1.The monitoring of nosocomial infections and control of an outbreak in a surgical intensive care unit

Objective To investigate the incidence and the risk factors of nosocomial infection in a surgical ICU (SICU) where infections occurred frequently. Methods A bacteriological investigation was prospectively done in all the inpatients of the SICU admitted in one month. Nosocomial infection was diagnosed according to Diagnostic Criteria of Nosocomial Infection published by Ministry of Health. The nurse who was responsible for the study should observe each patient carefully and fill in the progress note and monthly report of each ICU patient every day. When a case of infection was found, it should be recorded in the Record of Nosocomial Infection Case. The data were analyzed after the completion of the study. Results All the inpatients had undertaken prolonged use of interventional equipments such as urethral catheter, arterial or venous cannula, artificial ventilation, etc. The nosocomial infection rate was 22.7% in 22 inpatients. The daily infection incidence of urinary tract, blood, and lung was 26.8‰, 9.5‰, and 65.8‰, respectively. One patient was found to have pulmonary infection, urinary tract infection and bacteremia simultaneously. From the specimens from patients with nosocomial infection pathogenic bacteria were identified, including Stenotrophomonas maltophilia, Streptococcus viridans, Staphylococcus epidermidis, Pseudomonas aeruginosa, E. coli, Staphylococcus aureus, Achromobacter xylosoxidans, and Candida tropicalis were isolated. At the same time the drug sensitivity tests were carried out. During the monitoring period an outbreak of infection was controlled. Conclusions The aim of an objective monitoring is to solve the clinical problem and cut down the infection rate. In this group there are many risk factors, including old age, malignant tumor, major operation, severe pathological condition, long duration of stay, and so on. SICU is the department where rate of nosocomial infection is high. Comprehensive control measures must be carried out to lower the incidence of nosocomial infection.
2.Quality Study on Cinnabar-analysis of Dyeing Status for Cinnabar

OBJECTIVE:To analyze the dyeing status of cinnabar and its pieces,and provide reference for its quality clinical safetey appicaton. METHODS:TLC was used for the qualitative identification of amaranth,carmine,erythrosine,acid red 73, 808 udan and indirubin. HPLC-MS was used to detect the 808 udan :HPLC conditions were as follows,column was Acquity UPLC BEH C18 with mobile phase of cetonitrile-0.1% formic acid(70∶30,V/V)at a flow rate of 0.3 ml/min,the detection wave-length was 520 nm;MS conditions were as follows,ion source was electrospray ionization source,scanning mode was positive ion scanning with full scanning tandem mass spectrometry,nebulizer pressure was 30 psi,drying gas was nitrogen,ion spray voltage was 4 000 V,collision energy was 30 V,and the injection volume was 5 μl. The volumetric method was used for content determi-nation of HgS. RESULTS:TLC spots of amaranth,carmine,erythrosine,acid red 73,808 udan and indirubin were clear and well-separated. 4 batches of 808 udan dyeing were included in the 18 batches of samples,6 batches had non-compliance contents (including 3 batches of 808 udan dyeing). CONCLUSIONS:Dyeing-doped and other quality problems exist in the cinnabaris in markets,which should be noticed.

Objective To describe a case of lower limbs infection of Helcococcus Kunzii in North China University of Science and Technology Affiliated Hospital in October 2017 and to analyze the etiology and drug susceptibility??Methods The bacteria were identified by French Meriere mass spectrometer and BD Phoenix?100 automatic bacterial identification/drug sensitivity system??The drug resistance of the bacteria was detected by disk diffusion method,and collected the clinical information and related literature information of the Helcococcus Kunzi to analyze??Results The Gram?positive cocci was isolated from the foot secretion of a patient with venous thrombosis of lower limbs complicated with infection??The result of smear was Gram?positive cocci in double,agglomerate and varied in size??On the blood plate,there was a small colony of α hemolysis??Thixozyme negative,β galactosidase negative??The results of mass spectrometry identification and two kinds of fully automatic bacterial identification / drug sensitivity system were identified as Helcococcus Kunzii,the isolates was resistant to erythromycin and clindamycin among the 10 antibiotics tested??By searching,sorting out and analyzing the literature information, it is revealed that the infection of this bacterium is not related to sex??Middle?aged and elderly people are mostly infected??And the cocci easily infected patients with lower immunity??The infection of lower limbs is 47??37%,the infection of blood flow is 15??79% and the other ways of infection are 42??11%( lung, mammary gland, etc??)??Conclusion Helcococcus Kunzii is a conditional pathogen, and patients with lower immunity are more common??Erythromycin resistance could identify it from the green balloon bacteria??Moreover, the bacteria could cause a wide spectrum of diseases,and easily cause sepsis and lower limbs infections,which reminds us that we should be vigilant in clinical practice??
