Objective:This study aimed to investigate the role of mitochondrial fusion protein 2 (Mfn2) in chronic renal injury caused by cyclosporine A (CSA) and the possible mechanism.Methods:The model of chronic CSA nephropathy was established in rats. The rats were divided into solvent control group, CSA model group and normal control group. Serum creatinine and urinary protein levels were detected. The tubulointerstitial injury was observed by light microscopy. The changes of mitochondrial structure were observed by electron microscope, and the expression of Mfn2 was detected by Western blot.Results:The levels of serum creatinine and urinary protein in CSA model group at 2 and 4 weeks were significantly higher than those in control group and solvent group ( P<0.05). In CSA model group, tubulointerstitial injury was obvious, mitochondria swelling and deformation were found in tubular epithelial cells, and ridge disappeared. Compared with normal control group and solvent control group, the expression of Mfn2 in CSA model group was significantly decreased at 2 and 4 weeks ( P<0.05), and the expression of Mfn2 in CSA model group at 4 weeks was further decreased compared with 2 weeks ( P<0.05). Conclusions:Mfn2 may play an important role in the injury of tubular epithelial cells caused by cyclosporine.

Objective To know the distribution of arsenic and trace elements in drinking water in the endemic arsenism areas in Shanyin County, Shanxi Province. Methods Drinking water samples were collected from the chronic endemic arsenic poisoning areas of Silizhuang and Shanghexi, Shanyin County, Shanxi Province and HPLC-HG-AFS system was used to analyze the arsenic contents, valence and speciation. The trace elements were determined by ICP-MS. The data were analyzed by SPSS13.0 for windows. Results A total of 63 drinking water samples were determined, 39.68% of them exceeded the standard limit of arsenic level, inorganic arsenic was the main arsenical in drinking water, iAs3+/(iAs3++iAs5+) ratio was 56.17% in the water sample (arsenic was higher than 50 ?g/L) . The contents of Fe, Mn, Hg, Se and Pb in part of drinking water samples exceed the standard limits. No significant difference was seen between the arsenic results determined by HPLC-HG-AFS and ICP-MS. Conclusion Arsenic contamination of drinking water in Shanyin County, Shanxi Province is severe. The most frequently seen arsenic valence state in drinking water is iAs3+. Some trace elements in part of drinking water samples exceed the standard limits, this may aggravate arsenic poisoning.

Objective To investigate the infection rates of mycoplasma penetrans (Mpe),mycoplasma pneumoniae (Mp) and mycoplasma ferments (Mf) in patients with IgA nephropathy (IgAN),chronic kideny disease (CKD),and heathy people,to compare the difference of infection rate,and to analyze the association of mycoplasma infection and clinicopathological features in IgAN.Methods Blood samples were collected from 118 patients in IgAN group,90 patients in CKD group,and 89 cases in health control group.DNA of Mpe,Mp and Mf was detected in plasma by PCR.Positive cases were confirmed by Southern blot.According to mycoplasma infection,IgAN patients were divided into two groups,then analyzed the clinicopatholgical features.Results (1)Genus,Mpe,Mp,and Mf positive rates were 33.05％,16.1％,25.45 ％,and 8.47％ in IgAN group,respectively; 5.56％,2.22％,5.56％,and 2.22％ in CKD group,respectively; and 3.33 ％,1.11％,2.22％,and 0 in health group,respectively.Compared with CKD and health group,patients in IgAN group had a higher infection rate in Genus,Mpe,and Mp (P ＜ 0.05).In IgAN group,10 patients had three kinds of mycoplasmas infection at the same time,and positive rate was 8.47％ much higher than CKD group (positive rate was 2.22％) (P ＜ 0.05).(2) Based on mycoplasm detection results,IgAN patients were divided into two groups,overlapping infection group and mycoplasma negative group.In overlapping infection group,the mean age of onset was much younger than negative group.Compared with negative group,overlapping infection group had higher tonsillitis and urinary tract infection rate,more severe microscopic hematuria and tubulointerstitium lesion (P ＜ 0.05).Conclusions Patients with IgAN had higher infection rate of Genus,Mpe and Mp,compared with CKD patients and health people.Compared with mycoplasma negative group in IgAN patients,more severe microscopic hematuria and tubulointerstitium lesion in overlapping infection group,which suggested that infection of Mpe might have some possible connection with IgAN.

Objective To investigate whether systemic inflammatory response syndrome(SIRS) after cerebral infarction is associated with poor outcome and its associated clinical factors.Methods We prospectively studied 500 patients with cerebral infarction.recorded the associated clinical factors on presentation and calculated the mortality at 21 st day.Results There are 85 SIRS patients in 500 patients with cerebral infarction,31 with total anterior cerebral infarction(TACI),34 with partial anterior cerebral infarction(PACI),15 with posterior cerebral infarction(POCI)and 5 with lacunar cerebral infarction (LACI).The frequency of SIRS was positively correlated with mortality rate according to Oxfordshire Commanity Stroke Project(OCSP)subtypes in cerebral infarction patients with fever(Spearman correlation coefficient=1.0:P＜0.001).Single factor analysis showed that the risk factors for both SIRS and mortality of cerebral infarction were as follows:age,infection,48h Chinese Stroke Scale score,48h Glassgow score,OCSP subtypes and dysphagia.Diabetes Was the risk factor for SIRS.but had no effect on mortality.On Cox regression,48h Glassgow score was the sole independent risk factor of outcome.However.if SIRS was included in the formula.P＞0.05.ConclusionsSIRS is the predictor of poor outcome after acute cerebral infarction.It is important to prevent SIRS especially in TACI and POCI.Diabetes is the risk factor for SIBS.but has no effect on mortality.

Objective:To study the clinical effect of letrozole combined with triptorelin acetate (GnRH-a) on the serum estrogen level and ovulation quality of patients with polycystic ovary syndrome(PCOS).Methods:112 cases of PCOS patients in our hospital from August 2014 to January 2016 were selected and randomly divided into the control group (50 cases) and the observation group (62 cases) according to the wishes of patients.During the first 3～7 days of the menstrual cycle,the patients in the observation group were treated by LE,2.5 mg/d;the patients in the control group were given intramuscular injection of human menopausal gonadotropin (HMG),75 IU/d.when the diameter of the largest follicle (MFD)≥ 18 mm,the patients in the observation group were were given subcutaneous injection of 0.1 mg triptorelin acetate induced ovulation,patients in the control group were given intramuscular injection of Human chorionic gonadotropin HCG6000～10000IU induced ovulation.The ovulation induced ovulation effect and the serum hormone levels and pregnancy outcome were compared between two groups.Results:On the induced ovulation day,no significant difference was found in the endometrial thickness,the number of mature follicles,serum LH and P levels,rate of pregnancy and the luteal function between two groups of patients (P＞0.05),the number of dominant follicles,serum E2 and T levels,multiple pregnancy rate,the incidence of OHSS and ovarian cyst in observed group were significantly lower than those of the control group (P＜0.05).Conclusion:LE combined with GnRH-a could effectively improve the quality of ovulation in patients with PCOS,reduce the serum level of estrogen,and prevent the occurrence of OHSS,improve the outcome of pregnancy.

Objective To evaluate clinical effects of integrated traditional Chinese and western medicine therapy for epilepsy after stroke. Methods Literatures about the treatment of epilepsy after stroke with the combination of traditional Chinese medicine and western medicine which come from the database such as CNKI, VIP, WanFang and Pubmed, were studied and all the randomized controlled trials on comparing therapeutic effects of treating epilepsy between combined treatment of TCM and western medicine and western medicine were enrolled. The Cochrane Collaboration's software RevMan 5 was used for meta-analysis. Results 7 articles were included in the study, there is homogeneity among these articles(P>0.05), The fixed effect model was used for the meta analysis. the combined results were OR=5.17, 95%CI(3.02, 8.82), the overall effect of test Z=6.01, P<0.01, the difference between the treatment group and the control groups was significant, that means the overall effect of the combination of traditional Chinese medicine and Western medicine treatment was better than western medicine treatment; Funnel plot graphic displayed asymmetrically, which indicated the literatures publication bias. Conclusion The current data indicate that the curative effect of TCM and west medicine combined treatment is better than pure western medicine therapy for epilepsy after stroke, but as the literature publication bias, clinical studies need more high quality large samples to prove this conclusion.

Objective To explore the suicide ideation and related factors in elderly with depressive disorder.Methods A sample of 60 old patients meeting the criteria of ICD-10 for depressive disorder and 60 normal controls were assessed by the Self-rating Idea of Suicide Scale(SIOSS),Trait Coping Style Questionnaire (TCSQ),Self-Rating Anxiety Scale(SAS),Self-Rating Depression Scale(SDS) and Pittsburgh Sleep Quality Index(PSQI).Results The score of despair factor,optimistic factor,sleep factor,conceal factor and total score of SIOSS in patients with depressive disorders (6.22±3.28,2.43± 1.47,1.87± 1.58,1.17± 1.1 1 and 10.51±4.91,respectively) were significantly higher than those of normal controls(t=5.83-14.03,P＜0.01).The scores of SIOSS in patients were significantly positively related with the scores of SAS,SDS,PSQI and N C (r=0.311-0.792,all P＜0.05),but negatively related with the scores of PC(r=-0.761,P＜0.01).Moreover,regression analysis showed that the suicide ideation was increased in accordance with serious depression,anxiety,sleep quality,higher level of education and decreased ability to positive response in elderly with depressive disorder(R 2=0.853,F=62.768,P＜0.01).Conclusion The suicide ideation in elderly with depressive disorder is more serious than that of normal people,and may be influenced by depression,anxiety,sleep quality and coping style.

Objective To conduct a methodological evaluation on flowcytomotry fluorescence detection of serum CA242, and to evaluate its value in the diagnosis of pancreatic cancer.Methods The blood samples of 40 cases of pancreatic cancer, 49 cases of other tumors, 48 cases of benign digestive diseases and 159 healthy volunteers were collected.Fowcytometry fluorescence immunoassay was used to detect serum level of CA242, and it was compared with routine ELISA method to measure its sensitivity and specificity.Results The detection limit of CA242 by flowcytometry fluorescence immunoassay was 0.89 U/ml, the linear range was 1～ 500 U/ml after confirmation.The within-batch CV was 3.37％～ 5.30％, between batch CV was 7.43％ ～ 9.60％.When compared with routine ELISA, flowcytometry fluorescence immunoassay showed the equation of linear regression is Y =1.0398X-0.947, r =0.9687.Area under ROC curve was 0.811 ± 0.025 (95％ CI 0.763 ～0.859), with 18.625 U/ml as the best cutoff value, the specificity was 92.0％ and the sensitivity was 62.1％.Conclusions Flowcytometry fluorescence immunoassay for CA242 testing has the advantage of shorter detection time, miltiple sample and testing project detection, which is worth of clinical application.

Objective To evaluate the values of urinary liver-fatty acid binding protein (uL-FABP) and urinary neutrophil gelatinase-associated lipocalin (uNGAL) in diagnosis of acute kidney injury (AKI) caused by obstructive nephropathy and in the prediction of renal prognosis.Methods Clinical data of 30 patients with obstructive nephropathy were collected prospectively.uL-FABP and uNGAL were measured by ELISA at various time points.Risk factors of the renal outcome were evaluated.The patients were followed up for at least one year.Results Patients with AK1 had higher levels of uL-FABP and uNGAL compared to those without AKI [700.00(154.62-1216.14) μg/g· Cr vs 26.90 (16.77-41.38) μg/g·Cr; 1266.69 (671.57-3396.07) μg/g·Cr vs 179.12 (90.98-215.16) μg/g·Cr,all P ＜ 0.01].Positive correlations of uL-FABP and uNGAL with serum creatinine were found (r =0.552,0.553,all P ＜ 0.01).The AUCs of uL-FABP and uNGAL to detect AKI were 0.925 and 0.900.Patients with non complete renal recovery had higher levels of uL-FABP before operation and 72-hour after operation compared to those with complete renal recovery (all P ＜ 0.01).Before operation,the AUC of uL-FABP to detect renal prognosis was 0.948,sensitivity was 85.7％ and specificity was 90.9％.72-hour after operation,the AUC of uL-FABP to detect renal prognosis was 0.935,sensitivity was 85.7％ and specificity was 90.9％.Kaplan-Meier analysis revealed that uL-FABP before operation over 366.57 μg/g · Cr or uL-FABP 72-hour after operation over 223.60 μg/g · Cr were closely related to the poor progression of renal function.Conclusions uL-FABP and uL-NGAL have good accuracy in detecting AKI.The level of uL-FABP before operation and 72-hour after operation is helpful to predict the renal outcome of obstructive nephropathy.
