1.Causes and treatment in radial nerve injury

[Objective]To probe into the causes and treatments in the radial nerve injury.[Method]Twenty-nine patients with radial nerve injury were treated.Conservative measures were anti-inflammatory,repercussion,nervous pharmacotherapy,acupuncture and physiotherapy.Surgical interventions included open reduction and internal fixation,tenorrhapy,neurorrhaphy and decompression of nerve.[Result]The outcome of treatment was evaluated with the result of 19 excellent,8 good and 2 fair,in which nerve healing period was at an average of 4 months.[Conclusion]The overall effect was satisfactory in this series,among which,electro-neurogram were inconsistent with their symptoms and signs.It is important to asses the quality and degree in injury to the nerve for guiding the treatment.During internal fixation of the fractures of humerus,the exclusion and protection of the radial nerve should be carefully done.when removig the fixation the radial nerve would be protected by first dissecting normal radial nerve in distal and proximal segments,then,exposing gradually adherent segment within the scar tissue.In brief,the opportunity of injury to the radial nerve may be reduced by careful operation.
2.Histological research on regeneration of sciatic nerves after pasteurization treatment

Objective To observe the histomorphological changes of sciatic nerves following pasteurizing warm temperature treatment. Methods The 12 mm left sciatic nerve from healthy adult male Wistar rats was pasteurized at 60℃ for 30 minutes and observed at the first week (1-week group) and the sixth week (6-week group), 15 rats per group. Number, diameter and area ratio of the myelinated axons in the control (proximal to the treated part), treatment and peripheral nerves (distal to the treated part) were calculated. Objective To investigate a new method which could not only avoid the extrusion of the silicon implant, but also be benefit for the reconstruction of nasal tip and alar during nasal augmen-tation procedure. Methods Folded lower lateral cartilage flap combined with silicon implant was ap-plied for nasal dorsal augmentation and reconstruction of nasal tip and alar with its unique character at the same time. Results 12 cases were all primary healing, without infection, extrusion of implant and other complications. One month after operation, the projection of nasal tip was increased, which had better delicate and definite shape, natural appearing tip adding contour and height to nasal tip, pleas-ant definition. Conclusions Folded lower lateral cartilage flap combined with silicon implant is an ef-fective method to decrease the incidence of extrusion, when we want to have a little over-projected na-sal tip with prosthesis. It is also helpful for reconstructing the nasal tip and alar with its unique char-acter.
4.Effect of dexmedetomidine pretreatment on expression of Clara cell secretory protein during endo-toxin-induced acute lung injury in rats

Objective To evaluate the effect of dexmedetomidine pretreatment on expression of se-cretion protein of Clara cell secretory protein(CC16)during endotoxin-induced acute lung injury(ALI) in rats.Methods One hundred and twelve healthy Wistar rats of both sexes, aged 8-12 weeks, weighing 250-350 g, were divided into ALI group(n=56)and dexmedetomidine pretreatment group(group DEX, n= 56)using a random number table.ALI was induced by intravenously injecting lipopolysaccharide (LPS)5 mg∕kg over 1 min.Dexmedetomidine 10 μg∕kg was intravenously infused over 10 min starting from 10 min before LPS in group DEX.At 10 min before LPS injection and 0.5, 1, 2, 4, 6 and 24 h after LPS injection, 8 rats were sacrificed and lungs were removed for examination of the pathological changes(with a light microscope)and ultrastructure of Clara cells(with a transmission electron microscope)and for deter-mination of CC16 expression in bronchioles(by immunohistochemistry). Results Compared with the baseline at 10 min before LPS injection, the expression of CC16 in bronchioles was significantly down-regu-lated at 1, 2, 4, 6 and 24 h after LPS injection in group ALI and at 1, 2, 4 and 6 h after LPS injection in group DEX(P<0.01), the pulmonary small arterial hyperemia, alveolar septa edema, red blood cell exudation and inflammatory cell infiltration were found, the Clara cells in bronchioles were reduced, the secretory granules in the cytoplasm were reduced, and the mitochondria were swollen and deformed after in-jection of LPS.Compared with group ALI, the expression of CC16 in bronchioles was significantly up-regu-lated at 1, 2, 4, 6 and 24 h after LPS injection(P<0.01), the pathologic changes of lung tissues were significantly attenuated, and the number of Clara cells was increased in group DEX.Conclusion The mechanism by which dexmedetomidine pretreatment reduces endotoxin-induced ALI may be related to up-regulating CC16 expression in rats.
5.Risks and predictors of mortality among human immunodeficiency virus-infected children receiving highly active antiretroviral therapy in Yunnan Province

Objective To investigate the influence factors of mortality among human immunodeficiency virus (HIV)-infected children under highly active antiretroviral therapy (HAART).Methods Retrospective cohort study of 652 children initiated HAART from 2005 to 2014 was conducted,and enrolled patients were followed-up until December,2015.Survival data was analyzed using Kaplan-Meier method and Cox regression model was used to identify independent predictors of mortality among these children on HARRT.Chi-square test and Fisher's exact test were used for comparison between groups.Results Overall,26 of the children died over a follow-up period of 3 116.24 child-years,with a mortality rate of 0.83 per 100 child-years.Twelve (46%)of deaths occurred during the first six months after starting HAART.Cox regression analysis of variables showed that the World Health Organization (WHO) clinical stages Ⅲ/Ⅳ (hazard rate [HR] =10.717,95%confidence interal [95% CI]:4.189-4.749,P =0.000),baseline hemoglobin < 80 g/L (HR =14.768,95 % CI:5.721-38.125,P =0.000),tuberculosis co-infection (HR =4.794,95% CI:2.105-10.918,P =0.000),baseline CD4+T lymphocyte < 50 cells/μL (HR =4.219,95% CI:1.524-11.680,P =0.006),weight-for-age z-score <-2 (HR =2.983,95 % CI:1.094-8.135,P =0.033) were independently associated with death,whereas the age < 7 year-old at HAART initiation was protectire (HR =0.293,95% CI:0.126-0.684,P =0.005).Conclusions The mortality of children receiving HAART is strongly associated with WHO stages Ⅲ/Ⅳ,hemoglobin < 80 g/L,weight-for-age z-score <-2,tuberculosis co-infection and older age at treatment.
6.Mechanism of Youguiwan Monomer in Osteoarthritis: A Review

As a prescription for tonifying kidney yang, Youguiwan accords with the pathogenesis of osteoarthritis. Professor LIU Yuan-lu from the Affiliated Hospital of Liaoning University of Traditional Chinese Medicine found that modified Youguiwan had a significant effect on the clinical conservative treatment of osteoarthritis pain with Chinese medicine. There are teams studying the mechanism of Youguiwan in the treatment of osteoarthritis in China, but the profound study of the compound needs the mechanism of each component as support. Youguiwan contains 10 kinds of Chinese medicine, including Rehmanniae Radix Praeparata, Aconiti Lateralis Radix Praeparata, Cinnamomi Cortex, Dioscoreae Rhizoma, Corni Fructus, Cuscutae Semen, Angelicae Sinensis Radix, Eucommiae Cortex, Cervi Cornus Colla, and Lycii Fructus. With more and more attention to the study of Chinese medicine monomer, the amount of related research in this field is increasing. Therefore, it is of great significance to summarize the mechanism of Youguiwan in the treatment of osteoarthritis. Based on the sovereign, minister, assistant, and guide of Chinese medicine in Youguiwan, this paper used China National Knowledge Infrastructure (CNKI), Pharmacopoeia of the People's Republic of China (2020 edition Vol. 1), and ChemicalBook platform to understand the active components of each kind of Chinese medicine, and used PubMed to search. The latest research progress of effective monomers that had an intersection with the mechanism of osteoarthritis was summarized, and there were five pharmacological effects including anti-inflammation, pain inhibition, chondrocyte apoptosis, oxidative stress, and extracellular matrix degradation, involving several signaling pathways. The theory of sovereign, minister, assistant, and guide and the molecular mechanism were integrated and discussed, respectively, and finally, they were combined with traditional Chinese medicine theory, providing theoretical references for further study in this direction.
