1.A SEM-OBSERVATION ON THE EPITHELIAL SURFACE OF RAT PINEAL CAPSULE
Acta Anatomica Sinica 1953;0(01):-
Objective To explore the structural foundation of transport pathway of pineal secretions from the pineal body to the cerebrospinal fluid of subarachnoid space. Methods The pineal capsule of the superficial pineal body of 1.5-month and 12-month old rats was observed under scanning electron microscope(SEM).Results Cribriform and trumpet-shaped epithelial stomata were scattered on the pineal capsule.Cribriform epithelial stomata were seen mostly in 1.5-month old rats. They were composed of many round pores which pierced through the full-thickness of the periphery of the capsule endothelium. The pores were dense and ranged from 200-500 nm in diameter;trumpet-shaped epithelial stomata were seen in both 1.5-month and 12-month old rats.Trumpet-shaped epithelial stomata were located among epithelial cells of the pineal capsule. They were round or elliptic in shape and ranged from 1-4 ?m in diameter. On the surface of the pineal capsule, many secretory granules were observed. They were spherule and 8001 000 nm in diameter. Conclusion The releasing pattern of secretory granules of the pineal body could include a releasing of the whole membrane;the pineal secretions may be transported directly from the pineal body to the cerebrospinal fluid of subarachnoid space through the epithelial stomata of the pineal capsule.

AIM: A new HPLC-MS method was developed to determine finasteride in human plasma. METHODS: Two formulations of finasteride tablets were given to 20 healthy male volunteers according to a randomized 2-way cross-over design. The samples were extracted by ethyl acetate under basic conditions, then were separated by C18 column and determined by mass detector. RESULTS: The calibration curve of finasteride was linear and intra-day and inter-day RSD were less than 10 %. The pharmacokinetics parameters of the two formulations (4.5 ± 0.5) h for t1/2; (3.0 ± 0.7) and (2.8 ± 0.9) h for tmax, respectively. The results indicated that there was no significant difference on cmax, A UC0-24, t1/2 or tmax values between the two formulations. CONCLUTION: The relative bioavailability of tablets I with respect to tablets Ⅱ is (99.3 ± 9.2) % by the A UC0-24 measurement, and bioe quivalence is observed between the two tablets.

Objective:To explore the effect of improving oral care based on cluster management in ventilator-associated pneumonia (VAP).Methods:Totally 126 VAP patients from February 2019 to February 2020 in our hospital were selected. The patients admitted from February 2019 to August 2019 were the control group, and the patients from September 2019 to February 2020 were the observation group, 63 cases in each group. The control group was treated with cluster nursing, and the observation group was treated with cluster nursing with improved oral care. The EICU hospitalization time, mechanical ventilation time, plaque index, pathogen infection, oral cleaning score, oral bleeding ulcer and aspiration were compared between the two groups. Results:After nursing, the EICU hospitalization time and mechanical ventilation time in the observation group were (18.34±4.15), (8.56±2.14) days, which were shorter than (23.56±4.82), (12.04±3.10) days in the control group ( t value was -6.514, -7.333, P<0.05). During the nursing process, the incidence of bleeding ulcer and aspiration in the observation group were 12.70% (8/63), 6.35% (4/63), which were lower than 30.16% (19/63), 20.63% (13/63) in the control group ( χ 2 values were 5.704, 5.508, P<0.05). After nursing, the plaque index of the two groups was lower than that before nursing ( t values were 12.516, 6.654, P<0.05), and the plaque index of the observation group was lower than that of the control group ( t value was -6.860, P<0.05). During the nursing process, the incidence of pathogenic bacteria infection in the observation group was 28.57% (18/63), which was lower than 49.21% (31/63) in the control group ( χ 2 value was 5.644, P<0.05). After nursing, the oral cleaning score of the two groups was lower than that before nursing ( t values were 11.118, 6.240, P<0.05), and the score of the observation group was (14.38±3.60) points, which was lower than (18.20±4.11) points of the control group ( t value was -5.549, P<0.05). Conclusion:Auricular pressure therapy can effectively improve the constipation symptoms, shorten the time required to take effect for the main symptoms, and improve the quality of life of schizophrenic patients.

Based on the theory of ascending and descending of center qi, it is believed that yang deficiency and water dampness, as well as abnormal circulation of center qi, are the core pathogenesis of psoriasis. The common pathogenic evolution of psoriasis includes wood constraint and blood stagnation, lung metal fluid exhaustion. In the later stage of psoriasis, a mixture of deficiency and excess patterns, as well as cold and heat in complexity, are often observed. Treatment focuses on warming yang and resolving dampness to restore proper qi ascending and descending, and the therapeutic formulas such as Ganjiang Lingzhu Decoction (甘姜苓术汤), Zexie Decoction (泽泻汤), Zhenwu Decoction (真武汤), and Linggui Zhugan Decoction (苓桂术甘汤) are suggested based on the nature and location of the disease. To unblock the qi movement through discharging the liver and rectifying the lung, Xiaochaihu Decoction (小柴胡汤) and Danzhi Xiaoyao Powder (丹栀逍遥散) are commonly used as the foundation with modifications. To balance yin and yang through clearing the heart and nourishing the kidneys, a self-designed Changzhong Decoction (畅中汤) is utilized. Following the theory of ascending and descending of center qi, it is suggested to combine cold and warm medicinals and mediate the center earth, so as to promote the circulation of the center qi, restore the clear and the turbid, and keep four-dimensional smooth flow of qi, providing a reference for traditional Chinese medicine treatment for psoriasis.

A 65-year-old male patient was admitted for recurrent lymph node enlargement for 5 years and elevated creatinine for 6 months. This patient was diagnosed with angioimmunoblastic T-cell lymphoma 5 years ago and underwent multiple lines of anti-tumor therapy, including cytotoxic chemotherapy; epigenetic modifying drugs such as chidamide and azacitidine; the immunomodulator lenalidomide; and targeted therapy such as rituximab, a CD20-targeting antibody, and brentuximab vedotin, which targets CD30. Although the tumor was considered stable, multiple virus activation (including BK virus, JC virus, and cytomegalovirus) accompanied by the corresponding organ damage (polyomavirus nephropathy, cytomegalovirus retinitis, and progressive multifocal leukoencephalopathy) occurred during anti-tumor treatment. Anti-tumor therapy was suspended and ganciclovir was used. The serum viral load decreased and organ functions were stabilized. The purpose of this report was to raise clinicians′ awareness of opportunistic virus reactivation during anti-tumor treatment.
