Objective: To investigate the bone healing of rigid internal fixatio n(RIF) during Le Fort I Osteotomy.Methods: We regarded rabbits as study objects and evaluated the form of bone healing by bone histomorphometry.Results:There was significant difference in the degree of the new bone formation between groups,and the degrees in group A were much higher than that in group B at any stages .Conclusion:The use of RIF in orthognathic surgery can speed the bone healing.`
2.Valuation of dynamic MR imaging for diagnosis of pituitary microadenomas

Objective To evaluate the use of keyhole dynamic magnetic resonance (MR) imaging in evaluation of pituitary microadenomas. Methods Fifty three patients with pituitary microadenomas proved by operation or clinical findings such as headache, amenorrhea, lactating, fat or acromegaly were retrospectively studied on dynamic MR, conventional contrast enhanced MR, and unenhanced MR. Then the image data were analyzed by two experienced doctors who didn′t know the details and three degrees were made as follows: grade 0, no evidence of the tumor; gradeⅠ, asymmetry signals in the pituitary; and gradeⅡ, nodulated or sheets signals in the pituitary. The grading data were compared with statistical methods. Results In dynamic MR grading system: grade 0 was revealed in 1 patient, gradeⅠ in 29 patients and gradeⅡ in 23 patients. In conventional enhanced MR: grade 0 in 26 patients, grade Ⅰ in 17 patients and gradeⅡin 10 patients. In conventional unenhanced MR: grade 0 in 39 patients, grade Ⅰin 10 patients and grade Ⅱ in 4 patients. The diagnostic rate of dynamic MR, conventional enhanced MR, and unenhanced MR was 98.1%, 51.9%, and 26.4%, respectively. ( P

Objective To study the pathogenesis of pathogens of sexually transmitted disease (STD) in acute pelvic inflammatory disease (PID) Methods Neisseria gonorrhoeae, chlamydia trchomatis, ureaplasma urealyticum were cutured respectively in 130 patients with acute PID Results The pathogens of STD had been found in 55 patients The detective rate of Neisseria gonorrhoeae was 6 9% (9/130), about half of them were mixed infection; the detective rate of chlamydia trachomatis was 4 6% (6/130), all of them were mixed infection; the positive percentage of ureaplasma urealyticum was 37 7% (49/130), one third of them were mixed infection Conclusion The pathogens of STD might cause acute PID or only play a synergetic role in it
4.Comparison of the efficacies of open and laparoscopic surgery in the treatment of adult patients with congenital choledochal cyst

Objective To explore the clinical efficacies of open and laparoscopic surgery in the treatment of adult patients with congenital choledochal cyst.Methods The clinical data of 36 adult patients with congenital choledochal cysts who were admitted to the Sir Run Run Shaw Hospital from February 2008 to February 2015 were retrospectively analyzed.Twenty-four patients underwent common bile duct cyst resection + cholecystectomy +biliojejunal Roux-en-Y anastomosis (open surgery group),12 patients underwent laparoscopic common bile duct cyst resection + cholecystectomy + biliojejunal Roux-en-Y anastomosis (laparoscopy group).The intraoperative conditions,detection indexes and postoperative recovery indexes in the 2 groups were analyzed.The follow-up by regular outpatient examination and telephone interview were done up to March 2015.Non-normal distribution data were described as M(P25,P75).Comparison between groups was analyzed using the Mann-Whitney U test and Fisher exact probability.Measurement data with normal distribution were presented as (x) ± s and analyzed using t test.Count data were evaluated by the ratio and proportion.Results The operation time of the open surgery group and laparoscopy group were 238 minutes (191 minutes,283 minutes) and 270 minutes (225 minutes,326 minutes),with a significant difference between the 2 groups (Z =-1.360,P > 0.05).The volume of intraoperative blood loss in the open surgery group and laparoscopy group were 200 mL (113 mL,363 mL) and 75 mL(50 mL,138 mL),with a significant difference between the 2 groups (Z =-3.377,P <0.05).The level of C-reactive protein in the open surgery group and laparoscopy group were 94 mg/L (81 mg/L,104 mg/L) and 29 mg/L (21 mg/L,61 mg/L),with a significant difference between the 2 groups (Z =-4.296,P <0.05).The pain scores at postoperative day 3,time to anal exsufflation,time for fluid diet intake and duration of hospital stay of the open surgery group and laparoscopy group were 2.5 ± 0.9,(3.7 ± 1.0) days,(5.0 ± 1.6) days,(10.0 ± 2.7) days and 1.9 ±0.3,(2.6 ± 1.0) days,(3.6 ± 1.6) days,(7.8 ± 2.2) days,respectively,showing significant differences between the 2 groups (t =3.146,3.286,2.450,2.523,P < 0.05).Nine patients in the open surgery group and 3 patients in the laparoscopy group had the complications,with improvement of conditions after symptomatic treatment,showing no significant difference between the 2 groups (P > 0.05).All the 36 patients were followed up for a median time of 36 months (range,1-72 months) with good recovery,without long-term postoperative complications and re-operation.Conclusion Laparoscopic surgery in the treatment of adult patients with congenital choledochal cyst is safe and feasible,with the advantages of less intraoperative bleeding,less surgical trauma,fast recovery compared with open surgery.
5.Assessment of left ventricular twist in rabbits with ventricular aneurysm formed by two-dimensional speckle tracking echocardiography

ObjectiveTo evaluate the influence on left ventricular(LV) twist,and the relationship between LV twist and function in the rabbits with ventricular aneurysm formed after myocardial infarction by two-dimensional ultrasound speckle tracking imaging.MethodsTwenty-six healthy rabbits were divided into two groups,six as control group,twenty as experimental group.The model of myocardial infarction and ventricular aneurysm were established by ligaturing left anterior descending coronary artery and left circumflex artery.After four weeks,according to the pathological examination of whether ventricular aneurysm formed or not,the experimental group was divided into myocardial infarct group and ventricular aneurysm group.Echocardiography was performed to measure rotations (at the levels of mitral annulus and apex in LV short-axis view),LV twist,left ventricular cavity size and function.ResultsCompared with the myocardial infarct group,LV end-diastolic diameter(LVEDD) and LV end-diastolic volume(LVEDV) were larger in the ventricular aneurysm group,but LV ejection fraction (LVEF),LV fractional shortening (LVFS),apical rotation and LV twist were lower in the ventricular aneurysm group than the myocardial infarct group.Both apical rotation and LV twist had shown a great correlation with LVEF.The conclusion was also shown in the ROC curve that sensitivity and specificity of diagnosis cardiac dysfunction were 76.9% and 80.0% respectively when the apical rotation was 1.21°.Conclusions There was a closely relationship between the change of apical rotation and the LV systolic function.When the apical rotation dropped to 1.21°,an apparent decline of LV systolic function was implied.
6.Biocompatibility and osteoinductive activity of nano-hydroxyapatite/chitosan/poly(lactide-co-glycolide) scaffoldsin vitro

BACKGROUND:Studies have found that combination of two of chitosan (CS), nano-hydroxyapatite (nHA) and poly(lactide-co-glycolide) (PLGA) can improve the mechanical properties and biocompatibility of the composite stent in certain extent as wel as improve osteogenic differentiation of the cels, but there is a certain distance from the ideal bone tissue engineering scaffolds.
OBJECTIVE:To study biocompatibility and osteoinductive activity of nHA/CS/PLGA scaffolds with different proportions in vitro.
METHODS: nHA/CS/PLGA scaffolds were prepared at mass ratio of 10:10:80, 10:20:70, 20:10:70 respectively by particle leaching method. And human bone marrow stem cels (hBMSCs) were co-cultured with these scaffolds in vitro. Adhesion, proliferation, and osteoinductive activity of these scaffolds were examined qualitatively and quantitatively by growth curve of hBMSCs on scaffolds. Gene expression of alkaline phosphatase activity and osteocalcin was detected by RT-PCR.
RESULTS AND CONCLUSION: hBMSCs could be attached, proliferated, and osteoinduced better on the nHA/CS/PLGA scaffold with the mass ratio of 20:10:70, compared to the other two groups of scaffolds. The differences were significant statisticaly (P< 0.05). Alkaline phosphatase and osteocalcin expressions were respectively higher in the scaffold with the mass ratio of 20:10:70 after 9-27 days of co-culture and 15-27 days of co-culture, in comparison with the other two groups of scaffolds. These findings indicate that the nHA/CS/PLGA scaffolds with the mass ratio of 20:10:70 demonstrated preferable biocompatibility and osteogenic inductivity, which is expected to be a promising scaffold material for bone tissue engineering.
7.Clinical experience in facial nerve tumors:A review of 27 cases

Objective:To analyze the clinical manifestations and the diagnosis of the facial nerve tumor according to the clinical information, and evaluate the different surgical approaches depending on tumor location.Method:Twenty-seven cases of facial nerve tumors with general clinical informations available from 1999.9 to 2006.12 in the Shanghai EENT Hospital were reviewed retrospectively.Result:Twenty(74.1%) schwannomas,4 (14.8%)neuofibromas ,and 3(11.1%)hemangiomas were identified with histopathology postoperatively.During the course of the disease,23 patients(85.2%)suffered facial paralysis,both hearing loss and tinnitus affected 11 (40.7%)cases,5(18.5%)manifested infra-auricular mass and the others showed some of otalgia or vertigo or ear fullness or facial numbness/twitched. CT or/and MRI results in 24 cases indicated that the tumors originated from the facial nerve.Intra-operative findings showed that 24(88.9%)cases involved no less than 2 segments of the facial nerve,of these 24 cases 87.5%(21/24)involved the mastoid protion,70.8%(17/24)involved the tympanic protion, 62.5%(15/24)involved the geniculate ganglion, only 4.2%(1/24)involved the internal acoustic canal(IAC),and 3 cases (11.1%)had only one segments involved. In all of these 27 cases ,the tumors were completely excised,of which 13 were resected followed by an immediate facial nerve reconstruction,including 11 sural nerve cable graft,1 facial nerve end-to-end anastomosis and 1 hypoglossal-facial nerve end-to-end anastomosis.Tumors were removed with preservation of facial nerve continuity in 2 cases.Conclusion:Facial nerve tumor is a rare and benign lesion,and has numerous clinical manifestations.CT and MRI can help surgeons to make a right diagnosis preoperatively.When and how to give the patients an operation depends on the patients individually.
8.Meta-analysis on laparoscopy-assisted total gastrectomy for gastric cancer

Objective To evaluate the safety and efficacy of laparoscopy-assisted total gastrectomy (LATG) for gastric cancer by systematic review and meta-analysis.Methods The literature database before February,2012 was extensively searched to retrieve the comparative studies of LATG and open total gastrectomy(OTG) with a relevance of study goal.The inclusion and exclusion criteria were formulated.Aftera quality evaluation,the data was extracted.The Cochrane collaboration RevMan 5.1 version software was used for meta-analysis.Results There are ten studies meeting the inclusion criteria for meta-analysis.The total sample size of these studies was 942 cases.Compared to OTG,LATG experienced longer operation time [weighted mean difference(WMD) =41.12 min,95% confidence interval(CI):20.62-61.63,P <0.01)],less blood loss(WMD =-198.36 ml,95% CI:-300.94--95.78,P < 0.01),earlier time to flatus(WMD =-0.80 days,95% CI:-1.17--0.42,P<0.01),shorter hospital stay(WMD =-4.02days,95% CI:-6.03--2.01,P < 0.01) and decrease in overall complications [relative risk (RR) =0.55,95% CI:0.40-0.76,P < 0.01)].The number of dissected lymph nodes,proximal or distal resection margin and mortality were similar between the two groups.Conclusions Laparoscopy-assisted total gastrectomy is a safe and feasible procedure with less blood loss,less overall complications and quick recovery.
9.Cloning of PTD-NPY fusion gene and its secretory expression in Pichia pastoris.

The PTD-NPY fusion gene derived from HIV-1 TAT protein transduction domain and rat neuropeptide Y was amplified by overlap extension PCR, digested and subcloned into yeast expression vector pPICZ alpha A to construct recombinant expression plasmid pPICZ alpha-PTD-NPY. The cloned PTD-NPY fusion gene was identified by PCR and restriction enzyme digestion and sequenced. The exact recombinant plasmid was linearized by Sac I and integrated by electrotransformation into the genome of Pichia pastoris GS115 cells. Then, these positive recombinant yeast cells were induced by 10 mL/L methanol to express soluble PTD-NPY fusion protein. After 120 h of methanol induction, the SDS-PAGE electrophoresis result indicated PTD-NPY fusion protein was efficiently secreted into the medium. Western blotting analysis proved that the expressed fusion protein had specific NPY binding activity. The successful expression of PTD-NPY fusion protein in Pichia pastoris provided basis for its further application study.

10.Study of sleep quality and daytime sleepiness in maintenance hemodialysis patients

Objective To assess sleep quality and daytime sleepiness in patients on maintenance high flux hemodialysis, and discussed the associated factors. Methods A total of 112 high flux hemodialysis patients and 53 normal subjects were estimated by Pittsburgh sleep quality index (PSQI) and Epworth Sleep Scale (ESS) to assess the sleep quality and day time sleepiness. Global score of these questionnaires were analyzed. Seven components' scores and 9 reasons for sleep disturbances were compared between "good" (global PSQI ≤5) and "bad" (global PSQI>5) sleepers. Sleep quality was compared among different shifts of hemodialysis. The impact of clinical factors on sleep quality were analyzed by multivariate linear regression and logistic regression. Results Compared with control group, hemodialysis group had a higher PSQI (7.02±4.94 vs 3.28±2.79, P<0.05) and a lower ESS score [3(0-6) vs 8(4.25-11.75), P<0.05] . 58% patients were "bad" sleepers and sleep latency was longer (30 min vs 15 min, P<0.05). Insomnia was the main problem. Patients on morning shift, afternoon shift and night shift had similar subjective sleep quality. Age (OR=1.75, P=0.003), dialysis vintage (OR=1.26, P=0.008),hemoglobin (OR=0.64, P=0.008), calcium phosphate product (OR=1.60, P=0.02) were significantly related to sleep quality score. Conclusions Sleep disturbance is common in hemodialysis patients. Older age, longer dialysis vintage, anemia and higher calcium phosphate product are risk factors for poor sleep quality.
