To investigate some rare causes of obstructive jaundice in children. Three school children with obstructive jaundice caused by uncommon causes were analyzed. Despite pre operative physical examination, B Ultrasonic and CT examinations, the diagnosis was not established. The main clinical manifestation of three patients was obstructive jaundice. Postoperative pathological examination proved that one patient was suffering from cryptococcus neoformans infection of the common bile duct, another embryonal rabdomyosarcoma of common bile duct, and the third primary hydatid cysts in the common bile duct. They were all treated with surgical removal of the lesions, followed by reconstruction of the bile duct and other adjunctive treatment with satisfactory result. The results suggest that obstructive jaundice in children may be caused by some unusual diseases. Comprehensive analysis and corresponding treatments were recommended.

Objective To evaluate the clinical responses and side effects of non sealed colloidal 32 P internal radiation and macroaggregate albumin (MAA) for hepatocellular carcinoma (HCC). Methods Twenty six patients with HCC underwent ultrasound guided intratumoral injection therapy of 32 P colloid and MAA. The clinical responses and toxic effects were analyzed on basis of clinical manifestions and laboratory data, including the tumor size, histopathological changes, serum alpha fetoprotein (AFP) estimation, blood routine, hepatic and renal functions and immunocompetence assays before and after treatment. Results Clinical responses: alleviation of symptoms, decreased AFP levels, fibrosis and complete or partial necrosis within tumor lesions (CR 2, PR 13, MR 2, SD 7 and PD 2). The total response rate was 57.7% and the median survival period was 16 months. Hepatic lesions became resectable in 5 patients for whom successful operation was performed. The 1 ,2 and 3 year survival rates were 88.5%, 76.9% and 38.5%, respectively. No evidence of non sealed radionuclide induced toxic reactions or complications were observed. Conclusions The treatment of internal radiation with 32 P colloid and macroaggregate albumin is effective, simple, safe and applicable with wide indications for primary hepatic carcinoma.

Objective:To investigate the expression of IGF-IR and PKC and their clinical significance in laryngeal squamous cell carcinoma(LSCC).Method:The expression of IGF-IR and PKC protein were detected by immunohistochemistry and Western blot in 60 cases of LSCC, 25 cases of adjacent non-tumorous laryngeal epithelium and 10 cases of normal laryngeal epithelium.Result:The positive rate of IGF-IR and PKC expression in LSCC were significantly higher than that in normal laryngeal epithelium and adjacent non-tumorous laryngeal epithelium(P<0.05). IGF-IR positive expression were increased more in cases with stage Ⅲ-Ⅳ, cervical lymph node metastasis and well differentiation than that in cases with stageⅠ-Ⅱ, non-lymph node metastasis and poor/mediate differentiation, respectively(P<0.05).The positive rate of PKC in LSCC were correlated with clinical stage and cervical lymph node metastasis. There was a correlation between the expression of IGF-IR and PKC in LSCC.Conclusion:The overexpression of IGF-IR and PKC in the laryngeal carcinoma may play an important role in the carcinogenesis and development of LSCC. It is suggested that detecting the expression of IGF-IR and PKC can be used for early diagnosis and treatment of LSCC.

Objective To investigate the genotypes of aminoglycoside-modifying enzymes (AMEs) produced by Pseudomonas aeruginosa isolated from Huaibei Miner's General Hospital. Methods Minimum inhibitory concentration of 36 strains of P. aeruginosa to 3 aminoglycoside antibiotics was determined. AME genes were amplified by polymerase chain reaction (PCR). Results The P. aeruginosa isolates were resistant to multiple antibiotics. Most (62.2% to 81.1%) were resistant to aminoglycosides. And 27 strain (75.0%) carried one or more types of AME genes, including ant(3″)-Ⅰ (63.9%), aac(6')-Ⅱ (58.3%), aac(6')-Ⅰ (50.0%), aac(3)-Ⅱ (38.9%) and ant(2″)-Ⅰ (36.1%). The aac(3)-Ⅰ, aac(3)-Ⅲ, aac(3)-Ⅳ, aph(3')-Ⅳ genes were not identified. Conclusions The study indicates that the P. aeruginosa isolates in Huaibei are multi-resistant to aminoglycoside antibiotics. The prevalence of AMEs-positive strains is high.

Objective To use the multi-slice CT(MSCT)cerebral perfusion imaging and CT angiography(CTA)in the clinical diagnosis of transient ischemic attack(TIA).Methods 20 cases with transient ischemic attack within 48 h were examined with 16-slice CT perfusion imaging(CTPI)and CTA in head and neck. The interesting regional cerebral blood flow(rCBF),cerebral blood volume(rCBV)and the time to peak(rTP)were analyzed. Results The rTP of affected side delay compared with that of the healthy(t=3.04,P=0.008).CTA showed the responsible vascular stenosis or occlusion in varying degrees. There was not significant correlationship between rTP and vascular stenosis(r=0.32,P=0.17).Conclusion Cerebral perfusion imaging and head and neck CTA can provide valuable information for diagnosis and treatment of TIA.
10.Quantitative evaluation of the anal sphincter by the tendency presented in the manometric asymmetry variations.
Chinese Medical Journal 2002;115(3):413-417
OBJECTIVESTo assess the tendency of manometric asymmetry, and to quantitatively evaluate the function of the anal sphincter.
METHODSUsing the PC polygram HR at the states of relax and squeeze, we compared the values of pressure asymmetry in the anorectum of 27 normal children with that of 12 children with fecal incontinence.
RESULTSIn the two states, the anal sphincter in normal children showed not only the gradient of pressure, but also a gradient of pressure asymmetry. Children with fecal incontinence did not show any gradients.
CONCLUSIONGradients of pressure and pressure asymmetry can quantitatively evaluate the function of the anal sphincter.
