Objective To study the effects of selective inducible nitric oxide synthase (iNOS) inhibitor 1400W on nitric oxide (NO) exhalation and plasma nitrate concentration during endotoxemia in pigs. Methods Fourteen pigs of both sexes aged 12-16 weeks, weighing 40-60 kg were randomly divided into two groups : ( Ⅰ ) endotoxemia group ( n = 6) and ( Ⅱ ) 1400W group ( n = 8). The animals were anesthetized with pentobarbital and ketamine, intubated and mechanically ventilated. PaC02 was maintained at 35-45 mm Hg. Femoral artery and portal vein were cannulated for BP monitoring and blood sampling. E coli (LPS 20 mg L-1 ) was continuously infused iv at 4 ml h-1 for 24 h in both groups. In 1400W group, at 12 h of LPS infusion, continuous iv infusion of 1400W was started at 0.5 mg kg-1 h-1 . Blood samples were taken from artery and portal vein before LPS infusion (T0) and at 12 h (T, ) and 24 h (T2) of LPS infusion for determination of plasma nitrate concentration. Expired NO concentration was measured using NOA 280 NO analyzer. Results 1400W decreased LPS-induced increase in expired NO (P 0.05). Conclusion The selective iNOS inhibitor 1400W can inhibit iNOS activity and lead to decreased NO production.

BACKGROUND: Adipokines are secreted by adipose tissue, including a series of cytokines and chemokines. Additionally, adipokines have been found to play certain roles in pro- and anti-inflammatory regulation. OBJECTIVE: To analyze the regulatory effect of various adipokines in the inflammatory process of autoimmune response and immune diseases, thus providing the strategy for controlling and treating autoimmune diseases and searching effective indexes for assessing disease development, incidence of complications and prognosis. METHODS: CNKI and PubMed databases were retrieved for the articles addressing the role of adipokines in autoimmune diseases published from 1986 to 2016 using the keywords of adipokine, autoimmune in English and Chinese, respectively. RESULTS AND CONCLUSION: Adipose tissue plays a critical role in metabolism regulation, and its effect on the immune system has emerged through a series of studies concerning adipokines regulating immune response. Adipokines exert various functions, and regulate metabolism and inflammation. In the development and occurrence of autoimmune diseases, some factors for immune responses remain unclear, and in-depth studies on adipokines will be helpful for lifting the veil of autoimmune diseases.

The traditional denoising algorithm for ultrasound images would lost a lot of details and weak edge information when suppressing speckle noise. A new denoising algorithm of adaptive threshold based on curvelet transform is proposed in this paper. The algorithm utilizes differences of coefficients' local variance between texture and smooth region in each layer of ultrasound image to define fuzzy regions and membership functions. In the end, using the adaptive threshold that determine by the membership function to denoise the ultrasound image. The experimental text shows that the algorithm can reduce the speckle noise effectively and retain the detail information of original image at the same time, thus it can greatly enhance the performance of B ultrasound instrument.

Chinese Journal of General Surgery 2008;23(8):600-602
Objective To evaluate the prognostic value of lateral lymphadenectomy for advanced lower rectal cancer. Methods A total of 96 consecutive patients who underwent total mesorectal excision and lateral lymphadenectomy for advanced lower rectal cancer were retrospectively evaluated by using therapeutic index. A product of the frequency of lymph node metastasis and the cancer-related 5-year survival rate. Results The incidences of lymph node metastasis in mesorectal area, superior rectal artery area, inferior mesenteric artery area and lateral area was 21%,13%.10%and 15%respectively.The 5-year survival rate of patients with metastasis to the mesorectal area, superior rectal artery area, inferior mesenteric artery area and lateral area was 35%,25%,20%and 36%respectively.The therapeutic indexes for total mesorectal excision and lateral lymphadenectomy were 7.4 and 5.4,which were much higher than that of lymphadenectomy of the superior rectal artery area(3.3)and inferior mesenteric artery area(2.0).Local recurrence occurred in 19%(18 out of 96 cases)patients with advanced lower rectal cancer. Local recurrence rate of patients with positive lateral lymph node metastasis was 64%(9 of 14 cases),whereas it was 11%(9 of 82 cases)in those without lateral lymph node metastasis. The difference between these two groups was statistically significant(x2=22.308,P=0.000).Kaplan-Meier survival analysis showed significant improvements in median survival(80.9±2.1)m,95%CI:76.7~85.1 m vs(38±6.7)m,(95%CI:24.8~51.2 m)for patients with negative lateral lymph node metastasis over patients with lateral lymph node metastasis(log-rank,P=0.000). Conclusion The current results indicate that lateral pelvic lymphadenectomy could significantly reduce loeal recurrence and effectively improve the survival of patients with advanced lower rectal cancer. Except for total mesorectal excision, lateral pelvic lymphadenectomy should be followed in the surgery of pailents with advanced low rectal cancer.

Objective:To summarize the influencing factors and the preventative strategies of transurethral resection syndrome(TURS) in transurethral vaporization of the prostate(TVP). Methods:Among the consecutive 1360 patients who underwent TVP from Feb 1998 to 2004 Dec,using Circon vaporization device and vaporizaton-resection electrode, irrigating with 5% GS,TURS occured in 12 cases. The clinical materials were reviewed retrospectively. Results:At the end of operation, serum sodium declined and blood glucose rose apparently in the TURS cases.Of the 12 patients, mean operative time was 136 min(90-180min),prostate volume were Ⅲ?(50-75 g),perforation of the prostate capsule occurred in 8 cases, chill occued in 8 cases.TURS occurrence rate was relatively high in the high risk cases. The 12 patients were cured. Conclusion:The risk of TURS that still exists during TVP is relevant to: perforation of the prostate capsule, operative time, irrigating pressure, the high risk cases, prostate volume and(so on.)

Objective To investigate the application of colostomy care products in managing the leakage around PTCD drainage tube, and to discuss its clinical effect. Methods A total of 56 patients with malignant obstructive jaundice, who had received PTCD and suffered from postoperative leakage around PTCD drainage tube, were randomly and equally divided into the study group (n=28) and the control group (n=28). The colostomy care powder, the skin protective film and the colostomy bag were used for the patients of the study group, while iodophors, sterile gauze and mupirocin ointment were employed for the patients of the control group. The incidence of irritant dermatitis around PTCD drainage tube, the degree of comfort to the indwelling tube judged by the patient, and the nursing workload for PTCD drainage tube were determined, and the results were compared between the two groups. Results The incidence of irritant dermatitis around PTCD drainage tube in the study group was obviously lower than that in the control group. And the degree of comfort to the indwelling tube judged by the patient in the study group was much higher than that in the control group. The differences between the two groups were statistically significant (P<0.05). Conclusion Combination use of colostomy care powder, skin protective film and colostomy bag can effectively reduce the incidence of irritant dermatitis around PTCD drainage tube and improve the patient’s condition. Therefore, this method is worthy of popularization in clinical practice.
