2.Psychological intervention on the clinical curative effect of knee osteoarthritis inpatients

Objective To explore the effect of psychological intervention on knee osteoarthritis patients. Methods A total of 92 OA patients were included into a one-month randomized controlled followup study, 47 of them were treated with combined psychological intervention, the other 45 cases were routinely treated. All cases were investigated on their demographic information , physiological parameters and score of the short form-36 health survey questionnaires (SF-36), self-rating depression scale (SDS) and self-rating anxiety scale (SAS). Statistics method used t-test and variance analysis. Results There was no signi-ficant difference in terms of the physiologic parameters changes between before and after treatment (P>0.05) of the two groups. ① The SDS scores of the two groups decreased gradually after the treatment (P<0.05),while there was remarkable descending of SDS score in patients of psychological intervention group (P<0.05). ② The SAS scores of the two groups decreased gradually after the treatment (P<0.05). And there was no significant difference between the two groups (P>0.05). Compared with the non-intervention group, the fourfactors scores including physical function, pain, vitality and role limitations due to emotional problems of the intervention group were significantly improved (P<0.05). Anegative correlation was observed between SDS and PF, BP, GH, VT and SF (r=-0.739-0521,P<0.05). SAS was correlated negatively with VT and SF (r=-0.699~-0.475,P<0.05). Conclusion Psychological intervention could ameliorate the depression of knee osteoarthritis patients. The quality of life of knee osteoarthritis patients can be improved by psychological intervention.

4.The clinical diagnostic value of SYT-SSX gene detection combined with immunohistochemistry in synovial sarcoma

Objective To analyse the clinicopathological characteristics of synovial sarcoma (SS) from histomorphology,immunohistochemical (IHC) and fluorescence in situ hybridization (FISH),and to investigate the related factors influencing the prognosis of SS patients.Methods 94 cases were collected,including 60 SS cases and 34 in dined to SS cases.The expressions of common antibodies related to the diagnosis of SS Vimentin,CD99,AE1/AE3,EMA,CD34,Calponin,Ki-67 were detected by IHC,and the SYT-SSX fusion gene was detected by FISH.The clinical pathologic factors that affecting the prognosis of patients were analyzed through the statistical method.Results The positive rates of Vimentin,CD99,AE1/AE3,EMA,CD34,Calponin,Ki-67 were 100.00 %,74.47 %,36.17 %,28.72 %,17.02 %,90.43 % and 60.64 % respectively.FISH results showed that 81 patients were tested with SYT gene translocation,including 54 cases who were considered as SS,and 27 cases who were suspected/inclined to SS,and the number of positive cells surpassed 80 %.The gene translocation was not related with the patient' s age,sex,tumor position,histological type,differentiation of the cancer (all P > 0.05).Single factor survival analysis showed that the degree of differentiation,tumor diameter,the recurrence and metastasis had certain influences on patients' survival time (all P < 0.05).Multiple factors regression analysis showed that the degree of differentiation and tumor diameter were the risk factors influencing the prognosis of SS (risk coefficients > 1,P < 0.05).Conclusions IHC combined with FISH SYT-SSX fusion gene detection have the vital significance in the diagnosis of SS.The degree of differentiation and tumor size are the risk factors influencing the prognosis of SS.
5.Practice of Clinical Pharmacists Participating in Anti-infective Treatment for A Patient with Tropical Candi-demia

OBJECTIVE:To explore the role of clinical pharmacists participating in drug therapy for patients with severe infec-tions. METHODS:Clinical pharmacists participated in drug treatment for a patient with tropical candidemia and assisted physicians to adjust anti-infective treatment plan. According to the results of blood culture,clinical pharmacists suggested Piperacillin sodium and tazobactam sodium for injection 3.75 g,ivgtt,q8 h+Caspofungin acetate for injection 50 mg (initial dose of 70 mg),ivgtt, qd,for symptomatic treatment;increased the daily dose of Caspofungin acetate for injection to 50 mg,ivgtt,bid due to plasma ex-change;Caspofungin acetate for injection 50 mg,ivgtt,qd+Amphotericin B for injection 0.1 mg/kg,ivgtt,qd for anti-infective plan due to the possible“contradiction”of echinocandins;closely monitored ADR,such as allergy,erythra,renal function injury. RE-SULTS:Physicians adopted the suggestions of clinical pharmacists,vital sign of patient kept stable,and tropical candidemia was not detected in the blood culture;the patient was transferred to general ward for further treatment. CONCLUSIONS:Based on the results of blood calture,clinical symptoms and the characteristics of drug effects,clinical pharmacists participated in the treatment for the patient with severe infection,retrieved related treatment guideline,assisted physicians to adjust anti-infective plan and close-ly monitored possible ADR so as to guarantee the effectiveness and safety of anti-infective treatment.

7.Modeling of a controllable acute regional cerebral ischemia in rats and evaluation with CT perfusion imaging and histopathology

Objective To establish a stable and controllable model of acute regional cerebral ischemia in rats, and to evaluate it by CT perfusion imaging and histological study. Methods Twenty eight male Wistar rats were randomly divided into 4 groups, and there were 7 rats in each group. The sham operation rats were defined as the first group, rats suffered from cerebral ischemia for 15 minutes were classified as the second group, rats suffered from cerebral ischemia for 30 minutes and then reperfusion for 1 hour as the third group, and rats suffered from hypo perfusion for 6 hours as the fourth group. Cerebral ischemia or hypo perfusion were induced by inserting a nylon thread of different diameter into right middle cerebral artery (MCA) of rats under the monitoring of regional cerebral blood flow (rCBF) by the Laser Doppler Blood Perfusion Monitor (BPM). rCBF was also examined by dynamic CT perfusion imaging. At the end of the observation time, rats were decapitated, and three rats of each group were performed 2,3,5 triphenyltetrazolium chloride (TTC) staining and four rats were performed histological study. Results In the second group, rCBF was controlled within 5% to 22% under the monitoring by BMP and CT perfusion imaging showed the decreased rCBF in 7 rats, but TTC staining showed red appearance indicating no infarction focus formed. Electronic microscopic study revealed astrocytic swelling and a few of neuronal degeneration. In the third group, rCBF was controlled within 4% to 23% under the monitoring by BMP. There were more severe astrocytic swelling and a lot of neuronal degeneration. The abnormal areas in CT perfusion images were the same as TTC staining. In the fourth group, in accordance with less decrease ment of rCBF (from 38% to 55%) in 7 rats, there were obvious astrocytic swelling and subtle neuronal degeneration. TTC stain did not show ischemia area. All these abnormal changes were not observed in the sham operation rats. Conclusion The controllable acute regional cerebral ischemic model in rats is very stable and repeatable. It can be simulated into the ischemic state of different perfusion level. This model is suitable for the research of acute cerebral infarction and regional cerebral ischemia. The facts that parallel changes existed among BMP measurement, CT perfusion imaging, and brain histology indicated that CT perfusion imaging is accurate and sensitive in evaluating acute regional ischemia.
8.Effect of acupuncture at different acupoints on electric activities of rat cerebellar fastigial nuclear.

OJECTIVETo explore whether different acupuncture signals were afferent to the cerebellar fastigial nucleus (FN) neuron and to find out their corresponding effect features through observing the effect of spontaneous discharge of cerebellar FN neuron by needling at different acupoints.
METHODSTotally 120 male SD rats were anesthetized by 20% urethane and their right cerebellar FN were positioned (AP 11. 6 mm, RL 1. 0 mm, H 5. 6 mm). Extracelluar discharge was recorded by glass microelectrode (AP: -11. 6 mm, R: 1. 0 mm, H: 5.7 -7. 0 mm), using extracellular microelectrode recording method, recording the spontaneous discharge of cerebellar FN neurons as a baseline. Random order of needling at zusanli (ST36), quchi (Lil1), weishu (BL21), and zhongwan (CV12) were compared with the baseline before each acupuncture. Their effects on the discharge of cerebellar FN neurons were observed and compared with baselines.
RESULTSThe frequency of FN neuronal discharge could be elevated by needling at zusanli (ST36), quchi (LiI), weishu (BL21), and zhongwan (CV12) (P <0. 01, P <0. 05). The response rate of needling at Zhongwan (CV12, 56. 00%) was higher than that of needling at Zusanli (ST36), Quchi (Ll1), and Weishu (BL21) (35. 00%, 34. 62%, 36. 63%, respectively) with statistical difference (P <0. 05). The response rate of needling at zhongwan (CV12) was obviously higher than that of needing at other points (F = 2. 101, P < 0. 05).
CONCLUSIONSNeedling at zusanli (ST36 ), quchi (Lil), weishu (BL21), and zhongwan (CV12) could elevate the spontaneous discharge frequency of cerebellar FN neurons. Needling at Zhongwan (CV12) had advantageous roles in regulating cerebellar FN.

9.Parameter Optimization in Rectal Probe Electrostimulation to Release Spasticity after Spinal Cord Injury

Objective To screen the optimal parameters of rectal probe electrostimulation (RPES) to release spasticity after spinal cord injury. Methods 10 patients with muscular tension more than 2 level of the modified Ashworth scale after spinal cord injury received the RPES in 9 parameters combinations from orthogonal array, every patient underwent a combination per day. Their muscular tension, clonus and reflex were recorded before and after each treatment. The varieties were analyzed with orthogonal design variance analysis. Results The optimal parameter for muscular tension was 10 mA, 3 /min, 15 min; for reflex: 10 mA, 4 /min, 10 min; for clonus: 40 mA, 2 /min, 15 min; spasm score: 10 mA, 2 /min, 15 min. Conclusion The stimulus intensity most affects the muscular tension, reflex and spasm score, and the frequency of stimulation most affects the clonus. The parameter components were separately for muscular tension, reflex, clonus and spasm score.
10.Chemical constituents from Crepis crocea.

Thirteen compounds were isolated from the ethyl acetate fraction of Crepis crocea by column chromatographies on silica gel, Sephadex LH-20 and semi-preparative HPLC. The structures were elucidated on the basis of spectral analysis as tectorone I (1), 8β- (2-methyl- 2-hydroxy-3-oxobutanoyloxy) -glucozaluzanin C (2), tectoroside (3), luteolin-7-O-glucoside (4), cosmosiin (5), esculetin (6), 3,4-dihydroxybenzaldehyde (7), trans-4-hydroxycinnamic acid (8), Caffeic acid (9), methyl p-hydroxyphenyllactate (10), ethylp- hydroxyphenyllactate (11), cis-3,4-dihydroxy-β-ionion (12). All the compounds, except for compounds 4 and 9, were isolated from this plant for the first time, and tectorone I (1) is a new natural product.
