small area was removed by scar excision plus suture for several times, treated with transposition repair of local flap and autologous hair transplant carrying hair follicle. The largest area to repair alopecia reached 336 cm2. MAIN OUTCOME MEASURES: Postoperative complications, repairing effect and cicatrix restitution were observed after operation; The elimination of alopecia area, hair density and appearance were detected during follow-ups. RESULTS: Psilotic scar eliminated in all cases, but 11 cases underwent scalp scar excision, transposition of local flap and autologous hair transplant because of wide incisional scar or residual small alopecia. No obvious complications occurred. It was found by a postoperative follow-up of 1-3 years that hair growth was good. CONCLUSION: Skin soft tissue expansion is an ideal method to repair cicatricial alopecia with large area. In later stage, the combination of simple repair methods can obtain better curative effect.
2.Significance of total bilirubin/albumin in predicting bilirubin neurotoxicity

Objective To investigate the significance of total bilirubin/albumin in predicting bilirubin neurotoxicity.Methods Eighty-three cases with hyperbilirubinemia who treated from May 2007 to August 2010 were selected,the serum total bilirubin and albumin were detected and total bilirubin/albumin was calculated.According to brainstem auditory evoked potential(BAEP)results,the patients were divided into normal BAEP group and abnormal BAEP group and compared.Results There were 27 cases of abnormal BAEP group and 56 cases of normal BAEP group.Total bilirubin and total bilirubin/albumin in abnormal BAEP group were higher than those in normal BAEP group[(356.50±59.23)μmol/L vs.(318.70±55.12)μmol/L,(5.02±0.49)×10-3 vs.(4.56±0.43)×10-3],the differences were significant (P<0.05).Multiple regression analysis showed:abnormal BAEP was closely related to total bilirubin/albumin(r=0.72,P<0.05),whih abnormal BAEP was not obviously related to total bilirubin(r=0.19,P>0.05).Conclusion Total bilirubin/albumin can reflect serum unconjugated bilirubin level of neonatus with hyperbilirubinemia better than total bilirubin,and it can can be taken as one index to evaluate the risk factors of bilirubin neurotoxicity.
3.The tidal breathing lung function in very low birth weight preterm infants with bronchopulmonary dysplasia

Objective To investigate the development of tidal breathing lung function in very low birth weight infants with bronchopulmonary dysplasia(BDP).Methods Two hundred and sixty-two very low birth weight infants hospitalized in Wenzhou Children′s Hospital were enrolled as objects.All infants were given the tidal breathing pulmonary function test within a week before discharge and at the corrected age of 6 to 8 months.According to clinical diagnosis,they were divided into BPD group(n=65) and non-BPD group(n=197),and BPD group were divided into mild group(n=31),moderate group(n=20) and severe group(n=14) according to the severity.The lung function indexes were compared between different groups.Results The results of the test within a week before discharge displayed that the respiratory rates in the BPD groups were higher than those in the non-BPD group(all P<0.05).The peak expiratory flow(TPEF),tidal expiratory flow at 75%,50%,and 25% tidal volume(TEF75,TEF50,TEF25) in moderate to severe BPD group were higher than those in other groups(all P<0.05),while in mild BPD group were higher than those in non-BPD group(all P<0.05).The ratio of time to peak tidal expiratory flow to total expiratory time (TPEF/TE),ratio of volume to peak tidal expiratory flow to total expiratory volume (VPEF/VE) were lower in BPD group than those in non-BPD group,the decline was more obvious as the BPD was more serious(all P<0.05).No significant differences were found between groups in tidal volume(P>0.05).The result at the corrected age of 6 to 8 months displayed that TPEF,TEF75,TEF50,TEF25 in severe BPD group were still higher than those in any other group(all P<0.05),while TPEF/TE,VPEF/VE were lower than those in any other group(all P<0.05),no significant differences were found between other groups in other indicators(P>0.05).Conclusion Preterm infants BPD have different degrees of lung injury,but as the age grows some lung function indexes can improve gradually(at the corrected age of 6 to 8 months).However,early severe small airway obstruction is still serious.Therefore,it′s important to prevent and treat BPD for the prevention and regulation of respiratory tract disease in future.
4.Effects of early minimal enteral nutrition on the immune system in newborn infants under mechanical ventilation with intratracheal intubation

Objective To study the effect of minimal enteral nutrition on the immune system of newborn infants under mechanical ventilation (MV) with intratracheal intubation.Methods Eighty-one patients under MV with intratracheal intubation were divided into two groups:treatment group and control group.Forty patients in treatmet group accepted early minimal enteral nutrition,while other 41 patients in control group accepted normal feeding,then T-lymphocytes (CD3,CD4,CD8),CD4/CD8,immunoglobulins (IgG,IgA,IgM),occurane of secondary infection,time of beginning fully enteral feeding,occurance of abdominal distension were recorded.Results There were no significant differences of T-lymphocytes (CD3,CD4,CD8),CD4/CD8,immunoglobulins (IgG,IgA,IgM) changes between the two groups in which the time of patients beginning MV under intratracheal intubation was less than five days (P > 0.05) ; while comparing patients in the two groups whose MV time was equal or greater than five days,the changes of T-lymphocytes CD3[(37.3 ± 1.6)%,(42.6 ±2.8)%],CD4[(29.1 ±1.7)%,(34.6 ±2.1)%],CD4/CD8 (17.9 ±1.8,18.9 ± 1.6),immunoglobulins IgA[(55.42 ±3.98) mg/L,(129.00 ±7.76) mg/L],IgM[(130.29 ±10.92) mg/L,(317.33 ± 11.64) mg/L] in treatment group were more significant than those in control group(P <0.05) ;furthermore the differences of the secondary infection occurance,the time beginning fully enteral feeding,intravenous nutrition and hospital stays in the treatment group [20% (8/40),(8.56 ± 1.78) d,(10.56 ± 1.78) d,(12.63 ± 1.73) d] were lower significantly than those in the control group [48.78% (20/41),(10.20 ± 1.65) d,(12.15 ± 1.69) d,(14.15 ± 1.64) d] (P < 0.05).These were no significant differences on abdominal distension and gastric retention between the treatment group [20% (8/40),17.5% (7/40)]and the control group [24.39% (10/41),19.51% (8/41)] (P > 0.05).Conclusion Early minimal enteral nutrition in infants under long time MV with intratracheal intubation could improve immune function recovery,reduce the occurance rate of secondary infection,shorten the time beginning fully enteral feeding,intravenous nutrition and hospital stay.
5.Fluorescent quantitative analysis on the expression of miRNA-34s in human skin keloid tissue

BACKGROUND:Understanding the difference of miRNA-34s expression in normal tissue and tumor tissue wil contribute to screen out a miRNA with high sensitivity as the specific tumor molecular marker.
OBJECTIVE:To investigate the differential expression of miRNA-34s (miR-34a/b/c) between normal skin and keloid tissue using real-time fluorescent quantitative PCR, and to evaluate the role and mechanisms of miRNA-34s in keloid formation and development.
METHODS:Ten cases of keloid tissue and two cases of normal skin tissue were col ected as specimens. Total RNAs were extracted from keloid and nomal skin tissue by Trizol method, and miRNA-34s were further isolated by Ambion’s miRNA Isolation Kit. Real-time fluorescent quantitative PCR was applied to verify expression levels of microRNA-34s (miR-34a/b/c) in keloid tissue and normal skin tissue.
RESULTS AND CONCLUSION:miRNA-34s (miRNA-34a/b/c) expression was down-regulated in keloid tissue compared with normal skin tissue (P<0.01). The findings showed that miRNA-34s (miRNA-34a/b/c) are involved in keloid formation and development, and down-regulation of the family member may result in neoplastic growth of keloid.
6.Eficacy of levothyroxine replacement therapy in children with hypothyroidism

Objective To investigate the clinical efficacy of levothyroxine replacement therapy in children with hypothyroidism.Methods 45 children with hypothyroidism admitted from Apr.2015 to Dec.2016 were selected as observation group,and 45 healthy children in the same period were selected as the control group.Children in the control group received regular diet and were paid attention to nutritional balance.Children in the observation group were given levothyroxine sodium on the basis of dietary guidance.The two groups were treated continuously for 6 months.The levels of thyroid function,insulin-like growth factor,indicators for growth and development and inflammatory factors were measured before and after treatment in both groups.Results After treatment for 3 months and 6 months,the levels of FT3,FT4,IGF-1,IGFBP-3 and IGF-1/IGFBP-3 in the observation group increased significantly compared with those before treatment,and the levels of TSH in the observation group decreased significantly(P<0.05).The levels of FT3,lFT4,TSH,IGF-1,IGFBP-3 and IGF-1/IGFBP-3 in the observation group after treatment for 6 months were (7.17±0.06)pmol/L,(2.74±0.07)mU/L,(207.67±9.06)μg/L,(0.46±0.02) mg/L and(34.59±1.97).The difference was not statistically significant (P>0.05).After 3 months and 6 months of treatment,the height,body weight,osteocalcin and 25-(OH)2D3 levels in the observation group were significantly higher than those in the control group (P<0.05),and there was no significant difference between the two groups after 6 months (P>0.05).The difference was statistically significant (P<0.05).Conclusion Left thyroid hormone replacement therapy can significantly improve the thyroid function of children with hypothyroidism,increase the level of thyroid-associated hormone and insulin-like growth factor,promote the growth and development of children,which is worthy of clinical application.
7.Molecular identification of human/goat xenogeneic model.

OBJECTIVETo identify the human hematopoietic stem cells from the human/goat xenogeneic model with molecular techniques.
METHODSDNA and total RNA were extracted from 11 transplanted goat peripheral blood cells. Human CD(34), GPA and SRY genes were amplified with PCR in these samples, and CD(34), GPA mRNA transcripts were detected using RT-PCR in 5 and 6 goat peripheral blood cells, respectively. Southern blot analysis was performed in 8 goat DNAs to detect the human specific alpha-satellite sequence. Meanwhile FISH was also performed to detect the human cells in goat blood with a probe of human Y chromosome.
RESULTSHuman CD(34) and GPA genes could be detected with PCR in all the 11 goats, and SRY gene did in 5 goats transplanted with hematopoietic stem cells derived from male human babies. Southern blot showed that human specific alpha-satellite sequence was present in 8 goats. By RT-PCR, human CD(34) mRNA was detected in 5 experimental goats, GPA mRNA was found in the other 6 experimental goats and FISH assay showed that some peripheral blood cells of the human/goat xenogeneic model were positive.
CONCLUSIONExistence of human cells in the recipient goats was identified by molecular detection, which was feasible for the examination of human/goat xenogeneic models.

8.Appropriate age of primary and secondary school students for Cardiopulmonary resuscitation training

Objective To investigate the appropriate age of primary and secondary school students for cardiopulmonary resuscitation (CPR) training.Methods A total of 437 students aged 9-15 years at 3 to 6 grade in the primary schools or 1 to 2 grade in the secondary schools were selected from 2 Yuyao primary and secondary schools by stratified random sampling between March 2017 and January 2018.The numbers of students with the age of 9,10,11,12,13,14 and 15 y were 61,62,66,64,63,63 and 58,respectively.All students received chest compression training provided by Yuyao emergency department People's Hospital according to the 2015 Cardiopulmonary Resuscitation Guidelines.The training included 30 min theoretic teaching and 6 min practice in the simulator.The quality of chest compression performed by students was assessed;the depth,rate,position and retention of chest compression were recorded.Results The mean depth of chest compression in the students aged 9-15 years was 3.8,4.1,4.6,5.1,5.2,5.6 and 5.6 cm,respectively;the accuracy rate was 24.6%(14/61),25.8% (16/62),50.2% (33/66),70.5% (45/64),79.4%(50/63),88.9%(56/63) and 91.4(53/58),respectively.Compared with the students aged 9-11 years,the mean depth of chest compression was significantly increased and accuracy rate was significantly improved in the students aged 12-15 years (Compared with 9-y students,t=-8.936,-9.502,-10.640 and-11.370;x2=35.019,47.599,63.013 and 65.671;compared with 10-y students,t=-6.927,-8.179,-10.70 and-11.047;x2=24.977,35.967,50.916 and 52.727;compared with 1 1-y students,t=-3.095,-4.177,-6.785 and-6.995;x2=5.586,12.114,22.786 and 24.870;all P<0.05).The mean rate of chest compression was 110-116/min and its accuracy rate was 86.4%-95.2%;the accuracy rate of chest compression position was 90.9%-96.8% in all students,there were no significant differences among the 7 groups.The mean retention rate of chest compression in the 7 groups was 81.3%(122/150),67.3%(101/150),64.7% (94/150),48.0%(72/150),48.7%(73/150),33.3%(50/150) and 27.3%(41/150),respectively.Compared with the students aged 9-11 years,the mean retention rate of chest compression was significantly decreased in the students aged 12-15 years (compared with the 9-y students,x2=36.472,35.179,70.64 and 119.92;compared with 10-y students,x2=11.483,10.728,34.682 and 72.150;compared with 11-y students,x2=6.528,5.927,25.855 and 59.11;all P<0.05).Correlation analysis showed that the depth (r=0.96,0.89,0.91 and 0.86;P<0.01) and retention rate (r=-0.99,-0.90,-0.93 and-0.86;all P<0.01) of chest compression were significantly associated with the age,body weight,height and body mass index of students.Conclusion The students with an age of 12 years or more are able to effectively perform chest compression;thus,12 years and above might be the appropriate age for CPR training.
9.The clinical value of MRI in quantitative evaluation of anterior cruciate ligament mucoid degeneration

Objective:To explore the clinical application value of MRI in quantitative evaluation of anterior cruciate ligament mucoid degeneration (ACL-MD).Methods:From March to July 2020, 40 patients who were scheduled to undergo arthroscopic treatment were prospectively collected in the First Affiliated Hospital of Kunming Medical University.The anterior cruciate ligament tissue from the lateral edge of the tibial end was taken during the operation. Based on the pathologicalre sults, the patients were divided into the ACL-MD group ( n=19) and the normal group ( n=21). The sagittal plane three-dimensional steady-state rapid precession (3D-FIESTA), T 1 mapping, T 2 mapping, and T 2* mapping were performed before the knee joint surgery, and the scanned images were post-processed and analyzed to measure the T 1, T 2, and T 2* values of the tibial end of the anterior cruciate ligament.The relaxation time of the ACL-MD group and the normal group was compared using independent sample t test. The ROC curve was drawn using each parameter and the areas under the curve (AUC) for the diagnosis of ACL-MD were obtained.DeLong test was used to compare the differences of AUCs. Results:The T 1 [(1 291.9±273.4) ms], T 2 [(54.8±10.6) ms], and T 2* values [(30.6±6.4) ms] of anterior cruciate ligaments in the ACL-MD group were significantly higher than those in the normal group [ (1 087.0±121.0), (44.8±7.1), (20.4±4.8) ms; t=3.011, 3.473, 5.658, all P<0.001]. The AUCs of T 1, T 2, T 2* were 0.747, 0.764, 0.912, sensitivity of 63.2%, 63.2%, 100%, and the specificity of 100%, 95.2%, 76.2% in diagnosing ACL-MD. The AUC of the T 2* value was higher than those of the T 1 and T 2 values, and the differences were statistically significant ( Z=1.734, 2.162, P=0.043, 0.031). Conclusion:T 1, T 2, T 2*values measured by MRI quantitative imaging have high performance in assessing knee joint ACL-MD, and T 2* value has the largest AUC and the highest diagnostic efficiency.
