1.Analysis of Non-rational Prescriptions in Pharmacy Intravenous Admixture Service

OBJECTIVE:To analyze the non-irrational prescriptions in pharmacy intravenous admixture services to ensure patient's medication safety.METHODS:We examined the prescriptions in pharmacy intravenous admixture services from Sep.2006 to Feb.2007 and analyzed the non-irrational ones regarding incompatibility,improper choice of infusion carriers,improper medication frequency,and the incompatibility of Chinese medicines etc.RESULTS:The non-irrational prescriptions totaled 126 cases,of which,46 associated with incompatibility,38 with improper choice of infusion carriers,25 with improper medication frequency and 17 with incompatibility of Chinese medicines.Most of the irrational drug use behaviors were corrected by informing the clinic these medication errors.CONCLUSION:Examining prescriptions in pharmacy intravenous admixture services can improve the safety and effectiveness of patient's use of intravenous drugs.
2.A comparative study of levels of MDA, SOD, NO and XOD between gastric mucosa of Han and Tibetan patients with chronic gastritis at the same altitude in Qinghai province

Objective To compare the levels of four factors between gastric mucosa of Han and Tibetan patients with chronic gastritis residing in districts at the same altitude in Qinghai Province.Methods Levels of malonic aldehyde ( MDA) , superoxide dismutase (SOD) , nitric oxide ( NO) and xanthine oxidase (XOD) in gastric mucosa of Han and Tibetan patients with chronic gastritis resident in same altitude districts were examined, and a comparative study was performed.Results Levels of MDA and SOD were significantly higher in gastric mucosa of Han patients (2 2699 ± 1.58906 nmol/mgprot and 134.6313 ±48.35329 U/mgport, respectively) than those of the Tibetan (1.7095 ± 1.19474 nmol/mgprot and 109.5056 ± 28.26794 U/mgport, respectively) (P <0.05) , which indicated there were differences in metabolism of oxygen free radicals and antioxi-dant free radicals between 2 ethnics.However, there were no differences in NO and XOD levels between two ethnical groups, which suggested no ethnical difference in metabolism of the two factors.Conclusion There is ethnical difference in MDA and SOD metabolism in gastric mucosa of Han and Tibetan patients with chronic gastritis from same altitude district, which is presumably due to the different tolerance to the hypoxia at high altitude.
3.Stability of Cefoperazone Sodium /Tazobactam Sodium for Injections in Fructose-contained Sodium Chloride Injection or Invertose Injection

OBJECTIVE:To study the stability of Cefoperazone Sodium /Tazobactam Sodium in Fructose-contained 0.9% sodium chloride injection or Invertose Injection.METHODS:The contents of Cefoperazone Sodium /Tazobactam Sodium within 6 hours after mixing with Fructose-contained 0.9% sodium chloride injection or Invertose Injection were determined by HPLC,meanwhile the changes in the appearances and the pH values were observed.RESULTS:The contents of Cefoperazone Sodium and Tazobactam Sodium in Fructose-contained 0.9% sodium chloride injection or Invertose Injection were stable within 6 hours,and the mixed solutions were clear and their pH values were stable on the whole.CONCLUSION:Cefoperazone Sodium /Tazobactam Sodium can de mixed with Fructose-contained 0.9% sodium chloride injection or Invertose Injection for clinical use.
4.The practice and experience of clinical scenarios discussion teaching for medical students be-fore clinical probation

Objective To investigate the learning outcome of clinical scenarios discussion im-plemented in teaching medical students before clinical probation. Methods Using standardized pa-tients (SP) to simulate the clinical common difficult situation, all eight year program clinical medicine students (84) of Grade 2008 who were about to enter the clinical probation were given clinical commu-nication education guidance. Through five classification variables questionnaire and return visit, the teaching effect was evaluate. Epidata 3.0 was used to input data, and the SPSS 15.0 was used to make descriptive analysis of the questionnaire and proportion comparison. Results We found 35.4%(29) and 63.4%(52) of subjects liked and approval this training, respectively;90.2%(74) of the subjects thought the diffi-culty of this train was moderate; 77.3% (65) of subjects believed that it was necessary to set the clinical scenarios discussion before clinical probation. Conclusions The implementation of clinical scenarios discussion before clinical probation was effective on inducing the medical students to pay attention to the doctor-patient communication, and deep thinking about the communication notice mat-ters and cop-ing styles in clinical environment.
5.Navigation template for sacral fracture fixation using three-dimensional reconstruction and reverse engineering technique

Objective To provide a new method for sacral fracture fixation by means of 3D recon-struction and reverse engineering technique. Methods Pelvis images of 3D CT scan were obtained from patients with sacral fracture. The digital data were transferred into a computer workstation. 3D models of pelvis were reconstructed using Amira 3.1 software and saved in STL format. The 3D fracture models were imported into Imageware 10.0 software. Different situations of reduction (total reduction, half reduction and no-reduction) were simulated using Imageware 10.0 software. The extract locations and the best directions of inserting iliosacral lag screws were defined using Reverse Engineering according to the 3 situations to before navigation templates were designed according to the posterior anatomic features of the ilium and the insertion channels. Exact navi-gational templates were manufactured by rapid prototyping. Drill guides were sterilized and used intraoperatively to assist surgical navigation and placement of iliosacral lag screws. Results Accurate screw placement was confirmed with postoperative X-ray and CT scanning. The navigation templates were found to be helpful and highly accurate. Conclusion The navigation template may be a useful method for mini-invasive fixation of sacroiliac joint fracture.
6.Evaluation of reliability and validity of Chinese version of Diabetes, Hypertension and Hyperlipidemia (DHL) Knowledge Instrument in community diabetic patients

Objective To evaluate the reliability and validity of the Chinese version of the Diabetes,Hypertension and Hyperlipidemia(DHL) Knowledge Instrument in community diabetic patients.Methods A face-to-face interview was conducted among 513 patients with diabetes in the community from January to March 2016 by using the Chinese version of DHL Knowledge Instrument.Forty of them were randomly sampled and reinvestigated 4 weeks later.The analyses on Cronbach's α coefficient,test-retest reliability,content validity,discriminant validity and construct validity were performed to evaluate reliability and validity of the DHL Knowledge Instrument.Results The qualified questionnaires were collected from 488 participants,including 232 males and 256 females with a mean age of (66 ± 6)years.The overall scores of the Chinese version of DHL Knowledge Instrument was 67.7 ± 18.0 and the scores of diabetes,hypertension,hyperlipidemia,medications and general issues sub-scales were 82.0 ± 22.5,65.9 ± 25.2,38.2 ± 34.3,75.5 ± 20.8 and 72.1 ± 22.9,respectively.The standardized Cronbach's α coefficient of the total scale was 0.849 and the test-retest reliability of the total scale was 0.706 (P < 0.01).In term of content validity,the correlation coefficient was from 0.580 to 0.827 among total scale and sub-scales (P < 0.01).In term of discriminant validity,the difference between the high and the low score group on total scale and sub-scales were significant (t value =-13.486 to-35.528,all P < 0.01).In term of construct validity,based on exploratory factor analysis,the scale was revised.According to confirmatory factor analysis of revised scale,four factor model containing 20 items was well fitted (X2 =159.689,df =134,P =0.064);the GFI (goodness of fit index) =0.966,AGFI (adjusted goodness of fit index) =0.952,RMSEA (root mean square error of approximation) =0.020.Conclusion The Chinese version of DHL Knowledge Instrument possesses good reliability and validity and is suitable to evaluate the knowledge of community diabetic patients,and the modified version may work better than the original one.
7.A standardized breeding study of gray red-backed voles(Myodes rufocanus)in the laboratory

Objective To establish a breeding method ofMyodes rufocanus in the laboratory,collect their growth and reproduction data,and provide a basis for carrying out the experimental animalization.Methods Wild Myodes rufocanus caught in the Moranbong woodland were brought back to the laboratory.They were bred artificially in a large hard wall rodent negative pressure isolator.Their growth and reproduction data were recorded for evaluating the results of breeding.Results The Myodes rufocanus were successfully bred in the laboratory.The pregnancy rate was 54.55%.The average pregnancy length was 20.4 days(8 to 22 days).During one breeding period,they gave birth 2.9 times on average.The maximum number of births was 7 times,far more than the number tested under field conditions.The average litter size was 4.3±1.22.The highest litter number of a single nest was 8.The weaning rate of pups was 94.8%.The growth and development of pups were good.Conclusions The breeding method for Myodes rufocanus is established.The growth and reproduction data are tested too.The results of our study laid a foundation for the experimental animalization of Myodes rufocanus.
8.Meta analysis of prognostic factors and surgical extent of papillary thyroid microcarcinoma

ObjectiveTo study prognostic factors and surgical extent of papillary thyroid microcarcinona (PTMC). MethodsLiteratures about PTMC were searched. Relationship between prognostic factors and recurrence after initial operation was analyzed by meta analysis. The surgical extent of PTMC was discussed. Results A total of 12 retrospective studies were included. Meta analysis results showed that factors such as male, tumor size > 5 mm, nonincidental, positive lymphnode, multifocus, and capsular invasion were related to recurrence after initial surgery. Age above 45 years had nothing to do with the recurrence rate of PTMC. There was no signifi.cant difference in recurrence rate between prophylactic neck dissection groups and the groups without prophylactic neck dissection. ConclusionsMany factors are related to the prognosis of PTMC. The surgical extent depends on prognostic factors. Prophylactic neck dissection is not supposed to be conventional surgical procedure for PTMC. Bilateral central lymph node dissection is appropriate to PTMC with positive lymph node.
9.Construction, expression and biological assessment of BPI23-Fcgamma1 recombinant protein prokaryotic expression vector.

OBJECTIVETo construct pBV-BPI600-Fcgamma1(700) recombinant expression vector, to transform it into Escherichia coli DH5alpha, and to induce the expression of BPI23-Fcgamma1 anti-bacterial recombinant protein.
METHODSGenes coding for BPI23 and Fcgamma1 were amplified by RT-PCR from mRNA extracted from HL-60 cell and normal human leukocytes; recombinant cloning vector and recombinant expression vector were then constructed. pBV-BPI600-Fcgamma1(700) recombinant expression vector was transformed into the competent Escherichia coli DH5alpha and BPI23-Fcgamma1 recombinant protein was expressed by a temperature-induced method.
RESULTS(1) Expected amplified products BPI600hp and Fcgamma1(700bp) were obtained by RT-PCR method. (2) pUC18-BPI180, pUC18-BPI420 and pUC18-Fcgamma1(700) recombinant cloning vector were successfully constructed, and sequences were identical with the reported ones. 3) pBV-BPI600-Fcgamma1(700) recombinant expression vector was successfully constructed, and the enzyme digestion analysis showed an expected result. (4) The expression level of BPI23-Fcgamma1 recombinant protein accounted for 20% of total bacterial proteins. (5) The renatured BPI23-Fcgamma1 recombinant protein showed bacteriocidal activity and biological function of complement fixation, and opsonization.
CONCLUSIONpBV-BPI600-Fcgamma1(700) recombinant expression vector was successfully constructed, and BPI23-Fcgamma1 recombinant protein with double biological activity of BPI and IgGFc was expresed in Escherichia coli.

10.SHARP-2 gene silence by lentiviral-based short hairpin RNA interference prolongs survival time of rat kidney transplant recipients

Objective To investigate if rat enhancer of split- and hairy-related protein-2 (SHARP-2) short hairpin RNA interference (shRNAi) prolongs the survival time of rat kidney transplant recipients. Methods Gene recombinant procedures, transfection and co-transfection were carried out to introduce short hairpin RNA interference sequences target for SHARP-2 into 3rd generation self-inactivated lentiviral-ViraPower packaging mix. Limiting dilution method was used for viral titration. Real-time PCR was employed for quantification of gene expression. Rat kidney transplantation was utilized to investigate the effect of SHARP-2 gene silence on the recipient survival. Results A lentiviral-based shRNAi construct LV-SHARP-2iC showed 84% SHARP-2 gene silence efficiency in normal rat kidney cells. At multiplicity of infection 20, 57% T cells could be transfected by lentivirus with spinoculation method. In activated T cells, SHARP-2 g ene silence resulted in 61.3% and 68.7% reduction of intedeukin 2 (IL-2) and interferon γ (IFN-γ) gene expression. When donor kidney was perfused with 5×107 TU LV-SHARP-2iC, the median survival time prolonged for 4-5 days as compared to blank and scramble control groups. Conclusions A recombinant lentivirus LV-SHARP-2iC that effectively silence SHARP-2 gene expression is constructed successfully, leading to the inhibition of IL-2 and IFN-γ. LV-SHARP-2iC treatment can prolong the survival time of rat kidney transplant recipients.
