1.Clinical analysis of polycythemia vera among different nationalities in Xinjiang Uygur Autonomous Region

Objective To investigate and compare the clinical feature and natural course of polycythemia vera(PV)between Uygur and Han nationalities in Xinjiang Uygur Autonomous Region.Methods Clinical symptoms,signs,laboratory tests and prognosis of 37(20 Han,13 Uygur,4 other nationalities)patients with PV were analyzed.Results There were 22 males and 15 female.The mean age of all patients was 59.41±11.31(31-85)years old.The mean hemoglobulin level was(191.11±21.41)g/L.Erythremia combined with leukocytosis or thromboeytosis were observed in 23(62.2%)or 19(51.4%)patients respectively.Splenomegaly was found in 27(73.0%)patients and hepatomegaly in 9(24.3%).Serum LDH was elevated in 28(75.7%)patients.Twenty two patients(59.5%)had vascular thrombotic events.Two patients developed MF.N0 secondary neoplasm was identified.Two patients died of thrombosis.Over 60 years old,leukocytosis and thrombocytosis were the high risk factors associated with thrombosis.No significant differences of clinical manifestations and thrombotic events have been found between Uygur and Han patients in Xinjiang.Conclusion Over 60 years old,leukocytosis and thromboeytosis were probably the high risk factors of thrombosis in PV patients.There was no significant differenee in clinical feature and natural course of PV between Uygur and Han nationalities in Xinjiang Uygur Autonomous Region.
2.Comparison of the effect between PFNA with DHS in the treatment of intertrochanteric fracture in elderly case

Objective To compare the clinical effects of proximal femoral nail antirotation (PFNA) and dynamic hip screw (DHS) in the treatment of intertrochanteric fractures in elderly patients.And provide a reasonable basis for clinical treatment of elderly patients with intertrochanteric fractures.Methods The clinical and follow-up records of 58 elderly patients with intertrochanteric fractures treated by PFNA and DHS were retrospectively reviewed.For times of operation,blood loss in operation,length of incision,incidents of complication and Harris's hip functional standard score were compared and analyzed.Results PFNA group operation time[(59.61 ± 8.27)min],amount of bleeding[(234.51 ± 38.80)ml] were both better than the DHS group of[(83.54 ± 11.12)min and (446.57 ±54.01) ml] respectively.There were significant differences (t =9.80,18.10,all P < 0.05).The satisfactory rate was 92.8% in PFNA group,higher than the DHS group's 83.3 % (x2 =6.18,P < 0.05).There was significant difference between two groups in the incident of complication,DHS group was higher.Conclusion PFNA internal fixation with a minimally invasive,fixed solid and recovery fast,is the better internal fixation in the treatment of elderly intertrochanteric fracture patients.
3.Serial Processing of Exogenous and Endogenous Information Conflicts in Human Brain

Objective To investigate the cortical activities for processing exogenous stimulus-related information conflict and endogenous mental-related information conflict.Methods Event-related potentials (ERPs) were recorded while 15 healthy subjects performed a digit matching task. Each trial consisted of two sequentially presented digits (S1~S2), where S2 was either the same as S1 (S-) or different from S1 (S+) in digital value. Subjects pressed a button of a push-pad when S2 was the same as S1 and pressed the other button when S2 was different from S1 in the first session. Furtherly, they were required to calculate the difference between S1 and S2, and compare their calculation result with digit '3' in the second session. They pressed a button when the calculation result was equal to '3' (M-) and pressed the other button when it was not (M+). Three trial types were included: same numbers but their difference was not equal to '3' (S-M+), different numbers but their difference was '3' (S+M-) and different numbers but their difference was not '3' (S+M+). Results Following S2 onset, a component N270 was broadly elicited at all the scalps by S+, S+M- and S-M+. N270 and N400 were elicited in series by S+M+. The maximal amplitude of N270 was at the posterior scalp while the maximal amplitude of the N400 was at the central areas. Conclusion The different spatio-temporal distributions of the two negative potentials suggested that exogenous and endogenous information conflicts were serially processed in the human brain by the conflict processing system of multiple neural substrates.
4.The expression of BMP-2 mRNA of rat osteoblast cell sheets cultured with ascorbic acid

Objective:To study the effects of ascorbic acid on BMP-2 mRNA expression of osteoblast cell sheets.Methods:Rat os-teoblasts were primaryly cultured and identified;osteoblast cell sheets were built by physical scraping method in vitro;the osteoblast cell sheets were cultured with 1 5,50 and 85 mg/L ascorbic acid for 1 and 2 weeks respectively,and the expression of BMP-2 mRNA of the cell sheets was detected by RT-qPCR.Results:The obtained cells were conformed to be osteoblasts.The osteoblast cell sheets could be rolled into tube in vitro.The expression of BMP-2 mRNA of osteoblast cell sheets in experiment group,whether in week one or week two was higher than that in control group,50 mg/L group showed the highest expression(first week P <0.05;second week P>0.05);the expression of any group in week two was higher than that in week one(P <0.05).Conclusion:Ascorbic acid may pro-mote the expression of BMP-2 mRNA in osteoblast cell sheets.
5.Effects of Eldepryl on TH and GDNF expressions in substantia nigra and striatum in Parkinson’s disease model in rat

Objective To observe the effects of Eldepryl on expressions of tyrosine hydroxylase (TH) and glial cell line-derived neurotrophic factor (GDNF) in substantia nigra and striatum in Parkinson’s disease (PD) and to explore the protective mechanism of Eldepryl on dopaminergic neuron . Methods Healthy male Sprague-Dawley (SD) rats (n=72) were randomly divided into control group, model group and Eldepryl group (n=24 in each group). Each group was divided random?ly into 2 subgroups as 4 day treatment group and 8 day treatment group (n=12 in each subgrop). Pakinson’s disease model was established by injecting rotenone subcutaneously back the neck, rats in the control group were injected with an equal vol?ume of sunflower oil subcutaneously at the same location. Rats in the Eldepryl group were then given Eldepryl 0.5 mg·kg-1 in?tragastrically every day for 4 or 8 consecutive days and rats in model group and control group were given an equal volume of saline instead. The expression of TH and GDNF in substantia nigra and striatum were detected by immunohistochemistry and Western blotting. Results Immunohistochemistry and Western blotting showed that strong expression of TH positive cells with little expression of GDNF positive cells were seen in substantia nigra and striatum in rats of control group, and there was no significant difference between subgroup of 8 day treatment and 4 day treatment within control group. The expression of TH cells and GDNF were both significantly reduced in model group compared with those in control group (both P<0.05), and there was no significant difference between subgroup of 8 day treatment and 4 day treatment within each group. The ex?pression of TH positive cells were significantly reduced in Eldepryl group compared with those in control group, and were sig?nificantly increased compared with those in model group. The expression of GDNF positive cells were significantly increased in Eldepryl group compared with those in control group and model group (all P<0.05). And there were significantly more ex?pression of TH positive cells and GDNF positive cells at subgroup of 8 day treatment compared with those at subgroup of 4 day treatment within Eldepryl group with (all P<0.05). Conclusion These data suggest that Eldepryl can protect the dam?age of dopaminergic neurons in substantia nigra and striatum of PD rats. And its therapeutic mechanism may be associated with increased expression of GDNF.
7.Efflux pumps in multidrug resistant Escherichia coli

Objective To explore the prevalence of 11 efflux pumps in isolates of multidrugresistant Escherichia coli(MDR-ECO). Methods Efflux pumps emrB, emrD , emrE, mdfA, sugE, mdtl,tehA, oqxA, qacE△1, qacE and smr-2 were detected by polymerase chain reaction(PCR)in 20 MDR-ECOs isolated from clinical samples. Results Efflux pumps emrB, emrD, emrE, mdfA, sugE, mdtI, qacE △1, tehA and oqxA were detected, and 8 efflux pumps were found in the same strain. Conclusion Multidrug- resistance in Escherichia coli may be related with efflux pumps.
8.Dosimetry of three-dimensional confonnal, IMRT and simplified IMRT on rectal cancer post operative pelvic radiotherapy

Objective To compare the dosimetry difference among three-dimensional conformal radiotherapy (3 DCRT),simplified intensity-modulated radiotherapy (sIMRT) and intensity-modulated radiotherapy (IMRT) in whole pelvic irradiation in postoperative rectal carcinoma,in order to optimize the protocol for clinical practice.Methods From 2006 to 2008,10 patients with stage II and ID rectal cancer after radical resection (Dixon surgery) participated in this study.3DCRT,sIMRT and IMRT were performed for each patient.The dose distribution of target volume and normal tissues,conformal index (CI) and HI were analyzed using the dose-volume histogram (DVH).Results The CI for PTV of IMRT and sIMRT was superior to that of 3DCRT.3DCRT had the best HI in PTV target area dose distribution,while IMRT was similar with sIMRT,however,there were no significant difference among them.As regarded as the protection on organs at risk,for bladder,IMRT was superior to 3DCRT and slightly better than sIMRT;for small intestine,sIMRT showed better performance than 3DCRT while IMRT was better than sIMRT but with no significant difference;for colon,no dosimetry difference was found among three plans;for caput femoris,IMRT and sIMRT were better than 3DCRT.Additionally,sIMRT was similar to 3DCRT in MU of segments,but significantly lower than IMRT.The mean values of total MU for 3DCRT,sIMRT and IMRT were 569.73 ±48.69,542.97 ±69.78,and 770.25 ±73.12,respectively.Conclusions All of 3DCRT,sIMRT and IMRT could provide target area with sufficient and accurate dose,meanwhile they could also protect organs at risk well on rectal cancer after radical resection.Compared with 3DCRT plan and IMRT plan,sIMRT plan might be the optimal plan for clinical practice.
9.Endovascular therapy for complicated aortic dissection

Objective To explore the method of endovascular therapy for complicated aortic dissections.(Methods) The clinical data of 25 patients with complicated aortic dissections were analysed retrospectively.Results The patients′ ages ranged from 31-72 years with a mean of 50.2 years.Among the 25 cases,6 cases had severe ischemia of mesenteric artery,5 cases had renal artery ischemia,4 cases had severe(ischemia) of both legs,3 cases had renal arteries ischemia combined with superior mesenteric artery ischemia,2 had complicated aortic dissection combined with AAA,and in 5 cases the true aortic lumen was totally(compressed) by the false aneurysmal lumen.All patients underwent endovascular therapy,and the instant(technique) was successfully performed in all patients.Endoleak occurred in 3 cases after the stent-graft(deployment),it stopped spontaneously in 2 of them 7 days later,and 1 case with endoleak waiting for(treatment).In the other 22 patients,angiography after the operation showed that all the diseased area were sealed completely,and the viscera arterial blood supply was restored mainly via the true lumen.All the(patients) were cured and discharged.Conclusions In the management of complicated aortic dissections,(endoluminal) technique is simple,less traumatic,safe and has less complications as compared to the traditional operation.Improvement of the endoluminal technique is essential for successful treatment of these complicated cases.
10.Effect of multimodal warming regimen on development of intraoperative hypothermia in patients undergoing precise hepatectomy

Objective To evaluate the effect of multimodal warming regimen on the development of intraoperative hypothermia in the patients undergoing precise hepatectomy.Methods Sixty patients of both sexes,aged 28-64 yr,weighing 44-89 kg,of American Society of Anesthesiologists physical status Ⅰ or Ⅱ (Child-Pugh A),undergoing precise hepatectomy,were equally and randomly assigned into control group (group C) and muhi-model warming group (group M) using a random number table.A bed sheet was covered on the body exposed,and the abdominal cavity was washed using the room-temperature peritoneal layage fluid in group C.In group M,after admission to the operating room,the room temperature was raised to 25-26 ℃,the mattress was prewarmed using the medical heating blanket,the temperature was maintained at 40 ℃ during operation,the lower body was simultaneously covered using the forced-air warming system,and the temperature was maintained at 37-39 ℃;the solution used for infusion was warmed to 37 ℃ before anesthesia;the incisional area was washed with 0.9% sodium chloride injection which was prewarmed to 37 ℃ during operation.The development of hypothermia,hypotension and respiratory depression was recorded during operation.Before induction of anesthesia,at 1 h after anesthesia,at the end of operation,and at 5 min before discharge from postanesthesia care unit (PACU),blood gas analysis was performed,and the pH value,base excess and lactate level were recorded.The intraoperative blood loss,emergence time,duration of PACU stay,and occurrence of shivering,agitation,nausea and vomiting were also recorded.Results Compared with group C,the incidence of intraoperative hypothermia,and shivering,agitation,nausea and vomiting in PACU were significantly decreased,the emergence time and duration of PACU stay were significantly shortened,the pH value was significantly increased at 5 min before discharge from PACU,and the base excess and lactate level were significantly decreased at the end of operation and 5 min before discharge from PACU in group M (P<0.05 or 0.01).Conclusion Multimodal warming regimen can raise the quality of recovery from anesthesia and decrease the development of intraoperative hypothermia in the patients undergoing precise hepatectomy.
