1.Expression of glucose transporter-3 mRNA in rats' focal cerebral ischemic penumbra

Objective To investigate the expression of glucose transporter 3(GLUT 3) transcription level at different ischemic time in rats' focal cerebral ischemic penumbra and core of cortices.Methods Focal ischemic models of middle cerebral artery occlusion (MCAO) in rats were made by inserting nylon surgical thread. Brain samples were harvested from ischemic penumbra and core of cortices. The change of GLUT 3 mRNA was assessed by RT PCR.Results GLUT 3 mRNA in the penumbra regions slightly increased at 3 h after ischemia, and reached a peak at 24 h, in 96h following ischemia it came back to the normal level. As for ischemic core regions, GLUT 3 mRNA had a short time increase at 3 h after ischemia, and then rapidly decrease to the low level. Conclusion GLUT 3 mRNA expression was notably up regulated in the penumbra regions after focal cerebral ischemia,it may be a protective reaction against ischemic injury.
2.Evaluation of the clinic effects of perioperative psychological intervention combined appropriate sedation by target-controlled infusion of midazolam during operation via SCL-90 and OAA/S in patients undergoing thumb or finger reconstruction

Objective To evaluate the clinic effects of perioperative psychological intervention combined appropriate sedation by target-controlled infusion of midazolam during operation via symptom check List 90 (SCL-90) and observers assessment of alertness/sedation scale (OAA/S) in patients undergoing thumb or finger reconstruction.Methods Eighty patients undergoing thumb or finger reconstruction were randomly divided into group A,B,C,and D (n =20 cases per group).All patients were received brachial plexus block and spinal-epidural combined anesthesia.Patients in group A were given perioperative psychological intervention combined appropriate sedation by target-controlled infusion of midazolam during operation; patients in group B were received routine treatment; patients in group C were given perioperative psychological intervention; and patients in group D were sedated with midazolam during operation.The vital signs and the scores of OAA/S were observed during operation.The scores of visual analogue scale (VAS) and SCL-90 were recorded after operation.Results The OAA/S grade in groups A and D were stability than that in groups B and C (P < 0.05) ; the scores of VAS and the results of psychological status assessment by SCL-90 in group A were superior to groups B,C,and D (P < 0.05).Conclusions Perioperative psychological intervention combined appropriate sedation in operation on thumb or finger reconstruction is a excellence anaesthesia processing method.It might have beneficial effects on postoperative recovery profile,and strengthen the tolerance to pain and improve the patient's psychological status during operative period.
3.CT Manifestations and Pathological Features of Pneumonia-type Lung Cancer

Purpose To explore the CT manifestations and pathological features of pneumonia-type lung cancer, and to improve the diagnosis capability of pneumonia-type lung cancer. Materials and Methods CT and pathologic features of 33 cases of biopsy or surgical pathology confirmed pneumonia-type lung cancer patients were retrospectively analyzed and classified according to the new pulmonary adenocarcinoma classification. Results Among the 33 pneumonia-type lung cancer subjects, CT showed multiple distributions within both lungs in 22 cases, lateral lobe distribution in 9 cases, segmental distribution in 2 cases. Lung consolidation was found in all lesions, among them 21 cases appeared mainly as lower lobe consolidation, 32 cases as inflatable bronchial symptoms within the consolidation, including inflatable bronchoconstriction in 24 cases;consolidation with multiple pulmonary nodules found in 23 cases;ground glass shadowing around or distal to the consolidation found in 26 cases;combined with cysts or honeycomb lung symptom in 13 cases. Inhomogeneous mild enhancement of lung consolidation after enhancement found in 18 cases and inhomogeneous moderate enhancement in 15 cases;blood vessel branch shadow within the consolidation was visible in 21 cases. Pathology results revealed 26 cases of invasive mucinous adenocarcinoma, and 7 cases of invasive mucinous adenocarcinoma partial mixed with papillary or alveolar adenocarcinoma. Conclusion CT features of pneumonia-type lung cancer are single or multiple opacities, within which inflatable bronchial symptoms can be observed, with multiple nodules and ground glass shadowing, cysts or honeycomb symptom can also be found concomitant, the lesions expand, increase and spread to both lungs, taking its dynamic change features into consideration will also help to improve the diagnostic accuracy.
4.Genotyping and Homology Comparison of Meticillinresistant Staphylococcus aureus Isolated from Earthquake Victims

OBJECTIVE To investigate the genetic feature of meticillin-resistant Staphylococcus aures isolated from earthquake victims and compare them with those of common patients.METHODS The MRSA isolates underwent SCCmec(Staphylococcal Cassette Chromosome mec) multi-PCR and PVL gene detection.Diversilab method was used for homology comparison and antibiotic resistance was also analyzed.RESULTS All the MRSA isolates were confirmed to be type Ⅲ SCCmec with PVL gene negative.Homology analysis showed relatively consistent with community-acquired MRSA confirmed to be ST8 by MLST typing.CONCLUSIONS There may be cross-spread of MRSA between hospital and community.We should pay more attation even in emergent time.
5.Subclavicular angiography with brachial-axillary artery externally and temporally occluded in interventional treatment of hemoptysis

Objective To evaluate subclavicular angiography with brachial-axillary artery externally and temporally occluded in depicting hemoptysis-related ectopic arteries originating from shoulder, neck and chest wall in the interventional treatment of hemoptysis. Methods A retrospective analysis of the medical records and angiograms of 103 patients who underwent transarterial procedures for hemoptysis was carried out. The arteries originating from the subclavicular arteries supplying the shoulder, the neck and the chest wall, ectopic bronchial arteries (EBA) as well as non bronchial systemic arteries (NBSA), were depicted by subclavicular angiography with brachial-axillary artery externally and temporally occluded. The number and distribution of the responsible aberrant arteries were investigated. Results Of the hemoptysis-related arteries(n=372) depicted and embolized in the studies, 51.9% was normally-originated bronchial arteries(n=193),6.7% was ectopic bronchial arteries (n=25),41.4% was NBSA(n=154).Both anomalous and NBSA arising from subclavicular-axillary area(n=80) were accounted for 21.5% (80/372) of all aberrant arteries, and 44.7%(80/179) of all anomalous and NBSA (n=25+154=179).Of those arising from subclavicular-axillary arteries,56.3%(45/80) were found to be originated from the internal thoracic artery (n=45),98.2%(112/114)of responsible internal thoracic arteries and their distal branches were well depicted by subclavicular angiography with brachial-axillary artery externally and temporally occluded. Conclusion Subclavicular-axillary artery is a very important vessel where EBA and NBSA originate. Subclavicular angiography with brachial-axillary artery externally and temporally occluded could help to determine the abnormal arteries in the region quickly. The method should be taken as a routine procedure for interventional treatment of hemoptysis.
6.Insulin stimulates translocation of GLUT4 and glucose uptake in ischemic myocar dium in dog

Objective: To investigate whether insulin stimulates the translocation of glucose transporter-4 (GLUT4) and glucose uptak e in ischemic myocardium. Methods: Plasma concentration of gluc ose, lactate, free fatty acid and insulin were determined by autoanalyser, and G LUT4 was studied by Western blotting analysis. Results: Insulin increased GLUT4 significantly in sarcolemma of ischemic myocardium [(25±4)% vs (40±6)%], and GLUT4 content in intracellular membrane decreased proporti onally. The glucose uptake increased significantly in insulin-ischemic myocardi um. The uptake of insulin-ischemic myocardium was almost 2 times that of ischem ic myocardium. Conclusion: Insulin stimulation results in GLUT4 translocation and increases glucose uptake in ischemic myocardium. When myocardi al ischemia occurs, insulin is helpful in increasing myocardial glucose uptake a nd utilization.
7.Investigation of nasal mucosa diseases changes with age and local climate

[ABSTRACT]OBJECTIVETo study of the relationship of age and climate with the nasal mucosa lesions.METHODSAll the patients met with the inclusive criteria were registered and followed-up once a month and their nasal mucosal lesions were observed through nasal speculum and/or nasal endoscope. The recording data was analyzed by SPSS17.0 software.RESULTSA total of 259 patients, 555 person-times and 957 lesion spots were observed. The nasal mucosa was injured by airflow with the highest incidence rate from October to next year April. Patients numbers gradually increased after August, and significantly increased at November, December (P<0.05). Children aged from 2 to 12 years old and the adults aged from 40 to 80 years old were susceptible population (P<0.05), the incidence rate had a negative relation with humidity (P<0.05). The susceptible spots of the lesion were ranked as follows: Little area, inferior turbinate, mucosa of the both side of the deviated nasal septum, inferior meatus, middle meatus, middle turbinate and olfactory area. CONCLUSIONNasal mucosal injury relates to age and seasons. The nasal cavity airflow may be a factor of these lesions.
8.Association of glucocerebrosidase gene polymorphism with Parkinson's disease in Han, Uygur and Kazak people in Xinjiang

Objective To study the polymorphism of glucocerebrosidase ( GBA) gene of N370S, V394L, L444P, R120W, R359X, R496H, R353W and RecNcil in the patients with Parkinson's disease ( PD) in Han, Uygur and Kazak in Xinjiang and to investigate the relationship between GBA gene polymorphism and Parkinson's disease.Methods GBA gene polymorphism was analyzed by improved multiplex ligation detection reaction technique in 294 sporadic PD patients (100 cases of Uygur, 134 cases of Han, 60 cases of Kazak) and 305 healthy controls (109 cases of Uygur, 122 cases of Han, 74 cases of Kazak) in Xinjiang area.Results There were two L444P loci polymorphisms that were heterozygous mutations in 294 cases of PD patients and the mutation frequency was 0.7%.Three hundred and five cases of control group did not show L 444P polymorphism.There were no significant differences in L 444P genotype and allele frequency distribution between PD group and control group ( AA:99.3%vs 100.0%, GA:0.7%vs 0, P>0.05;G:0.3%vs 0, A:99.7%vs 100.0%, P>0.05);L444P genotype and allele frequency distribution in Han and Uygur patients with PD showed no significant differences ( AA:99.3% vs 99.0%, GA:0.7%vs 1.0%, P>0.05;G:0.4%vs 0.5%, A:99.6%vs 99.5%,P>0.05);N370S, V394L, R120W, R359X, R496H, R353W, RecNcil loci polymorphisms were not found in the PD and control groups.Conclusion The GBA gene of N370S, V394L, R120W, R359X, R496H, R353W, RecNcil showed no polymorphism in Xinjiang Han and Uygur population and there was no association of L 444P polymorphism with Parkinson's disease in Han and Uygur populations in Xinjiang .
9.Approach to frailty in the elderly in primary care and the community.

Frailty is a distinct clinical syndrome wherein the individual has low reserves and is highly vulnerable to internal and external stressors. Although it is associated with disability and multiple comorbidities, it can also be present in individuals who seem healthy. Frailty is multidimensional and its pathophysiology is complex. Early identification and intervention can potentially decrease or reverse frailty, especially in the early stages. Primary care physicians, community nurses and community social networks have important roles in the identification of pre-frail and frail elderly through the use of simple frailty screening tools and rapid geriatric assessments. Appropriate interventions that can be initiated in a primary care setting include a targeted medical review for reversible medical causes of frailty, medication appropriateness, nutritional advice and exercise prescription. With ongoing training and education, the multidisciplinary engagement and coordination of care of the elderly in the community can help to build resilience and combat frailty in our rapidly ageing society.

10.Effect of double opposing rhomboid flap in repairing quasi-circular facial skin defects

Objective:To explore the clinical efficacy of double opposing rhomboid flap in repairing facial skin defects.Methods:From January 2020 to December 2020, 30 cases of facial skin lesions were removed in the Department of Plastic and Aesthetic Surgery, the Affiliated Hospital of Qingdao University, including 12 males and 18 females, aged 14-65 years, with an average age of 34.2 years. The diameter of the facial skin defect wound was 0.5-2.0 cm. The patients with facial skin defect were repaired with double opposing rhomboid flap. All patients were followed up for 3-12 months.Results:The incisions of 30 patients were healed in one stage, and double opposing rhomboid flaps survived. Following-up for 3 to 12 months showed that the operation area was flat, the incision scar was not obvious, the texture and color of the operation area and the surrounding skin matched well, the surrounding organs were not deformed, the lesions were not recurrent, and the cosmetic effect was satisfactory.Conclusions:The double opposing rhomboid flap is an effective method to repair quasi-circular facial skin defects, which is worthy of clinical application.
