1.Observation on the effect of dexmedetomidine on regional cerebral oxygen saturation and postoperative cognitive function in elder patients received prostatectomy by using laparoscope

Objective: To research the effect of dexmedetomidine on regional cerebral oxygen saturation(rSO2)and postoperative cognitive function in elder patients received prostatectomy by using laparoscope. Methods: 60 elderly patients received prostatectomy by using laparoscope were divided into observation group (30cases) and control group (30cases) as random number table. In observation group, the dexmedetomidine was injected by venous pump after endotracheal intubation of general anesthesia was implemented, while the equivalent normal saline (NS) was injected by venous pump in control group. The rSO2, PaCO2 and PaO2 of different time point between the two groups were observed and compared. The usages of propofol and remifentanil, the situation of postoperative restlessness, and situation of nausea and vomiting between the two groups were compared. Besides, the scores of MoCA of different time point between the two groups also were compared. Results: The usages of propofol and remifentanil of observation group were significantly lower than that of control group (t=3.449, t=9.466, P<0.05), respectively. The occurrence rate of postoperative restlessness and occurrence rate of nausea and vomiting were 10.0% and 6.7%, respectively, while they were 13.3% and 3.3%in control group, and the differences of them between the two groups were no significant (x2=0.162, x2=0.351, P>0.05). The score of MoCA of observation group was significant higher than that of control group (t=8.938, P<0.05) at postoperative 1d. Conclusion: In the elder patients received prostatectomy by using laparoscope, the dexmedetomidine doesn't affect the rSO2 level of patients, while it contributes to fast recover the postoperative cognitive function of patients.
2.A observation of early postoperative cognitive dysfunction and postoperative analgesia situation after intercostal nerve block was applied on elderly patient received thoracic surgery

Objective:To investigate the effect of early postoperative cognitive dysfunction and postoperative analgesia situation after intercostal nerve block was applied on elderly patient received thoracic surgery.Methods: 105 elderly patients underwent thoracic surgeries were divided into observation group (35 patients received intercostal nerve block combined with general anesthesia), control A group (35 patients received epidural anesthesia combined with general anesthesia) and control B group (35 patients received routine general anesthesia). The cognitive function, postoperative pain, intraoperative mean artery pressure (MAP) and heart rate of the patients among different groups were respectively compared.Results: The cognitive function scores in postoperative 12h, 24h, 72h of observation group were significantly higher than that of control A group (t=20.917,t=27.780, t=74.081,P<0.05), respectively. And all of these data also were significantly higher than that of control B group (t=37.922,t=48.969,t=62.653,P<0.05), respectively. The differences of MAP, HR value between observation group and control A group were statistically significant (t=18.927,t=22.380,P<0.05), respectively. And the differences of them between observation group and control B group also were statistically significant (t=31.051, t=19.932, P<0.05), respectively. Besides, the differences of pain scores in postoperative 6h, 12h, 24h, 48h between observation group and control A group were not statistically significant, while all of pain scores of observation group were significantly lower than that of control B group (t=18.731,t=19.035, t=21.093,t=17.036;P<0.05).Conclusion:Intercostal nerve block combined with general anesthesia can ensure more stable intraoperative vital signs for elderly patients underwent thoracic surgeries, and it contributes to improve early cognitive function and possesses better postoperative analgesic effect for elderly patients.
3.Diminished capacity of mesenchymal stem cells in lesions of patients with psoriasis to inhibit T cell proliferation

Objective To evaluate the inhibitory effect of mesenchymal stem cells (MSCs) in lesions of patients with psoriasis on T lymphocyte proliferation.Methods Tissue specimens were obtained from the lesions of 15 patients with psoriasis vulgaris (7 at progressive stage and 8 at resting stage) and normal skin of 15 human controls from the Department of Urology and Plastic Surgery,Taiyuan City Centre Hospital.MSCs were isolated from these skin specimens,cultured,and identified using flow cytometry and in vitro differentiation assay.Enzyme-linked immunosorbent assay (ELISA) was performed to detect the concentration of interleukin (IL)-6,IL-1 1,hepatocyte growth factor (HGF) and transforming growth factor (TGF)-β1 in the culture supernatant of third-passage MSCs.Peripheral blood T cells were obtained from a healthy adult and cocultured with the third-passage MSCs for four days.Then,cells were counted and methyl thiazolyl tetrazolium (MTT) assay was conducted to evaluate the proliferation of T cells.One-way analysis of variance (ANOVA) and Student-Newman-Keuls (SNK) test were carried out to compare the proliferation of T lymphocytes,and two independent samples t test to compare the concentrations of cytokines.Results Inverted microscopy revealed that the patient-and control-derived MSCs shared similar morphological properties and multi-directional differentiation capacity,along with the expression of CD29,CD44,CD73,CD90 and CD105,but absence of CD34,CD45 and HLA-DR on cell surface.After coculture with MSCs from the patients and controls for four days,the count of T lymphocytes per milliliter was (1.67 ± 0.34) × 105 and (1.04 ± 0.29) × 105 respectively (P< 0.01),and the proliferative activity (expressed as absorbence at 492 nm)was 0.317 ± 0.021 and 0.275 ± 0.007 respectively (P < 0.01).Compared with the control-derived MSCs,the patient-derived MSCs showed a significantly higher level of IL-1 1 ((181.37 ± 31.74) vs.(130.07 ± 29.20) ng/L,t =5.32,P < 0.01),but a lower level of lL-6 ((61.67±17.53) vs.(76.74±18.96) ng/L,t=2.61,P<0.05)and HGF ((319.24 ± 41.03) vs.(352.35 ± 51.47) ng/L,t =2.25,P< 0.05),as well as a similar level of TFG-β1,in the culture supernatant.Conclusions The inhibitory effect of MSCs in psoriatic lesions on T lymphocyte proliferation is diminished,which may contribute to the pathogenesis of psoriasis.
4.Preliminary observation on anti-tumor necrosis factor α therapy in fulminant ulcerative colitis

Objective To report three cases of fulminant ulcerative colitis (FUC) treated with infliximab (IFX) and the positive result for clinical purpose. Methods Three patients with FUC were infused with IFX (5 mg/kg) at interval of 0, 2, 6 wk. Sulfasalazine or probiotics was used for the maintanance of remission. The mucosal healing was evaluated by endoscopy and patholoic examination. ResultsComplete remission was found in 3 patients with FUC. Eight weeks after IFX withdraw, complete remission was found in two of three cases demonstrated by endoscopy, except for one case died from refractory ventricular tachycardia. The remaing 2 cases showed no active manifestation during 8 months' follow-up. Conclusion IFX therapy results in complete remission in 3 cases with FUC. However, further randomized control study is warranted for concrete evaluation on salty and application clinically.
5.Effects of temperature on the bioactivity of fat graft and adipose-derived stem cells situated in fat graft

Objective To observe the impacts of different storage temperatures on the activities of adipose tissues and the extraction and proliferation of adipose-derived stem cells (ADSCs) situated in harvested fat.Methods Adipose tissues obtained by liposuctions in healthy females were kept in different temperature;all groups of adipose tissues were studied in context of the histomorphology and activity of GAPD as well as the extraction efficiency and proliferation of ADSCs.Results Adipose tissues cryopreserved at 4 ℃ within 24 hours could retain their activities effectively,but their activities would decrease rapidly if they preserved at room temperature.The efficiency of adipose tissues extracting stem cells and their proliferations were affected by storage temperatures and times.If adipose tissues were preserved at room temperature for over 1 hour,the extraction efficiency and proliferation of their ADSCs would be lower significantly.Conclusions Adipose tissues cryopreserved at 4 ℃ can effectively ensure their activities and their extraction efficiency and proliferation of ADSCs.It is important to preserve adipose tissues at low temperature after harvesting fat graft.
6.Facial expression recognition based on feature selection by quadratic mutual information.

To solve the problem of imprecise positioning of feature point and of the feature data redundancy in facial expression recognition by active appearance models (AAM), the automatic adjustment of initial model for AAM fitting is proposed in this paper. The specific aims are to improve the precision of positioning and to more effectively reflect the variation of expressions by acquired features. The problem of feature selection is resolved by adopting quadratic mutual information and reducing the feature dimension. The support vector machine (SVM) classifier is used for expression recognition. The experimental results on CAS-PEAL facial expression database show that the proposed method effectively improves the performance of facial expression recognition, the maximum recognition rate being 83.33%.

7. Treatment of enlarged facial pores

Enlarged facial pores remain a common skin concern. Its occurrence is mainly associated with increased sebum production, skin aging (skin laxity and decreased skin elasticity) , increased hair follicle size and other factors, and current treatments for enlarged facial pores mainly target these factors. This review retrospectively summarizes current common treatments, including oral antiandrogens, topical retinoids, chemical peels, lasers, radiofrequency and focused ultrasound. The increasing development of photoelectric devices offers newer and more reliable choices.
8.Connexin 43 gene in the in vivo treatment of cerebral glioma in C6 rats.

OBJECTIVETo study the role of connexin gene (Cx43) in the suppression of C6 glioma.
METHODSCx43 gene depleted parental C6 rats (control group) and C6 cells transfected with Cx43 cDNA (transfection group) were implanted into the right caudate nucleus of SD rats. Rats bearing cerebral C6 gliomas were treated with Cx43 cDNA (treatment group) with another group treated with empty vector (empty vector group) serving as control. The general manifestation, survival time, MRI dynamic scanning and histopathological changes in all rats were observed. Cx43 mRNA and its protein were examined by in situ hybridization and immunohistochemistry. Proliferation activity was monitored by the average number of AgNOR stain. Cell apoptosis was examined by the Tolt-mediated x-duTP nick end labeling (TUNEL) method.
RESULTSAll rats in the control and empty vector groups died of cerebral glioma within 3 weeks after implantation of C6 cells. Six in the transfection group and 8 in the treatment group were alive beyond 120 days with complete disappearance of the tumor foci, except one in this group having some residue of tumor. In the glioma of transfection and treatment groups, Cx43 gene expression was up-regulated, proliferation activity reduced while the apoptotic cells did not increase.
CONCLUSIONThe development of glioma is greatly suppressed by the transfection of Cx43 gene, which has great effectiveness in rats bearing cerebral malignant gliomas. This could become a target of choice in the gene treatment of malignant gliomas.

9.Analysis of species distribution and antibiotic susceptibility trends of 1.172 bacterial uropathogens isolated in urine cultures on an outpatient basis

Objective:To assess the bacterial profiles and antimicrobial susceptibility patterns in uropathogens, and help to inform the empiric treatment decisions for urinary tract infection in outpatient settings.Methods:A single institutional retrospective analysis was performed on positive urine cultures from outpatient settings between January 1998 and December 2018. To analyze changes over time, trends analysis were undertaken on bacterial profiles, antimicrobial susceptibility and resistance.Results:A total of 1.172 pathogenic bacteria were isolated after exclusion of duplicate strains originated from the same patient, including 991(84.6%) Gram-negative bacterial strains and 181(15.4%) Gram-positive strains. The most common Gram-negative uropathogens were Escherichia coli (60.8%) and Klebsiella pneumonia (8.1%). Enterococcus faecalis (4.6%) was the predominant Gram-positive strain. The detection rate of Escherichia coli increased significantly, from 50.8% to 63.2% ( χ2=7.978, P=0.046), and no significant difference was observed in the distribution of major uropathogenic bacteria over the 20 years (all P>0.05). The proportion of extended-spectrum β-lactamase (ESBLs) producing strains increased significantly across the 20 years ( P<0.05). The resistance rates of Escherichia coli to amoxicillin and clavulanate potassium, aztreonam, ceftazidime, ciprofloxacin and sulbactam + cefoperazone increased significantly (all P<0.05). All the isolates sustained high susceptibility to tazobactam + piperacillin, amikacin, imipenem and nitrofurantoin (95.0%, 95.7%, 97.9% and 91.1%). Similar to those of Escherichia coli, Klebsiella pneumoniae remained a high and stable sensitivity to tazobactam+piperacillin, amikacin and imipenem during the 20 years (79.1%, 88.0% and 80.3%). However, the proportion of ESBLs producing strains increased significantly ( P<0.05). Among Gram-positive bacteria isolates, the sensitivity rates of Enterococcus faecalis to ampicillin, nitrofurantoin and penicillin G were 100.0%. No vancomycin resistant strain was detected in Gram-positive bacteria. Conclusions:From 1998 to 2018, Escherichia coli and Klebsiella pneumoniae are the most common Gram-negative bacteria uropathogens obtained in outpatient settings. Significant increases of resistance to some antimicrobial agents such as second- and third-generation cephalosporins and fluoroquinolones are observed during the 20 years and high susceptibilities to tazobactam+piperacillin, amikacin, imipenem and nitrofurantoin sustain over time. Local treatment strategies of urinary tract infections on outpatient basis should be made according to epidemiology of drug resistance and individual characteristics to control the spread and curb the prevalence of drug resistant.

This specification covers the manipulation of pediatric Tuina, the position of acupoints, the effects of acupoints and the diagnosis and treatment of pediatric Tuina, including indications, contraindications, cautious use, operation steps and methods.
