1.Clinical application of Carba NP test in detecting CRE strains

Objective To investigate the clinical application of Carba NP test in detecting carbapenem-resistant enterobacteriaceae.Methods Routine method was adopted to identify the 51 strains from clinical isolates and K-B test was conducted to screen out the carbapenem-resistant enterobacteriaceae and detect antimicrobial susceptibility.E-test was used to detect the minimum inhibitory concentration (MIC)of carbapenems.Modified Hodge test (MHT)and Carba NPⅠ test were used to screen out the phenotypes of carbapenemase,respectively. Then Carba NPⅡ test was used to classify carbapenemase which were positive in Carba NP I test.E-test was used for the detection of metalloenzymes.Polymerase chain reaction (PCR)was used for the detection of resistance genes. Results Totally 41 strains were positive in Carba NP 1 test and classified as B-class by Carba NPⅡ test.They were positive in metalloenzyme by E-test and carried NDM-1 gene by PCR.Conclusion Carba NP test can be used conveniently,has high consistency with PCR method,and the result is easy to determine.Therefore,it can be used as a routine laboratory method.

3.Perioperative treatment of breast carcinoma complicared with hyperthyroidism:76 cases

Objective To study the perioperative treatment of breast carcinoma complicated with hyperthyroidism.Methods The perioperative treatment of 76 cases of breast carcinoma complicated with hyperthyroidism in the period of 26 years was retrospectively studied.Results Hyperthyroidism was treated by surgical methods or medical methods in perioperative period.73 cases didn't have any correlative postoperative complications related to hyperthyroidism,3 cases showed postoperative thyroid crisis or clinical signs similar to thyroid crisis,and no death happened.Conclusions For breast carcinoma complicated with hyperthyroidism,if clinical symptoms of hyperthyroidism are apparent,hyperthyroidism should be treated first by surgical methods,and then breast surgery can be considered until basal metabolic rate resumes normal.If clinical symptoms of hyperthyroidism aren't remarkable,breast surgery can be performed directly on the premise of heart control.
4.Study on preparation of acellular matrix material fixed by oxidized sodium alginate and its cytocompatibility.

This study was intended to investigate the crosslinking characteristics of a new crosslinking agent-oxidized sodium alginate (ADA), which might provide an ideal biological crosslinking reagent for the construction of soft tissue bioprostheses. Glutaraldehyde and genipin, which have been typically used in developing bioprostheses, were used as controls. The porcine aortas were treated by these three crosslinking agents for 15 min to 72 h and the fixation index was determined. Subsequently, the mechanical property and cytocompatibility of fixed tissues were also tested. The results indicated that fixed tissues by ADA were comparable as glutaraldehyde and superior to genipin controls in fixative efficiency. It was also found that tissues fixed by ADA were comparable as genipin and superior to glutaraldehyde controls in cytocompatibility and were similar to natural tissues in mechanical property. The results of in vitro study demonstrated that ADA could be a promising crosslinking reagent for biological tissue fixation.

5.Hygienic status of central air conditioning ventilation systemsin public places in urban areas of Ma'anshan City

Objective:
To investigate the hygienic status of the central air conditioner ventilation system in public places in urban areas of Ma'anshan City, Anhui Province, so as to provide insights into formulation of supervision and management interventions.
Methods:
A total of 15 public places with the central air conditioner ventilation system were randomly sampled from main urban areas in Ma'anshan City in 2022. Sampling and detection were performed following the standard GB/T 18204.5—2013 Examination methods for public places Part 5: Central air conditioning ventilation system, including total number of bacteria and total number of fungus on the inner surface of wind pipes, total number of bacteria and fungus, particulate matter (PM10) and β-hemolytic streptococci in the air supply system, and Legionella pneumophila in the cooling water, and the detection indicators were assessed following the WS 394—2012 Guideline for hygiene of the central air conditioner ventilation system in public places. The eligible rate of samples, the detection rate of L. pneumophila were analyzed.
Results:
A total of 368 samples were collected from 15 public places, and the qualified rate was 50.54%, no places had all eligible measurement indicators. The qualified rates of total bacteria and fungus numbers on the inner surface of wind pipes were 52.67% and 59.33%. The qualified rates of total bacterium number, total fungus number, PM10 and β-hemolytic streptococci were 12.00%, 28.00%, 90.00% and 96.00% in the air supply system. The qualified rates of samples in administrative workplaces, hotels, bathing places and malls (supermarkets) were 32.50%, 59.24%, 61.09% and 68.92%, the qualified rates of total bacteria on the inner surface of air ducts were 8.33%, 72.46%, 66.67% and 61.90%, and the qualified rates of total fungus numbers in air supply were 0, 21.70%, 33.30% and 71.40%, respectively, with statistical significance (P<0.05). A total of 18 cooling water samples were collected, and L. pneumophila was detected in three samples (16.67%).
Conclusions
Poor hygiene is seen in the central air conditioning ventilation systems in public places in main urban areas of Ma'anshan City. High attention needs to be paid to contamination of bacterium, fungus and L. pneumophila, and expansion of supervision coverage and improved supervision intensity are recommended.
6.Allogeneic bone marrow mesenchymal stem cell transplantation can improve the function of the aging heart

BACKGROUND:Bone marrow mesenchymal stem cels can secrete a variety of factors in the local lesion, and these factors can promote cel proliferation and inhibit cel apoptosis. OBJECTIVE: To observe the curative effect of bone marrow mesenchymal stem cel transplantation on the aging heart of rats and to explore the possible mechanism of action. METHODS:Thirty Sprague-Dawley rats were randomized into three groups: normal blank group, model group and treatment group. Aging models were made in the latter two groups by injection of D-galactose. Rats in the treatment group were given alogeneic bone marrow mesenchymal stem cel injection, once a week, totaly four times. At 1 week after final injection, the heart tissues were sliced into sections to observe the pathological changes using hematoxylin-eosin staining. Western blot assay was used to detect the expression of basic fibroblast growth factor in the heart tissues. Real-time PCR was used to measure the expression of p53 mRNA in the heart tissues. RESULTS AND CONCLUSION:Bone marrow mesenchymal stem cel transplantation could improve the pathological morphology of the aging heart. Compared with the model group, the expression of basic fibroblast growth factor in the heart tissues was significantly higher in the treatment group (P < 0.05), but the mRNA expression of p53 was lower (P < 0.05). It is speculated that bone marrow mesenchymal stem cels can interact with heart cels to secrete basic fibroblast growth factor and reduce p53 mRNA expression, thereby playing a curative effect on the aging heart.
7.Literature review on clinical features and associated complications of dorsal pancreas agenesis

Objective To analysis the clinical features of dorsal pancreas agenesis ( DPA) and the associated diabetes, pancreatitis and other congenital organ malformations.Methods Chinese databases of Sinomed, CQVIP and CNKI using the term of short pancreas, pancreas agenesis, bulbar pancreas and dorsal pancreas, and English databases of PubMed using the term of dorsal pancreas agenesis, short pancreas and pancreas hypoplasia were searched.The clinical manifestation, pancreatic head characteristics and associations with diabetes, pancreatitis and other congenital organ malformations were analyzed.Results Six related publications from Chinese databases were searched and 21 patients were included with 2 cases excluded.Sixty-one publications from English database were searched and 71 patients were included.Thus, a total of 91 patients with DPA were analyzed.Abdominal pain was the most common manifestation, which was reported by 61.5% of the patients. 15.3% patients were identified during regular physical examination. Other manifestations including jaundice, fatigue, abdominal discomfort and diabetes were rare.After removing cases with insufficient information, 39 patients (61.9%) carried abnormal pancreatic head.Prevalence of diabetes or impaired glucose tolerance was 56.7% and the percentage of insulin-dependent diabetes in patients with abnormal glycaemia was 47.3%(n=18).20 patients (26.7%) were associated with pancreatitis, including 15 patients (75.0%) with acute pancreatitis, 1 patient (5.0%) with recurrent pancreatitis, and 4 patients (20.0%) with chronic pancreatitis. Thirty-three patients ( 36.2%) suffered other congenital organ malformations, including 21 patients (63.6%) with splenic malformation, 8 patients (24.2%) with heart malformation, and 17 patients (51.5%) with multi-organs malformations like gastrointestinal malformation, azygos vein and inferior cava vena fusion, duodenal and biliary atresia and renal absence.Conclusions The main diagnostic criteria of DPA was the absence of dorsal pancreatic duct.Diabetes was the most common complication followed by pancreatitis.
8.On the construction of the core competence training system of ICU specialized nurses in Yunnan Province based on patient safety

Objective:To construct an objective and scientific core competence training system for intensive care unit (ICU) specialized nurses in Yunnan Province, and to provide references for formulating a homogeneous and standardized ICU specialized nurse training strategy and assessment.Methods:Based on the theoretical basis of the nurse-patient safety competency framework, the relevant literature of nursing job competence was consulted, the core competency training system of ICU specialized nurses in Yunnan Province was constructed, and the expert letter questionnaire was designed through theoretical analysis. From January to May 2020, 15 experts of critical care medicine from Yunnan Province were consulted by Delphi method to establish a core competency training system for ICU specialized nurses in Yunnan Province. SPSS 21.0 was used to input and analyze the data.Results:A total of 2 rounds of Delphi expert consultation were carried out. The final core competency indicators for ICU specialized nurses included effective communication, teamwork, safety risk management, nursing quality improvement and professionalism, including 5 first-level indicators, 14 second-level indicators and 59 third-level indicators. The effective recovery rates of the two rounds of Delphi expert consultation questionnaires were 100.00% (15/15) and 86.67% (13/15), the expert authority coefficient values were 0.863 and 0.881 respectively, the Kendall harmony coefficients of the first-level index, the second-level index and the third-level index were 0.381, 0.299 and 0.250, and χ2 value was 22.86, 58.22 and 217.30, with statistically significant differences ( P<0.001). The 5 first-level indicators were safety risk management (0.360), professionalism (0.280), teamwork (0.200), nursing quality improvement (0.120), and effective communication (0.040) in descending order. Conclusions:The established core competence training system for ICU specialized nurses in Yunnan Province based on patient safety is reliable and scientific, which can provide objective basis for the on-the-job training, evaluation and development of patient safety education courses for ICU specialized nurses in Yunnan Province.
9.Relation between physical fitness and frequency of breakfast consumption among Chinese Han college students

Objective:
To examine the relation between physical fitness and frequency of having breakfast among Chinese Han college students so as to provide proper intervention and theoretical basis for improving college students’ physical fitness.
Methods:
The data from 2014 Chinese National Survey on Students’ Constitution and Health (CNSSCH) was used. The data of 47 172 Han students were calculated, the chi-square test wasused to compare the differences of the physical fitness passing rate in different subgroups of college students. The Log-binomial model was established to assess the relation between physical fitness and breakfast consumption.
Results:
The unqualified rate of physical fitness was 22.8%. Boys(24.7%), urban students (25.7%), and those who exercised less than one hour per day (23.3%) had lower physical fitness passing rates than girls(20.9%), rural students (19.9%) and those who exercised more than one hour per day (20.3%)respectively(χ2=101.38, 223.33, 32.20,P<0.01). The unqualified rate of physical fitness increased by age. The unqualified rate of physical fitness in eastern, central and western regions was 21.2%, 21.7% and 25.1% respectively. The unqualified rate of physical fitness for those with normal nutritional status (18.2%) was significantly lower than those who were underweight (26.0%) or overweight/obese(42.6%)(χ2=1 660.50, P<0.01). In this study, 53.2% of college students had breakfast every day,unqualified rate of physical fitness was higher among students with less regular consumption of breakfast(χ2=393.50, P<0.01). The Log-binomial model showed that the college students who had 3-5 breakfasts(PR=1.16, 95%CI=1.11-1.20), 1-2 breakfasts(PR=1.34, 95%CI=1.28-1.40), never had breakfast (PR=1.43, 95%CI=1.32-1.53) per week had higher physical fitness failure rates than those who had breakfast every day.
Conclusion
There is a significant association between physical fitness and breakfast consumption among college students. Developing the good habit of having breakfast is not only the component of healthy lifestyle but also an indispensable part to the improvement of college students’ physical fitness.

Objective:To establish the acute symptomatic osteoporotic thoracolumbar fracture (ASOTLF) classification system, and to examine the reliability and evaluate the effect of clinical application.Methods:A retrospective case series study was conducted to analyze the clinical data of 1 293 patients with osteoporotic thoracolumbar fracture(OTLF) admitted to Honghui Hospital from January 2016 to December 2018. There were 514 males and 779 females, aged 57-90 years [(71.4±6.3)years]. The T value of bone mass density was -5.0--2.5 SD [(-3.1±-0.4)SD]. According to the clinical symptoms a and fracture morphology, OTLF was divided into 4 types, namely type I(I occult fracture), type II(compressed fracture), type III (burst fracture) and type IV(unstable fracture). The type II was subdivided into three subtypes (type IIA, IIB, IIC), and the Type III into two subtypes (type IIIA, IIIB). of all patients, 75 patients (5.8%) were with type I, 500 (38.7%) with type II A, 134 (10.4%) with type IIB, 97 (7.5%) with type IIC, 442 (34.2%) with type IIIA, 27(2.1%) with type IIIB and 18 (1.4%) with type IV. After testing the validity of the classification, different treatment methods were utilized according to the classification, including percutaneous vertebroplasty (PVP) for Type I, PVP after postural reduction for Type II, percutaneous kyphoplasty (PKP) for Type IIIA, posterior reduction and decompression, bone graft fusion and bone cement-augmented screw fixation for Type IIIB, and posterior reduction, bone graft fusion and bone cement-augmented screw fixation for Type IV. The visual analog score (VAS), Oswestry disability index (ODI), Frankel grade of spinal cord injury, local Cobb Angle, and vertebral body angle (vertebral body angle) were recorded in all patients and in each type of patients before surgery, at 1 month after surgery and at the last follow-up. The neurological function recovery and complications were also recorded.Results:The patients were followed up for 24-43 months [(29.9±5.1)months]. A total of 3 000 assessments in two rounds were conducted by three observers. The overall κ value of inter-observer credibility was 0.83, and the overall κ value of intra-observer credibility was 0.88. The VAS and ODI of all patients were (5.8±0.7)points and 72.5±6.6 before surgery, (1.8±0.6)points and 25.0±6.3 at 1 month after surgery, and (1.5±0.6)points and 19.5±6.2 at the last follow-up, respectively (all P<0.05). The Cobb angle and vertebral body angle of all patients were (13.0±9.1)° and (8.0±4.6)° before surgery, (7.9±5.2)° and (4.6±2.9)° at 1 month after surgery, and (9.1±6.0)° and (5.8±3.0)° at the last follow-up, respectively (all P<0.05). At the last follow-up, VAS, ODI, Cobb Angle and VBA of each type of patients were significantly improved compared with those before surgery (all P<0.05). The spinal cord compression symptoms were found 1 patient with type IV and 5 patients with type IIIB preoperatively. At the last follow-up, neurological function improved from grade C to grade E in 1 patient and from grade D to grade E in 5 patients ( P<0.05). The lower limb radiation pain or numbness in 3 patients with type IV and 22 patients with type III preoperatively were fully recovered after surgical treatment at the last follow-up except for three patients. Conclusions:The ASOTLF classification is established and has high consistency and reliability. The classification-oriented treatment strategy has achieved a relatively satisfactory effect, indicating that the classification has a certain guiding significance for treatment of OTLF.
