Objective: To explore the relationship between the combinative intervention therapy and clinical curative effect, through used function magnetic resonance imaging (fMRI) to observe the change of detection data for patients with cerebral infarction pre and post-intervention therapy of acupuncture combined with traditional Chinese medicine (TCM). Methods: 40 patients with cerebral infarction were divided into control group (20cases, received normal treatment)and observation group (20cases, received combinative treatment of acupuncture and TCM). Pre and post therapy, the patients of every group received evaluation about state of illness and detection of fMRI. The score of national institute of health stroke scale (NIHSS) was used to evaluate the degree of neurologic impairment, and analyze the imaging manifestations of fMRI on patients with cerebral infarction at basal ganglia who received acupuncture combined with TCM. Some cerebral functional imaging techniques, included blood oxygenation level dependent( BOLD), fMRI and diffusion tensor imaging (DTI), were used to grasp the changes of relevant functional region of brain and different brain regions connected with function region of patients with cerebral infarction before and after therapy. Results: The NIHSS score of observation group was significantly less than that of the control group (t=5.450, P<0.05), and the total efficiency of observation group (95%) was significantly higher than that of the control group (75%) (x2=4.329, P<0.05). And the differences of the activation frequency, activation volume, ADCavg values and the values of FA between the observation group and the control group were statistically significant (t=5.425, 2.760, 3.453, 5.513, P<0.05). Conclusion: Acupuncture combined with traditional Chinese medicine can efficiently improve the status of neurologic impairment for patients with cerebral infarction, and the clinical efficacy before and after treatment can preferably be observed by fMRI.

Objective To investigate the influence of anti-depression therapy on inflammatory cytokines and quality of life(QOL) in patients with acute coronary syndrome(ACS) complicating depression.Methods Ninety-four patients with ACS complicating depression(HAMD total score ≥8) were randomly divided into the treatment group and control group,47 cases in each group.The control group was given the conventional drug therapy,pantoprazole 40 mg /d,on this basis the treatment group was given sertraline 50 mg/d.The treatment course in the two groups was 6 months.The change of HAMD score,inflammatory cytokines,QOL and incidence of adverse cardiovascular events were observed and compared between the two groups.Results The HAMD score after 3-,6-month treatment in the treatment group was significantly lower than that in the control group (P<0.05).The levels of inflammatory cytokines after treatment in the two groups were all reduced,while the IL-6 and TNF-α levels in the treatment group were lower than those in the control group (P<0.05);the scores of all dimensions and total scores of SAQ after 6-month treatment in the treatment group were significantly improved,and the anginal attack situation,treatment satisfaction degree and total score were significantly higher than those in the control group (P<0.05).During 12-month follow-up,the incidence rate of major and minor cardiovascular events,alimentary tract hemorrhage events and total events had no statistical difference between the two groups(P>0.05).Conclusion The antidepressant therapy is safe and effective for treating ACS patients,can effectively improve the depression emotion,inhibits inflammatory reaction,relieves somatic symptoms,and improves QOL.

Objective To explore change of diaphragm electromyograms in patients with obstructive sleep apnea-hypoventilation syndrome (OSAHS) before and after nocturnal sleep, as well as effective nasal continuous positive airway pressure (n-CPAP) ventilation treatment for more than two months. Methods Diaphragm electromyogram was recorded with chest surface electrodes in 22 patients with moderate and severe OSAHS and 24 normal people, and phrenic nerve conduction time (PNCT) and diaphragm compound muscle action potential (CMAP) provoked by unilateral magnetic stimulation (UMS) were measured for them before and after sleep. Measurements were repeated for five patients with severe OSAHS after effective OSAHS patients before and after nocturnal sleep than that in normal people bilaterally, (8.4±0. 6)ms and (8.4±0. 9)ms vs (7. 3±0. 8)ms and (7. 3±0. 8) ms for the left side; and (8.4±1.3) ms and (8. 9 ± 0. 8) ms vs (7.2 ± 0. 8) ms and (7.2 ± 0. 8 ) ms for the right side ( P < 0. 01 ), respectively ; and amplitude of CMAP was significantly lower in OSAHS patients, (0. 60±0. 20)mV and (0. 64±0. 29)mV vs (0. 98 ± 0. 28)mV and (0. 97±0. 27)mV for the left side; and (0. 53±0. 23)mV and (0. 56±0. 26)mV vs (0. 93 ±0. 29) mV and ( 0. 94 ± 0. 29 ) mV for the right side, respectively ( P < 0. 01 ) ; but no significant significantly shortened bilaterally in five patients with severe OSAHS after effective n-CPAP ventilation treatment for more than two months, (8.6±0. 7)ms vs (7.4±0. 5)ms for the lfet side and (7. 8±0. 6)ms vs (6.4 ± 0. 6) ms for the fight side ( P < 0. 05 ), respectively. Conclusions Both phrenic nerve conduction and diaphragm muscle function are weakened in patients with OSAHA, which may be related to hypoxia and/ or disturbance of sleep structure at night.

BACKGROUND:Inflammatory cellactivation and the generation of oxygen free radicals are important factors of lung ischemia-reperfusion injury. Adding the drugs with anti-inflammation and antioxidant effects into the lung preservation solution used, can improve the protection fluid, and play a crucial role in the study of lung ischemia-reperfusion injury and protection of the function of transplanted lung. OBJECTIVE:To discussion the effect of harmine in canine model of pulmonary ischemia-reperfusion injury. METHODS:Twelve healthy hybrid dogs were randomly divided into two groups, with six rats in each group. A canine model of lung ischemia-reperfusion injury was established, and the protecting liquid was perfused with the clockwise irrigation method. Control group:low potassium dextran protective fluid;experimental group:low potassium dextran+harmine protective fluid. After 2 hours of ischemia, the left lung circulation was recovered. The left lung tissue and blood samples were col ected from two groups of animal models after reperfusion, and their cytokines levels and the lung wet/dry weight ratio were detected and calculated. Bronchoalveolar lavage fluid was col ected to observe the pathological indicators. The main pulmonary artery pressure, left and right pulmonary artery pressure were recorded by continuous monitoring. RESULTS AND CONCLUSION:There were no statistical y significant differences in the interleukin-17, tumor necrosis factorαand endothelin 1 content in the left lung tissue and the blood between the two groups at 2 and 4 hours after reperfusion (P>0.05). After 4 hours of reperfusion in both groups, the neutrophil number, the number of lymphocytes, alveolar edema index, and vascular wal damage in bronchoalveolar lavage fluid showed no statistical y significant differences (P>0.05). Through the analysis of variance, the main pulmonary artery pressure, left pulmonary artery pressure and right pulmonary artery pressure also had no statistical significance between the two groups (P>0.05). By the analysis of cytokines, pathological indicators, lung wet/dry weight ratio, and pulmonary arterial pressure, harmine has no significant lung protection effect in canine model of lung ischemia-reperfusion injury.

Carotid ultrasound examination is a very mature examination method, it is widely used in clinical diagnosis and health checkup. Carotid ultrasound examination plays an important role in the prediction of the occurrence of cardiovascular disease. However, there are still certain problems in the application of the examination in the domestic health checkup population, which mainly include that the method and the standard are not unified, and the reporting form is not standardized. Carotid ultrasound examination must be standardized as soon as possible.

Objective To review the characteristics and outcomes of primary total hip replacement(pTHR) in patients with ankylosing spondylitis(AS). Methods Eighteen patients (26 hips) with AS underwent pTHR from January 1991 to September 2003 were reviewed retrospectively. Sixteen patients (24 hips) were followed-up. The mean duration of follow-up was 85 months (ranging from 23 to 169 months). All patients were evaluated using Harris score standardization. Results There were 15 hips with hip ankylosis( 57.7%). The average time of the operation was 167.4 minutes (ranging from 90 to 350 minutes),the average volume of bleeding in the operation was 773.9 ml (ranging from 300 to 2000 ml),the postoperative average drainage was 488.6 ml(ranging from 50 to 1130 ml). The rate of excellent and good was 87.5 percent. Conclusion The pTHR is a very effective treatment for patients with AS. The perfect rehabilitation care is one of the main factors influencing the outcomes of pTHR for patients with AS.

Objective To improve the understanding of primary pulmonary artery sarcoma(PAS)and its early diagnosis and treatment.Methods The clinical data of 3 PAS patients in Beijing Anzhen Hospital and related literature,the clinical features,diagnosis and therapy of the disease were retrospectively studied.Results Pulmonary artery sarcoma had similar clinical presentations to pulmonary thromboembolism(PTE).Although CT pulmonary angiography and Color Doppler echocardiography etc were helpful to the diagnosis of the entity,the confirmed diagnosis needed the operation.The surgery was main therapy for the disease.Conclusion Pulmonary artery sarcoma is easily misdiagnosed as PTE.The clinicians should pay more attention to the disease so as to early diagnose and treat it.

Aim To express h-ERA and h-ERA C-terminal domain in E.coli and prepare antiserum against human ERA. Methods Human era(h-era) cDNA and h-ERA C-terminal domain gene were amplified by PCR and ligated with prokaryotic expression vector pRSET-C and pDH respectively. E.coli TAP106 transformed with the recombinant plasmid pDH-h-era-C was induced at 42℃ to express h-ERA C-terminal domain protein. Antiserum against h-ERA was prepared by immunizing rabbit with h-ERA C-terminal domain protein purified by SDS-PAGE. E.coli BL21(DE3) transformed with the recombinant plasmid pRSET-C-h-era was induced with IPTG to express (His)6-h-ERA fusion protein for Western-blot analysis.Results The expressed h-ERA C-terminal domain and (His)6-h-ERA fusion protein occupied about 40％ and 80％ of total bacterial protein respectively. The(His)6-h-ERA fusion protein expressed in E.coli can be detected with the rabbit antiserum. Conclusion The h-ERA C-terminal domain protein and (His)6-h-ERA fusion protein were expressed with high efficiency in E.coli, and the rabbit antiserum against h-ERA was prepared successfully.

Objective To study the levothyroxine doses and related factors in the treatment of pregnant women with subclinical hypothyroidism (SCH).Methods Fifty-six pregnant women with SCH (diagnosed before 12 weeks of gestation) were recruited and divided into 2 groups according to the baseline TSH levels,SCH group 1 (2.5 mIU/L ≤ TSH ≤ 5.0 mIU/L,n =24) and SCH group 2 (TSH＞5.0 mIU/L,n =32).Thyroid autoantibodies [thyroid peroxidase antibody(TPOAb) and thyroglobulin antibody(TGAb)] were detected.All the subjects were treated with levothyroxine and the doses were adjusted according to the TSH level.The therapeutic target was to keep the TSH levels under control,0.3 to 2.5 mIU/L for the first trimester and 0.3 to 3.0 mIU/L for the second and third trimesters.Results There was a positive correlation between the levothyroxine doses and baseline TSH levels (r =0.533,P＜0.01) in pregnant women with SCH.A significant difference in the levothyroxine doses between SCH group 1 and SCH group 2 was found [(0.583 ± 0.341) vs (0.961 ± 0.405) μg/kg,t =-3.695,P＜ 0.01].The levothyroxine doses in SCH group 2 were 64.84％ higher than those in group 1.There was a significant difference in the levothyroxine doses between thyroid autoantibody negative and positive subjects [(0.680 ± 0.370) vs (0.918 ±0.440) μg/kg,t =-2.197,P =0.032].The levothyroxine doses in thyroid autoantibody positive subjects were 35 ％ higher than those in the negative subjects.In addition,there was a significant difference in the levothyroxine doses between subjects with negative and positive thyroid autoantibody [(0.421 ± 0.192) vs (0.720 ± 0.385)μg/kg,t =-2.331,P =0.029] in SCH group 1.While in SCH group 2,the difference did not reach statistical significance.Conclusion The baseline TSH levels and status of thyroid autoantibodies may affect the levothyroxine dosage in pregnant women with SCH.
