1.A comparative study on decoctrate of berberine in Gegen Qinlian decoction

Objective To compare the solution rate of berberine in Gegen Qinlian decoction and Chinese goldthread decoction,to clarify the scientific nature of the traditional Chinese medicine compound.Methods High performance liquid chromatography(HPLC)was used to detect the content of berberine in Gegen Qinlian decoction and Chinese goldthread decoction.Chromatographic condition was C1 8 chromatographic column,mobile phase was acetonitrile:0.05mol/L of potassium dihydrogen phosphate(48:52)(0.4g sodium dodecyl sulfate was added to per 1 00mL solution,adjusted pH to 4.0 with glacial acetic acid),detection wavelength was 345nm,the sample size was 1 0μL,flow rate was 1 .0mL/min,column temperature was 30℃.Results The average decoct rate of berberine in Gegen Qinlian decoction was 0.73%,while that was 0.45% in Chinese goldthread decoction,there was significant difference between the two groups(P <0.01 ).Conclusion The efficacy of TCMcompound Gegen Qinlian decoction is greater than the efficacy of the unilateral simple accumulation,so traditional Chinese medicine compound has certain scientific nature.
2.Repressor SmeT of Efflux Pump SmeDEF in Stenotrophomonas maltophilia

OBJECTIVE To investigate the repressor SmeT and other sequences of efflux pump SmeDEF in the clinic isolates of Stenotrophomonas maltophilia to involve in antibiotic resistance. METHODS The mRNA level of smeD examined the resistance profile of the clinical strains to antimicrobials was by agar dilution and their reaction to pump inhibitor was investigated by RT-PCR,and the smeD-smeT fragment was amplified by PCR,and then sequenced.Adding poly(A) to 3′ end of RNA,applying 3′RACE,nested PCR,then sequence-analysis. RESULTS The amino acid sequences of the forepart of SmeT were conservative in all the 7 strains.In antibiotic-resistant strains with positive reaction to pump inhibitor,the intergenic sequences of smeD-smeT were different from those in sensitive ones.The variations in(-165 to-82)nt of smeT and several nucleic acids before the start codon of smeT,possibly involved in antimicrobial resistance.All isolates didn′t have Leu166Gln mutation within the SmeT protein of D457R,which was reported possibly associated with antibiotic resistance.The resistant strains with efflux positively inhibited had Asp218Glu substitution,different from the negatively-inhibited ones. CONCLUSIONS The expression level of SmeDEF is related with the antibiotic resistance of S.maltophilia.The possible resistance mechanism includes the variations in the intergenic smeD-smeT and/or smeT codon sequence.
3.Analysis on the influence factors of renal insufficiency in patients with hepatocellular carcinoma during microwave therapy

Objective To explore the influence of renal insufficiency with microwave therapy in the treatment of liver cancer .Methods A retrospective clinical analysis with 80 patients who underwent microwave therapy of liver cancer ,was performed from June 2012 to March 2014 in the department of hepatobiliary and pan-creatic surgery ,the Affiliated Tumor Hospital of Harbin Medical University .According to perioperative presence of renal insufficiency,we divided the patients into renal dysfunction group (A)(n=44)and normal renal function group(B)(n=36);Preoperative ALT,TBIL,BUN,intraoperative hypotension,intraoperative urinary volume,age, the distance between the tumor and the main vessels in the liver ,postoperative microwave ablation time the differ-ences of the risk factors ,such as microwave ablation range and sex of the two groups were compared during micro -wave therapy .Results There was no statistical significant difference between the two groups on intraoperative u -rinary volume(P>0.05);The preoperative ALT,TBIL,BUN,intraoperative hypotension,age,microwave ablation time,the distance between the tumor and the main vessels in the liver ,scular microwave ablation range ,sex of two groups showed statistical differences (P<0.05).BUN with preoperative ALT,TBIL,preoperative BUN,intraoper-ative hypotension,age,the distance between the tumor and the main vessels in the liver ,time of microwave abla-tion,microwave ablation range,sex influence factors were positively correlated (r=0.63,0.60,0.58,0.49,0.45, 0.40,0.35,0.46,0.52,P<0.05).Strength and influencing factors of BUN are the distance between the tumor and the main vessels in the liver,microwave ablation range,intraoperative hypotension,preoperative ALT,sex,pre-operative TBIL,microwave ablation time,age,preoperative BUN(β=0.52,0.42,0.37,0.31,0.29,0.25,0.20, 0.18,0.16,P<0.05).Conclusion Microwave therapy leads to renal insufficiency in the treatment of liver cancer performs many aspects , including preoperative ALT , TBIL, preoperative preoperative BUN , intraoperative hypotension ,age,the distance between the tumor and the main vessels in the liver ,time of microwave ablation ,mi-crowave ablation range ,sex,which is closely related to the factors affecting the risk of kidney .
4.Study on changes of middle cerebral arterial hemodynamics before and after intraluminal stent-assisted angioplasty in patients with their stenosis by color Doppler ultrasonography

Objective To evaluate the changes of middle cerebral arterial (MCA) hemodynamics before and after intraluminal stent-assisted angioplasty for their stenosis. Methods A total of 11 patients with MCA stenosis treated by intraluminal stent-assisted angioplasty were selected. In these cases, their stenosis before treatment and the state of stents after treatment were observed using transcranial color Doppler ultrasonography(TCCD). Meanwhile, the blood flow velocities (including peak systolic velocity, end diastolic velocity and mean velocity) were measured and studied comparatively at stenosis and post-stenosis place before and after the treatment, respectively. Results ①Among 11 cases implanted stents, 10 stents were in good state after treatment, only one expanded incompletely. ②The blood flow velocities were significantly higher in patients at stenosis place than in normal adults, and lower at post-stenosis place before treatment (P<0.05). The blood flow velocity lowered obviously at stenosis place after treatment (P<0.05) ,among which 90.9% were restored. The one which expanded incompletely was slightly higher. The blood flow velocity increased at post-stenosis place after treatment (P<0.05) and all returned to normal.Conclusions TCCD can observe the MCA stenosis and the state of stent,and analyze the changes of their hemodynamics.
5.Cloning and characterization of the group 5 allergens from Dermatophagoides farinae

Objective To obtain the gene coding for the group 5 allergens from Dermatophagoides farinae ( Derf5 ) and predict its molecular characteristics. Methods The total RNA of D. farinae were extracted, and the gene Derf5 was amplified by RT-PCR with the primers designed according to previous sequence published in GenBank. The target gene was linked into pMD19-T Simple plasmid, sequenced and analyzed by bioinformatics software. Results The sequence homology reached to 97.8% between our sequenced result with one complete open reading fragment (ORF) and the reference. The gene encode an extracellular hydrophobic protein with 132 amino acid resides, one signal peptide from 1 to 19 position and one transmembrane domain from 1 to 19 position. The secondary structure was composed of extended strand (1. 52% ), random coil (7.58%) and alpha helix (90.91%). The encoded protein was deduced to have two Casein kinase Ⅱ phosphorylation sites. The similarity of the amino acid sequence of the group 5 allergens were 78% between D. farinae and D. pteronyssinus. Conclusion The gene Derf5 was cloned successfully, and its characteristics was primarily predicted.
6.Preparation of monoclonal antibodies against recombinant Der f 7 allergen

Objective:To clone and express the Der f 7 recombinant protein from Dermatophagoides farinae and prepare the Der f 7 monoclonal antibody.Methods: The Der f 7 recombinant protein was expressed in prokaryotic expression system of pET28a(+)-Der f 7.BALB /c mice were immunized with the recombinant protein.Myeloma cells and spleen cells were fused,and hybridoma cells were screened by ELISA.Hybridoma cells were injected into the mice abdominal cavity to obtain ascites.It was purified by protein A agarose medium ascites,and then to dentified the titer and purity of the antibody.The specificity of the antibody was identified by Western blot.Results: Three hybridoma cells which stably secret recombinant Der f 7 monoclonal antibody were obtained.The monoclonal antibody had high purity,the titer was higher than 1∶243 000.Western blot showed that Der f 7 recombinant protein could be recognized well.Conclusion: We successfully obtained Der f 7 monoclonal antibody,which can be used for the quantification and localization of Der f 7 allergen and the diagnosis and treatment of allergic diseases.
7.Relationship etween Electroacupuncture and Spleen Function on Leukocyte

Objective: To explore the underlying mechanism by which acupuncture regulates the peripheral leukocyte count and observe the relationship between the effects of electroacupuncture (EA) and the spleen ultrastructure in leukopenia. Methods: Leukopenia models of rabbits were established by injecting cyclophosphamide (CY) into rabbits' ear vein,and then the rabbits were treated with acupuncture. The peripheral leukocyte count and classification were measured daily. At last, after the animals were anesthesized, the abdominal cavity was opened, and a piece of spleen tissue was cut. The diameter of splenic sinusoid basal lamina eyehole was measured under electric microscope. Results: The peripheral blood leukocyte count in the EA group increased significantly with shift to right of the granulocyte nuclei compared with model control group (P<0.01). Moreover, the calibers of splenic sinusoid basal lamina eyehole in the EA group were larger than those in the model control group (P<0.05). Conclusion: Acupuncture can enhance the peripheral leukocyte count by promoting spleen activity in the early phase of leukopenia.
8.Three-dimensional Quantitative Measurement of Spiral CT in Evaluating Tumor Size and T Stage of Stage I Non-small Cell Lung Cancer

Purpose To investigate the value of three-dimensional quantitative measurement of spiral CT in evaluating tumor size and preoperative T stage in stage I non-small cell lung cancer (NSCLC). Materials and Methods The complete data of 125 patients with stage I NSCLC confirmed surgically and pathologically were compared in terms of maximum tumor diameter and T stage analysis by means of three-dimensional quantitative CT measurement, two-dimensional measurement and general pathology measurement. Results The mean maximum tumor diameter of these 125 patients measured by quantitative three-dimensional CT measurement, two-dimensional measurement and general pathology measurement were (26.21±8.14) mm, (27.03±9.90) mm and (25.60±9.31) mm, respectively. The difference in mean maximum tumor diameter by two-dimensional measurement and three-dimensional quantitative measurement was significant, and remained so when two-dimensional measurement and pathology measurement was compared (t=2.377, P<0.05;t=2.961, P<0.01), but that between three-dimensional quantitative measurement and pathology measurement was not significant (t=1.281, P>0.05). Bland-Altman analysis showed that three-dimensional quantitative measurement had higher consistency than two-dimensional measurement when compared with the gold standard pathology measurement. When three-dimensional quantitative measurement was taken to be the staging criterion, 20% results (25 cases) obtained by two-dimensional measurement proved to be inconsistent. Conclusion Compared with two-dimensional measurement, quantitative three-dimensional CT measurement can provide more accurate information in maximum tumor diameter and T stage for patients with stage I NSCLC, therefore can be applied as a more accurate criterion in preoperative staging and prognosis of stage I NSCLC.
9.The reconstruction study of EEG signal based on sparse approximation & compressive sensing.

OBJECTIVEDue to random sampling of non-adaptive, high-quality reconstruction of the original signal, one-dimensional non-stationary multi-channel EEG signal can be achieved automatic detection and analysis.
METHODSA new multicomponent redundant dictionaries with the atoms of the Gaussian function and its first and second derivatives was built in the paper, and reconstructed signal base on compressed sensing measurement model.
RESULTSThe selected dictionary atoms can more effectively match the EEG signals in a variety of transient characteristics of the waveform, allowing the formation of EEG signal is more sparse matching pursuit decomposition. With the theory based on compressed sensing signal sampling, only half of the original signal with different sample size can be used to reconstruct the original signal quality, the important instantaneous features of the waveform can well be maintained.
CONCLUSIONSignal sampling based on the theory of compressed sensing contains enough information of the original signal, using the prior conditions of EEG signals (or compressibility) sparsity, high-dimensional signal and original image can be reconstructed through a certain decoding of linear or nonlinear model.

10.Needle-knife fistulotomy for difficult cannulation during ERCP

Objective To evaluate the effectiveness and safety of needle-knife fistulotomy (NKF)for difficult cannulation during endoscopic retrograde cholangiopancreatography (ERCP). Methods Data of patients, who received NKF on the back of major papilla when bile duct could not be accessed by conventional cannulation and/or other pre-cut methods during ERCP, were retrospectively reviewed. The success rate of deep cannulation and its complications were observed and analyzed. Results NKF was performed in 108patients due to difficult cannulation, which succeeded in 97 (91.2%) in access to the bile duct and failed in 11 patients with malignant biliary strictures. The failure rate in patients with distal malignant obstruction was higher (25. 8%, 8/31) than those with proximal lesions (5.3%, 3/57) (P = 0. 014, χ2 = 5. 983).Post-ERCP pancreatitis occurred in 5 cases (4. 6%), with the incidence significantly higher in NKF-failure group (18. 2%, 2/11) than that in NKF-suocess group (3.1% ,3/97) (P = 0. 006, χ2 = 7.418). Intestinal perforation occurred in 1 patient and cholangitis developed in 4 others, which all recovered after conservative managements. Conclusion NKF for difficult cannulations in ERCP is safe and effective, especially in hands of experienced operators, but cannulation success rate is relatively low in distal malignant biliary obstruction.
