1.Study on the effects of tonsillectomy and adenoidectomy on the body's immune function of preschool children

Objective To investigate the effects of tonsillectomy and adenoidectomy on the body's immune function of preschool children.Methods Using paired comparison method,40 preschool children with OSAHS[aver-age age (4.5 ±1.2)years]were selected.The peripheral venous blood's white blood cells,neutrophils,lymphocytes, monocytes,immune active cells(total T lymphocytes and total B lymphocytes),immunoglobulin(IgA,IgG,IgM)and the content of C3 and C4 levels in the different periods (after 1 month,3 months )before and after tonsillectomy and adenoidectomy were detected.And the immune parameters of 20 healthy children with matched age were selected as control group,the analysis comparison was conducted.Results In preoperative and postoperative 1 month,3 months compared with the control group,the above immune parameters had no statistically significant differences (t =0.332, 0.212,0.336,0.415,0.579,1.295,0.874,1.143,1.296,1.058,1.261,all P >0.05).At postoperative 1 month, the immune active cells(total T lymphocytes and total B lymphocytes),immunoglobulin (IgA,IgG,IgM)and the content of C3 and C4 levels declined slightly compared with preoperation,but within the normal range,back to normal at postoperative 3 months.Conclusion Tonsillectomy and adenoidectomy will not suppress the development of preschool children's immune systems,and the resection is feasible.

3.Mycoplasma pneumoniae induced coagulopathy and thrombosis in children

Mycoplasma pneumoniae infection can cause coagulation dysfunction and thrombosis, involving organ including deep vein, lung and brain, etc.Central venous catheterization is the most important risk factor for deep venous thrombosis.Lung consolidation more than 2/3 is a high risk of pulmonary embolismin mycoplasma pneumoniae pneumonia.Elevated blood D-dimer over 5 mg/L is an independent risk factor for predicting thrombus.Color Doppler ultrasound and CT angiography(CTA) are the main methods to determine thrombosis.Deep venous thrombosis or pulmonary embolism should be treated with individualized anticoagulant and thrombolytic therapy.
4.Effect of electronic colonoscopy on indicators of cardiovascular events in elderly patients

Objective To observe the effect of electronic colonoscopy on heart rate,blood pressure,arterial hemoglobin oxygen saturation and electrocardiography in elderly patients.Methods 60 patients were divided into two groups,elderly group (over 60 years old,n=30) and non-elderly group (< 60 years old,n =30).The changes in heart rate,blood pressure,arterial hemoglobin oxygen saturation and electrocardiography before and during colonoscopy were compared between the two groups.The increased heart rate was observed in both groups(the non-elderly group:t=10.746,P<0.001; the elderly group:t=4.892,P<0.001),and the increment of heart rate was higher in non-elderly group than in elderly group [(6.9±3.7) /min vs.(4.0±3.8) /min,t=2.88,P< 0.01].Results Systolic and diastolic pressures were increased in both groups during colonoscopy as compared with pre-colonoscopy(t=3.88,5.88,6.55 and 7.43,respectively,P<0.01 or 0.001),but there were no significant differences in the increment between the two groups(t=0.31 and 0.32,bothP>0.05).Arterial hemoglobin oxygen saturation was decreased in both groups during colonoscopy as compared with pre-colonoscopy(t=3.05 and 2.98,P<0.05 or 0.01),and there was no significant difference in the decrease between the two groups(t=-0.13,P>0.05).Abnormal ECG can be detected in both groups,but had no statistical significance between the two groups (20.0% vs.16.7%,x2 =0.11,P> 0.05).Conclusions Electronic colonoscopy can be performed relatively safely in the elderly.
5.Arthroscopic reconstruction of the anterior cruciate ligament using multi-strands of hamstring tendons

Objective To investigate the clinical effect of arthroscopic anterior cruciate ligament(ACL) reconstruction using autologous multi-strands of hamstring tendon.Method From May 2002 to Dec.2006,arthroscopic reconstruction of the anterior cruciate ligament using multi-strands of hamstring tendon was performed on 39 patients.Among them,28 cases were followed up for 7 to 48 months(average 13.5±2.3 months).Result The median Lysholm knee score was improved from 47.3±3.6 to 91.3±2.9 after operation.Conclusion Arthruscopic reconstruction of the anterior cruciate ligament using muhi-strands of hamstring tendons is a good alternative method with minimal invasion and convenient tendon harvesting and fewer complication and reliable clinical effect.
6.The analysis of identification for non-tuberculous Mycobacterium with molecular assay genotype Mvcobacterium kit

Objective To identify non-tuberculous Mycobacterium(NTM) rapidly with HAIN molecular assay genotype Mycobacterium kit,and investigate the advantages and disadvantages of this method.Methods Seventy-four clinical NTM isolates were collected from hospitals in Zhejiang and Anhui province.Clinical strains were identified with HAIN molecular assay genotype Mycobacterium kit.16S rRNA gene sequencing was used to estimate and compare with this method.Results The results of kit showed that there were thirty-one M.intracellulare strains,twelve M.chelonae strains,eight M.fortuitum strains,six M.kansasii strains,five M.avium strains,three M.smegmatis strains,two M.phlei strains,two M.scrofulaceum strains and one M.gordon strain.Four strains were identified as Mycobacterium without further identification.Eight M.tuberculosis strains were identified correctly too.Compared with 16S rRNA gene sequencing,except for four strains identified as Mycobacterium,others 70 strains got the same results as 16S rRNA gene sequencing,the coincidence was 94.59%,and it could further identify thirteen Mycobacterium chelonae complex and eight Mycobacterium kansasii complex to subspecies M.abscessus and M.kansasii,respectively.If only to identify strains under the identification range of this kit,the coincidence reach to 100%,Conclusion The method of HAIN molecular assay genotype Mycobacterium kit is simple and accurate,the time is shorter and should widely be applied clinically.

Objective To develop a machine simultaneously integrating mechanized environmental cleaning and automatic mollusciciding and to evaluate its effectiveness of field application,so as to provide a novel Oncomelania hupensis snail control technique in the large?scale marshlands. Methods The machine simultaneously integrating mechanized environmental clean?ing and automatic mollusciciding,which was suitable for use in complex marshland areas,was developed according to the mech?anization and automation principles,and was used for O. hupensis snail control in the marshland. The effect of the machine on environmental cleaning and plough was evaluated,and the distribution of living snails was observed at various soil layers follow? ing plough. The snail control effects of plough alone and plough followed by mollusciciding were compared. Results The ma?chine could simultaneously complete the procedures of getting vegetation down and cut vegetation into pieces,plough and snail control by spraying niclosamide. After plough,the constituent ratios of living snails were 36.31% ,25.60% ,22.62% and 15.48% in the soil layers at depths of 0-5,6-10,11-15 cm and 16-20 cm respectively,and 61.91% living snails were found in the 0-10 cm soil layers. Seven and fifteen days after the experiment,the mortality rates of snails were 9.38% and 8.29% in the plough alone group,and 63.04% and 80.70% in the plough + mollusciciding group respectively(c27 d = 42.74,c215 d =155.56,both P values < 0.01). Thirty days after the experiment,the densities of snails were 3.02 snails/0.1 m2 and 0.53 snails/0.1 m2 in the soil surface of the plough alone group and the plough + mollusciciding group,which decreased by 64.92% and 93.60% ,respectively,and the decrease rate of snail density was approximately 30% higher in the plough + mollusciciding group than that in the plough alone group. Conclusions The machine simultaneously integrating mechanized environmental cleaning and automatic mollusciciding achieves the integration of mechanical environmental cleaning and automatic niclosamide spraying in the complex marshland areas,which provides a novel technique of field snail control in the large?scale setting in Chi?na.
8.Three-dimensional ultrasound guided catheter ablation of premature ventricular components originating from left anterior ventricular papillary muscles via transspetal puncture

Objective To investigate ablation characteristics of PVC/VT originating from left ventricle anterior papillary muscles.Methods This study included 10 patients of PVC/VT originating from left ventricle anterior papillary muscles from January 2015 to June 2016 in Beijing Anzhen Hospital.Electrophysiological mapping and radiofrequency ablation were completed using three-dimensional anatomical mapping system combined with three-dimensional intracardiac ultrasound technology.ECG and abaltion target diagram characteristics as well as the special anatomy were explored.Results All the 10 patients were successfully ablated and followed up for 12 months.One patient had recurrence within 12 months and no complications were recorded.The target sites localized at the tip (n =1),middle portion(n =4)or the base (n =5) of the LV-APM.Among 7 patients,the target sites were located at the anterior septal papillary muscle and in 3 patients were located in the free papillary muscle.9 patients were successfully ablated via anterograde trans-septal catheterization after the failure of retrograde approach.Premature QRS wave time were 152.80 ± 11.72 ms and 6 patients presented sharp potential at the targets during PVC/VT.Conclusions PVC/VT originating from left ventricle anterior papillary muscles have similar ECG and diagram characteristics that is different from which originating from left anterior fascicle.It is recommended to get the target via transseptalpuncure approach.Ablation target could be clearly positioned by three-dimensional intracardiac ultrasound technology.
9.Surveillance and risk assessment system of schistosomiasis in Jiangsu Prov-inceⅢRisk of schistosomiasis transmission in the area along the Yangtze River in Yang-zhou City

Objective To evaluate the potential risk of schistosomiasis transmission in the area along the Yangtze River in Yangzhou City,so as to provide evidences for establishing a post?transmission surveillance system for schistosomiasis in marsh?land regions. Methods The water infectivity,floating boatmen and fishermen infection,reservoir host infection and wild feces contamination were investigated in five districts/counties along the Yangtze River in Yangzhou City,including Guangling,Hanji?ang,Jiangdu,Yizheng and Development Zone,and the transmission factors and risky characteristics were assessed after inter?ruption of schistosomiasis transmission in marshland regions. Results A total of 15 key water regions were identified in the area along the Yangtze River in Yangzhou City in 2015. A total of 1 500 sentinel mice were placed,after breeding,their overall sur?vival rate was 99.33%;1 490 were dissected,with no schistosome infection. Of the 5 576 floating boatmen and fishermen exam?ined,no schistosome infection was observed,and among the 3 566 domestic animals(including 171 cattle,1 895 sheep and 1 500 pigs),no infections were detected. During the period between January and March,2016,there were 3 200 mouse traps placed on 8 marshlands,and 62 wild mice were captured from 6 marshlands,with a capture rate of 1.94%,and no schistosome?infected wild mice were seen. In addition,there were 35 pieces of fresh wild feces captured from 7 marshlands,including 11 pieces of bovine feces(31.43%),17 pieces of sheep feces(48.57%),2 pieces of dog feces(5.71%)and 5 pieces of other feces (14.29%),and no infections were detected. Conclusions There is a low risk of schistosomiasis transmission in the area along the Yangtze River in Yangzhou City. However,the contamination of feces from bovine and sheep that are freely pastured on marshlands is a big threat to schistosomiasis control.
10.Diagnosis and treatment of tumor-associated acute liver failure in children

Liver is the most common location for hematogenous spread of solid tumors. Clinically acute hepatic failure ( ALF) secondary to either a solid organ tumor or a hematologic malignancy is rare,but with a poor prognosis and high mortality. The time between the onset of symptoms and ALF was relatively rapid. Malignancy is one of the causes of pediatric acute liver failure (PALF). Common clinical symptoms were jaundice,haemorrhage,and hepatic encephalopathy. It is lack of specific clinical features and difficult to diagnose in early phase. Delayed diagnosis and treatment contribute to poor prognosis. How to combine the treatment of primary tumors and liver protection is critical issue to clinician. The effective therapy for patients with PALF is liver transplantation. In recent years,the development of artificial liver support is to provide a facilitating recovery chance and to prolong the window of opportunity for liver transplantation.
