1.The short-term prognosis evaluation model for the first ischemic cerebral stroke patients

Objective To explore common risk factors of the first acute ischemic cerebral stroke patients′neurological deficits and build a short-term prognosis model. Methods 89 hospitalized patients with acute is-chemic cerebral stroke were chosen for study from September 2014 to December 2015 in the Fifth Affiliated Hos-pital of Zhengzhou University. Our study′s evaluation methods were using the unified questionnaires , the NIHSS score and the mRS scale. Results Traditional risk factors were no significant difference among the three groups (P > 0.05); 6 kinds of hematology indexes such as WBC count had significant difference in NIHSS score (P <0.05) and prognosis(P < 0.05); 6 kinds of hematology indexes such as D-D and the NIHSS score had a signifi-cant effect on prognosis (OR = 1.800, 0.976, 1.112, 1.327, 5.564, 6.456, 1.227); the area under ROC curve was 0.976, which proved the model had a good predictive value. Conclusion Traditional risk factors had no significant difference among the different neurological deficits groups; 6 kinds of hematology indexes such as D-D and NIHSS score on admission had a significant influence on prognosis; the model predicted the short-term prognosis of acute ischemic cerebral stroke more accurately.
2.Short-term effects of Aidi injection adjuvant GP methods chemotherapy in treatment for advanced non-small cell lung cancer

Objective:To study the short-term effects,toxic side effects and immune function of Aidi injection adjuvant GP methods chemotherapy for advanced non-small cell lung cancer(NSCLC).Methods:130 patients with advanced NSCLC were divided into study group and the control group according to treatment methods.The study group were given Aidi injection +GP,and the control group only treated with GP.The short-term effects,toxic side effects gradeⅢ-Ⅳand immune function changes were compared between the two groups.Results:The overall response rate was 40.28%in study group,higher than 37.93% in control group(P>0.05).The two groups showed differences in the rates of leucocyte decrease,gastrointestinal reaction and platelets decrease(P<0.05).The levels of CD4,CD8 and CD4/CD8 didn′t changed obviously in study group(P>0.05),while in the control group the levels of CD4,CD8 and CD4/CD8 changed more significantly than before(P<0.05).The two groups also produced differences in CD4,CD8 and CD4/CD8 after treatment( P<0.05 ) .Conclusion: The short-term effects of Aidi injection adjuvant GP chemotherapy regimen for advanced NSCLC is similar to only GP chemotherapy,but it can obviously reduce the incidence of toxic side effects and prevent the immune func-tion.
3.Correlation Between Integron and Drug-resistance in Clinical Isolates of Acinetobacter baumannii

OBJECTIVE To investigate the drug-resistant rates of clinical Acinetobacter baumannii strains isolated from our hospital and to understand the integrons distribution carried by A.baumannii.To identify the type of the integrons and analyze the correlation between integron and drug resistance in A.baumannii clinical isolates. METHODS Collected clinical isolates and the drug-sensitivity tests were completed by the method of KB.Amplified integrase were tested by PCR and identified the integron category.Correlation between integron in A.baumannii and drug resistance was analyzed.The open reading frame(ORF) of integron were amplified and observed the polymorphism of integron,Part of amplified products were sequenced. RESULTS Apart from imipenem and cefoperazone /sulbactam,the drug-resistant rate of 85 A.baumannii clinical isolates to else 15 kinds of antibacterials surpassed 30.0%.67.1% A.baumannii clinical isolates carried the integrons,and confirmed all of them belonged to integron Ⅰ after identification,but integrons Ⅱ and Ⅲ were not found.The drug-resistant rates of A.baumannii with integron to 14 kinds of antibacterials were higher than those of A.baumannii without integron.The length of amplified products in variable region of ORF of integron varied from 0.15 to 2.5 kb.The sequencing results confirmed the integron carried multidrug-resistant gene code. CONCLUSIONS Drug-resistant rate of A.baumannii clinical isolates in our hospital is higher and the majority of them are multidrug-resistant strains.The drug-resistant rates of A.baumannii with integron to 14 kinds of antibacterial are higher.The multidrug-resistant A.baumannii is associated with integrons.
4.Expression and significance of IGF-IR and PKC in laryngeal squamous cell carcinoma

Objective:To investigate the expression of IGF-IR and PKC and their clinical significance in laryngeal squamous cell carcinoma(LSCC).Method:The expression of IGF-IR and PKC protein were detected by immunohistochemistry and Western blot in 60 cases of LSCC, 25 cases of adjacent non-tumorous laryngeal epithelium and 10 cases of normal laryngeal epithelium.Result:The positive rate of IGF-IR and PKC expression in LSCC were significantly higher than that in normal laryngeal epithelium and adjacent non-tumorous laryngeal epithelium(P<0.05). IGF-IR positive expression were increased more in cases with stage Ⅲ-Ⅳ, cervical lymph node metastasis and well differentiation than that in cases with stageⅠ-Ⅱ, non-lymph node metastasis and poor/mediate differentiation, respectively(P<0.05).The positive rate of PKC in LSCC were correlated with clinical stage and cervical lymph node metastasis.There was a correlation between the expression of IGF-IR and PKC in LSCC.Conclusion:The overexpression of IGF-IR and PKC in the laryngeal carcinoma may play an important role in the carcinogenesis and development of LSCC. It is suggested that detecting the expression of IGF-IR and PKC can be used for early diagnosis and treatment of LSCC.
5.Research about sexual knowledge nursing intervention for femal workers in Dongguan

Objective To know the master condition about sexual knowledge in female workers in Dongguan, and then carry out certain effective community nursing intervention to populize the related knowledge in female workers. Methods Divided 3000 femle workers with 18-35 years old who had selected in 9 factories in Dongguan into the DV group and the teacher education group randomly, there were 1500 cases in each group. Video displaied was used in the DV group in certain propert time, teacher education about sexual knowledge was used in the teacher education group, compared the master condi-tion of sexual knowledge before and after the interventioni by questionnair in the two groups respectively, and then summarized the results. Results There were no significant differences between the two groups about sexual knowledge before the intervention, while after the intervention, the master conditon of sexual knowl-edge in the DV group were better than those of in the teacher education group. Conclusions Video dis-play is a kind of saving, effectiveness, open-end and circular mode for female workers to pepulize sexual-related knowledge, which should be ranging used.
6.Analysis on the clinical effect of apatinib mesylate tablet in the treatment of advanced non-small cell lung cancer

Objective To investigate the effect of lapatinib mesylate tablets in the patients with advanced non-small cell lung cancer. Methods A total of 55 patients with advanced non-small cell lung cancer who were given chemoth·erapy in our hospital from January 2015 to May 2017 were selected. Among them, 27 patients who were given paclitaxel combined with cisplatin for treatment were assigned to the conventional group. 28 patients who were given apatinib mesylate for treatment were assigned to the intervention group. The patient's therapeutic effect, related index changes, adverse reactions and quality of life were observed. Results ORR, DCR, total incidence rate of adverse reactions and quality of life in the intervention group were higher than those in the conventional group (P<0.05); the levels of CEA, VEGF and MMP-9 in the two groups after treatment were lower than those before treatment (P<0.05). Patients in the intervention group had lower levels of CEA, VEGF, and MMP-9 after treatment (P<0.05). Conclusion The clinical effect of lapatinib mesylate in the treatment of advanced non-small cell lung cancer is significant.
7.Clinical efficacy of preoperative neoadjuvant chemoradiotherapy for unresectable locally advanced adherent colon cancer in 40 patients

Objective To investigate the efficacy and toxicities of neoadjuvant chemoradiotherapy (neoCRT) in the management of unresectable locally advanced adherent colon cancer (LAACC).Methods A retrospective analysis was performed on the clinical records of 40 patients with initially diagnosed unresectable LAACC who received preoperative neoCRT in our center from October 2010 to December 2015.Results Thirty-nine patients completed the preoperative neoCRT.Thirty-four patients underwent radical resection after neoCRT, and the R0 resection rate, pathological complete response rate (pCR), tumor downstaging rate, nodal downstaging rate, and clinical downstaging rate were 91%, 24%(8/34patients), 76%(26/34patients),100%(32/32patients), and 94%(32/34patients), respectively.Among the 21 patients with bladder invasion, the full bladder was preserved in 7 patients (33%) and partial cystectomy was performed in 11 patients (52%).During the course of neoCRT, the grade 3-4 hematologic toxicity rate, grade 3 hand-foot syndrome rate, grade 3 radiodermatitis, and incomplete intestinal obstruction rate were 23%, 3%, 3%, and 5%, respectively.The 3-year sample size was 25 patients.For all the patients, the 3-year overall survival (OS) and progression-free survival (PFS) rates were 75% and 80%, respectively.Of the 34 patients who received surgical radical resection, the 3-year OS and disease-free survival (DFS) rates were 87% and 81%, respectively.In addition, local tumor recurrence was identified in 3 patients, and distant metastasis was identified in 6 patients.Conclusions NeoCRT is an effective treatment for unresectable LAACC that results in significant tumor downstaging and enhanced R0 resection rate without an increase in surgical complications.The patients treated with radical surgical resection after neoCRT show a satisfactory short-term outcome.Further studies will be required to determine the clinical value of neoCRT in treating LAACC.
8.Mid-term outcomes of a prospective phase Ⅱ trial of preoperative sandwich-like neoadjuvant chemoradiotherapy for locally advanced rectal cancer

Objective To evaluate the mid-to long-term survival benefits of preoperative sandwich-like neoadjuvant chemoradiotherapy (CRT) in patients with locally advanced rectal cancer (LARC).Methods A total of 45 LARC patients who underwent neoadjuvant sandwich CRT in the form of XELOX regimen prior to,concurrently with,and following volumetric modulated arc radiotherapy (VMAT) in 2012 were enrolled in this study.VMAT was given at a gross tumor volume dose of 50 Gy in 25 fractions,and a clinical target volume dose of 45-46 Gy in 25 fractions.Total mesorectal excision was performed 6 to 8 weeks after completion of VMAT.The overall survival (OS) and disease-free survival (DFS) were determined by the Kaplan-Meier method,and survival comparison and univariate prognostic analysis were performed using the log-rank test.Results The median follow-up time was 46.7 months.There was no local recurrence detected among the patients.The 3-year distant metastasis (DM) rate was 18%,and the 3-year OS and DFS were 96% and 84%,respectively.Univariate analysis indicated that perineural invasion,N1-N2 pathology (pathological stage Ⅲ),and Ca-199>35 U/ml before treatment were risk factors for DM (P=0.000,0.000,and 0.013,respectively).Conclusions The significant short-term efficacy of preoperative sandwich-like neoadjuvant CRT can be extended to a positive mid-term survival in LARC patients.However,further phase Ⅲ clinical studies will be needed to confirm this finding.

10.Surveillance of antibiotic resistance in clinical isolates from the First Affiliated Hospital of Anhui Medical University during 2017

Objective To investigate the antibiotic resistance of clinical isolates in the First Affiliated Hospital of Anhui Medical University during 2017. Methods Antimicrobial susceptibility testing was carried out according to a unified protocol using automated system or Kirby-Bauer method. Results were interpreted according to the breakpoints of CLSI 2017. The data were analyzed by WHONET 5.6 software. Results A total of 6 495 non-duplicate clinical isolates were collected in 2017. There were 1 727 strains (26.6%) of gram-positive bacteria and 4 768 strains (73.4%) of gram-negative bacteria. The most frequently isolated microorganisms were E. coli (19.8%), followed by Klebsiella pneumoniae and Acinetobacter baumannii. The strains were mainly isolated from respiratory tract (37.0%) and urine (23.1%). The prevalence of MRSA and MRCNS in Staphylococcus aureus and coagulase-negative Staphylococcus was 50.1% and 82.1%, respectively. No Staphylococcus strains were found resistant to vancomycin or linezolid. E. faecalis and E. faecium accounted for 49.9% and 40.4% of total Enterococcus isolates. The prevalence of ESBLs-producing strains was 57.6% in E. coli, 27.1% in Klebsiella spp. and 33.0% in Proteus mirabilis. Enterobacteriaceae strains were still highly susceptible to carbapenems antibiotics. The Klebsiella pneumoniae isolates in 2017 showed significantly higher resistance rate to imipenem and meropenem than the strains in 2016. However, Pseudomonas aeruginosa and Acinetobacterbaumannii strains showed lower resistance rates to carbapenems than the strains in 2016. Conclusions The bacterial isolates in 2017 pose serious threat to clinical antibiotic therapy. More attention should be paid to rational use of antimicrobial agents and infection control measures.
