Objective To compare the results between double-threaded compression screws and cannulated lag screws for Garden type Ⅰ/Ⅱ femoral neck fractures. Methods Data of 56 patients with Garden type Ⅰ/Ⅱ femoral neck fractures who were treated with internal fixation from July 2012 to March 2016 were retrospectively analyzed.According to different fixation, the patients were divided into two groups:double-threaded screws group (group A) including 27 cases (7 males and 20 females, with an average age of 55.5±10.0 years,14 cases Garden type Ⅰ and 13 cases Garden type Ⅱ,the proportion of osteoporosis 10/27;And lag screws group (group B) including 29 cases (8 males and 21 females, with an average age of 57.2±9.5 years,11 cases Garden type Ⅰ and 18 cases Garden type Ⅱ,the proportion of osteoporosis 15/29.The 2 groups were compared with respect to bony fusion time, nonunion, avascular necrosis of femoral head, shortening of femoral neck and Harris Hip Score. Results The average follow-up time was 20.3±8.7 m in Group A and 24.5±7.9 m in Group B.The average bony fusion time was 5.3±0.6 m in Group A and 4.1±0.8 m in Group B,and there was significant difference between the groups (t=5.758,P=0.00).1 avascular necrosis of femoral head was found in both groups. In group A,there were 3 cases of bone nonunion (11.1%,3/27).There were one bone nonunion (3.4%,1/29) case in group B,with no statistically significant difference between groups.Shortening of femoral neck occurred in both groups, the average horizontal shortening was 5.8±2.9 mm and vertical shortening 7.0±3.4 mm in Group A,which were lower than group B (8.2±3.1 mm,9.4±3.6 mm),and the differences showed statistically significant (t=-2.94,P=0.005;t=-2.48,P=0.016).In group A,the average Harris Hip score was 88.2±8.3,while that in group B was 89.9±7.2.There was no statistically significant difference between two groups. Conclusion It seems to us that in the treatment of Garden type Ⅰ/Ⅱ femoral neck fractures, doublethreaded compression screws fixation could reduce femoral neck shortening, but there is no difference on functional outcome compared to cannulated lag screws fixation. On the contrary,double-threaded compression screws fixation is not conducive to fracture healing due to lack of sliding compression.

OBJECTIVEThis study aimed to observe the postoperative pain rate and degree of pain in preschool children with cleft lip and palate, and investigate the effect of nursing intervention on pain relief.
METHODSA total of 120 hospitalized cases of three- to seven-year-old preschool children with cleft lip and palate were selected from May to October 2011. The subjects were randomly divided into the control group and experimental groups 1, 2, and 3. The control group used conventional nursing methods, experimental group 1 used analgesic drug treatment, experimental group 2 used psychological nursing interventions, and experimental group 3 used both psychological nursing intervention and analgesic drug treatment. After 6, 12, 24, and 48 h, pain self-assessment, pain parent-assessment, and pain nurse-assessment were calculated for the four groups using the pain assessment forms, and their ratings were compared.
RESULTSThe postoperative pain rates of the four groups ranged from 50.0% to 73.3%. The difference among the four groups was statistically significant (P < 0.001). The differences among the control group and experimental groups 1 and 2 were not statistically significant (P = 0.871), whereas the differences among experimental group 3 and the other groups were statistically significant (P < 0.001).
CONCLUSIONPostoperative pain in preschool children with cleft lip and palate is common. Psychological nursing intervention with analgesic treatment is effective in relieving postoperative pain.

The design space of the high shear wet granulation process was established and validated within the framework of quality by design (QbD). The system of microcrystalline cellulose-de-ioned water was used in this study. The median granule size and bulk density of granules were identified as critical quality attributes. Plackeet-Burmann experimental design was used to screen these factors as follows: dry mixing time, the impeller and chopper speed of dry mixing, water amount, water addition time, wet massing time, the impeller and chopper speed of wet massing and drying time. And the optimization was implemented with the central composite experimental design based on screened critical process parameters. The design space of the high shear wet granulation process was established based on the quadratic polynomial regression model. Since the P-values of both models were less than 0.05 and values of lack of fit were more than 0.1, the relationship between critical quality attributes and critical process parameters could be well described by the two models. The reliability of design space, illustrated by overlay plot, was improved with the addition of 95% confidence interval. For those granules whose process parameters were in the design space, the granule size could be controlled within 250 to 355 μm, and the bulk density could be controlled within a range of 0.4 to 0.6 g x cm(-3). The robustness and flexibility of the high shear wet granulation process have been enhanced via the establishment of the design space based on the QbD concept.

METHODSummarizing the drug properties of bear bile by reference to medical literatures for drug properties of TCM bear bile and Science of Traditional Chinese Medicine (China Press of Traditional Chinese Medicine, 2007). Analyzing and summarizing the pharmacological effects of main bile acid constituents according to relevant literatures for studies on pharmacological effects of main bile acid constituents in CNKI database. Predicating the drug properties of these bile acid constituents by using the drug property predication model established by the study group according the pharmacological effects of main bile acid constituents in the paper, and compare the prediction results with the drug properties of bear bile.
RESULTBile acid constituents in bear bile were mostly cold in property, bitter in taste, and the combination of their drug properties could reflect the combined drug properties of bear bile.
CONCLUSIONAll of these bile acid constituents in bear bile could show part of effects of bear bile. Attention shall be given to regulate the medication scheme in clinical application according to actual conditions.

Objective To investigate the effects of blocking gastrin receptor on the proliferation, apoptosis and expression of key proteins in the related pathway in gastric cancer cell lines. Methods In the experimental group, the gastric cancer cell lines SGC-7901 and AGS cells were treated with 5 mmol/L proglumide,a kind of a gastrin receptor antagonist. And the normal cultured gastric cancer cells SGC-7901 and AGS were used in control group. The growth of each group was detected by MTT assay;the cell growth curve was drawn by flow cytometry;the cell cycle of each group was detected by flow cytometry.Annexin V-FITC/PI double staining was used to detect the cell growth of apoptosis. The relative mRNA expression of β-catenin,nuclear factor-P65,mammalian target of rapamycin and glycogen synthase kinase 3 beta in Wnt,NF-κB and PI3K-AKT-MTOR pathways were detected by RT-qPCR.The expression of β-catenin protein was detected by Western blotting. Results After treatment with proglumide, the growth of the cells in the experimental group was lower than that in the control group;and the proportion of S phase cells in the cell cycle was also lower than that in the control group, but the proportion of cells in G0/G1 phase was higher than that in the control group(P<0.05).The percentage of apoptotic cells was also increased after treatment with proglumide(P<0.05).Furthermore,proglumide treatment significantly reduced the expression of β-catenin at both mRNA and protein levels(P<0.05).Conclusion Blocking gastrin receptor can down-regulate the expression of β-catenin,inhibit the cell proliferation and promote the cell apoptosis in gastric cancer cells.
