1.A REPORT ON 16 CASES OF CERVICAL SPONDYLOSIS TREATED WITH THE USE OF PCB

An evaluation of 16 patients with cervical spondylosis undergoing PCB (a new anterior cervical instrumental system combining an intradiscal cage with an integrated plate,PCB) is presented. Four patients underwent surgery for cervical myelopathy and 12 for cervical radiculopathy.There were 11 males and 5 females.The average age was 46 years (ranging from 31~67 years).The time of follow up was 3~16 months after operation.Fixation at one level was performed in 12 patients,and at two levels in 4. The results revealed that the technique was easy to learn and implement,without intraoperative complications.After surgery no screw backout or device failure was identified.Symptoms improved significantly in 14 patients.Complications in donor site were rare. An external collar support was unnecessary after operation.
2.Treatment of cervical spine fractures by anterior plates with titanium mesh and bone graft

Objective To observe the clinical effects of anterior plates with titanium mesh and bone graft (auto and allo) in treatment of cervical spine fractures. Methods 42 cases of fractures of cervical spine treated with anterior plates with titanium mesh. Results The bone graft (auto and allo) were followed up for 4 to 28 months (mean 17 months). The preoperative and postoperative nerve functions were evaluated according to JOA criteria. The cervical curvatures, sink or shift of the mesh and bone fusion were evaluated by X rays. There were no significant complications during perioperative periods. The functions of spinal cord were improved, the cervical curvatures were restored, and no sinking or shifting of the mesh was observed. There was also no loosening of plate or screw. The fusion began 3.5 months postoperatively, and a good fusion could be observed 1 year postoperatively. Conclusion By application of anterior plates with titanium mesh and bone graft (auto and allo) in treatment of cervical spine fractures, the cervical curvature and stability can be restored, the iliac lesion and long operation duration caused by the harvest from ilium can be avoided, better fusion and quicker recovery can be obtained.
3.Observations on the Efficacy of Xi-Cleft Point Acupuncture plus Surrounding Electroacupuncture in Treating Occipital Neuralgia

Objective To investigate the clinical efficacy of Xi-Cleft point acupuncture plus surrounding electro- acupuncture in treating occipital neuralgia.Methods Forty-eight patients with occipital neuralgia were randomly allocated to treatment and control groups, 24 cases each. The treatment group received Xi-Cleft point acupuncture plus surrounding electroacupuncture and the control group, surrounding electroacupuncture alone. The VAS score was recorded in the two groups before and after treatment. The clinical therapeutic effects were compared between the two groups.Results There was a statistically significant pre-/post-treatment difference in the VAS score in the two groups (P<0.05). There was a statistically significant post-treatment difference in the VAS score between the treatment and control groups (P<0.05). The total efficacy rate was 95.8% in the treatment group and 83.3% in the control group; there was a statistically significant difference between the two groups (P<0.05).Conclusion Xi-Cleft point acupuncture plus surrounding electroacupuncture is an effective way to treat occipital neuralgia.
4.Preparation of RFP-loaded a novel polylactone/β-tricalcium phosphate composite scaffold

Objective To explore a biodegradable drug?loaded composite scaffold and determine its bone regeneration and local long?term drug release ability. Methods In this study, RFP?loaded a novel poly (caprolactone)?b?poly (lactide?co?gly?colide)/β?TCP composite scaffold was produced using particle?leaching/freeze?drying technique. And the obtained composite scaffold was characterized by SEM, TGA, UV spectrophotometer, universal testing machine and so on. New Zealand rabbits were used to prepare bone defects, which was treated by the implantation of active artificial bone loaded with or without RFP. The blank group was untreated. Then the repairing effects of the materials were examined. Results The obtained porous scaf?fold has inter?connected and uniformly distributed pores, and the diameters of pores range from 200 μm to 300 μm. Many mi?cropores (10 μm to 50 μm) can be observed on the wall of macropores. The total porosity of the porous composite scaffold can reach as high as 83.4%; the β?TCP content of the scaffolds is 51.2%. Meanwhile, the addition of β?TCP avoided volume shrinkage compared with b?PLGC scaffold; Additionally, the porous composite scaffold has good compressive strength ( 240 kPa) and compressive modulus (1.0 MPa); And the drug loading of the scaffold was 3.2%,which could smoothly release drug for 63 days after a period of burst release for a week. All defects in the experimental groups were radiographically repaired. There were significant differences between the experimental groups and the control group. Conclusion RFP?loaded poly (cap?rolactone)?b?poly (lactide?co?glycolide)/β?TCP composite scaffold is expected to benefit in drug therapy and bone repair in the treatment of bone tuberculosis.
5.Therapeutic Observation of Electroacupuncture for Herpes Zoster

Objective To observe the clinical efficacy of electroacupuncture in treating herpes zoster.Method Ninety patients with herpes zoster were randomized into treatment group 1, treatment group 2, and a control group, 30 cases in each group. Treatment group 1 was intervened by surround needling at Ashi points (deep needling, by 33 mm) plus electroacupuncture; treatment group 2 was by surround needling at Ashi points (superficial needling by 16 mm) plus electroacupuncture; the control group was by oral administration of Valacyclovir Hydrochloride tablets plus Vitamin B1. After 10-day treatment, the blister-breaking time, crust-forming time, and the time of falling-off of the crust, and Visual Analogue Scale (VAS) were observed, and the clinical efficacies were compared.Result The recovery rate and total effective rate were respectively 73.3% and 93.3% in treatment group 1, versus 46.7% and 86.7% in treatment group 2, and 33.3% and 63.3% in the control group. The recovery rate of treatment group 1 was significantly different from that of treatment group 2 and the control group (P<0.01,P<0.05). The total effective rates of treatment group 1 and 2 were both significantly different from that of the control group (P<0.01,P<0.05). The blister-breaking time, crust-forming time, and crust falling-off time of treatment group 1 were significantly different from that of treatment group 2 and the control group (P<0.01,P<0.05). The crust-forming time and falling-off time of treatment group 2 were significantly different from that of the control group (P<0.05). The VAS scores were markedly changed after the intervention in the three groups (P<0.01). After the treatment, the VAS scores of treatment group 1 and 2 were significantly different from that of the control group (P<0.01). The VAS score of treatment group 1 was markedly different from that of treatment group 2 after the treatment (P<0.05).Conclusion Electroacupuncture is an effective method in treating herpes zoster, while deep surround needling at Ashi points plus electroacupuncture can effectively inhibit the development of blisters, promote crust-forming, release the neural pain, and reduce the incidence of postherpetic neuralgia.
6.Neuroprotective effects of Buchang Naoxintong Capsule on focal cerebral ischemia/reperfusion in rats

Objective To explore the neuroprotective effects of Buchang Naoxintong Capsule on focal cerebral ischemia/reperfusion in rats.Methods Rat model of ischemic reperfusion of cerebral middle artery was established according to Zea Longa's pattern. Rats were randomly divided into pseudo-operation group, ischemia/reperfusion group and Buchang Naoxintong Capsule treated group. The ischemia/reperfusion group and Buchang Naoxintong Capsule treated group were both divided into five subgroups according to reperfusion time or administration time such as 1 d, 3 d, 7 d, 10 d and 15 d. Using immunohistochemical staining method and color image analysis, we measured the expression of vessel endothelium growth factor (VEGF) and Laminin in the region of ischemia. Pathological changes were observed with HE staining and transmission electron microscope. 7.Effect of the diet recovery time on postural hypotension after gynecological laparoscopic operation

Objective To explore the effect of the diet recovery time on postural hypotension after gynecological laparoscopic operation. Methods A total of 300 patients undergoing gynecological laparoscopy were randomly divided into observation group and control group according to the order of admission,150 cases in each group. Patients in observation group took liquid food since recovery from anesthetic, their appetite and dietary levels restored to preoperative level within 24 h after surgery from liquid diet to regular diet. Patients in control group were given routine nursing care of diet after belly operation. Results The diet recovery time and the anal exhaust time of patients in observation group were (21.2±3.5) h and (15.5±4.7) h, the control group were (46.8±5.4) h and (23.4±5.4) h, there was significant difference between two groups (t=7.543,5.126, P<0.05).The incidence rate of orthostatic hypotension in observation group was 9.3% (14/150), the control group was 33.3% (50/150), there wassignificant difference between two groups (χ2=8.907, P<0.05). Conclusions Patients undergoing gynecological laparoscopy took food as soon as possible, their appetite and dietary levels restored to preoperative level within 24 h after operation. It didn′ t increase abdominal distension, but helped the intestinal function recovery and helped to reduce the incidence of postoperative orthostatic hypotension.
8.Clinical Observations on Acupuncture at Heel Vessels for the Treatment of Cervical Vertigo

ObjectiveTo investigate the clinical efficacy of points Shenmai(BL62)and Zhaohai(KI6)in treating cervical vertigo. MethodSixty-eight patients meeting the diagnostic criteria were randomly allocated to treatment and controlgroups, 34 cases each. The treatment group received conventional acupuncture at heel vessel points Shenmai and Zhaohai plus cervical Huatuo jiaji points and the control group, conventional acupuncture at cervical Huatuo jiaji points alone. In the two groups, the vertigo symptom score was recorded, and the clinical therapeutic effects and pre-/post-treatment change in the score were observed.ResultThe total efficacy rate was 94.1% in the treatment group and 79.4% in the control group. The therapeutic effect was significantly better in the treatment group than in the control group (P<0.05). The vertigo symptom score decreased significantly in the two groups after the course of treatment compared with before (P<0.01). Vertigo improved significantly more inthe treatment group than in the control group (P<0.05).ConclusionAcupuncture at points Shenmai and Zhaohai is significantly effective by comparison with conventional acupuncture in clinicaltreatment of cervical vertigo.
9.Anterior locking plates with titanium mesh in treatment of 23 patients with cervical fractures and dislocations: X-ray and 1-year follow-up evaluations

From January 2005 to January 2008, 23 cases of fractures and dislocations of cervical spine treated with anterior decompression, reconstruction of bone graft with titanium mesh, and titanium locking plates fixation in Department of Orthopaedics, the 309 Hospital of Chinese PLA were retrospectively analyzed, including 16 males and 7 females, aged 41.9 years (ranging 25 ?65 years). The clinical effect and application value of this treatment was evaluated; the conditions at preoperative, immediate postoperative and follow-up were evaluated by JOA grade, and these radiographs were taken. Lordosis (kyphosis) of the fusion segment with Cobb's angle, and lordosis (kyphosis) of cervical spine with D value were measured to evaluate effect of reconstruction and cervical spine physiological curve. All cases were followed-up for 20.5 months (ranging 1-3 years). Clinical stability was reached in all cases during the follow-up. There were significant differences between preoperative and immediate postoperative, between preoperative and follow-up groups in terms of JOA grade, Cobb's angle and D value (P < 0.05). The JOA grade of immediate postoperative was improved compared with that during follow-up (P < 0.05). The mean improvement rate was 63% at 1 year of follow up. A good curative effect was obtained by using anterior locking plate with titanium mesh systems in treatment of cervical fractures and dislocations and a good effect of restoring and maintaining the lordosis of cervical spine can be also obtained.
10.Experimental Study of Efficacy of a DBM-rifampicin Composite Graft in Healing of Segmental Radial Defect in Rabbits

Objective To evaluate the antibiotics release characteristic and osteogenesis ability of anti-tuberculosis composite graft by observing the healing effect of segmental radial defect in rabbits.Methods Rabbit decalcified bone matrix(DBM) was made under the method of Urist and was combined with rifampicin-gelatin solution. The antibiotics release characteristics were observed by PBS release test in vitro and K-B disc diffusion test in vivo. The osteogenesis ability was measured by critical radius bone defect of rabbit.Results It was detected in release test that the antibiotic releasing time could maintain at least 28 days in vivo and in vitro and the release ratio was 87%. The rabbit critical radius bone defect could be repaired by this anti-tuberculosis composite graft perfectly.Conclusion Good antibiotics release characteristics and bone formation ability showed during this anti-tuberculosis composite graft and it definitely could be used widely in further clinics.
