1.Contrast-enhanced ultrasound for evaluating area of coagulation necrosis after microwave ablation of rabbit liver:Comparison with pathological findings

Objective To observe area of coagulation necrosis after microwave ablation(MWA)of rabbit liver showed on contrast-enhanced ultrasound(CEUS)in comparison with pathological findings.Methods Twenty-five healthy male experimental rabbits were equally randomly divided into L0,M0,H0,M7 and M14 groups.MWA was achieved with different power and time,and then CEUS and pathological examinations were performed,respectively.Four target areas of rabbit liver were ablated under 20 W for 1 min in L0 group,under 30 W for 1 min in M0 group and under 30 W for 3 min in H0 group,and the ablated areas were observed on the same day after MWA,while 2 target areas of rabbit liver were ablated under 30 W for 1 min in M7 and M14 groups,and the ablated areas were observed 7 days and 14 days after MWA,respectively.The ablated foci and ablated foci+edema band(inflammatory reaction band,IRB)showed on CEUS and pathology were compared,and their differences and changes with time going were analyzed.Results On the day of ablation,the extent of ablation foci,ablation foci+IRB as well as IRB in L0,M0 and H0 groups showed on CEUS were all larger than pathological findings under light microscopy(all P<0.05),and the difference value of the length of the ablation foci between 2 methods increased sequentially among group L0,M0 and H0(all adjusted P<0.05).Seven days after ablation,the area of ablation foci measured with CEUS in M 7 group was slightly larger than that measured with pathology(adjusted P=0.045),but no significant difference of the length nor the short diameter was found(both adjusted P>0.05).Fourteen days after ablation,no significant difference of the length,the short diameter nor the area of ablation foci was detected between 2 methods in M14 group(all adjusted P>0.05).The length,short diameter and area of ablation foci in M0 group measured with CEUS or pathology were all smaller than those in M 7 and M14 groups(all adjusted P<0.05),while no significant difference was found between the latter 2 groups(all adjusted P>0.05).Conclusion On the day of MWA of rabbit liver with different power and time settings,the areas of ablation foci,ablation foci+IRB and IRB showed on CEUS were larger than pathological findings.The range of ablation foci showed on CEUS 7 days after ablation was basically in line with pathology,indicating that CUES should be reexamined 7 days after ablation to evaluate the actual extent of coagulation and necrosis of ablation foci.
2.A clinical study on the treatment of relapsed/refractory acute myeloid leukemia with venetoclax regimen

Objective:To explored the feasibility and efficacy of a rapid ramp-up,2-week maximum regimen of venetoclax(VEN)plus low-dose cytarabine(LDAC)for treating relapsed/refractory acute myeloid leukemia(R/R AML).Methods:We retrospectively analyzed patients with venetoclax-na?ve R/R AML treated with VEN+LDAC between October 2018 and November 2023.On the first day,patients received 200 mg of VEN,and the dose was quickly increased to 400 mg for the rest of the treatment;cytarabine was administered subcutaneously at a low dose of 20 mg/m2/day.The treatment duration was 10 or 14 days,depending on the condition of bone marrow hyperplasia determined on the 8th day of treatment.No patients received venetoclax monotherapy.All patients responding to salvage therapy received VEN+LDAC until disease progression or transplantation.Results:Among the patients,the median follow-up duration was 27.5 months.No clinical mani-festations of tumor lysis syndrome(TLS)occurred during the treatment.The overall response rate(ORR)was 68.75%,including four com-plete responses(CR),one complete remission with incomplete hematologic recovery(CRi),and six partial responses(PR).The median num-ber of best treatment result cycles was one cycle.The median overall survival(OS)in the whole cohort was 5.8(0.5-47.2)months;the medi-an progression-free survival(PFS)was 22.2(7.3-42.9)months.The major adverse events were grade 3-4 hematologic adverse events and in-fections.Conclusions:The 8th-day myelosuppression-adjusted VEN+LDAC regimen is a feasible salvage option with a reasonable safety pro-file in patients with venetoclax-na?ve R/R AML.Most patients tolerated the 14-day treatment;the response was generally rapid in the re-sponding patients.

Acute lung injury/acute respiratory distress syndrome (ALI/ARDS) is characterized by diffuse alveolar injury primarily caused by an excessive inflammatory response. Regrettably, the lack of effective pharmacotherapy currently available contributes to the high mortality rate in patients with this condition. Xuebijing (XBJ), a traditional Chinese medicine recognized for its potent anti-inflammatory properties, exhibits promise as a potential therapeutic agent for ALI/ARDS. This study aimed to explore the preventive effects of XBJ on ALI and its underlying mechanism. To this end, we established an LPS-induced ALI model and treated ALI mice with XBJ. Our results demonstrated that pre-treatment with XBJ significantly alleviated lung inflammation and increased the survival rate of ALI mice by 37.5%. Moreover, XBJ substantially suppressed the production of TNF-α, IL-6, and IL-1β in the lung tissue. Subsequently, we performed a network pharmacology analysis and identified identified 109 potential target genes of XBJ that were mainly involved in multiple signaling pathways related to programmed cell death and anti-inflammatory responses. Furthermore, we found that XBJ exerted its inhibitory effect on gasdermin-E-mediated pyroptosis of lung cells by suppressing TNF-α production. Therefore, this study not only establishes the preventive efficacy of XBJ in ALI but also reveals its role in protecting alveolar epithelial cells against gasdermin-E-mediated pyroptosis by reducing TNF-α release.

5. Quantitative Analysis of Relationship Between Gut Flora Dysbiosis and Severity of Liver Cirrhosis

Background: The incidence of liver cirrhosis is increasing year by year in China. Decompensated liver cirrhosis and its related complications seriously threaten people's health. Aims: To investigate the changes of gut flora in cirrhotic patients and its correlation with the severity and prognosis of the disease. Methods: One hundred patients with liver cirrhosis admitted from Dec. 2010 to Jul. 2011 at the Nanfang Hospital, Southern Medical University were enrolled. Fifty healthy subjects were served as controls. Using genus-specific primers and genomic DNA extracted from the fecal samples, a quantitative analysis of the gut flora was conducted by real-time PCR. Relationships between the changes of gut flora and grade of liver function, prognostic score, inflammatory indicator and complications of cirrhosis were analyzed. Results: Compared with healthy subjects, fecal Enterobacterium was significantly enriched, while fecal Lactobacillus, Bacteroides and Clostridium were significantly reduced in cirrhotic patients (all P<0.05). Enterobacterium was positively correlated with the grade of liver function, Child-Turcotte-Pugh (CTP) score, model for end-stage liver disease (MELD) score, C-reactive protein (CRP), and the occurrence of spontaneous peritonitis, respectively (all P<0.05), while Clostridium was negatively correlated with the above mentioned indices (all P<0.05). Conclusions: Gut flora dysbiosis, including increase in Enterobacterium and decrease in Clostridium, is correlated with the severity and prognosis of patients with liver cirrhosis. It may aggravate liver cirrhosis by promoting an inflammatory response.
6. Quantitative Analysis of Major Shifts in Gut Microbiota Before and After Helicobacter pylori Eradication

Background: Helicobacter pylori (Hp) infection occurs in about half of the world's population, and Hp eradication therapy is an important method for treating Hp infection-related diseases. Aims: To explore the major shifts in gut microbiota before and after Hp eradication by using bismuth-containing quadruple therapy. Methods: Fifty Hp-positive patients in whom the Hp was successfully eradicated by bismuth-containing quadruple therapy from January 2016 to January 2017 at the Third People's Hospital of Shenzhen were enrolled. Using genus-specific primers and genomic DNA extracted from the fecal samples collected before and within 3 days after eradication therapy, a quantitative analysis of the major shifts in gut microbiota was accomplished by real-time PCR. Results: Compared with those before Hp eradication, fecal Enterobacterium and Enterococcus were significantly increased and Bifidobacterium, Bacteroides, Clostridium and total bacteria were significantly decreased after Hp eradication (P<0.05). No significant change was observed in Lactobacillus (P>0.05). Conclusions: Eradication of Hp infection by bismuth-containing quadruple therapy may cause changes in gut microbiota composition and resulting in dysbiosis.

Trauma is the leading cause of death for people under 40 years old in the world. At present, the rescue and treatment system of trauma patients in China is not yet well established, and the mortality of trauma patients is higher than those in the developed countries. Improving the treatment system is the key to reducing the trauma mortality. In order to innovate the service mode of trauma first aid, further promote the establishment of regional trauma first aid system, improve the ability of trauma treatment, reduce the mortality and disability rate of trauma patients in Jiangxi Province, recently Health Commission of Jiangxi Province and the First Affiliated Hospital of Nanchang University have reached a consensus on the establishment of Jiangxi trauma first aid center. In order to provide reference for the construction of trauma treatment system, the author analyzes the following aspects including functional positioning, basic requirements, organization management, and evaluation of core indicators.

9.The application of antiskid steel plate on supination-eversion ankle fracture

Objective To study the application effect of antiskid steel plate internal fixation via exposing lateral and posterior ankle fracture through fibula posterolateral approach in the treatment of Lauge-Hansen supination-eversion ankle fracture.Methods From January 2009 to December 2012,exposed the lateral and posterior ankle fracture through fibula posterolateral approach of 36 patients suffering from the Lauge-Hansen supination-eversion ankle fracture,and fixed the lateral and posterior ankle by the antiskid steel plate.Adopt Olerud-Molander ankle function scoring system to evaluate the postoperative effect of the ankle.Results All the patients were followed up for 7-27 months,with an average follow-up of 19 months.There was no complications such as incision infection and cutaneous necrosis.Only 1 patient occurred with peroneus myotenositis,and the symptom was relieved after taking out the steel plate.The fracture was healed within 16 weeks.Among the 36 cases,22 cases were excellent,9 cases were good,4 cases were medium and 1 case was poor.The fine rate was 86.1%(31/36).Conclusion Using the antiskid steel plate to treat the lateral and posterior ankle fracture through fibula posterolateral approach,can fully expose a single incision,fix the lateral and posterior ankle and reduce the tension in suture,improve the incision healing rate and fracture stability,provide a new method for treating ankle fracture.
10.Investigation and reflections on the pre-graduation practice for preventive medicine specialty

Pre-graduation practice is an important part of teaching work for preventive medical science.The article is about investigation on the pre-graduation practice of 85 preventive medical students just graduated,and some suggestions for improvement.It is found that the overall teaching effect is good,but there are some problems,mainly on the construction of practice base,practice contents and time,and also graduation design.
